Epidemiology and Histology Aspects of Breast Cancers of Women in Ivory Coast

Breast cancer is a major public health problem both in developed countries, where it represents the first female cancer, and in developing countries, where its incidence is increasing. Purpose: The authors report the results of a study whose objective was to describe the epidemiological and pathological features of women’s breast cancers in Ivory Coast. Material and Methods: This is a retrospective descriptive study on breast cancer histologically confirmed and made from the records of pathology laboratories of the University Hospital in Abidjan. The study period was 24 years (1984-2007). The parameters studied were: frequency, age, risk factors, the macroscopic and histological aspects. Results: Women’s breast cancer was the second cancer among women (13.69%) after cervical cancer and represented 6.51% of cancers. The average age was 45.21 years ranging from 10 years to 85 years. Subjects from 35 to 44 years were the most affected (30.7%) and 8% of patients were under 30 years (n=61). Multiparity (53.55%) and lower socioeconomic level (79.63%) were associated. At the macroscopic level nodular aspects represented 47.62% of cases (n=301). Carcinomas were 92% (n=710) of histological types with 94.36% (n=692) of invasive carcinomas. Sarcomas (5.7%), lymphoma (2.1%) and Paget’s disease (0.3%) were also observed. With regard to prognostic, the grading of Scarff-Bloom and Richardson showed 26.75% (n=156) grade I, 60.64% (n=354) grade II and 12.52% (n=73%) grade III. Conclusion: In consideration of prognosis of advanced forms, an effort should be made for diagnosis and early treatment of women’s breast cancer in Ivory Coast.

This case report is about a 61 years old woman who consulted for hematuria, the cystoscopy performed revealed an intra-diverticular bladder tumor. After tumor resection, an anatomopathological exam revealed the urothelial carcinoma pT1. A few months later she presented with a local reccurence of bladder tumor. We proposed her anterior pelvectomy but she refused it and she was lost to follow-up. The aim of this study is to show the anatomopathological and evolutionary characteristics and the therapeutic difficulties of IDBT. These tumors are in the majority of cases infiltrating from the outset with a high potential for recurrence after transurethral resection of bladder (TURB). The standard treatments are TURB and total cystectomy.

Renal biopsy is an invasive procedure used to evaluate the activity and the therapeutic management of kidney disease and kidney transplantation. Objective: The aim of this study was to describe the epidemiological, clinical, and pathological features of kidney disease diagnosed by biopsy in the Ivory Coast. Materials and Methods: This was a descriptive and prospective study conducted between January 2015 and December 2018 in the Department of Anatomy and Cytology of the Pathology of Cocody (Abidjan) and Bouake. Specimens were obtained from the nephrology department of Ivory Coast and from teaching hospitals in Togo, Guinea, Burkina Faso, and Mali. Samples were analyzed according to standard renal biopsy procedures. One kidney fragment was fixed in acetic acid formalin (AAF) for optical microscopy examination. The other fragment, soaked in physiological water, was immediately sent for immunofluorescence examination. All renal pathologies were included in this study. The study parameters were frequency, age, sex, origin, occupation, clinical and biological signs, and pathological aspects with optical microscopy, and immunofluorescence. Results: 153 cases (2.23%) of kidney biopsies were examined on a total of 10,573 specimens. Ivory Coast specimens accounted for 91.5% of cases (n = 140). Nephrotic syndrome (49%) was the most common clinical sign and indication for renal biopsy, followed by acute renal failure (16.3%), chronic renal failure (19.6%), and a combination of chronic renal failure with hypertension (11.18%) and glomerulonephritis (4%). Mean proteinuria was 3.03 g/24h (range, 0.14 to 11.5 g/24h). Histologically, 90.8% (n = 139) were glomerular nephropathies, including 26.6% HIV-associated nephropathy, 17.3% focal segmental glomerulosclerosis, 13.6% nephroangiosclerosis, 11.5% post-infectious glomerulonephritis, 9.3% membranous glomerulonephritis, and 21.6% miscellaneous glomerular nephropathies (n = 30). Biological Profile and Cardiovascular Risk in Patients Receiving Neuroleptics at the Psychiatric Department of the University Hospital Center of Brazzaville

Several studies report the problem of cardiovascular tolerance of treatments with neuroleptics, given the important number of morbidities in patients with mental illnesses. This preliminary work aimed to describe the epidemiological and biological profile of patients taking neuroleptics and followed in the psychiatry department of Brazzaville University Hospital, from the angle of cardiovascular risk. Fifty (50) patients (17 men and 33 women), with a mean age of 33.9 10.7 years, were included. Epidemiological data (sex, age, tobacco or alcohol consumption) were collected on pre-established survey forms. Biochemical (total cholesterol, HDL-c, triglycerides and atherogenicity index) and inflammatory parameters (ultra-sensitive CRP, troponin I and NT-ProBNP) were investigated using enzymatic and indirect immunofluorescence technical, respectively. The results obtained showed that 54% of patients were obese, 94% were non-smokers, and 12% had high blood pressure. 10% of patients had high total cholesterol levels and 90% had HDL cholesterol levels below 60 mg/dl. Triglycerides and atherogenicity index were significantly elevated in relation to Body Mass Index (BMI). Ultrasensitive CRP was elevated in 38% of patients. In conclusion, this study revealed an association between lipid parameters (triglycerides and atherogenicity index) in relation to BMI in patients taking neuroleptics followed in the Psychiatry Department of University Hospital Center of Brazzaville.

Asymptotic Normality of the Nelson-Aalen and the Kaplan-Meier Estimators in Competing Risks

This paper studies the asymptotic normality of the Nelson-Aalen and the Kaplan-Meier estimators in a competing risks context in presence of independent right-censorship. To prove our results, we use Robelledo’s theorem which makes it possible to apply the central limit theorem to certain types of particular martingales. From the results obtained, confidence bounds for the hazard and the survival functions are provided.

The siltation of reservoir causes many problems of management and environmental disturbances that are important to minimize. Solving these problems requires in particular a good knowledge of mud bank instabilities. In partnership with the Laboratoire National d'Hydraulique (LNH), a subsidiary of Electricite de France (E.D.F.), authors realize a study of soft soils sliding entailed by reservoir emptying. Using a geotechnical flat sliding mechanism, the theoretical formulation of the safety factor can be established when the sediment mass is submerged and emergent. The measurement campaign carried out makes it possible to obtain bathymetric and topographic surveys and a geotechnical characterization of the soil for the whole reservoir of Grangent dam, located on the Loire (France). The proposed model appears to agree satisfactorily with the instability of the zones emerged because of the emptying and with the stability of the profile after emptying. In this paper, stochastic processes developed by Aalen [1]?[2] are adapted to the Nelson-Aalen and Kaplan-Meier?[3] estimators in a context of competing risks. We focus only on the probability distributions of complete downtime individuals whose causes are known and which bring us to consider a partition of individuals into sub-groups for each cause. We then study the asymptotic properties of nonparametric estimators obtained.

Hysterectomy for Uterine Inversion: About Two Cases at University Hospital of Cocody (Abidjan—Ivory Coast)

It is a rare clinical condition that may occur to two possible types: puerperal uterine inversion (IUP) and no puerperal uterine inversion (IUNP). Obstetrician will observe often only once in his professional practice. The risk of maternal mortality by hemorrhage is high. We report two cases of uterine inversion requiring a hysterectomy, including a postpartum mode and the other gynecological mode with young women. Hysterectomy remained a last resort treatment in the two types, to reduce morbidity. In the case of uterine inversion, the treatments without hysterectomy could be facilitated if the management is early and adequate. Furthermore, the support should be multidisciplinary and fast in the two types.

Molecular Characterization and Prevalence of Trypanosoma Species in Cattle from a Northern Livestock Area in Cote d’Ivoire

Background: African animal trypanosomiasis (AAT) is caused mainly by Trypanosoma congolense, T. vivax, and T. brucei brucei and is the major constraint for livestock productivity in Sub-Saharan African countries. Information about animal trypanosomiasis status in Ivory Coast is missing, especially regarding molecular epidemiology. Therefore, this study intended to apply molecular tools to identify and characterize trypanosomes in Ivory Coast for sustainable control. Methods: 363 cattle blood samples were collected from Ferkessedougou Region in northern Ivory Coast in 2012. Buffy coat technique (BCT) and species-specific PCR assays were used to detect trypanosome species. Results: Out of 363 cattle examined with BCT, 33 were found positive with all trypanosomes species accounting for an average of 9.09% prevalence whereas polymerase chain reaction (PCR) using species-specific primers showed that 81 out of 363 cattle were infected with trypanosomes with an overall prevalence of 22.31%. Trypanosoma congolense savanah type, T. Vivax and T. brucei sl. accounted for 28.39%, 49.38% and 23.45% of the infection rate respectively. No infection with T. congo forest?type was detected. T. vivax infection was the most prevalence in the area investigated compared to the two other trypanosome species. Mixed infections with different trypanosomes species were detected accounting for 7.32% of prevalence. Regarding sexrelated prevalence, male cattles were slightly more infected than female but the difference was not significant. Conclusion: Our results showed that there was a high prevalence of AAT in livestock in Ferkessedougou Area. There is therefore a need to strengthen control policies and institute measures that help prevent the spread of the parasites for sustainable control of animal trypanosome in this area.
