Child-Pugh-Turcott versus Meld score for predicting survival in a retrospective cohort of black African cirrhotic patients

AIM: To compare the performance of the Child-Pugh-Turcott (CPT) score to that of the model for end-stage liver disease (MELD) score in predicting survival of a retrospective cohort of 172 Black African patients with cirrhosis on a short and mid-term ***: Univariate and multivariate (Cox model) analyses were used to identify factors related to mortality. Relationship between the two scores was appreciated by calculating the correlation coefficient. The Kaplan Meier method and the log rank test were used to elaborate and compare survival respectively. The Areas Under the Curves were used to compare the performance between scores at 3, 6 and 12 ***: The study population comprised 172 patients, of which 68.9% were male. The mean age of the patient was 47.5 ± 13 years. Hepatitis B virus infection was the cause of cirrhosis in 70% of the cases. The overall mortality was 31.4% over 11 years of follow up. Independent factors significantly associated with mortality were: CPT score (HR = 3.3, 95% CI [1.7-6.2]) (P 1.5 mg/dL versus serum creatine 21 vs MELD < 21). The area under the curves (AUC) that predict survival was 0.72 and 0.75 at 3 mo (P = 0.68), 0.64 and 0.62 at 6 mo (P = 0.67), 0.69 and 0.64 at 12 mo (P = 0.38) respectively for the CPT score and the MELD ***: The CPT score displays the sameprognostic significance as does the MELD score in black African patients with cirrhosis. Moreover, its handling appears less cumbersome in clinical practice as compared to the latter.

Comparitive evaluation of different systems of medicines and the present scenario of chikungunya in Kerala

Utilization of psychoacoustic parameters for occupancy-based acoustic evaluation in eating establishments

Psychoacoustic parameters are mostly used for determining the sound quality of mechanical *** have also started to be used for evaluating soundscapes of open areas and enclosed *** research aims to find out the relationship between the psychoacoustic parameters and acoustic comfort in non-acoustic enclosed public spaces,specifically in eating *** on-site measurements and laboratory listening tests were conducted for five eating *** on-site measurements,a simultaneous questionnaire study was also carried *** and objective data were comparatively *** tests were based on auralizations with calibrated simulation *** provided a research model,that allows control over the acoustic environment without having to make real changes in the physical elements of the eating *** auralization sound files were presented to listening test participants with headphones and they evaluated soundscapes with different psychoacoustic *** results,better acoustic comfort was found to be related with higher sharpness,lower reverberation time,lower loudness and lower roughness values for the examined parameter ranges.

Paper-based devices have attracted extensive attention due to the growing demand for disposable flexible ***,we integrate semiconducting devices on cellulose paper substrate through a simple abrasion technique that yields high-performance photodetectors.A solvent-free WS_(2) film deposited on paper favors an effective electron-hole separation and hampers *** as-prepared paper-based WS2 photodetectors exhibit a sensitive photoresponse over a wide spectral range spanning from ultraviolet(365 nm)to near-infrared(940 nm).Their responsivity value reaches up to~270 mA W^(−1) at 35 V under a power density of 35 mW cm^(−2).A high performance photodetector was achieved by controlling the environmental exposure as the ambient oxygen molecules were found to decrease the photoresponse and stability of the WS_(2) ***,we have built a spectrometer using such a paperbased WS_(2) device as the photodetecting component to illustrate its potential *** present work could promote the development of cost-effective disposable photodetection devices.

The objective of this study was to investigate the effects of diammonium glycyrrhizinate(GL) on the pharmacokinetics of aconitine in rats and the potential *** oral administration of GL(50 mg/kg),the peak plasma concentration(C),area under the plasma concentration-time curve from zero up to timeτ(AUC) and absolute bioavailability of aconitine(0.2 mg/kg) significantly increased 1.64,1.63 and 1.85 fold,respectively,but there was no significant change in half life(t) or clearance(CL).In the other two routes of administration via tail vein and the hepatic portal vein,GL(15 mg/kg) did not affect any of the pharmacokinetic parameters of aconitine(0.02 mg/kg). Thus,GL can enhance the absorption of aconitine,leading to higher oral bioavailability and plasma levels but does not influence its ***,in vitro everted gut sac model and ussing chamber model were used to investigate the potential *** from bidirectional transport and inhibition studies demonstrated that P-glycoprotein was the main efflux transporter involved in the absorption of aconitine in *** absorption enhancement effect of GL should be mainly attributed to inhibiting the activity of P-glycoprotein rather than the influence on the paracellular or transcellular transport.

Fast clogging problem of open check dams and a new type suggestion:curved footed type open check dam

Driftwood is one of the important physical components in mountainous rivers which causes severe hazards due to the clogging of bridges,culverts,and narrow sections during ***,the understanding of driftwood dynamics and mitigation measures are crucial for managing wood in *** check dams are the most commonly used engineering measure for preventing driftwood from reaching downstream ***,these open check dams frequently lose their sediment transport function when they are blocked by sediment and driftwood,especially during major flood *** paper proposes a new type of open check dam for preventing from ***,flume experiments were conducted to examine the influence of different types of open check dams on the characteristics of driftwood *** the model with wood length(LWD)=16.5 cm,wood diameter(D)=15 mm,and wood number(N)=172,the highest trapping efficiency was observed with 90.1%and 87.2%retention rates for the classical debris flow breaker and curved footed open check dams,*** tests showed that through this proposed design,woody debris blockage in a very short time was prevented from the accumulation of woods beside the *** addition to this,most of the sediment passed through the check dam and most of the driftwood got *** can be briefly stated that the geometrical design of the structure plays an important role and can be chosen carefully to optimize trapping *** designing this type of open check dams in mountain river basins,it may provide a better understanding of the driftwood accumulation and basis for the optimal design of these *** development of the solution proposed in this work can pave the way for designing different types of open check dams for effective flood management.

Hematological profile of COVID-19 infected children before and after the spread of the Omicron variant in Istanbul

Objective:To examine the effect of the COVID-19 virus,especially the Omicron variant,on hematological parameters of hospitalized pediatric patients during the COVID-19 ***:Medical records of pediatric COVID-19 patients hospitalized at Kartal Dr.Lütfi K?rdar City Hospital in Istanbul,Turkey,between March 2020 and May 2022 were retrospectively reviewed to analyze data regarding demographics,SARS-Co V-2 infection polymerase chain reaction(PCR)test results,reversetranscriptase(RT)-PCR for other respiratory agents,duration of hospital stay,and hematological and biochemical laboratory ***:Out of 467 children with a confirmed diagnosis of SARSCo V-2 infection,94(20.1%)had Omicron infection and 373(79.9%)were infected with other variants;the Omicron group had younger patients than the remaining samples(P<0.001).The most frequent clinical symptoms in all children were cough(53.5%)and fever(32.3%),followed by vomiting(20.8%).Lung involvement in the Omicron group(10.6%)was significantly lower than in the remaining samples(29.8%)(P<0.001).Hemoglobin and lymphocyte levels were lower in the Omicron-infected group(both P<0.001),while prothrombin time,activated partial thromboplastin time,international normalized ratio,and D-dimer levels were significantly higher in this group(P<0.001,P<0.001,P<0.001,and P=0.023,respectively).In terms of lung involvement,those with lung involvement were significantly older(P<0.001).Conclusions:Although lung involvement was less common with Omicron infection,this group had greater hematological system involvement,such as anemia,lymphopenia,D-dimer elevation,and coagulation disorders.

Aerodynamics of non-slender delta and reverse delta wings:Wing thickness,anhedral angle and cropping ratio

The effects of thickness-to-chord(t=c)ratio,anhedral angle(d),and cropping ratio from trailing-edge(Cr%)on the aerodynamics of non-slender reverse delta wings in comparison to non-slender delta wings with sweep angle of 45°were characterized in a low-speed wind tunnel using force and pressure *** measurements were conducted for total of 8 different delta and reverse delta *** different t/c ratios of 5.9%and 1.1%,and two different anhedral angles ofd=15°and 30°for non-cropped and cropped at Cr=30%conditions were *** results indicate that the reverse delta wings generate higher lift-to-drag ratio and have better longitudinal static stability characteristics compared to the delta *** wing thickness has favorable effect on longitudinal static stability for the reverse delta wing whereas longitudinal static stability is not influenced by wing thickness for the delta *** reverse delta wings,the anhe-draled wing without cropping has adverse effect on aerodynamic performance and decreases the lift-to-drag *** in anhedraled wing causes significant improvement in lift-to-drag ratio,shift in aerodynamic and pressure centers towards the trailing-edge,and enhancement in longitudi-nal static stability.

Production of nanocrystalline silver particles by hydrogen reduction of silver nitrate aerosol droplets

Nanocrystalline silver particles were produced by hydrogen reduction of silver nitrate aerosol droplets formed by high frequency ultrasonic *** dependences of the particle size,morphology and crystallite size on the precursor concentration and the reaction temperature were *** spray pyrolysis process was combined with hydrogen reduction to research the effects on the silver particle *** silver particles including slight oxide structure were prepared at temperature as low as 200 ℃ from silver nitrate under hydrogen atmosphere.X-ray diffraction（XRD） studies showed that pure silver particles were obtained above 200 ℃ reaction *** crystallite sizes of the samples ranged from 29 to 47 *** results indicate that the crystallite sizes hardly ever depended on the reaction *** slightly enlarged by increasing precursor *** observations showed that particles were obtained in spherical morphology with particle sizes between 210 and 525 *** temperature and precursor concentration strongly influenced the particle size.
