Retinal ganglion cell complex changes using spectral domain optical coherence tomography in diabetic patients without retinopathy

AiM：To assess the ganglion cell complex（GCC）thickness in diabetic eyes without retinopathy. METHODS：Two groups included 45 diabetic eyes without retinopathy and 21 non diabetic eyes. All subjects underwent full medical and ophthalmological history,full ophthalmological examination,measuring GCC thickness and central foveal thickness（CFT）using the RTVue~？ spectral domainoptical coherence tomography（SD-OCT）,and HbA1C ***：GCC focal loss volume（FLV%）was significantly more in diabetic eyes（22.2% below normal）than normal eyes（P=0.024）. No statistically significant difference was found between the diabetic group and the control group regarding GCC global loss volume（GLV%）（P=0.160）. CFT was positively correlated to the average,superior and inferior GCC（P=0.001,0.000 and 0.001 respectively）and negatively correlated to GLV% and FLV%（P=0.002 and0.031 respectively）in diabetic eyes. C/D ratio in diabetic eyes was negatively correlated to average,superior and inferior GCC（P=0.015,0.007 and 0.017 respectively）. The FLV% was negatively correlated to the refraction and level of Hb A1c（P=0.019 and 0.013 respectively）and positively correlated to the best corrected visual acuity（BCVA）in log MAR in diabetic group（P=0.004）.CONCLUSiON：Significant GCC thinning in diabetes predates retinal vasculopathy,which is mainly focal rather than diffuse. it has no preference to either the superior or inferior halves of the macula. increase of myopic error is significantly accompanied with increased focal GCC loss. GCC loss is accompanied with increased C/D ratio in diabetic eyes.

Optimal Tuning of FOPiD-Like Fuzzy Controller for High-Performance Fractional-Order Systems

This paper addresses improvements in fractional order(FO)system *** the classical proportional-integral-derivative(PiD)-like fuzzy controller can provide adequate results for both transient and steady-state responses in both linear and nonlinear systems,the FOPiD fuzzy controller has been proven to provide better *** high performance was obtained thanks to the combinative benefits of FO and fuzzy-logic *** paper describes how the optimal gains and FO parameters of the FOPiD controller were obtained by the use of a modern optimizer,social spider optimization,in order to improve the response of fractional dynamical *** group of systems had usually produced multimodal error surfaces/functions that occasionally had many variant local *** integral time of absolute error(iTAE)used in this study was the error *** results showed that the strategy adopted produced superior performance regarding the lowest iTAE *** reached a value of 88.22 while the best value obtained in previous work was 98.87.A further comparison between the current work and previous studies concerning transient-analysis factors of the model’s response showed that the strategy proposed was the only one that was able to produce fast rise time,low-percentage overshoot,and very small steady-state ***,the other strategies were good for one factor,but not for the others.

Hydrocarbons,carbon monoxide and other pollutants from the transportation sector harm human health in many *** cell(FC)has been evolving rapidly over the past two decades due to its efficient mechanism to transform the chemical energy in hydrogen-rich compounds into electrical *** main drawback of the standalone FC is its slow dynamic response and its inability to supply rapid variations in the load ***,adding energy storage systems is ***,to manage and distribute the power-sharing among the hybrid proton exchange membrane(PEM)fuel cell(FC),battery storage(BS),and supercapacitor(SC),an energy management strategy(EMS)is *** this research work,an optimal EMS based on a spotted hyena optimizer(SHO)for hybrid PEM fuel cell/BS/SC is *** main goal of an EMS is to improve the performance of hybrid FC/BS/SC and to reduce the amount of hydrogen *** prove the superiority of the SHO method,the obtained results are compared with the chimp optimizer(CO),the artificial ecosystem-based optimizer(AEO),the seagull optimization algorithm(SOA),the sooty tern optimization algorithm(STOA),and the coyote optimization algorithm(COA).Two main metrics are used as a benchmark for the comparison:the minimum consumed hydrogen and the efficiency of the *** main findings confirm that the minimum amount of hydrogen consumption and maximum efficiency are achieved by the proposed SHO based EMS.

A reliable approach based on a multi-verse optimization algorithm(MVO)for designing load frequency control incorporated in multi-interconnected power system comprising wind power and photovoltaic(PV)plants is presented in this *** has been applied for optimizing the control parameters of the load frequency controller(LFC)of the multi-source power system(MSPS).The MSPS includes thermal,gas,and hydro power plants for energy ***,the MSPS is integrated with renewable energy sources(RES).The MVO algorithm is applied to acquire the ideal parameters of the controller for controlling a single area and a multi-area MSPS integrated with *** link is utilized in shunt with AC multi-areas interconnection tie *** proposed scheme has achieved robust performance against the disturbance in loading conditions,variation of system parameters,and size of step load perturbation(SLP).Meanwhile,the simulation outcomes showed a good dynamic performance of the proposed controller.

Radiotherapy in Non-Functioning Pituitary Macroadenoma： Mansoura Experience

OBJECTiVE The current retrospective study aims to evaluate the management of non-functioning the assessment of experience on pituitary macroadenoma through clinical, biochemical, radiological features, and treatment outcome of patients, and to identify prognostic factors affecting progression-free survival （PFS）. METHODS Data of 55 patients macroadenoma presented to the with non-functioning pituitary Clinical Oncology and Nuclear Medicine department between 1998 and 2009 were investigated. RESULTS The most common symptom was visual disturbance （38.2%） followed by headache （27.3%）. The presence of male predominance was observed （1.4：1）. Ten patients received radio-therapy （RT） only. Extrasellar extension was the more common treatment. The overall response rate was 72.8% with completed response at 16.4%. Memory and intellectual sequelae were the most common late complications of treatment （14%）. The ten-year PFS was at 84.6%. PFS was found to be significantly better with higher dose of RT （up to 54 Gy）, treatment by both surgery and RT, absence of visual field defect, and tumor localized to sella, whereas it was not significantly affected by age and sex. CONCLUSiON The data confirmed that the prevalence of mass effect and hypopituitarism in patients with non-functioning pituitary macroadenoma is elevated. Conventional external RT up to 54 Gy is safe and effective in controlling non-functioning pituitary macro- adenoma with tolerable and acceptable morbidity.

Treatment of Mild to Moderate Hallux Valgus, Percutaneous Osteotomy versus Distal Chevron Osteotomy

Objective: The aim of this study was to compare two different surgical techniques for treating mild to moderate cases of hallux valgus. Methods: Forty-one patients (58 feet) with mild to moderate hallux valgus were treated by either percutaneous technique (n = 24) patients (33 feet) or by distal chevron osteotomy (n = 17) (25 feet). Results: in the percutaneous group, after a mean follow up of 49.36 months, the mean correction of hallux valgus angle (HV) was 26.69°in the intermetatarsal angle, average correction was 9.45°. The mean improvement of American Orthopedic Foot and Ankle Score (AOFAS) was 46.45 points. in the chevron osteotomy group, the mean follow up of 51.56 months, the mean correction of hallux valgus angle was 26.78°in the intermetatarsal angle, average correction was 9°. The mean improvement of AOFAS was 44.76 points. Conclusion: Both techniques gave satisfactory results. However, the percutaneous technique is safer than the distal chevron osteotomy.

The main goal of this research is to develop and apply a robust Artificial Neural Networks(ANNs)model for predicting the characteristics of the osmotically drying treated potato and apple samples as a model heat-sensitive product in vacuum contact *** salt and sugar solutions were used as the osmotic solutions at 27◦*** of experiments were performed at various temperatures of 35◦C,40◦C,and 55◦C for conduction heat input under vacuum(−760 mm Hg)*** experiments were also performed in a pure vacuum without heat *** moisture content(DMC),effective moisture diffusivity,and mass flux were considered as the performance parameters in this *** revealed that the osmotic dehydration using a concentrated sugar solution shows a higher reduction in the initial moisture loss of 19.87%compared to 5.3%in the salt ***,a significant enhancement of drying performance of about 27%in DMC was observed for both samples at vacuum and 40◦C compared to pure vacuum drying *** the experimental data,a robust artificial neural network(ANN)was proposed to describe the osmotic dehydration’s behavior on the drying *** ANN model outputs are the dimensionless moisture contents(DMC),the diffusivity,and the mass *** the ANN inputs were the drying time,the percent of sugar solution,and the percent of salt *** the ANN apple’s model,the minimum root mean square error(RMSE)values were 0.0261,0.0349 and 0.0406,for DMC,diffusivity,and mass flux,*** the best correlation coefficients of the above three parameters’determination values were 0.9909,0.9867 and 0.9744,*** the ANN potato’s model,the minimum RMSE values were 0.0124,0.0140 and 0.0333,for DMC,diffusivity,and mass flux,*** the best correlation coefficients of the parameters’values were found 0.9969,0.9968 and 0.9736,***,the ANN model’s prediction has a perfect agreement with th

Tadalafil(TAD)and dapoxetine HCl(DAP)are recently co-formulated and both show native ***,a novel,accurate,specific and sensitive reversed-phase high performance liquid chromatographic method with fluorescence detection was developed and validated for their separation and quantitation in dosage form and human plasma using avanafil as an internal standard(iS).Separation was achieved using isocratic elution within 7.0 min on C18column and acetonitrile-0.15%triethylamine(40∶60,v/v;pH 4)as a mobile *** flow rate was 1.0 mL/min and the detection was time-programmed at 330,410 and 370 nm for TAD,DAP and iS,respectively,after excitation at 236 *** linear ranges from 0.01 to 30.00μg/mL for each drug with the limits of detection of 4.20 and 7.20 ng/mL for TAD and DAP,*** method was validated in accordance to the international Conference on Harmonization(iCH)guidelines and was successfully applied to spiked human plasma with mean recoveries of 98.17%and 98.83%for TAD and DAP respectively.

Evaluation of Lead Hepatotoxicity;Histological, Histochemical and Ultrastructural Study

Lead is one of the most well-known naturally occurring environmental heavy metals. This experimental study was designed to evaluate lead induced toxic effects on hepatocytes and lobular architecture as judged microscopically. Material and Methods: This study was conducted in anatomy department, Benha faculty of medicine, Benha University, Egypt from May to October 2013 on 30 normal adult albino rats divided into 3 groups;one control and 2 experimental groups. The experimental groups were given 0.13% lead acetate solution in drinking water for 4 and 8 weeks, respectively. Animals were scarified and livers were removed and used to identify microscopic changes. Specimens were stained with Hematoxylin and eosin, with Masson trichrome stain for study of fibrous tissue and with periodic acid shiff's (PAS) to study the glycogen content. Other specimens were prepared for ultrastructural study. Results: Mild lymphocytic infiltration, vacuolar degeneration and mild increase of periportal fibrosis with mild depletion of glycogen content and partial disappearance of glycogen vacuoles were reported in animals received contaminated water for 4 weeks. Animals maintained for 8 weeks on contaminated water showed hepatic changes in the form of abundant lymphocytic infiltration, increased cellular polymorphism, pyknotic nuclei and areas of cell necrosis with evident moderate periportal fibrosis and marked vacuolar degeneration associated with marked depletion of glycogen content. Ultrastructural study revealed mitochondrial edema, appearance of interstitial inflammatory cells, and appearance of scattered variable sized lead electron-dense inclusion bodies. Conclusion: it could be concluded that chronic exposure to lead imposes a potent toxic effect on liver cells manifested as glycogen depletion, cellular infiltration and liver architecture in the form of initiation of periportal fibrosis that may progress to liver cirrhosis.
