Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases

AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.

Disconnections due to voltage drops in the grid cannot be permitted if wind turbines(WTs)contribute significantly to electricity pro-duction,as this increases the risk of production loss and destabilizes the *** mitigate the negative effects of these occurrences,WTs must be able to ride through the low-voltage conditions and inject reactive current to provide dynamic voltage *** paper investigates the low-voltage ride-through(LVRT)capability enhancement of a Type-3 WT utilizing a dynamic voltage restorer(DVR).During the grid voltage drop,the DVR quickly injects a compensating voltage to keep the stator voltage *** paper proposes an active disturbance rejection control(ADRC)scheme to control the rotor-side,grid-side and DVR-side converters in a wind–DVR integrated *** performance of the Type-3 WT with DVR topology is evaluated under various test conditions using MATLAB®/Simulink®.These simulation results are also compared with the experimental results for the LVRT capability performed on a WT emulator equipped with a crowbar and direct current(DC)*** simulation results demonstrate a favourable transient and steady-state response of the Type-3 wind turbine quantities defined by the LVRT codes,as well as improved reactive power support under balanced fault *** the most severe voltage drop of 95%,the stator currents,rotor currents and DC bus voltage are 1.25 pu,1.40 pu and 1.09 UDC,respectively,conforming to the values of the LVRT *** controlled by the ADRC technique significantly increases the LVRT capabilities of a Type-3 doubly-fed induction generator-based WT under symmetrical voltage dip *** setting up ADRC controllers might be challenging,the proposed method has been shown to be extremely effective in reducing all kinds of internal and external disturbances.

Predictors of long-term survival after pancreaticoduodenectomy for peri-ampullary adenocarcinoma: A retrospective study of 5-year survivors

Background: Pancreaticoduodenectomy(PD) is the standard curative treatment for periampullary tumors. The aim of this study is to report the incidence and predictors of long-term survival( ≥ 5 years) after PD. Methods: This study included patients who underwent PD for pathologically proven periampullary adenocarcinomas. Patients were divided into 2 groups: group(I) patients who survived less than 5 years and group(II) patients who survived ≥ 5 years. Results: There were 47(20.6%) long-term survivors( ≥ 5 years) among 228 patients underwent PD for periampullary adenocarcinoma. Patients with ampullary adenocarcinoma represented 31(66.0%) of the long-term survivors. Primary analysis showed that favourable factors for long-term survival include age < 60 years old, serum CEA < 5 ng/mL, serum CA 19-9 < 37 U/mL, non-cirrhotic liver, tumor size < 2 cm, site of primary tumor, postoperative pancreatic fistula, R0 resection, postoperative chemotherapy, and no recurrence. Multivariate analysis demonstrated that CA 19-9 < 37 U/mL [OR(95% CI) = 1.712(1.24 8–2.34 8), P = 0.001], smaller tumor size [OR(95% CI) = 1.335(1.032–1.726), P = 0.028] and R0 resection [OR(95% CI) = 3.098(2.095–4.582), P < 0.001] were independent factors for survival ≥ 5 years. The prognosis was best for ampullary adenocarcinoma, for which the median survival was 54 months and 5-year survival rate was 39.0%, and the poorest was pancreatic head adenocarcinoma, for which the median survival was 27 months and 5-year survival rate was 7%. Conclusions: The majority of long-term survivors after PD for periampullary adenocarcinoma are patients with ampullary tumor. CA 19-9 < 37 U/mL, smaller tumor size, and R0 resection were found to be independent factors for long-term survival ≥ 5 years.

Lymphocytic colitis: A clue to bacterial etiology

AIM： To find out the role of bacteria as a possible etiological factor in lymphocytic colitis. METHODS： Twenty patients with histopathological diagnosis of lymphocytic colitis and 10 normal controls were included in this study. Colonoscopic biopsies were obtained from three sites （hepatic and splenic flexures and rectosigmoid region）. Each biopsy was divided into two parts. A fresh part was incubated on special cultures for bacterial growth. The other part was used for the preparation of histologic tissue sections that were examined for the presence of bacteria with the help of Giemsa stain. RESULTS： Culture of tissue biopsies revealed bacterial growth in 18 out of 20 patients with lymphocytic colitis mostly Escherichia coil （14/18）, which was found in all rectosigmoid specimens （14/14）, but only in 8/14 and 6/14 of splenic and hepatic flexure specimens respectively. In two of these cases, E coli was associated with proteus. Proteus was found only in one case, Klebsiella in two cases, and Staphylococcus aureus in one case. In the control group, only 2 out of 10 controls showed the growth of E coli in their biopsy cultures. Histopathology showed rod-shaped bacilli in the tissue sections of 12 out of 14 cases with positive E coli in their specimen＇s culture. None of the controls showed these bacteria in histopathological sections. CONCLUSION： This preliminary study reports an association between E coli and lymphocytic colitis, based on histological and culture observations. Serotyping and molecular studies are in process to assess the role of E coli in the pathogenesis of lymphocytic colitis.

Distribution and ecological risk assessment of some heavy metals in coastal surface sediments along the Red Sea,Egypt

The ecological risk assessment for Al,Zn,Cu,Ni,V,Pb,Cd,and Hg in surface sediment collected from the Egyptian Red Sea coast was evaluated using the Geo-accumulation Index(Igeo).Sediment Enrichment Factor（SEF） and Potential Ecological Risk Index（PERI） *** predominant heavy metal,aluminum,showed high concentrations along both of Aqaba Gulf（4378.8 ± 2554.1 μg/g） and southern part of the Red Sea（2972.8 + 1527.5 μg/g）.while it recorded the lowest concentration in Suez Gulf（829.7 ± 398.2 μg/g）.The determined heavy metal concentrations had the order of Al > Zn >-Ni > V > Pb > Cu > Cd > *** statistical analyses showed some correlations among the heavy metals *** international sediment quality guidelines were used to estimate the quality of the collected ***,the recorded average heavy metals concentrations were lower than those of the permissible contents for sediment quality *** Geo-accumulation index calculations(Igeo) proved that the investigated region could be classified as an unpolluted *** Enrichment Factor(KSEF) study showed high values in Suez Gulf *** single pollution index analysis of heavy metals in the sediments（Ci_f） indicated that Al,Zn,V,and Pb were of natural origin,while Ni,Cd and Hg were seriously affected by human ***,amongst,all the determined heavy metals,Cd and Hg gave moderate ecological risk indicators.

Duodeno-jejunal junction dyssynergia: Description of a novel syndrome

AIM： To investigate the hypothesis that duodeno-jejunal dyssynergia existed at the duodeno-jejunal junction.METHODS： Of 112 patients who complained of epigastric distension and discomfort after meals, we encountered nine patients in whom the duodeno-jejunal junction did not open on duodenal contraction. Seven healthy volunteers were included in the study. A condom which was inserted into the ist duodenum was filled up to 10 mL with saline in increments of 2 mL and pressure response to duodenal distension was recorded from the duodenum, duodeno-jejunal junction and the jejunum.RESULTS： In healthy volunteers, duodenal distension with 2 and 4 mL did not produce pressure changes, while 6 and up to 10 mL distension effected significant duodenal pressure increase, duodeno-jejunal junction pressure decrease but no jejunal pressure change. In patients, resting pressure and duodeno-jejunal junction and jejunal pressure response to 2 and 4 mL duodenal distension were similar to those of healthy volunteers. Six and up to 10 mL 1^st duodenal distension produced significant duodenal and duodeno-jejunal junction pressure increase and no jejunal pressure change.CONCLUSION： Duodeno-jejunal junction failed to open on duodenal contraction, a condition we call ＇duodenojejunal junction dyssynergia syndrome＇ which probably leads to stagnation of chyme in the duodenum and explains patients＇ manifestations.

Geochemistry of lamprophyre dykes, Wadi Sikait area, South Eastern Desert, Egypt

The Wadi Sikait area lies at about 95 km southwest of Marsa Alam City along the Red Sea Coast, Eastern Desert, Egypt. It is occupied by Precambrian rocks of ophiolitic mélange, metamorphosed sandstones (MSS), gab-bros and monzogranites which were later intruded by lamprophyre dykes and quartz veins. The lamprophyre dykes were extruded in NW-SE and NE-SW trends cutting monzogranites and metamor-phosed sandstones. The lamprophyres are porphyritic and composed of clinopyroxene, olivine and amphibole phenocrysts enclosed in a fine-grained groundmass of clinopyroxene, amphibole, opaque and lithium mica. The al-teration products are represented by amphibole (tremolite-actinolite and hornblende), carbonate, epidote, chlorite, iddingsite, clay minerals, limonite and serpentine. The Sikait lamprophyre dykes can be classified as alkaline lamprophyres characterized by silica contents rang-ing from 41.65 wt% to 50.88 wt% and Na2O>K2O. They are enriched in LILE, LREE and HFSE, but strongly de-pleted in compatible elements such as Cr and Ni relative to the primitive mantle. Sikait lamprophyres have moderate Zr/Hf (35.6-52.8) and Nb/Ta (20.5-22.5) ratios. Most of these features are attributed to the origin of these dykes from the metasomatized mantle affected by subduction-related fluid. These lamprophyres are compositionally similar to Salu lamprophyres in eastern China. The Sikait lamprophyre samples have high LREE (320×10-6-419×10-6) relative to HREE (20×10-6-33×10-6) with ratios (LREE/HREE=11.6-18.7) and no negative Eu anomaly (Eu/Eu*=0.9-1.04). The relative presence of posi-tive Ce anomaly (Ce/Ce*=1.04) in lamprophyre samples suggests the oxidizing condition under which the REEs were precipitated due to the common occurrence of fluorite and apatite.

The cavernoso-anal reflex： response of the anal sphincters to cavernosus muscles＇ stimulation

Aim： To prove the hypothesis that cavernosus muscles＇ contraction during coitus affects the reflex contraction of anal sphincters. Methods： electromyographic response of external and internal anal sphincters to ischiocavernosus and bulbocavernosus muscle stimulation was studied in 17 healthy volunteers （10 men, 7 women, mean aged 38.3 ± 11.6 years）. The test was repeated after individual anesthetization of anal sphincters and the two cavernosus muscles,and after using saline instead of lidocaine. Results： Upon stimulation of each of the two cavernosus muscles,external and internal anal sphincters recorded increased electromyographic activity. Anal sphincters did not respond to stimulation of the anesthetized cavernosus muscles nor did anesthetized anal sphincters respond to cavernosus muscles＇ stimulation. Saline infiltration did not affect anal sphincteric response to cavernosal muscles＇ ***： Cavernosus muscles＇ contraction is suggested to evoke anal sphincteric contraction, which seems to be a reflex and mediated through the ＂cavernoso-anal reflex＂. Anal sphincteric contraction during coitus presumably acts to close the anal canal to thwart flatus or fecal leak.
