Atypical Presentation of Prostatic Cancer with Left Axillary and Supraclavicular Lymphadenopathy

Introduction: The burden of prostatic cancer is rising in Sudan. Usually, they present late in their disease with urinary tract obstruction, hematuria, bony pain, or cachexia because there is no screening program. Here we present a patient with prostatic cancer who presented with left axillary mass as his main concern. Case Description: 82-year-old Sudanese male presented with a left axillary and left supraclavicular lymphadenopathy of a few months’ duration. He underwent a decisional biopsy which showed metastatic adenocarcinoma. Upper and lower GI endoscopy were performed but the latter was complicated by a sigmoid perforation with peritonitis. During laparatomy, multiple enlarged pelvic lymphnodes were encountered and a biopsy result suggested a metastatic prostatic neoplasm. Later, prostatic biopsy confirmed the diagnosis. The patient was treated with bilateral orchidectomy. Clinical discussion: Lymphatic metastasis to axillary lymph nodes is a very rare manifestation of prostate cancer and only a few cases have been reported in the literature. It can cause diagnostic difficulty since prostate cancer typically metastasis to the pelvic lymph node and very rarely involves he supradiaphragmatic lymph node. Conclusion: Metastatic prostatic carcinoma should be considered among the causes of supra-diaphragmatic lymph adenopathy. Careful physical and imaging examinations combined with PSA and pathological analysis are essential in the diagnosis of advanced prostate cancer with unusual presentation.

Report on the Diatoms and Dinoflagellates Distribution along the Algerian Coasts: Inter-Region Comparison

The phytoplankton group’s composition is an excellent indicator of the state of coastal waters. The aim of this study is to explore spatial variability of phytoplankton in shallow coastal areas from different regions(East, Center and West) of the Algerian coast. The quantitative and qualitative study of the phytoplankton population was conducted on samples taken during the 2012–2013 period by the research vessel GRINE BELKACEM. The qualitative results show a significant diversity of diatoms and dinoflagellates, which rank first and second in the total flora(Fr > 50%). This finding is also confirmed quantitatively by the abundance values in both 2013 and 2012 samples which largely exceeds the average value in most other areas of the western region(56%) and slightly exceeding 52% in all central areas which represent an equi-distribution between diatoms and dinoflagellates. The Dinoflagellates contribute significantly to the quantitative richness in the eastern region(>60%). The minimum value of Dia/Dino index, recorded in the eastern region confirms the dominance of the dinoflagellates especially in Skikda(0.31)(Skikda).However, our results reveal the presence, in smaller proportion, of other groups such as the cocolithophoridae and euglenophyceae.

目的研究舒眠胶囊中枢镇静催眠作用及其机制。方法分别设空白对照组、阳性药地西泮组及舒眠胶囊低、中、高剂量组。雄性昆明小鼠连续灌胃给药,分别进行小鼠自主活动实验、戊巴比妥钠阈上和阈下剂量睡眠实验;采用酶联免疫吸附法(ELISA)及比色法测定小鼠脑组织γ-氨基丁酸(GABA)、谷氨酸(Glu)含量。雄性SD大鼠腹腔注射350 mg/(kg·d)对氯苯丙氨酸(PCPA)制备大鼠失眠模型,造模后按实验分组分别连续灌胃给药,观察大鼠一般状况和体质量变化,旷场实验测试大鼠运动总路程和站立次数;Western blotting和免疫组化检测大鼠海马5-HT1A受体(5-HT1A R)蛋白表达。结果与空白对照组相比,连续给药14 d,1.6、3.2、6.4 mg/(kg·d)舒眠胶囊各剂量组均可明显减少小鼠的活动次数及站立次数(P<0.01),高剂量组明显延长阈上剂量戊巴比妥钠致小鼠睡眠持续时间(P<0.01);舒眠胶囊各剂量组可剂量依赖性升高小鼠脑组织GABA水平(P<0.05),高、中剂量组均明显降低小鼠海马Glu水平(P<0.05),高剂量组明显降低皮质Glu水平(P<0.05)。失眠模型大鼠出现活动萎靡不振,昼夜活动节律消失,毛发变硬,粗糙无光泽,掉毛现象严重,体质量减轻等现象;连续给药7 d,舒眠胶囊各剂量组大鼠精神状况得到改善,毛发相对润泽,掉毛现象改善;与空白对照组比较,腹腔注射PCPA后,大鼠体质量增量显著降低(P<0.01);与模型组比较,舒眠胶囊各剂量组大鼠体质量增量明显增加(P<0.01)。模型组大鼠在旷场实验中运动距离延长,海马5-HT1AR蛋白表达水平下降;而地西泮组和舒眠胶囊各剂量组运动距离显著减少(P<0.01),且地西泮及舒眠胶囊高、中剂量组海马5-HT1AR蛋白表达明显上调(P<0.05)。结论舒眠胶囊有镇静、催眠、改善实验性动物失眠等作用,其机制可能与升高大脑组织GABA水平,降低Glu含量,上调海马5-HT1AR蛋白表达有关。

Pattern Recognition of Rainfall Using Wavelet Transform in Bangladesh

The aim of the study is to explore the regional variation of changing patterns of rainfall in Bangladesh using wavelet transform. The study is completed using rainfall variation of the five regions of Bangladesh as Dhaka, Cox’s Bazar, Rajshahi, Bogra and Sylhet. The duration of the study period was 69 years for Dhaka, 64 years for Cox’s Bazar, 40 years for Rajshahi, 54 years for Bogra and 55 years for Sylhet. The results of the wavelet analysis reveals that, in Rajshahi the amount of rainfall are decreasing in a significant rate among the other study regions. It also explores the annual periodicity of rainfall for all the study regions along with a special 6-month periodicity in the Cox’s Bazar. In addition, this analysis also explores a dominating 3 - 4 year cycle of rainfall in all the study regions. Besides the climate change in Cox’s Bazar and Sylhet are pretty much alarming.

Atrial Electromechanical Delay in Healthy Individuals in Fasting and Postprandial States

Background: Atrial Electro-Mechanical Delay (AEMD) has been defined as the delay between the electrical stimulation and mechanic contractility that may be attributed to several abnormalities in the atria. It is associated with atrial remodeling that finally leads to atrial fibrillation (AF). Methods: Fifty five subjects were enrolled in this descriptive observational cross sectional study, (mean age was 33.27 ± 7.49 years, 33 males). All patients were evaluated by precise history taking, clinical examination, twelve lead surface electrocardiography (ECG). ECG gated Echocardiography with measurement of atrial electromechanical coupling (PA), left atrial intra-AEMD over a period of 1 year from July 2017 to July 2018 in Mansoura specialized medical hospital. Results: It was found that PA lateral (50.98 ± 8.07 vs. 50.38 ± 7.47 msec, p 0.026), PA Septal (34.87 ± 6.33 vs. 34.16 ± 6.32 msec, p 0.003) and, left intra-AEMD (PA lateral–PA Septal) (16.72 ± 7.34 vs. 16.21 ± 6.57 msec, p 0.01), values were higher in post prandial condition than in fasting state. Conclusion: Determining electromechanical intervals as the atrial conduction interval with transthoracic echocardiography is a simple, easy bedside method to be evaluated. PA lateral, PA septal and left atrium EMD are prolonged in postprandial condition than during fasting and it may be predictor of occurrence of postprandial palpitation and hidden atrial fibrillation.

Application of Solar Energy for Recovery of Water from Atmospheric Air in Climatic Zones of Saudi Arabia

In the present work, an investigation on the application of solar energy to heat a sandy bed impregnated with calcium chloride for recovery of water from atmospheric air is presented. The study also aimed at evaluating the effects of different parameters on the productivity of the system during regeneration. These parameters include system design characteristics and the climatic conditions. An experimental unit has been designed and installed for this purpose in climatic conditions of Taif area, Saudi Arabia. The experimental unit which has a surface area of 0.5 m2, comprises a solar/desiccant collector unit containing sandy bed impregnated with calcium chloride. The sandy layer impregnated with desiccant is subjected to ambient atmosphere to absorb water vapor in the night. During the sunshine period, the layer is covered with glass layer where desiccant is regenerated and water vapor is condensed on the glass surface. Ambient temperature, bed temperature and temperature of glass surface are recorded. Also, the productivity of the system has been evaluated. Desiccant concentration at start of regeneration is selected on the basis of the climatic data of Al-Hada region, which is located at Taif area, Saudi Arabia. Experimental measurements show that about 1.0 liter per m2 of pure water can be regenerated from the desiccant bed at the climatic conditions of Taif. Liquid desiccant with initial concentration of 30% can be regenerated to a final concentration of about 44%. Desiccant concentration at start of regeneration is selected on the basis of the climatic data of Al-Hada region. The climate of Taif city is dry compared with that for Al-Hada region. This method for extracting water from atmospheric air is more suitable for Al-Hada region especially in the fall and winter.

The Patterns of Blood Groups and Its Association with Blood Donation in Saudi Arabia

Background: Blood groups system has key importance for transfusion safety and clinical practice. Therefore, the objective of the present study was to evaluate the distribution of ABO blood groups and its association with blood donation in Saudi Arabia. Methodology: In this study, we collected information regarding blood donation from 715 Saudi volunteers living in the city of Hail, Northern Saudi Arabia. Data about blood donation were obtained from 715 Saudi volunteers living in the city of Hail. Results: The majority of the study population was found with group B+ representing 174 participants, followed by A+, O+, AB+, B-ve, A-ve, AB-ve and O-ve, representing 165, 153, 47, 29, 20, 9, and 7 participants, in this order. Conclusion: The major blood group among Saudi population is group B+ followed by group A+ and O+. Individuals with rare blood groups were less likely to participate in blood donation programs, which necessitate the need for educational programs using motivating factors.

Evaluation of Real-Time PCR and Culturing for the Detection of Leptospires in Canine Samples

Validated real-time PCRs detecting leptospires for veterinary purposes are not readily available. This paper describes the prospective evaluation of a SYBR Green-based real-time PCR on serum samples collected from experimentally infected dogs. Compared to culturing, the assay had a diagnostic sensitivity and specificity of 91.7% and 90.0%, respectively. Culturing for part is complementary to PCR and preferably both should be applied for diagnosis and vaccine challenge experiments. In a subsequent prospective study on the dynamics of experimental infections with serovars Canicola and Copenhageni and serovar complex Bananal-Liangguang in young and adult dogs, the PCR was applied on serum samples in conjunction with culturing on blood, urine and kidney samples with the following results: 1) Leptospires persisted longer in the blood of young dogs than of adult ones;2) Numbers of viable leptospires in the blood are rapidly reduced but DNA remains occasionally detectable up to 7 days post infection;3) Appearance of viable leptospires in the urine follows a biphasic dynamics;4) EDTA hampers effective culturing from blood samples;5) Serovar Canicola persists longer in the blood of dogs than Copenhageni and Bananal-Liangguang. Together with a markedly higher recovery rate from kidney samples (71% compared to respectively 33% and 0% in young dogs), this probably reflects the adaptation power of Canicola to its canine maintenance host. Appearance of leptospires in the urine samples indicates that experimental infections have been successful. PCR presents a valuable adjunct to the diagnosis of veterinary leptospirosis and the follow-up in vaccine protection experiments.

Preservability of rabbit semen after chilled storage in tris based extender enriched with different concentrations of Propolis ethanolic extract (PEE)

Objective:To maintain semen quality of male rabbits during chilled storage by enrichment the tris based diluent with different concentrations of propolis ethanolic ***: Total phenolic and total flavonoid contents, as well as antioxidant activity was determined in propolis ethanolic extract (PEE). The extract was analysed by HPLC for separation and identification of target metabolites. Semen was collected from 10 rabbit bucks, pooled, then divided into five aliquots (each of 500 μL) and diluted each in 5 mL Tris-citric acid-glucose-egg yolk extender (TCGY). The 1st aliquot served as control while PEE was added at concentration of 0.8, 1.2, 1.6 and 2.0 mg/5 mL tris extender in the aliquot 2, 3, 4 and 5 respectively. Diluted semen samples were subjected to cooling at 4℃ for 72 h. Sperm motility, sperm viability, sperm abnormality, sperm membrane integrity and acrosome integrity were evaluated in chilled semen allover the chilling ***:The resluts revealed presence of a considerable amount of total phenolic compounds (98.67 mg GAE/g extract) and total flavonoids (70.16 mg CE/g extract) which were parallel to an antioxidant activity assessed as ABTS, DPPH and FRAP (198.65, 180.18 and 306.17 mM TE/g extract respectively). The dominant phenolic acid was chlorogenic acids (3.959 mg/g extract). Other compounds were found in less amounts rosmarinic acid (3.959 mg/g extract), myrcetin (1.946 mg/g extract), kaempferol (1.089 mg/g extract) and apeginin-7-glucoside (1.113 mg/g extract). Obtained results clearly demonstrated that the addition of (1.2-1.6) mg PEE in the chilled extended rabbit semen proved to be beneficial for maintaining semen characteristics compared to control and the addition of 0.8 and 2 mg ***: The enrichment of rabbit semen tris-basic extender with (1.2-1.6) mg PEE/5 mL tris-extender (as the best and safe concentrations) maintain the sperm characteristics in good condition all over 72 h of chilling.
