The Impact of Obesity on Performing Lymphadenectomy and Its Outcome in Ovarian Cancer Patients

Background: This study discusses the effect of obesity on the number of lymph nodes harvested during systematic LND and the LND-related complications in ovarian cancer patients. Methods: This retrospective study enrolled women with ovarian cancer who were consecutively subjected to open surgical resection that included systematic LND (pelvic and para-aortic) in the Oncology Center in Mansoura University (OCMU) during the period between January 2012 and June 2017. Patients were categorized according to the recommendations of World Health Organization by their BMI as non-obese (BMI 30.0 kg/m2) and obese (BMI ≥ 30.0 kg/m2). Results: Seventy-seven women with ovarian cancer were enrolled in the study according to our inclusion and exclusion criteria. 43 females (55.8%) were grouped as obese and 34 (44.2%) as non-obese. Lymph nodes retrieved in total and in different stations separately (pelvic and para-aortic) were all similar among patients in both groups. LND-related intraoperative complications were observed in 8 patients (18.6%) in the obese group and 3 patients in the non-obese group (8.8%) (P = 0.347). Hospital stay was the same in the two groups with a median of 4 days (IQR 3 - 5). Postoperative complications occurred in 13 patients (30.2%) in the obese group and only in one patient (3%) in the non-obese group (P = 0.004). Conclusion: Obese ovarian cancer patients may safely undergo comprehensive staging involving extensive lymph node dissection in open surgeries without significant increase in the rates of intraoperative complications. Whereas, postoperative complications (wound infection and thromboembolic events) tend to occur at higher rates with obese patients. Trial registration: This study was retrospectively registered and approved at faculty of Medicine Mansoura University, Egypt with IRB approval number R.18.02.46.

Objective: To study the effect of cerebral ischemic vascular insult on the localization of language areas as regard to automatic speech, initiation, and mutism. Materials and Methods: Seventy ischemic stroke patients were selected within the first 48 h of the onset of stroke with left hemispheric stroke with aphasia. Inclusion criteria: any age above seven years, both sexes, first ever clinical stroke, admitted within the first 2 days of stroke insult. Patients underwent careful history taking, neurological examination, routine laboratory investigations, echo cardiography, and carotid duplex and language assessments. Results: Automatic speech disorders were mainly seen along left insular, left supramarginal and left frontal inferior orbital lesions. Initiation difficulty is seen mainly along as left insular, left temporal superior and left periventricular lesions. Mutism is seen along left external capsule, left internal capsule, left pallidum, left frontal inferior orbital and left inferior trigone lesions. Conclusions: Deep periventricular white matter and insular area in the dominant hemisphere play an important role in many language tasks. Thus aphasia is not a mere cortical function.

Adjacent treatment fields are commonly employed in external beam radiation therapy, such as the mantle and inverted-Y fields for the treatment of Hodgkin's disease. In some cases, the adjacent fields are orthogonal, such as the craniospinal fields used in the treatment of medulloblastoma. Another example is the irradiation of head and neck tumors when the lateral neck fields are placed adjacent to the anterior supraclavicular field. In each of these situations, there is a possibility of introducing very large dosage errors across the junction. Consequently, this region is at risk for tumor recurrence if it is underdosed or severe complications if it is overdosed. Four millimeter overlap and gap resulted in an unacceptable dose in homogeneity in the junction. As a result of this study, the magnitudes of hot and cold spots might be clinically acceptable for 3 mm gap between photon fields.

Background:Recognition and treatment of emotional distress,which affects 20 to 40%of outpatients with type 1 or type 2 diabetes,are important because of its association with worse diabetes self-care,poor glycemic control,increased rates of mortality and diabetes-related complications,and a rise in healthcare ***,although the symptoms of emotional distress improve in diabetic patients after psychological and pharmacological interventions,evidence of benefits in glycemic control is still ***:The aim of this study was to assess the metabolic control in diabetic patients with emotional distress attending primary mental care clinics(PMCC)in the Eastern Province of Saudi ***:This study was a single group pretest-posttest design conducted on adult diabetic patients with emotional distress attending *** of 194 attendees,62 patients who fulfilled the selection criteria were *** were collected from the patients’records using a worksheet designed for the *** parameters including BMI,BP,FBS,HbA1c and lipid profile were recorded before and after the management of emotional ***-demographic characteristics,complications related to diabetes and co-morbidities were also *** t-test was used appropriately to compare continuous variables.A p-value of less than 0.05 was considered significant in all statistical ***:The majority of the patients had type 2 DM(98.2%)and the presence of co-morbidities was high(83.9%).A comparison of the metabolic parameters before and after management of emotional distress,showed a significant reduction in HbA1c(p=0.020)and elevation in HDL(p=0.010).No differences in BMI,BP,FBS,TC,TG or LDL were ***:Treating emotional distress in diabetic patients is associated with an improvement in HbA1c and HDL.

蒙古白丽蘑Leucocalocybe mongolica是生长于蒙古高原草原上的一种珍贵的食药用真菌,它以形成蘑菇圈的独特方式生长,是蒙古草原特有的珍稀物种。由于其资源濒危,亟待保护与可持续利用研究。因此采用"功能基因—活性成分—分子机理"整体性模式对该菌进行了研究。通过全基因组的denovo与全长转录组测序,首次获得蒙古白丽蘑全基因组信息,基因组大小为45.98Mb,保护26个组装片段,contig N50长度为3.6Mb,预测到11599个基因;其中碳水化合物酶相关基因在子实体形成过程中起重要作用,但该菌有基因丢失现象。全基因组分析发现384个碳水化合物酶相关基因;基于两条KEGG有42个基因注释到甾类和萜类合成通路;甾醇24-C-甲基转移酶与异戊二烯半胱氨酸羧甲基转移酶基因在两条通路中有高表达;子实体阶段的甾类与萜类合成基因并无特异表达,并获得了高多态性的该菌的SSR分子标记8个;基于基因组发现1821个分子标记位点。通过药理研究发现蒙古白丽蘑具有显著的抗肿瘤、增强免疫及催乳作用,其中石油醚提取物和甾类成分效果显著,其抗肿瘤机理主要通过对色氨酸代谢、5-羟色胺能突触、氨基酸的生物合成与花生四烯酸代谢等多个代谢通路和对肠道微生物影响、增强免疫、促进细胞凋亡以及抑制肿瘤血管新生等多种途径;并且甾类成分对免疫抑制小鼠有显著的恢复作用,可以激活巨噬细胞的吞噬能力,从而达到提高免疫功能的作用,其中以4,6,8(14),22(23)-四稀3-酮-麦角甾烷的治疗效果最佳;石油醚提取物和水提取物通过上调乳腺组织中α-乳白蛋白、β-酪蛋白、催乳素受体、信号转导和转录激活因子5、雌激素受体和孕激素受体的转录起到显著的催乳作用。本研究为蒙古白丽蘑的保护及可持续利用提供了研究基础,确定甾类成分是其特征性和活性成分。该菌对某些甾类和萜类代谢酶的高表达特性与富含多种甾类成分及甾类显著的药理活性,三者具有高度的一致性,本研究在蒙古白丽蘑及其它菌物药资源的可持续利用和新产品开发利用方面具有重要价值和意义。

The absorption of acid gas using reactive amines is among the most widely used types of capturing ***,the absorption process requires intensive energy expenditure majorly in the solvent regeneration *** study simultaneously evaluated the regeneration energy of MDEA and PZ/MDEA solvents in terms of heat of absorption,sensible heat,and vaporization *** Hysys version 8.8 simulation tool is applied to model the full acid gas removal plant for the chemical absorption *** new energy balance technique presents around the absorption and desorption columns to bring a new perspective of energy distribution in the capturing of acid gas *** analysis of regeneration energy and its three contributors is performed at several operation parameters such as absorber and stripper pressures,lean amine circulation rate,solvent concentration,reflux ratio,and CO2 and H2 S *** results show that the heat of absorption of PZ/MDEA system is higher than that for MDEA system for the same operating *** sensible heat is the main contributor in the required regeneration energy of MDEA solvent *** simulation results have been validated against data taken from real plant and *** product specifications of our simulation corroborate with real plant data in an excellent approach;additionally,the profile temperature of the absorber and the stripper columns are in good agreement with *** overall results highlight the direction of the effects of each parameter on the heat of absorption,sensible heat,and vaporization heat.

Face verification systems are critical in a wide range of applications,such as security systems and biometric ***,these systems are vulnerable to adversarial attacks,which can significantly compromise their accuracy and *** attacks are designed to deceive the face verification system by adding subtle perturbations to the input *** perturbations can be imperceptible to the human eye but can cause the systemtomisclassifyor fail torecognize thepersoninthe *** this issue,weproposeanovel system called VeriFace that comprises two defense mechanisms,adversarial detection,and adversarial *** first mechanism,adversarial detection,is designed to identify whether an input image has been subjected to adversarial *** second mechanism,adversarial removal,is designed to remove these perturbations from the input image to ensure the face verification system can accurately recognize the person in the *** evaluate the effectiveness of the VeriFace system,we conducted experiments on different types of adversarial attacks using the Labelled Faces in the Wild(LFW)*** results show that the VeriFace adversarial detector can accurately identify adversarial imageswith a high detection accuracy of 100%.Additionally,our proposedVeriFace adversarial removalmethod has a significantly lower attack success rate of 6.5%compared to state-of-the-art removalmethods.

Sparsity adaptive matching pursuit(SAMP)is a greedy reconstruction algorithm for compressive sensing *** reconstructs signals without prior information of sparsity and presents better reconstruction performance for noisy signals compared to other greedy ***,SAMP still suffers from relatively poor reconstruction quality especially at high compression *** the proposed research,the Wilkinson matrix is used as a sensing matrix to improve the reconstruction quality and to increase the compression ratio of the SAMP ***,the idea of block compressive sensing(BCS)is combined with the SAMP technique to improve the performance of the SAMP *** simulations have been conducted to evaluate the proposed BCS-SAMP technique and to compare its results with those of several compressed sensing *** results show that the proposed BCS-SAMP technique improves the reconstruction quality by up to six decibels(d B)relative to the conventional SAMP *** addition,the reconstruction quality of the proposed BCS-SAMP is highly comparable to that of iterative ***,the computation time of the proposed BCS-SAMP is less than that of the iterative techniques,especially at lower measurement fractions.

Production of porous silica by the combustion of rice husk ash for tundish lining

In order to study the production of porous silica compacts by the combustion of rice husk ash （RHA） for tundish lining, the experimental design technique was used to evaluate the effect of firing temperature, soaking time and compaction pressure on controlling both the porosity degree and compressive strength of rice husk ash compacts. The results revealed that while the porosity degree of the compacts decreased with the increase in the entire studied parameters, the compressive strength exhibited another trend especially at a lower soaking time. At a lower soaking time, the increase in firing temperature led to a slight decrease in compressive strength and then increasing thereafter. The porous silica compacts having 30% porosity and 〉 2.5 MPa compressive strength suitable for tundish lining could be obtained from the combustion of rice husk ash compacts.
