Heart disease remains a leading cause of morbidity and mortality worldwide, highlighting the need for improved diagnostic methods. Traditional diagnostics face limitations such as reliance on single-modality data and vulnerability to apparatus faults, which can reduce accuracy, especially with poor-quality images. Additionally, these methods often require significant time and expertise, making them less accessible in resource-limited settings. Emerging technologies like artificial intelligence and machine learning offer promising solutions by integrating multi-modality data and enhancing diagnostic precision, ultimately improving patient outcomes and reducing healthcare costs. This study introduces Heart-Net, a multi-modal deep learning framework designed to enhance heart disease diagnosis by integrating data from Cardiac Magnetic Resonance Imaging (MRI) and Electrocardiogram (ECG). Heart-Net uses a 3D U-Net for MRI analysis and a Temporal Convolutional Graph Neural Network (TCGN) for ECG feature extraction, combining these through an attention mechanism to emphasiz. relevant features. Classification is performed using Optimiz.d TCGN. This approach improves early detection, reduces diagnostic errors, and supports personaliz.d risk assessments and continuous health monitoring. The proposed approach results show that Heart-Net significantly outperforms traditional single-modality models, achieving accuracies of 92.56% for Heartnet Dataset I (HNET-DSI), 93.45% for Heartnet Dataset II (HNET-DSII), and 91.89% for Heartnet Dataset III (HNET-DSIII), mitigating the impact of apparatus faults and image quality issues. These findings underscore the potential of Heart-Net to revolutioniz. heart disease diagnostics and improve clinical outcomes. Copyright © 2024 The Authors. Published by Tech Science Press.

Effect of Modified HIF-1<i>α</i>Linked to Carbonic Anhydrase IX Inhibitor and Glycosylated Cisplatin on Solid Tumors: Short Review

Many cancer cells in solid tumors are hypoxic or pseudohypoxic and create acidic environment for malignancy progression. Under low oxygen conditions (hypoxia), hypoxia-inducible factors (HIFs) play pathological roles in cancer cell survival and spreading. HIF regulates several genes such as genes of glucose transporters that enhance anaerobic glycolysis, angiogenesis, erythropoiesis and carbonic anhydrase IX (CA-IX). CA-IX is a cell-surface glycoprotein that catalyz.s the hydration of CO2 to protons and bicarbonate ions (respiratory acidification). This process is involved in adaptation to acidosis and implicated in cancer progression. Therefore, CA-IX inhibitors (such as sulfonamide-based compounds) showed hoping results in reduction malignancy progression. The article aims to reversal the malignant hypoxic environment in solid tumors to create a condition of weakness within the cancer for further focused cisplatin potency. This article suggests the use of modified synthesiz.d HIF as a drug delivery molecule for both carbonic anhydrase IX inhibitor and glycosylated cisplatin that damages the DNA of malignant cell. HIF molecule has high affinity to bind with CA IX-expressing malignant cells, which is followed by cell entrance via endocytosis. Once the HIF-Cisplatin-CA-inhibitor complex enters the cell, the carbonic anhydrase inhibitor will improve the cellular pH that makes the environment unsuitable for HIF 1α function and it may be ubiquitinated. So, the raise in target genes transcription will be arrested. On the other hand, once the synthetiz.d HIF is degraded, the cisplatin molecules will be released inside the malignant cell and start to damage its DNA. This approach may be a good solution for many solid tumors.

Narrow Band Imaging Can Be Used for Anastomotic Assessment

Background: Anastomotic leak is the most dreaded complication and it accounts for one third of post-operative deaths in colorectal surgery. There is strong need of research in this field in order to reduce anastomotic leak rate. If there was a test available to signify probability of leak at the time of surgery, it would potentially avoid a leak at a later date. Aim: We propose that intra operative use of narrow band imaging colonoscopy in left sided colonic anastomosis can predict and detect anastomosis related problems, problems i.e. vascularity, anastomotic lumen patency and rigidness of anastomosis which in turn lead to correction at the time of surgery resulting in significant decrease in anastomotic leak rate associated with morbidity and mortality. Methodology: A pilot, single blind, prospective, randomised controlled trial. Aim is to divide patient in 2 groups, containing 15 patients in each group. One group will be assessed by intra-operative NBI colonoscopy and other with standard technique. Analysis: This will be performed using SPSS with the help of statistician. Significance: This area of research is particularly important because it improves survival by reducing morbidity and mortality associated with anastomotic leak. If a leak is avoided then it does not only mean early recovery but it also saves NHS in terms of ICU care, NHS bed, inpatient nursing care and home help, unfortunately all required if leak is to occur. As a result, this research will have better financial implication with significantly improve patient care.

Colonoscopy a True Therapeutic Icon

Colonic polypectomy remained the first colonoscopic therapeutic intervention which was successfully accomplished with the help of an early snare. This therapeutic procedure was executed by Dehyle in Europe and Shinya in USA in 1971 [1]. Thus, these two colonoscopists acquired the pioneer’s position of therapeutic colonoscopy. Now the colonoscopy has achieved a special status in treating colorectal diseases because of its unique therapeutic aspect. The common uses of therapeutic colonoscopy are broadly classified in to the four major categories because of its unique therapeutic properties. These uses include resection and ablation, hemostasis, decompression with or without recanaliz.tion and foreign body extraction. Colonic perforation remained an absolute contraindication to therapeutic colonoscopy which is analogous to diagnostic colonoscopy. Good Communication plays a vital role in accomplishment of successful therapy with the help of colonoscopy. Consent process remained a complex process which should be well organiz.d and tailored as per patient’s needs. There are lot of advancements in colonoscopy in recent years and many new procedures and treatment modalities have become available to treat colorectal diseases with the help of colonoscope. The recent advancement in colonoscopy has led to having many novel techniques. This therapeutic colonoscopy has made a significant change in current colorectal cancer management as polyps can be treated in the early stage, thus reducing the incidence of colorectal cancer. This is the basis of the United Kingdom existing bowel screening program, which is only feasible because of colonoscopy-induced interventions. The successful uses of these techniques obviate the need for a major surgical operation and colorectal disease can be treated with minimally invasive colonoscopic methods. These attributes make colonoscopy a true therapeutic icon.

Reaction of N,N’-dimethylformamide dimethyl acetal (DMFDMA) with malononitrile dimer 8 (1:1) mole afforded 9 while, this reaction when carried out in (2:1) mole to give amidine 11 which can be used for the preparation of pyrimidine 13, amidine 14 and pyridine 19 when reacted with 4-nitroaniline, 4-methylaniline and alkoxide respectively. Malononitrile dimer reacted with diaz.nium chloride to give pyridaz.ne 21, which can be reacted with DMFDMA, AcOH/HCl and cyanoacetamide to give pyridaz.ne 22, 23 and pyrido[4,3-c] pyridaz.ne 24 respectively. The latter reacted with DMFDMA to afford tricyclic compound 25.

Rituximab is a monoclonal antibody that targets CD20, which is a specific B-cell surface antigen. It was the first monoclonal antibody that was approved for the treatment of non-Hodgkin lymphoma, rheumatoid arthritis, and other cutaneous lymphoid malignancies. There are many off-label uses of rituximab, such as systemic lupus erythematosus, autoimmune hemolytic anemia, multiple sclerosis, graft-versus-host disease, chronic lymphocytic leukemia, and chronic immune-mediated thrombocytopenia. Among the rare side effects associated with rituximab treatment is vasculitis, more specifically, leukocytoclastic vasculitis. Here, we describe a 21-year-old Saudi female with leukocytoclastic vasculitis occurring three months after treatment with rituximab.

Prognostic Value of Combined;Cox-2, Cyclin D1 and P21 Expression in Colorectal Cancer (CRC) Patients: An Immunohistochemical Study

Background: Cyclooxygenase (COX) is a rate limiting enz.me in synthesis of prostanoids pathway and it has 2 isoforms (COX-1 and COX-2). It has many biological roles in inflammation and oncogenesis. Cox-2 was incriminated in performing disturbances in the cell cycle control in CRC, but its role of in CRC needs clarification of the mechanisms by which Cox-2 might affect the process of colorectal carcinogenesis. Cyclin D1 is an oncogene that regulates G1 phase progression to S phase of the cell cycle. Its stimulatory role on cell cycle is antagoniz.d by Cyclin D1-dependent kinase (CDK) inhibitors like p21. P21 plays an essential role in cell cycle regulation;it may have a pro-apoptotic or an antiapoptotic role in cancer. P21 was found to have many roles in cancer;invasion metastases, cellular senescence and stem cells aging. The roles of combined expression of Cox-2, Cyclin D1 and P21 in CRC tissues and their role of prognosis and patients survival are not sufficiently clarified. Aim of the Study: To evaluate tissue expression of Cox-2, Cyclin D1 and P21 in CRC and to correlate such expression with pathological parameters, clinical and prognostic data of the patients. Methods: Cox-2, Cyclin D1 and P21 are evaluated in colon cancer tissues. Correlations between their level of expressions pathological parameters, clinical and prognostic data of patients were analyz.d. Results: Cox-2, Cyclin D1 over-expression was associated with higher grade, higher incidence of occurrence of lymph node & distant metastasis and advanced stage (P = 0.000). Cox-2 was related to higher tumor recurrence rate (P = 0.04) and decreased overall patients survival rate (p = 0.002). (r correlation coefficient = +0.987). Conclusion: Cox-2 and Cyclin D1 are markers of poor prognosis colon cancer patients.

Objective:To evaluate the combination therapy of pyronaridine tetraphosphate and diminaz.ne aceturate against Babesia in vitro and in ***:Bioinformatic analysis was performed using atom pair *** in vitro combination test was performed against Babesia bovis and Theileria ***,the in vivo chemotherapeutic efficacy of pyronaridine tetraphosphate in combination with diminaz.ne aceturate was investigated against the growth of Babesia microti in mice using a fluorescence inhibitory ***:Pyronaridine tetraphosphate and diminaz.ne aceturate exhibited nearly similar molecular *** in vitro combination of pyronaridine tetraphosphate and diminaz.ne aceturate was synergistic on Babesia bovis and additive on Theileria *** addition,5 mg/kg pyronaridine tetraphosphate combined with 10 mg/kg diminaz.ne aceturate inhibited Babesia microti growth significantly compared with those observed after treatment with 25 mg/kg diminaz.ne aceturate alone from day 6 post treatment to day 12 post *** combination therapy also normaliz.d the hematological parameters of infected ***:An oral dose of pyronaridine tetraphosphate combined with a subcutaneous dose of diminaz.ne aceturate inhibits Babesia in vitro and in mice,suggesting it might be a new paradigm for the treatment of babesiosis.

Background: Dermatologists’ assessments of how their patients’ lives are affected by the skin disease are of importance for informing clinical decisions. However, there is no information about how often quality of life (QoL) issues are discussed in outpatient consultations. Objective: To examine the relationship between the extent of QoL- related discussion during dermatology outpatient consultations, and the current impact of the disease on patients’ lives. Patients and methods: A total of 238 consultations were observed in a teaching dermatology outpatient department for QoL- related discussion initiated by either the clinician or the patient. Following the consultation, all patients were posted a Dermatology Life Quality Index (DLQI) questionnaire to complete and return within 1 week. Results: QoL discussion was absent in 40% of consultations. Consultants initiated the fewest QoL discussions with patients, and nurses the most (P < 0.0001). One hundred and twentyeight (54% ) patients returned the DLQI, 114 (48% ) of which were evaluable. The mean score was 5.6, SD ± 6.6, median 3, range 0- 29. The mean DLQI score for the patients with whom there was no QoL discussion was 4.0 ± 4.7, n = 55, compared with the mean DLQI score for patients with whom QoL was discussed 6.8 ± 7.2, n = 59 (P < 0.001). Conclusion: This study demonstrates that little information concerning QoL is elicited during dermatology outpatient consultations.
