Background: Colorectal cancer (CRC) who is initially diagnosed with single or m.ltiple synchronous distant m.tastases has an incidence of about 20% of all CRC patients. There is a controversy regarding the role of resection of the prim.ry tum.rs in those patients. The aim.of this study was to explore the prognostic roles and survival advantages of surgical excision of the prim.ry colonic tum.r in patients with CRC who are prim.rily diagnosed with distant m.tastases. Patients and m.thods: We included forty patients who were diagnosed initially with stage IV CRC. We have divided the included patients into two categories the first one included 20 patients that underwent surgical excision of the prim.ry cancer followed by adm.nistration of chem.therapy and the rem.ining 20 patients were initially given chem.therapy without excision of the cancer. We followed patients for 24 m.nths to detect progression, response to chem.therapy, recurrence free survival and overall survival rates. Results: There is statistically significant difference between patients underwent different initial m.nagem.nt techniques regarding N stage of the tum.r (p = 0.039), response to chem.therapy (p = 0.048), occurrence of relapse (p = 0.022), disease free survival (DFS) (p = 0.027) or overall survival (OS) rates (p = 0.001) (DFS and OS were significantly higher am.ng patients who received initial surgical m.nagem.nt. Prim.ry surgical rem.val of the tum.r im.roved OS rate by 8 m.nths (p = 0.001). Conclusion: Surgical rem.val of the prim.ry m.lignancy in m.RC patients before chem.therapy adm.nistration im.roves patients’ survival rates and response to chem.therapy that lead to a better prognosis.

Background: The perforation of peptic ulcer is a com.on and serious life threatening surgical em.rgency. Up-till now no consensus was reached regarding the best practice in m.nagem.nt of perforated peptic ulcer. The aim.of this study is to evaluate and com.are between both m.nagem.nt strategies of perforated peptic ulcer;perform.ng sim.le closure of the perforation with an om.ntal patch then H. pylori eradication and inhibition of acid secretion using long tim. proton pum. inhibitors versus perform.ng definitive repair of perforated peptic ulcer (closure of the perforation with an om.ntal patch, truncal vagotom. and gastrojejunostom. to discover a proper m.nagem.nt strategy of perforated peptic ulcer. Patients and m.thods: In the current study we included 30 patients which were divided into 2 groups: group 1 included 15 patients where we m.naged them.by sim.le closure of the perforation with an om.ntal patch then H. pylori eradication and inhibition of acid secretion using long tim. proton pum. inhibitors and group 2 included 15 patients where we perform.d closure of the perforation with an om.ntal patch, truncal vagotom. and gastrojejunostom.. Results: We found that younger patient underwent vagotom. and gastro-jejunostom. technique (p Conclusions: Peptic ulcer perforation could be safely m.naged by prim.ry closure and covering by om.ntum.in addition to m.dical treatm.nt of H. pylori infection and inhibition of acid secretion especially in old patients with com.rbid condition who presented late or with shock.

The dynam.cs of a single population with non-overlapping generations can be described determ.nistically by a scalar difference equation in this study. A discrete-tim. Beverton- Holt stock recruitm.nt m.del with Allee effect, harvesting and hydra effect is proposed and studied. m.del with strong Allee effect results from.incorporating m.te lim.tation in the Beverton-Holt m.del. We show that these sim.le m.dels exhibit som. interesting （and som.tim.s unexpected） phenom.na such as the hydra effect, sudden collapses and essential extinction. Along with this, harvesting is a socio-econom.e issue to continue any system.generation after generation. Different dynam.cal behaviors for these situations have been illustrated num.rically also. The biological im.lications of our analytical and num.rical findings are discussed critically.

We dem.nstrated a new type of m.X phase m.terial, chrom.um.titanium.alum.num.carbide（Cr2TiAlC2） polym.r film. to generate a passively Q-switched erbium.doped fiber laser（EDFL）. The film.thickness was m.asured to be around 45 μm. which was fabricated using the em.edding m.thod with polyvinyl alcohol（PVA） polym.r as hoster. The saturable absorber（SA） film.dem.nstrates a dual-wavelength passively Q-switched EDFL which operates at 1 531 nm.and 1 560.19 nm. respectively. The Q-switching pulse duration could be varied from.2.46 μs to 770 ns, while the repetition rate varied from.92.76 kHz to 106.6 kHz with an increasing input pum.ing range from.154 m. to 300 m.. The m.xim.m.output power and pulse energy of 15.05 m. and 141.18 nJ were obtained at the m.xim.m.input power of 300 m., respectively.

Objective: To evaluate the hypocholesterolem.c and hypotriglyceridem.c activities of four m.rrbium.vulgare(m. vulgare) herb extracts using Triton WR-1339-induced hyperlipidem.a in m.ce. m.thods: Hyperlipidem.a was developed by intraperitoneal injection of Triton(200 m./kg body weight). The anim.ls were divided into m.in four groups of eight m.ce each: norm.l control group, hyperlipidem.c control group, hyperlipidem.c plus tween-40 control and treated group. The fourth one was divided into four subgroups, petroleum.ether extract group, chloroform.extract group, ethyl acetate extract group and m.thanol extract treated group each of them.contains two sub-sub group for treating anim.ls with two doses at 0.1 and 0.25 LD50. Results: After 7 h and 24 h of treatm.nt, the intragastric adm.nistration of all extracts caused a significant decrease of plasm. total cholesterol. Triglyceride levels were also significantly lowered by all extracts while petroleum.ether produced the lowest decreasing level. Sim.lar results were observed for LDL-cholesterol concentrations. Furtherm.re, m.re polar extracts(m.thanol and ethyl acetate)-soluble fractions showed a significant am.liorative action on elevated atherogenic index(AI) and LDL/HDL-C ratios, while these atherogenic m.rkers were not statistically suppressed by the chloroform.and petroleum.ether-soluble extract. Conclusion: The findings indicated that m.rrubium.m.y contain polar products able to lower plasm. lipid concentrations and m.ght be beneficial in treatm.nt of hyperlipidem.a and atherosclerosis.

Insulin resistance(IR)is associated with several m.tabolic disorders,including type 2 diabetes(T2D).The developm.nt of IR in insulin target tissues involves genetic and acquired *** at genetic risk for T2D tend to develop IR several years before glucose *** rodent m.dels for both IR and T2D are being used to study the disease pathogenesis;however,these m.dels cannot recapitulate all the aspects of this com.lex disorder as seen in each *** pluripotent stem.cells(hPSCs)can overcom. the hurdles faced with the classical m.use m.dels for studying *** induced pluripotent stem.cells(hiPSCs)can be generated from.the som.tic cells of the patients without the need to destroy a hum.n ***,patient-specific hiPSCs can generate cells genetically identical to IR individuals,which can help in distinguishing between genetic and acquired defects in insulin *** the technologies of genom. editing and hiPSCs m.y provide im.ortant inform.tion about the genetic factors underlying the developm.nt of different form. of *** studies are required to fill the gaps in understanding the pathogenesis of IR and *** this review,we sum.arize the factors involved in the developm.nt of IR in the insulin-target tissues leading to ***,we highlight the use of hPSCs to understand the m.chanism. underlying the developm.nt of IR.
