Since direct-acting antiviral agents(DAAs)have been introduced into hepatitis C virus treatment,the sustained viral response(SVR)rate has significantly increased to more than 95%.Scientific evidence supports the idea that SVR after interferon therapy has beneficial effects related to cirrhosis progression,resulting in a reduction in the incidence of hepatocellular carcinoma(HCC).However,a significant debate exists related to DAA impact on HCC *** reviewed the current literature highlighting the controversial data related to DAA association with de novo HCC occurrence or recurrence and possible pathophysiology of HCC related to *** a review of the published literature,we believe that the current evidence does not confirm or repudiate a higher rate of de novo HCC occurrence or recurrence related to DAA *** trials are needed to determine if there is an association between HCC occurrence or recurrence and DAA or if it is related to preexisting liver cirrhosis.

BACKGROUND Helicobacter pylori(***)is a significant human pathogen that is responsible for a variety of illnesses,including mucosa-associated lymphoid tissue lymphoma,gastric cancer,peptic ulcers,and *** To investigate the frequency of *** infection and its resistance patterns among Egyptian patients and to determine the influence of *** virulence genetic determinants on the eradication success of 14-d triple therapy *** *** infections were investigated in 72 patients with gastroduodenal complications suggestive of *** *** cagA and vacA genotypes of cultured strains were studied using polymerase chain *** patients underwent 14 d of triple-therapy *** treatment response was examined using histology and a rapid urease test 6 wk after therapy *** The intention-to-treat eradication rate was 59.2%(95%CI:48.2%-70.3%).Rates of *** resistance to clarithromycin,amoxicillin,and metronidazole were 52.8%,81.9%,and 100%,*** eradication of *** was more significantly associated with vacA s1-positive strains[adjusted odds ratio(aOR)=0.507,95%CI:0.175-0.822].A significant association was found between failed eradication rate and *** strains resistant to clarithromycin(aOR=0.204,95%CI:-0.005 to 0.412)and amoxicillin(aOR=0.223,95%CI:0.026-0.537).CONCLUSION This study’s low *** eradication rate following 14-d triple therapy is concerning and *** pan-resistance to metronidazole followed by the high resistance to ciprofloxacin,amoxicillin,and clarithromycin in this research is challenging and of great concern.

Owing to the significance of improving fuel economy, reducing emissions, and extending the durability of engine components, this study focused on the tribological performance of nano-additives. In this study, copper (Cu) and graphene (Gr) nanomaterials were dispersed in a fully formulated engine oil (5W-30) with different concentrations. The tribological trials were investigated under various speeds and loads, utilizing a reciprocating tribometer to mimic the ring/liner interfaces in the engine. The frictional surface morphologies were com-prehensively analyzed using electron probe X-ray microanalysis (EPMA), field emission scanning electron microscopy (FESEM), energy dispersive spectrometer (EDS), and three dimensional (3D) surface profilometry to explore the mechanisms responsible for improving the tribological performance of the frictional sliding parts in the engine. The tribological test results illustrated that lubrication by nano-additives reduced the wear rate (WR) and friction coefficient (COF) by 25%–30% and 26.5%–32.6%, respectively, as compared with 5W-30. The results showed that this is a promising approach for increasing the durability and lifespan of frictional sliding components and fuel economy in automobile engines.

In this paper we first prove that a near-ring admits a derivation if and only if it is zero-symmetric. Also, we prove some commutativity theorems for a non-necessarily 3-prime near-ring R with a suitably-constrained derivation d satisfying the condition that d（a） is not a left zero-divisor in R for some a ∈ R. As consequences, we generalize several commutativity theorems for 3-prime near-rings admitting derivations.

· AIM: To evaluate the safety and efficacy of dexamethasone implant in patients with non-infectious posterior uveitis with cystoid macular edema(CME).·METHODS: Retrospective analysis of patients reports with CME secondary to non-infectious uveitis treated with dexamethasone implant. Data included type of posterior uveitis, any systemic immunosuppressive therapy, Early Treatment Diabetic Retinopathy Study(ETDRS) best-corrected visual acuity(BCVA), central macular thickness(CMT) on optical coherence tomography(OCT) and signs of intraocular inflammation at baseline and then at 2wk postoperatively and monthly thereafter. Follow-up is up to 10 mo. Any per-operative and post-operative complications were recorded.·RESULTS: Six eyes of 4 patients with CME due to non-infectious posterior uveitis treated with dexamethasone implant. Diagnosis included idiopathic panuveitis, birdshot chorioretinopathy and idiopathic intermediate uveitis. At baseline mean ETDRS BCVA was63 letters and mean CMT 556 μm at 2wk postoperatively mean ETDRS BCVA improved to 70 letters and mean CMT decreased to 329 μm. All eyes showed clinical evidence of decreased inflammation. The duration of effect of the implant was 5 to 6mo and retreatment was required in 2 eyes. Two patients required antiglaucoma therapy for increased intraocular pressures.·CONCLUSION: In patients with non-infectious posterior uveitis dexamethasone implant can be a short-term effective treatment option for controlling intraocular inflammation.

Purpose:Evaluation of the patency and position of perforated lacrimal punctal plugs implanted for treating punctal stenosis together with quantitative assessment of the precorneal tear film using anterior segment optical coherence tomography(AS-OCT).Methods:In a prospective study,the lower punctum of 54 eyes of 29 patients implanted with perforated punctal plugs were examined using AS-OCT during the early postoperative *** tear meniscus height(TMH)and tear meniscus area(TMA)were ***,the patency of the plug,its position,TMH and TMA were evaluated,and the results were correlated with postoperative *** scale was used for epiphora ***:Using AS-OCT,48(88.9%)plugs were found in proper position while 6(11.1%)were *** lumen of the plugs was completely patent in 47(87%)plugs,partially obstructed in 2(3.7%)plugs and completely occluded in 5(9.2%)*** was a statistically significant postoperative decrease of TMH and TMA(P<0.001)and postoperative epiphora Munk score(P<0.001).Conclusion:AS-OCT is a valuable,reliable,and noninvasive investigative tool that can detect the proper positioning,patency,and contents of the implanted perforated lacrimal punctal plugs in addition to measurement of TMH and TMA.

Objective:This research study explores the perceptions of gerontological nursing competencies,attitudes toward older individuals,and the willingness to provide care for the elderly among internship nursing *** and Methods:A convenience sample of 350 internship nursing students from Cairo University Faculty of Nursing participated in the *** were collected using a set of comprehensive tools,including personal data and work experience assessment,the Hartford geriatric nurse competency tool,Kogan’s attitudes toward old people scale,and the modified elderly patient care ***:Descriptive analysis revealed balanced gender representation,with 60%identifying as *** percent of participants reported prior experience in gerontological ***-assessed competency scores indicated moderate proficiency in communication,physiological changes,and functional status *** such as pain management and restraint use demonstrated potential gaps in self-perceived *** toward older individuals were predominantly positive,and the willingness to provide care showed positive tendencies,with a minor degree of hesitation ***:The findings underscore the need for targeted interventions in nursing education to address competency gaps,enhance positive attitudes,and alleviate potential hesitations in caregiving for older *** the elderly population continues to grow,nursing education programs must prepare future practitioners to deliver comprehensive and compassionate care tailored to the unique needs of older adults.

BACKGROUND Choledocholithiasis develops in up to 20%of patients with gall bladder *** challenge in diagnosis usually occurs with small stones that may be missed by magnetic resonance cholangiopancreatography(MRCP).Endoscopic ultrasound(EUS)is accurate in detecting common bile duct(CBD)stones missed by MRCP,especially the small ones or those impacted at the distal CBD or the papillary *** To evaluate the accuracy of EUS in detecting CBD stones missed by *** Patients with an intermediate likelihood of choledocholithiasis according to ESGE guidelines and those with acute pancreatitis of undetermined cause were *** presence of choledocholithiasis was evaluated by MRCP and EUS,and then results were confirmed by endoscopic retrograde cholangiopancreatography(ERCP).The sensitivity and specificity of EUS and MRCP were compared regarding the presence of stones,the size,and the number of detected *** Ninety out of 100 involved patients had choledocholithiasis,while ten patients were excluded as they had pancreatic or gall bladder masses during EUS *** choledocholithiasis patients,the mean age was 52.37±14.64 years,and 52.2%were *** patients had biliary obstruction(74.4%),while only 23(25.6%)patients had unexplained *** overall prevalence of choledocholithiasis was 83.3%by EUS,41.1%by MRCP,and 74.4%by ***,the number and size of CBD stones could be detected accurately in 78.2%and 75.6%by EUS and 41.1%and 70.3%by MRCP,*** sensitivity of EUS was higher than that of MRCP(98.51%vs 55.22%),and their predictive value was statistically different(P<0.001).Combination of both tools raised the sensitivity to 97.22%and specificity to 100%.CONCLUSION EUS could be a useful tool in assessing patients with suspected choledocholithiasis especially if combined with ***,its usefulness depends on its availability and the experience of the local centers.

The excitation mechanism of coronal quasi-period fast-propagating(QFP)wave trains remains *** Atmospheric Imaging Assembly onboard the Solar Dynamics Observatory observations,we study a narrow and a broad QFP wave trains excited one after another during the successive eruptions of filaments hosted within a fan-spine magnetic system on October 20,*** consecutive occurrence of these two types of QFP wave trains in the same event provides an excellent opportunity to explore their excitation mechanisms and compare their physical *** observational results reveal that narrow and broad QFP wave trains exhibit distinct speeds,periods,energy fluxes,and relative intensity amplitudes,although originating from the same active region and being associated with the same GOES C2.9 *** wavelet analysis,we find that the narrow QFP wave train shares a similar period with the flare itself,suggesting its possible excitation through the pulsed energy release in the magnetic reconnection process that generated the accompanying *** the other hand,the broad QFP wave train appears to be associated with the energy pulses released by the successive expansion and unwinding of filament ***,it is plausible that the broad QFP wave train was also excited by the sequential stretching of closed magnetic field lines driven by the erupting *** findings shed light on the different excitation mechanisms and origins of the QFP wave trains.
