BACKGROUND Nonalcoholic steatohepatitis-related cirrhosis is one of the liver complications in type 2 diabetes mellitus(T2DM)and reported to be a risk factor for developing hepatocellular carcinoma(HCC).A reliable screening biomarker of liver cirrhosis(LC)and HCC among T2DM patients is important to reduce the morbidity and mortality of this disease.MicroRNA(miRNA)is considered a key player in HCC and T2DM,and it might be a hidden culprit in diabetes-associated HCC,making it a promising reliable prognostic tool.AIM To investigate the signature of serum miRNAs as early biomarkers for the screening of HCC among diabetic patients.METHODS Expression profiles of miRNAs in serum samples of diabetic LC and diabetic HCC patients were assessed using Illumina sequencing;then,RT-qPCR was used to validate significantly altered miRNAs between the two groups.Candidate miRNAs were tested in serum samples of 200 T2DM patients,270 LC patients,200 HCC patients,and 225 healthy control subjects.Additionally,receiver operating characteristic(ROC)analysis,with area under the curve(AUC),was performed to assess the diagnostic performance of the screened miRNAs for discriminating HCC from LC and nonmalignant patients(LC+T2DM).RESULTS Expression of the sequenced miRNAs in serum was different in HCC vs LCpositive T2DM patients.Two miRNAs(miR-34a,miR-221)were significantly upregulated and five miRNAs(miR-16,miR-23-3p,miR-122-5p,miR-198,miR-199a-3p)were significantly down-regulated in HCC compared to LC patients.Analysis of ROC curve demonstrated that the combination of these seven miRNAs can be used as a reliable biomarker for detection of HCC in diabetic patients,as it could identify HCC with high diagnostic accuracy in diabetic LC patients(AUC=0.993)and in diabetic nonmalignant patients(AUC=0.961).CONCLUSION This study validates a panel of serum miRNAs that can be used as a reliable noninvasive screening biomarker of HCC among T2DM cirrhotic and noncirrhotic patients.The study recommends further research to shed light on a possible role of

Minimally Invasive 360 Degrees Decompression for Ligamentous, Bony and Discogenic Lumbar Canal Stenosis

Background: For decades, traditional open surgical techniques were used to treat lumbar disc herniation and lumbar canal stenosis (LCS). However, seeking for better outcomes for patients and avoiding extensive bony loss with its sequences had raised minimally invasive technique for treating these disorders as an alternative surgery. Methods: This is a retrospective study in which 54 patients of LCS were operated upon via unilateral minimally invasive technique to decompress the canal in a 360 degrees fashion through laminotomy, deroofing of opposite laminar side, sublaminar ligamintectomy, bilateral foraminotomies and discectomy. We used VAS scores and ODI to assess clinical outcomes with a period of one year follow-up. Results: Our results demonstrated that minimally invasive techniques for treating these disorders are effective procedures. Minimally invasive 360 degrees decompression for treating LCS had better outcomes regarding postoperative back pain, smaller incisions, less bony loss and early ambulation. Conclusion: Minimally invasive techniques for treating lumbar canal stenosis of different causes could be considered a better option instead of traditional full laminectomy with better outcomes as regards respecting the anatomical layers such as posterior spinal integrity and musculature, postoperative pain, accompanied with less blood loss, shorter hospital stays, and shorter recovery periods.

Effect of dietary inclusion of sugar syrup on production performance,egg quality and blood biochemical parameters in laying hens

The effects of dietary inclusion of sugar syrup on quality of egg,cholesterol level,production performance,serum total protein and blood biochemical parameters were evaluated in laying hens.A total of300 commercial Lohmann LSL hens(30 weeks of age) were randomly distributed into 3 dietary treatments which consisted of a normal corn diet containing cornesoy and 2 diets containing 5% and 10%sugar syrup.Each treatment was replicated 5 times(n=20).Egg production,feed intake,body weight and egg weight of laying hens fed different diets were recorded.The experiment lasted for 20 weeks.The Haugh unit scores of hens fed diets with sugar syrup were significantly increased(P < 0.05) compared with the control treatment.The sugar syrup had no significant effect on liver enzymes,total protein,blood glucose and creatinine in all treatments.The eggs laid by hens fed sugar syrup diets had lower cholesterol level(P < 0.05) compared with those laid by hens fed the control diet.Electrophoresis analysis showed that comparable electrophoretic patterns were noticed between serum proteins of treatment groups.From the results,it can be concluded that sugar syrup diets and corn diets have similar effects on feed intake,body weight,production of eggs and blood biochemical parameters in layer hens,which suggests sugar syrup can be used as an energy source for replacing part of corn in poultry layer diets.

Dear Editor,Recently,there has been a rise in interest in the roles of cytokines and cytokine receptors in malignant human tumors.Cytokine receptors,especially typeⅡinterleukin-4 receptor(IL-4R),have recently been examined as novel therapeutic targets of human cancers[1-3].The typeⅡIL-4R complex is composed of IL-4Rαand interleukin-13 receptorα1(IL-13Rα1),and the expression of IL-4Rαand IL-13Rα1 was higher in cancer tissues compared with normal counterpart tissues[3,4].

Cystic pancreatic lesions involve a wide variety of pathological entities that include neoplastic and non-neoplastic lesions.The proper diagnosis,differentiation,and staging of these cystic lesions are considered a crucial issue in planning further management.There are great challenges for their diagnostic models.In our time,new emerging methods for this diagnosis have been discovered.Endoscopic ultrasonography-guided fine-needle aspiration cytology with chemical and molecular analysis of cyst fluid and EUS-guided fine needlebased confocal laser endomicroscopy,through the needle microforceps biopsy,and single-operator cho-langioscopy/pancreatoscopy are promising methods that have been used in the diagnosis of cystic pancreatic lesions.Hereby we discuss the diagnosis of cystic pancreatic lesions and the benefits of various diagnostic models.

Relative Explosive Strength of Some Explosive Mixtures Containing Urea and/or Peroxides

Several mixtures,based on urea derivatives and some inorganic oxidants,including also alumina,were studied by means of ballistic mortar techniques with TNT as the reference standard.The detonation pressure(P),detonation velocity(D),detonation energy(Q),and volume of gaseous product at standard temperature and pressure(STP),V,were calculated using EXPLO5V6.3 thermochemical code.The performance of the mixtures studied was discussed in relation to their thermal reactivity,determined by means of differential thermal analysis(DTA).It is shown that the presence of hydrogen peroxide in the form of its complex with urea(i.***.as UHP)has a positive influence on the explosive strength of the corresponding mixtures which is linked to the hydroxy-radical formation in the mixtures during their initiation reaction.These radicals might initiate(at least partially)powdered aluminum into oxidation in the CJ plane of the detonation wave.Mixtures containing UHP and magnesium are dangerous because of potential auto-ignition.

Changes of MicroRNA-377 in Diabetic Nephropathy

Background: MicroRNAs (miRNAs) play important roles in modulating gene expression. In cultured mesangial cells, MiR-377 exhibited the greatest change on exposure to high glucose and led to increased fibronectin production in in-vitro diabetic nephropathy. Our aim was to identify the pattern of microRNA-377 changes in human diabetic patients with different stages of diabetic nephropathy. Methods: The study included 45 patients divided into 5 groups;patients with stage 1 & 2 diabetic nephropathy (DN), stage 3 & 4 DN, diabetics without DN, stage 1 & 2 chronic kidney disease (CKD), and stage 3 & 4 CKD. The following tests were done to all patients;serum creatinine, estimated glomerular filtration rate (eGFR), albumin to creatinine ratio, blood sugar, urine analysis, glycated hemoglobin, fundus examination. The RNA was extracted from plasma samples, TaqMan microRNA (miRNA) assays from applied Bio systems were used for analysis of MicroRNA-377. Results: This study found that patients with the highest median of the interquartile range (IQR) of miR-377 were those of group 1 with significant difference between them and all other groups. MiR-377 was significantly correlated with glycated hemoglobin but not with eGFR. Conclusion: Plasma miR-377 is highly significantly increased in human diabetic patients with early rather than late diabetic nephropathy, diabetics without nephropathy, or those at any stage of other causes of CKD. Our findings confirm the role of miR-377 as a potentially novel target in the development of diabetic nephropathy in humans.

In-hospital outcomes of transapical versus surgical aortic valve replacement: from the U.S. national inpatient sample

OBJECTIVE To compare the outcomes of transapical transcatheter aortic valve replacement(TA-TAVR)and surgical aortic valve replacement(SAVR)using a large US population sample.METHODS The U.S.National Inpatient Sample was queried for all patients who underwent TA-TAVR or SAVR during the years2016-2017.The primary outcome was all-cause in-hospital mortality.Secondary outcomes were in-hospital stroke,pericardiocentesis,pacemaker insertion,mechanical ventilation,vascular complications,major bleeding,acute kidney injury,length of stay,and cost of hospitalization.Outcomes were modeled using multi-variable logistic regression for binary outcomes and generalized linear models for continuous outcomes.RESULTS A total of 1560 TA-TAVR and 44,280 SAVR patients were included.Patients who underwent TA-TAVR were older and frailer.Compared to SAVR,TA-TAVR correlated with a higher mortality(4.5%vs.2.7%,effect size(SMD)=0.1)and higher periprocedural complications.Following multivariable analysis,both TA-TAVR and SAVR had a similar adjusted risk for in-hospital mortality.TA-TAVR correlated with lower odds of bleeding with(adjusted OR(aOR)=0.26;95%CI:0.18-0.38;P<0.001),and a shorter length of stay(adjusted mean ratio(aMR)=0.77;95%CI:0.69-0.84;P<0.001),but higher cost(aMR=1.18;95%CI:1.10-1.28;P<0.001).No significant differences in other study outcomes.In subgroup analysis,TA-TAVR in patients with chronic lung disease had higher odds for mortality(aOR=3.11;95%CI:1.37-7.08;P=0.007).CONCLUSION The risk-adjusted analysis showed that TA-TAVR has no advantage over SAVR except for patients with chronic lung disease where TA-TAVR has higher mortality.

Inhibition of glucose-and fructose-mediated protein glycation by infusions and ethanolic extracts of ten culinary herbs and spices

Objective: To investigate the inhibitory activity of ten culinary herbs and spices namely on glucose-mediated glycation(GMG) and fructose-mediated glycation(FMG) of bovine serum albumin.Methods: Fluorescence was used as an index of albumin glycation using glucose and fructose as substrates in the presence of infusions and ethanolic extracts of ten culinary herbs and spices. Antioxidant activity of the extracts was evaluated using reducing power,metal ion chelating and superoxide radical scavenging assays. Phytochemicals profile was analysed using 13 standard methods.Results: FMG was found to be significantly higher than GMG(95 and 84 AU,respectively; P 0.05) was found in the percentage glycation inhibitory activity of infusions compared to ethanolic extracts. The mean percentage inhibitory activity of the extracts for GMG(45.9%) and for FMG(45.1%) was not significantly different(P > 0.05). Qualitative phytochemical analysis showed the presence of alkaloids, flavonoids, tannins, terpenoids, anthraquinones, steroids, reducing sugars, proteins, phenols,saponins, phlobatannins, and cardiac glycosides.Conclusions: The higher rate of fluorescence generation by fructation suggests that glycation by fructose deserves much attention as a glycating agent. Data herein showed that the extracts inhibited GMG and FMG. Thus, these edible plants could be a natural source of antioxidants and anti-glycation agent for preventing advanced glycation endproducts-mediated complications.
