Comparative Study between Endoscopic Ultrasonography and Transabdominal Ultrasonography in the Assessment of Pancreatobiliary Diseases (with Literatures Review)

Background: Endoscopic ultrasonography (EUS) and transadominal ultrasonography (TUS) are two imaging investigations which can be used to assess pancreatobiliary status, although both of these imaging techniques are operator and machine dependent, but they have different sensitivity and specificity in detecting pancreatobiliary diseases. Objective: To compare the diagnostic value of EUS versus TUS in the assessment of pancreatobiliary diseases. Patients and Methods: This study was conducted in KCGH (Kurdistan Center for Gastroenterology and Hepatology) in Sulaimani city. Iraqi Kurdistan;the duration of study was 14 months and conducted after approval of Iraqi board ethical committee. One hundred cases were enrolled in the study: 52 of them were females and 48 were males;their ages ranged between 16 - 90 years;informed consent was taken from all patients;all patients underwent proper clinical evaluation;TUS, EUS and Oesophagogastroduodenoscopy (OGD), and in some of them (25 patients) Endoscopic Retrograde Cholangiopancreatography (ERCP), CT scan of abdomen and laboratory investigations were done. Results: EUS detected more cases with common bile duct (CBD) dilatation, CBD stones and pancreatic space occupying lesion (SOL) than TUS, but the differences in the results were not statistically significant. Also EUS detected more cases of intra-abdominal lymph nodes (LNs) and ampullary/peri-ampullary neoplasms than TUS and the differences in the results were statistically significant. Conclusions: Eus is more sensitive than TUS in diagnosing CBD dilatation;it has higher sensitivity than TUS in suspected pancreatobiliary neoplasms;furthermore, EUS can be used for staging and resectability assessment of pancreatobiliary neoplasia.

The construction of gas and oil plants include many items such as steel structures, equipments, piping, tanks and vessels at different conditions such as offshore, onshore, above and underground shall be protected against corrosion by coating. The protective value of a coating system is proportional to the surface preparation;the objectives of the surface preparation are the surface cleanliness and the surface profile of the substrate. The surface preparation is usually achieved by blast cleaning using different types of abrasive. Due to its high hardness and low cost, silica sand has been used extensively for impact abrasion. It is, however, very brittle with most grain disintegrating on impact, causing excessive levels of siliceous dust which is a major health hazard and causes serious environmental pollution. Consequently, it has been banned as impact abrasive material in most countries around the world. Considerable research has been done in the last twenty years to find an impact abrasive material as hard as silica and that respects. So it was important to search for sand substitution among the local abundant minerals in Egypt. Waste product obtained from ilmenite ore extracted from the mine was found to be the most suitable alternative abrasive. This work shows the abrasive types, the occurrence of the ilmenite ore. The chemical properties, physical properties and microscopic examination of the waste were investigated. The obtained result indicates that, the waste product obtained from ilmenite ore can be utilized for abrasion as sand substitution.

Preliminary Assessment of Goat Piroplasmosis in Benadir Region, Somalia

Haemoparasites are major-constraints on livestock production in tropical and sub-tropical countries. This study was conducted during 2012-2013 to determine the prevalence of blood parasites in goats of small-holders in Benadir region, Somalia and update epidemiological data that had already lost during the civil war in the country. A total of 100 blood samples were collected from goat in Wadajir (47 goat) and Dharkeynley (53 goat) districts, using venipuncture of jugular vein. The samples were examined for the presence of blood parasites using light microscopy. Some ticks when presented in the sampled animals—were also collected for tick identification. Analysis of blood smears revealed 100% samples positive for blood parasites. Out of these cases, 22 samples (22%) were harboring single infection of Babesia spp. and 14 samples (14%) were having single infection of Theileria spp. Interestingly the Remaining 64 blood samples (64%) showed mixed infection of Babesia spp. with Theileria spp. Rhipicephalus evertsi (72.84%), Rhipicephalus pulchellus (34.57%), Amblyomma lepidum (3.70%) and Hyalomma rufipes (1.23%) were identified from the investigated goats. In conclusion, the findings of this study indicated that, the prevalence of tick and tick-borne diseases were considered to be high in Benadir region of Somalia. A further area wide in-depth study is recommended in the country.

Epilepsy is one of the most prevalent neurological disorders with no age, racial, social class, and neither national nor geographic boundaries. There are 50 million sufferers in the world today with 2.4 million new cases occur each year. Electroencephalogram (EEG) has become a traditional procedure to investigate abnormal functioning of brain activity. Epileptic EEG is usually characterized by short transients and sharp waves as spikes. Identification of such event splays a crucial role in epilepsy diagnosis and treatment. The present study proposes a method to detect three epileptic spike types in EEG recordings based mainly on Template Matching Algorithm including multiple signal-processing approaches. The method was applied to real clinical EEG data of epileptic patients and evaluated according to sensitivity, specificity, selectivity and average detection rate. The promising results illuminate that hybrid processing approaches in temporal, frequency and spatial domains can be a real solution to identify fast EEG transients.

Background: Coronary artery disease (CAD) is a paramount cause of death. Global and regional left ventricular (LV) systolic function is an important non-invasive marker of CAD. We can use tissue Doppler imaging (TDI) to measure the low velocities generated by myocardium which are: S'-wave, E'-wave, and A'-wave. We aimed to examine the value of the S'-wave dispersion of TDI across mitral valve annular velocities to predict angiographically significant coronary artery obstruction. Patients and Methods: We included 100 patients with symptoms suggesting CAD. All patients had undergone full history taken and clinical examination;ECG, echocardiographic with assessment of LV function, and Doppler derived mitral valve velocities;TDI with measuring of S'-wave and coronary angiography. Lesions with ≥70% or more stenosis in major epicardial artery or ≥50% stenosis in the left main coronary artery were considered significant. Patients were classified into two groups according to the presence or absence of significant coronary stenosis. Results: Clinical and conventional echocardiographic and Tissue Doppler measures were comparable between the two groups;there was statisticallysignificant difference between Group I and Group II (P value;patients with significant coronary arteries lesions have higher S' wave dispersion as compared with patients with normal or mild significant coronary lesions. Conclusion: S'-wave dispersion may consider a good predictor of angiographically significant coronary artery disease.

A pressure dependent Schrodinger equation is used to find the conditions that lead to superconductivity. When no pressure is exerted, the superconductor resistance vanishes beyond a critical temperature related to the repulsive force potential of the electron gass, where one assuming the electron total energy to be thermal, where applying mechanical pressure destroys Sc when it exceeds a certain critical value. However when the electron total energy is an assumed to be that of the free electron model and that the pressure is thermal and mechanical, the situation is different. The quantum expression for resistance shows that the increase of mechanical pressure increases the critical temperature. Such phenomenon is observed in high temperature cupper group.

Outcomes and Prognostic Factors of Small Cell Lung Cancer: A Retrospective Study

Background: Small cell lung cancer (SCLC) is a high grade neuroendocrine tumor, and has aggressive nature, so the majority of cases are presented with extensive disease. SCLC was staged into 2 categories: limited-stage disease (LS-SCLC) and extensive disease (ES-SCLC). Despite SCLC is sensitive to ra-diotherapy and chemotherapy, SCLC has high tendency for rapid dissemina-tion to regional and distant sites. Median survival time ranged from 2 - 4 months in patients with untreated SCLC. Multiagent chemotherapy was the primary treatment for SCLC. Aim of the work: This retrospective study was conducted to evaluate and analyze clinical features, treatment outcome, sur-vival and prognostic factors affecting survival in patients with SCLC presented to Clinical Oncology and Nuclear Medicine department, Chest department and Medical oncology unit in Mansoura Oncology Centre during the period from 2000-2015. Methods: Data of patients were collected from their files. The information obtained included demographic features, treatment received;its toxicity and outcome, survival and its prognostic factors. Demographic data were: age, Eastern Cooperative Oncology Group Performance Status (ECOG-PS), smoking status, stage of disease. Data also included disease presentation and metastatic sites. Several factors affecting survival were analysed as age, sex, stage, PS, smoking status and LDH. Results: Sixty-three patients were enrolled in this study. Median age was 56.2 ± 6. Strong male predominance (92.1%) was observed;84.1% of them were smoker. Thirty six patients (57.2%) were of ECOG-PS of 0 - 1. ES-SCLC was reported in 65% of cases and LDH was high (>1.5 xN) in 47.6%. The most common symptom was chest pain (38.1%) followed by cough (31.8%), weight loss (30%). Fifteen patients had single metastatic site (23.8%) and bone was the most common site of metastasis (reported in 8 patients) followed by brain, lung and liver. 2-year overall survival rate was 35% with median survival time of 14 months. 针对机械仿真过程中重复操作较多,以及仿真效率低的问题,设计了一种基于RPA(Robotic Process Automation)技术的机械仿真流程自动化软件,通过OCR文字识别技术,并结合外设模拟操作,可以实现机械仿真流程的自动化执行。对软件的总体架构,集成开发环境以及服务端进行了设计,并对OCR文字识别算法进行了优化,同时,为验证该系统的可行性,对系统进行了测试,实现了机械仿真流程的自动化执行,证明该系统在保证仿真质量的前提下,可以极大地提升仿真效率。

This paper presents a novel fault-tolerant networked control system architecture consisting of two cells working in-line. This architecture is fault-tolerant at the level of the controllers as well as the sensors. Each cell is based on the sensor-to-actuator approach and has an additional supervisor node. It is proven, via analysis as well as OMNeT++ simulations that the production line succeeds in meeting all control system requirements with no dropped or over-delayed packets. A reliability analysis is then undertaken to quantitatively estimate the increase in reliability due to the introduction of fault-tolerance.
