The impact of chemotherapy-naïve open radical cystectomy delay and perioperative transfusion on the recurrence-free survival: A perioperative parameters-based nomogram

Objective: To develop and internally validate a nomogram to predict recurrence-free survival (RFS) including the time to radical cystectomy (RC) and perioperative blood transfusion (PBT) as potential predictors.Methods: Patients who underwent open RC and ileal conduit between January 1996 to December 2016 were split into developing (n=948) and validating (n=237) cohorts. The time to radical cystectomy (TTC) was defined as the interval between the onset of symptoms and RC. The regression coefficients of the independent predictors obtained by Cox regression were used to construct the nomogram. Discrimination, validation, and clinical usefulness in the validation cohort were assessed by the area under the curve, the calibration plot, and decision curve analysis.Results: In the developing dataset, the 1-, 5-, and 10-year RFS were 83.0%, 47.2%, and 44.4%, respectively. On multivariate analysis, independent predictors were TTC (hazards ratio [HR] 1.07, 95% confidence interval [CI] 1.05-1.08, p<0.001), PBT (one unit: HR 1.40, 95% CI 1.03-1.90, p=0.03;two or more units: HR 1.72, 95% CI 1.29-2.29, p<0.001), bilateral hydronephrosis (HR 1.54, 95% CI 1.21-1.97, p<0.001), squamous cell carcinoma (HR 0.60, 95% CI 0.45-0.81, p=0.001), pT3-T4 (HR 1.77, 95% CI 1.41-2.22, p<0.001), lymph node status (HR 1.53, 95% CI 1.21-1.95, p<0.001), and lymphovascular invasion (HR 1.28, 95% CI 1.01-1.62, p=0.044). The areas under the curve in the validation dataset were 79.3%, 69.6%, and 76.2%, for 1-, 5-, and 10-year RFS, respectively. Calibration plots showed considerable correspondence between predicted and actual survival probabilities. The decision curve analysis revealed a better net benefit of the nomogram.Conclusion: A nomogram with good discrimination, validation, and clinical utility was constructed utilizing TTC and PBT in addition to standard pathological criteria.

Does local vaginal estrogen after tension-free transobturator vaginal tape reduce overactive bladder symptoms in postmenopausal women? A prospective randomized, controlled study

Objective:We aimed to evaluate the efficacy of topical estrogen after transvaginal tension-free vaginal tape-obturator(TVT-O)in the treatment of de novo overactive bladder symptoms that appear after surgery.Methods:This is a prospective randomized controlled study performed in the Urology and Gynecology Departments,Kasr Al Ainy Hospital,Cairo University,Cairo,Egypt.Two hundred and ten postmenopausal females presenting during the period between January 2017 and November 2020 with stress urinary incontinence were included in the study.Patients were divided into two groups,105 patients in Group A(treatment group)and 105 patients in Group B(control group).Patients in Group A underwent transvaginal TVT-O followed by local vaginal estrogen treatment for 6 months,while patients in Group B underwent transvaginal TVT-O only.The study included any postmenopausal female with urodynamic stress urinary incontinence.All patients had to fulfill a 3-day bladder diary,overactive bladder symptoms score,urine analysis,urodynamic study,and post-voiding residual urine measurement by abdominal ultrasound preoperatively and at 3-month and 6-month follow-ups.Results:At 6-month follow-up,daytime frequency was reduced to 8%in Group A(increased to 21%in Group B)with a statistically significant difference between both groups(p=0.009).At 6-month follow-up,nocturia was 8%in Group A(11%in Group B)with no statistically significant difference between both groups(p=0.469).There was a statistically significant difference between both groups as regards to urinary urgency at 6-month follow-up(p=0.024).There was a statistically significant difference in postoperative wound healing events as regards to cure,hyperemia,gapping,and wound infection 1 week after intervention between both groups(p=0.008).No local or systemic side-effects were reported from local estrogen use.Conclusion:Local vaginal estrogen treatment given to postmenopausal patients after midurethral sling procedures can reduce the symptoms of daytime frequency and urinary urgency.Long-ter

Wireless technology is transforming the future of transportation through the development of the Internet of Vehicles(IoV).However,intricate security challenges are intertwinedwith technological progress:Vehicular ad hoc Networks(VANETs),a core component of IoV,face security issues,particularly the Black Hole Attack(BHA).This malicious attack disrupts the seamless flow of data and threatens the network’s overall reliability;also,BHA strategically disrupts communication pathways by dropping data packets from legitimate nodes altogether.Recognizing the importance of this challenge,we have introduced a new solution called ad hoc On-Demand Distance Vector-Reputation-based mechanism Local Outlier Factor(AODV-RL).The significance of AODVRL lies in its unique approach:it verifies and confirms the trustworthiness of network components,providing robust protection against BHA.An additional safety layer is established by implementing the Local Outlier Factor(LOF),which detects and addresses abnormal network behaviors.Rigorous testing of our solution has revealed its remarkable ability to enhance communication in VANETs.Specifically,Our experimental results achieve message delivery ratios of up to 94.25%andminimal packet loss ratios of just 0.297%.Based on our experimental results,the proposedmechanismsignificantly improves VANET communication reliability and security.These results promise a more secure and dependable future for IoV,capable of transforming transportation safety and efficiency.

In the current landscape of the COVID-19 pandemic,the utilization of deep learning in medical imaging,especially in chest computed tomography(CT)scan analysis for virus detection,has become increasingly significant.Despite its potential,deep learning’s“black box”nature has been a major impediment to its broader acceptance in clinical environments,where transparency in decision-making is imperative.To bridge this gap,our research integrates Explainable AI(XAI)techniques,specifically the Local Interpretable Model-Agnostic Explanations(LIME)method,with advanced deep learning models.This integration forms a sophisticated and transparent framework for COVID-19 identification,enhancing the capability of standard Convolutional Neural Network(CNN)models through transfer learning and data augmentation.Our approach leverages the refined DenseNet201 architecture for superior feature extraction and employs data augmentation strategies to foster robust model generalization.The pivotal element of our methodology is the use of LIME,which demystifies the AI decision-making process,providing clinicians with clear,interpretable insights into the AI’s reasoning.This unique combination of an optimized Deep Neural Network(DNN)with LIME not only elevates the precision in detecting COVID-19 cases but also equips healthcare professionals with a deeper understanding of the diagnostic process.Our method,validated on the SARS-COV-2 CT-Scan dataset,demonstrates exceptional diagnostic accuracy,with performance metrics that reinforce its potential for seamless integration into modern healthcare systems.This innovative approach marks a significant advancement in creating explainable and trustworthy AI tools for medical decisionmaking in the ongoing battle against COVID-19.

Kaoline-based catalyst for a high stability desulfurization of sour heavy naphtha in a three-phase oscillatory baffled reactor

To meet the rapidly increasing demand for energy and the dramatic depletion of conventional crude oil,it is imperative to utilize sour naphtha.With no coke being produced,oxidative desulfurization(ODS)of naphtha lowers its sulfur content and average molecular weight.In this article,we outline a method for heavy naphtha non-extractive ODS using a very stable catalyst.The technique involves the use of a solid catalyst with oxygen gas as the oxidant.This necessitated relatively high mixing intensities;hence a three-phase Oscillatory Baffled Reactor(OBR)was used.The catalyst was based on the zeolite ZSM-5,prepared from natural kaolin by a series of delamination and activation steps and impregnated with Fe.A TiO2 nanolayer was applied,using the sol-gel method,to prevent rapid deactivation.The reactor performance was evaluated to minimize the sulfur content in the naphtha fuel.Due to the protective coating,the sulfur conversion stabilized at 90%.The results of this work establish the use of natural clay-based catalysts in a continuous,three-phase ODS,particularly with regard to proving long-term stability.It also showed that modest ODS can be achieved using an environmentally friendly oxidant,at mild operating conditions,whilst maintaining stability.

RPL-Based IoT Networks under Decreased Rank Attack:Performance Analysis in Static and Mobile Environments

The RPL(IPv6 Routing Protocol for Low-Power and Lossy Networks)protocol is essential for efficient communi-cation within the Internet of Things(IoT)ecosystem.Despite its significance,RPL’s susceptibility to attacks remains a concern.This paper presents a comprehensive simulation-based analysis of the RPL protocol’s vulnerability to the decreased rank attack in both static andmobilenetwork environments.We employ the Random Direction Mobility Model(RDM)for mobile scenarios within the Cooja simulator.Our systematic evaluation focuses on critical performance metrics,including Packet Delivery Ratio(PDR),Average End to End Delay(AE2ED),throughput,Expected Transmission Count(ETX),and Average Power Consumption(APC).Our findings illuminate the disruptive impact of this attack on the routing hierarchy,resulting in decreased PDR and throughput,increased AE2ED,ETX,and APC.These results underscore the urgent need for robust security measures to protect RPL-based IoT networks.Furthermore,our study emphasizes the exacerbated impact of the attack in mobile scenarios,highlighting the evolving security requirements of IoT networks.

Nanosecond pulse， dual-wavelength Q-switched 1.5 μm fiber laser using MAX phase CrTiAlCsaturable absorber

We demonstrated a new type of MAX phase material， Chromium Titanium Aluminum Carbide (Cr2TiAlC2) polymer film to generate a passively Q-switched erbium-doped fiber laser. The film thickness was measured around 45 μm which was fabricated using the embedding method with PVA polymer as hoster. The SA film demonstrates a dual-wavelength passively Q-switched erbium-doped fiber laser （EDFL） which operates at 1531 nm and 1560.19 nm， respectively. The Q-switching pulse duration could be varied from 2.46 μs to 770 ns while the repetition rate varied from 92.76 kHz to 106.6 kHz with an increasing input pumping range of 154 mW to 300 mW. The maximum output power and pulse energy of 15.05 mW and 141.18 nJ were obtained at a maximum input power of 300 mW， respectively. To the best of our knowledge， this is the first time that on dual-wavelength Q-switched EDF laser by Cr2TiAlC2as SA.

The development of bio-sourced materials is essential to ensuring sustainable construction;it is considered a locomotive of the green economy.Furthermore,it is an abundant material in our country,to which very little attention is being given.This work aims to valorize the waste of the trunks of banana trees to be used in construction.Firstly,the physicochemical properties of the fiber,such as the percentage of crystallization and its morphology,have been determined by X-ray diffraction tests and scanning electron microscopy to confirm the potential and the impact of the mode of drying on the quality of the banana fibers,with the purpose to promote the use of this material in construction.Secondly,the results obtained with the gypsum matrix allowed us to note a preponderant improvement in the composite’s thermal properties thanks to the variation of the banana fiber additive.Thirdly,the impact of the nature of the banana fiber distribution(either fiber mixed in matrix or fiber series model)on the flexural and compressive strengths of the composites was studied.The results obtained indicate that the insulation gain reaches up to 40%.It depends on the volume fraction and type of distribution of the banana fibers.However,the thermal inertia of the composites developed,represented by thermal diffusivity and thermal effusivity,was studied.Results indicate a gain of 40%and 25%,respectively,in terms of thermal diffusivity and thermal effusivity of the developed composites compared to plaster alone.Concerning the mechanical properties,the flexural strength depends on the percentage of the volume fraction of banana fibers used,and it can reach 20%more than the flexural strength of plaster;nevertheless,there is a significant loss in terms of the compressive strength of the studied composites.The results obtained are confirmed by the microstructure of the fiber banana.In fact,the morphology of the banana fibers was improved by the drying process.It reduces the amorphous area and improves the cellulosic crystalline surfaces,w

Dry face milling of titanium alloys

In machining titanium alloys, cutting tools generally wear out very rapidlybecause of the high cutting temperature resulted from the low thermal conductivity and density ofthe work material. In order to increase the tool life, it is necessary to suppress the cutting heatas much as possible by applying an abundant amount of coolant, but this will entail serioustechno-environmental and biological problems. To study the performance and avoid these limitations,a PVD-coated insert was used to the dry face mill of (alpha + beta) titanium alloys. As a result itwas found that the inserts exhibit an excellent cutting performance at low cutting speeds and feedrates, and there is no significant difference in the dominant insert failure mode between the wetand dry cutting in discontinuous cutting.

Relative performance of novel blast wave mitigation system to conventional system based on mitigation percent criteria

Recent researches focused on developing robust blast load mitigation systems due to the threats of terrorist attacks.One of the main embraced strategies is the structural systems that use mitigation techniques.They are developed from a combination of structural elements and described herein as conventional systems.Among the promising techniques is that redirect the waves propagation through hollow tubes.The blast wave propagation through tubes provides an efficient system since it combines many blast wave phenomena,such as reflection,diffraction,and interaction.In this research,a novel blast load mitigation system,employed as a protection fence,is developed using a technique similar to the technique of the bent tube in manipulating the shock-wave.The relative performance of the novel system to the conventional system is evaluated based on mitigation percent criteria.Performances of both systems are calculated through numerical simulation.The proposed novel system proved to satisfy high performance in mitigating the generated blast waves from charges weight up to 500 kg TNT at relatively small standoff distances(5 m and 8 m).It mitigates at least 94%of the blast waves,which means that only 6%of that blast impulse is considered as the applied load on the targeted structure.
