The Effect of Early versus Traditional Hospital Discharge on Activities of Daily Living for Women Undergoing Elective Cesarean Section. An Observational Cohort Study

Background: Cesarean section is one of the most common surgeries performed worldwide and its consequences and complications represent a major public health concern. The policy of enhanced recovery after surgery has been implemented after cesarean section to optimize perioperative care. Enhanced recovery also aims to reduce the length of hospital stay, which might imply substantial healthcare savings. Rising hospital costs are one of the factors in early discharge. Patients & Methods: This was a prospective cohort study. 158 patients met the inclusion criteria of the study, 81 patients were discharged after 48 hours and 77 patients were discharged after 24 hours. All patients received the same postoperative care and instructions. All the study participants received a phone call 1, 3, and 5 days after discharge to objectively assess the activities of daily living through Katz index of independence in activities of daily living. All patients were offered a postpartum visit 1 week after discharge during which the wound was assessed by the Southampton wound assessment scale. Moreover, other maternal complications such as mastitis, puerperal sepsis, or pyelonephritis were assessed and cases who were readmitted were documented. Initiation of successful breastfeeding and neonatal readmission were reported as well. Results: 158 patients met the inclusion/exclusion criteria of the study. There was no significant difference between the two arms of the study regarding baseline characteristics. Katz index of independence showed that the resumption of activities of daily living after CS was similar in both arms of the study on days 1, 3 and 5 after discharge. Time till the passage of flatus and stool after the cesarean section was significantly shorter among the early discharge arm (9.31 VS 14.68, p value < 0.001 & 13.25 VS 24.82, p value < 0.001 respectively). Plant diseases are a major impendence to food security,and due to a lack of key infrastructure in many regions of the world,quick identification is still *** losses owing to illnesses are a severe problem for both large farming structures and rural communities,motivating our *** of the large range of diseases,identifying and classifying diseases with human eyes is not only time-consuming and labor intensive,but also prone to being mistaken with a high error *** learning-enabled breakthroughs in computer vision have cleared the road for smartphone-assisted plant disease and *** proposed work describes a deep learning approach for detection plant ***,we proposed a deep learning model strategy for detecting plant disease and classification of plant leaf *** our research,we focused on detecting plant diseases in five crops divided into 25 different types of classes(wheat,cotton,grape,corn,and cucumbers).In this task,we used a public image database of healthy and diseased plant leaves acquired under realistic *** our work,a deep convolutional neural model AlexNet and Particle Swarm optimization was trained for this task we found that the metrics(accuracy,specificity,Sensitivity,precision,and Fscore)of the tested deep learning networks achieves an accuracy of 98.83%,specificity of 98.56%,Sensitivity of 98.78%,precision of 98.67%,and F-score of 98.47%,demonstrating the feasibility of this approach.

Herein,iron oxide/hydroxides deposits(gossans)were utilized,for the first time,in the fabrication of magnetite nanoparticles(MNPs)to load modified coal(MC).The as-synthesized MNPs@MC composite was characterized via different techniques and utilized for the Cr(Ⅵ)*** studies supported by theoretical treatment were applied to offer a new overview of the Cr(Ⅵ)adsorption geometry and mechanism at 25-45℃.Experimental results suggested that the Cr(Ⅵ)uptake was mainly governed by adsorption-reduction coupled *** Langmuir model fitted well the Cr(Ⅵ)adsorption data with maximum adsorption capacities extended from 115.24 to 129.63 mg·g^(-1).Theoretical calculations indicated that Cr(Ⅵ)ions were adsorbed on the MNPs@MC following the theory of the advanced monolayer statistical *** number of ions removed per site ranged from 1.88 to1.23 suggesting the involvement of vertical geometry and multi-ionic mechanism at all *** increment of the active sites density and the adsorption capacity at saturation with improving temperature reflected an endothermic ***,the Cr(Ⅵ)adsorption was controlled by physical forces as the adsorption energies were less than 40 kJ·mol^(-1).The calculated free enthalpy,*** internal energy explained the spontaneous nature and the viability of Cr(Ⅵ)adsorption on the MNPs@MC *** results offer a new approach in utilizing the iron-rich deposits as gossans in the preparation of magnetic and low-cost adsorbents for wastewater remediation.

The global burden of hepatitis B virus(HBV)and hepatitis C virus(HCV)infections and coinfection represents a major public health concern,particularly in resource-limited *** of HCV by 2030 has become foreseeable,with effective direct-acting antiviral oral therapies and the availability of affordable generics in low-and-middle-income countries(LMICs).However,access to oral nucleos(t)ide therapy for HBV remains critical and is limited outside the existing global HIV program platforms despite affordable *** of mother-to-child transmission of HBV through scaling up of birth dose implementation in LMICs is essential to achieve the 2030 elimination *** individuals living with HBV and/or HCV in resource-limited settings are unaware of their infection,and with improved access to medications,the most significant barrier remains access to affordable diagnostics and preventive *** coronavirus disease 2019 pandemic interrupted hepatitis elimination programs,albeit offered opportunities for improved diagnostic capacities and raised political awareness of the critical need for strengthening health care services and universal health *** review underpins the HBV and HCV management challenges in resource-limited settings,highlighting the current status and suggested future elimination strategies in some of these *** efforts should continue to improve awareness and political *** resources should be secured to access and implement comprehensive strategies for diagnosis and linkage to care in resource-constrained settings to fulfill the 2030 elimination goal.

BACKGROUND Right ventricular(RV)function is frequently overlooked during dilated cardiomyopathy(DCM)*** To evaluate RV function in children with idiopathic DCM using relatively recent echocardiographic *** We prospectively studied the cardiac function in 50 children with idiopathic DCM and 50 healthy children as a control group,using four-dimensional echocardiography(4-DE),Tissue Doppler Imaging(TDI),and two-dimensional-speckles tracking echocardiography(2-D-STE).RV EF was measured by *** The auto left(LV)ejection fractions(EF)measured by 2-D-STE were significantly lower in the patients'group than in the *** sphericity index was also significantly lower in children with DCM than in the *** EF measured by 4-DE was significantly lower in the patient's group than the *** S wave,e´/a'ratio,myocardial performance index(MPI),and tricuspid annular plane systolic excursion(TAPSE)were significantly impaired in children with DCM than in *** LV and RV global longitudinal strains(GLS)were significantly reduced in children with DCM than in *** was significantly associated with the duration since diagnosis,tricuspid annulus S wave,RV MPI,and TAPSE,but not with the age of the patients,RV EF,or e´/a'*** There was impairment of the RV LGS and other systolic and diastolic parameters in children with *** and TDI can help to detect the early decline of RV function.

Effect of reperfusion strategy on QT dispersion in patients with acute myocardial infarction:Impact on in-hospital arrhythmia

BACKGROUND Myocardial ischemia and ST-elevation myocardial infarction(STEMI)increase QT dispersion(QTD)and corrected QT dispersion(QTcD),and are also associated with ventricular *** To evaluate the effects of reperfusion strategy[primary percutaneous coronary intervention(PPCI)or fibrinolytic therapy]on QTD and QTcD in STEMI patients and assess the impact of the chosen strategy on the occurrence of in-hospital *** This prospective,observational,multicenter study included 240 patients admitted with STEMI who were treated with either PPCI(group I)or fibrinolytic therapy(group II).QTD and QTcD were measured on admission and 24 hr after reperfusion,and patients were observed to detect in-hospital *** There were significant reductions in QTD and QTcD from admission to 24 hr in both group I and group II *** and QTcD were found to be shorter in group I patients at 24 hr than those in group II(53±19 msec vs 60±18 msec,P=0.005 and 60±21 msec vs 69+22 msec,P=0.003,respectively).The occurrence of in-hospital arrhythmia was significantly more frequent in group II than in group I(25 patients,20.8%vs 8 patients,6.7%,P=0.001).Furthermore,QTD and QTcD were higher in patients with in-hospital arrhythmia than those without(P=0.001 and P=0.02,respectively).CONCLUSION In STEMI patients,PPCI and fibrinolytic therapy effectively reduced QTD and QTcD,with a higher observed reduction using *** was associated with a lower incidence of in-hospital arrhythmia than fibrinolytic *** addition,QTD and QTcD were shorter in patients not experiencing in-hospital arrhythmia than those with arrhythmia.
