Total Hip Replacement in Young Adults Less Than Fifty Year Old: Our Experience

Background: We report a retrospective study of 47 total prosthesis of hip put in place among the young adult of less than fifty years in the service of Traumatology and Orthopedics at the Wing IV of the CHU Ibn Rochd of Casablanca, over a period from January 2008 to March 2014, with an average rate of decline of 42 months. Results: The deliberate group of 43 patients (47 hips including 4 bilateral) consisted of 26 women, either 60, 47% and 17 men, either 39, 53%. The average age of our patients was 36 years. In our series, the predominant etiology has been conducted for the indication of the hip replacement was the aseptic osteonecrosis of the femoral head which has represented 31.91%. With regards to the choice of implants, 85% of PTH were non-cemented, the cemented PTH has been used in 15% of cases. The Friction Torque metal/polyethylene was used in all cases. The functional outcome was appreciated by the rating of Merle of Aubigne before and after arthroplasty. The average rate of decline of 42 months, and the results were generally satisfactory. They were excellent in 27.78% of cases, very good in 19.44% of cases and good in 44.44% of cases. The results on the Pain were remarkable: the rating of the pain going 2.8 in pre-operative at 5.6 at the latest decline. The results on the walking and mobility were also good. It was noted as a complication: 4 cases of prosthetic dislocation, 1 case of aseptic descellement bipolar, 2 cases of early sepsis on hardware, 3 cases of paralysis of the sciatic external popliteal and 4 cases of periprothetiques ossification. In total, 5 total prosthesis of hip were occasions. The young age does not seem to be an obstacle to the prosthetic surgery. Conclusion: However, a major question remains concerning the distant future of these prostheses in young active subjects, because they will be submitted for many years in a job which can cause the descellement of prosthetic parts.

Post-partum haemorrhage (PPH) is one of the leading causes of maternal death in sub-Saharan Africa. In developing countries, PPH is responsible for about 30% of maternal deaths. The main causes of PPH are uterine atony, placental implantation anomalies and coagulation disorders. Acting on the causes of post-partum haemorrhage would significantly reduce maternal mortality. To prevent PPH, the World Health Organization (WHO) recommends the use of uterotonics as a preventive measure. although parenteral Oxytocin is recommended as the first line Oxytocic for the prevention of PPH, the use of misoprostol is increasingly used in gynaecology and obstetrics, not only for the prevention of postpartum haemorrhage, but also for many other obstetric indications. The aim of this study was to assess the knowledge and level of use of misoprostol by healthcare providers in the gynaecology and maternity departments of South Kivu in the practice of gynaecology and obstetrics. Materials and methods We conducted a descriptive study from January 03 up to February 04, 2023. The study of population was made up of healthcare workers in South Kivu. A questionnaire containing questions relating to socio-demographic informations and knowledge of misoprostol was prepared and encoded in the kobo collect software. To access the questionnaire, it was compulsory to read the research protocol and give consent by ticking the “yes” button. all those who ticked “no” were denied access to the questionnaire. The link was sent, with a request to take part in the survey, to groups in the social networks of doctors and midwives in South Kivu. For areas not covered by the internet, a printed format was distributed and then encoded by data entry operators. For the paper format, respondents were also asked to indicate their consent by ticking the “yes” box. all the encoded data was automatically compiled on the server and then analysed and interpreted by the research team. Subsequent to demyelination in multiple sclerosis, myelin repair occurs but, as lesions age, the ability to remyelinate diminishes. Molecular pathways underl ying oligodendrocyte behaviour during CNS remyelination remain to be elucidated. In this study, we report for the first time constitutive expression of the CXC/ α chemokine receptors, CXCR1, CXCR2 and CXCR3, on oligodendrocytes in normal a dult human CNS tissue, the levels of which were upregulated in multiple sclerosi s and other neurological diseases (OND). In addition, both immature (A2B5+ /O4 + ) and more mature (CNPase+ ) human oligodendrocytes in vitro expressed the s ame three receptors. The respective ligands to CXCR1, CXCR2 and CXCM3 [i.e. CXCL 8/IL- 8, CXCL1/GRO- α and CXCL10/IP- 10), were absent in CNS tissue from no rmals and subjects with OND, but were present at high levels on hypertrophic (re active) astrocytes at the edge of active (but not silent) multiple sclerosis les ions. Astrocytes in vitro could be induced to express chemokines following stimu lation with pro- inflammatory cytokines. CXCL8 and CXCL1 production by human as trocytes at both the RNA and protein levels could be induced by interleukin (IL) - 1β , while CXCL10 was induced by both IL- 1β and interferon- γ . Since these cytokines are integral to inflammatory events occurring at the margins of active multiple sclerosis lesions, their upregulation in these regions may under lie the dynamics of chemokine expression observed herein. The simultaneous expression of differen t CXC chemokine receptors on oligodendrocytes, and their ligands on astrocytes a round multiple sclerosis lesions, may bespeak novel functional roles for these i mmune system molecules in the recruitment of oligodendrocytes and remyelination.

Background: In developing countries, postpartum hemorrhage is responsible for 30% of maternal deaths. although the World Health Organization recommends the use of oxytocin for the prevention of postpartum hemorrhage, the use of misoprostol is increasingly common. The objective of this study was to determine the frequency of postpartum hemorrhage in parturients delivering at Saint-Vincent Hospital and to compare the effectiveness of misoprostol use versus oxytocin in preventing postpartum hemorrhage. Material and Methods: We conducted a comparative longitudinal study at the Saint Vincent Hospital comparing 10 units of intramuscular oxytocin with 600 micrograms of sublingual misoprostol. The study was conducted from 01 January 2017 to 31 December 2019, a period of 3 years. The study population consisted of 2161 consenting women. Of these, 1289 received 10 IU of intramuscular oxytocin and 872 received 600 micrograms of misoprostol. The collected data were entered using Microsoft Excel 2013 and analysed using SPSS version 21 software. Results: The frequency of administration of Misoprostol and oxytocin in parturients was 40.4% and 59.6% respectively in this study. One hundred and fourteen cases of postpartum hemorrhage (114/2161 or 5.3%) were noted among the parturients. The average age of parturients who received oxytocin was 24.36 ± 4.45 years vs 24.63 ± 5.11 years among parturients who received Misoprostol;(p = 0.190). The mean parity was 2.52 ± 1.46 vs 2.66 ± 1.44;(p = 0.020). We noted a high proportion (78.3%) of postpartum hemorrhage from the oxytocin group vs. 21.7% from the Misoprostol group (OR 2.5-fold), with a statistically significant difference (p 0.001). We noted high proportions of uterine atony (92.3%) from the oxytocin group vs 7.7% from the Misoprostol group (p = 0.004). Uterine atony was the actual factor associated with postpartum hemorrhage (OR = 10.0895% CI: 1.78 - 57.10;p = 0.009). Conclusion: Misoprostol 600 Microgram administered sublingually im
