Intracranial arachnoid cysts (AC) are believed to be congenital and chronic subdural hematomas tend to occur in elderly patients with a history of mild head injury. The association between these two entities sporadically occur in relatively young patients but rare in elderly patients. We report a 65-year-old man who presented with headache and dizziness of 2 months’ duration with a history of head injury. brain computed tomography (CT) a CSDH in right side and a hygroma in left side. After first operation with burr holes in both sides, the patient underwent an early recurrence of acute subdural hematoma in the right side. The evacuation of this hematoma by a craniotomy allowed seeing an AC that we resected partially with complete recovery of the patient. The association CSDH/AC is rare and possible in elderly patients and there is no consensus on treatment.

Purpose:To compare cartilage tympanoplasty(CT)combined with eustachian tube balloon dilatation(ETbD)and cartilage tympanoplasty alone as a surgical treatment modality for adhesive otitis media(AdOM)in terms of graft healing,audiological outcomes,and impact on life style,using Chronic Otitis Media Outcome Test 15(COMOT-15).Methods:50 patients with AdOM were randomly classified into 2 groups:25 patients for cartilage tympanoplasty only(CT group)and 25 patients for cartilage tympanoplasty combined with eustachian tube balloon dilatation(CT t ETbD group).Clinical outcomes in both groups were compared at 3 and 6 months of follow ***:There was no significant difference in graft healing between the two *** COMOT-15 scores significantly decreased in both groups with a significant difference between the groups with regard to the decrease in COMOT-15 scores at 3 and 6 months of follow-up(PbG was 26.5±5.4 and 27.1±4.6 db,and the mean postoperative AbG at 6 months was 19.4±4.4 and 14.6±3.9 db in the CT and the CT t ETbD groups,*** difference in the magnitude of AbG reduction in the two groups was significant at 3 and 6 months of postoperative follow-up(PbD ***:ETbD can increase the success rate of cartilage tympanoplasty in patients with AdOM by enhancing the audiological outcome and quality of life.

The context of recognizing handwritten city names,this research addresses the challenges posed by the manual inscription of bangladeshi city names in the bangla *** today’s technology-driven era,where precise tools for reading handwritten text are essential,this study focuses on leveraging deep learning to understand the intricacies of bangla *** existing dearth of dedicated datasets has impeded the progress of bangla handwritten city name recognition systems,particularly in critical areas such as postal automation and document ***,no prior research has specifically targeted the unique needs of bangla handwritten city name *** bridge this gap,the study collects real-world images from diverse sources to construct a comprehensive dataset for bangla Hand Written City name *** emphasis on practical data for system training enhances *** research further conducts a comparative analysis,pitting state-of-the-art(SOTA)deep learning models,including EfficientNetb0,VGG16,ResNet50,DenseNet201,InceptionV3,and Xception,against a custom Convolutional Neural Networks(CNN)model named“Our CNN.”The results showcase the superior performance of“Our CNN,”with a test accuracy of 99.97% and an outstanding F1 score of 99.95%.These metrics underscore its potential for automating city name recognition,particularly in postal *** study concludes by highlighting the significance of meticulous dataset curation and the promising outlook for custom CNN *** encourages future research avenues,including dataset expansion,algorithm refinement,exploration of recurrent neural networks and attention mechanisms,real-world deployment of models,and extension to other regional languages and *** recommendations offer exciting possibilities for advancing the field of handwritten recognition technology and hold practical implications for enhancing global postal services.

Obstacle removal in crowd evacuation is critical to safety and the evacuation system efficiency. Recently, manyresearchers proposed game theoreticmodels to avoid and remove obstacles for crowd evacuation. Game theoreticalmodels aim to study and analyze the strategic behaviors of individuals within a crowd and their interactionsduring the evacuation. Game theoretical models have some limitations in the context of crowd evacuation. Thesemodels consider a group of individuals as homogeneous objects with the same goals, involve complex mathematicalformulation, and cannot model real-world scenarios such as panic, environmental information, crowds that movedynamically, etc. The proposed work presents a game theoretic model integrating an agent-based model to removethe obstacles from exits. The proposed model considered the parameters named: (1) obstacle size, length, andwidth, (2) removal time, (3) evacuation time, (4) crowd density, (5) obstacle identification, and (6) route *** proposed work conducts various experiments considering different conditions, such as obstacle types, obstacleremoval, and several obstacles. Evaluation results show the proposed model’s effectiveness compared with existingliterature in reducing the overall evacuation time, cell selection, and obstacle removal. The study is potentially usefulfor public safety situations such as emergency evacuations during disasters and calamities.

The determination of the ethanol content in food products is of fundamental importance for HALAL certification. In this work, an analytical method for the determination of ethanol in water by headspace gas chromatography with flame ionization detector (HS-GC-FID) has been developed and validated for the use in characterization of ethanol reference materials. The validation study was carried out in the linear calibration range 100 - 1500 mg/kg using the NIST SRM 2900, nominal 95.6%. The studied performance characteristics of the method were the limit of detection, LOD, the limit of quantification LOQ, selectivity, linearity, precision, recovery and bias. The validation results showed that the method is selective, precise, accurate and free from any significant bias. The LOD and LOQ were 1.27 and 3.86 mg/kg respectively and the estimated expanded uncertainty was 2% indicating that the method is fit for the purpose of certification of ethanol in water reference materials.

Effect of Trimetazidine on Functional Capacity in Patient with Ischaemic Cardiomyopathy (TOFCAPI)

Objective: This study aimed to evaluate the efficacy of trimetazidine on exercise capacity via a six-minute walk test in patients with ischaemic cardiomyopathy and also evaluate the effect of trimetazidine on left ventricular function via echocardiography in the same population. Methods: This prospective observational study, conducted at the National Institute of Cardiovascular Diseases in Dhaka, bangladesh, enrolled 200 patients with ischaemic cardiomyopathy and a depressed left ventricular ejection fraction (LVEF Results: In this study (n = 200) of ischaemic cardiomyopathy patients, the mean age was 58 years, with 76% of the patients being male. All study subjects received GDMT (Guideline-Directed Medical Therapy) for angina and heart failure. Those who received the modified released form of trimetazidine developed lesions during the 1st and 2nd follow-ups, during which the LVEF, LVIDd, and six-minute walk distance significantly improved (p Conclusion: The findings of the present study demonstrated that the addition of modified-release trimetazidine to GDMT can improve exercise capacity and left ventricular function in patients with ischaemic cardiomyopathy.
