Purpose:Overcrowding in emergency department(ED)is a concerning global problem and has been identified as a national crisis in some *** emergency sorting systems designed successfully in the *** in 2004,a group of branches in South African triage scale(SATS)*** effectiveness of the case sorting system of SATS was evaluated to reduce the patient's length of stay(LOS)and mortality rate within the ED at Suez Canal University ***:The study was designed as an intervention study that included a systematic random sample of patients who presented to the ED in Suez Canal University *** study was implemented in three phases:pre-intervention phase,115 patients were assessed by the traditional protocols;intervention phase,a structured training program was provided to the ED staff,including a workshop and lectures;and post-intervention phase,230 patients were assessed by *** the patients were retriaged 2 h later,calculating the LOS per patient and the *** was collected and entered using Microsoft Excel *** data from the triage sheet were analyzed using the SPSS software program version ***:The LOS in the ED was about 183.78 min before the intervention;while after the training program and the application of SATS,it was reduced to 51.39 *** 15.7%of the patients died before the intervention;however,after the intervention the ratio decreased to 10.7%***:SATS is better at assessing patients without missing important ***,it resulted in a decrease in the LOS and reduction in the mortality rate compared to the traditional protocol.

OBJECTIVE:"Polytrauma" patients are of a higher risk of complications and death than the summation of expected mortality and morbidity of their individual injuries. The ideal goal in trauma resuscitation care is to identify and treat all injuries. With clinical and technological advanced imaging available for diagnosis and treatment of traumatic patients, point of care-rapid ultrasound in shock and hypotension (RUSH) significantly affects modern trauma services and patient outcomes. This study aims to evaluate the accuracy of RUSH and patient outcomes by early detection of the causes of unstable polytrauma.;METHODS:This cross-sectional, prospective study included 100 unstable polytrauma patients admitted in Suez Canal University Hospital. Clinical exam, RUSH and pan-computed tomography (pan-CT) were conducted. The result of CT was taken as the standard. Patients were managed according to the advanced trauma life support (ATLS) guidelines and treated of life threatening conditions if present. Patients were followed up for 28 days for a short outcome.;RESULTS:The most diagnostic causes of unstability in polytrauma patients by RUSH are hypovolemic shock (64%), followed by obstructive shock (14%), distributive shock (12%) and cardiogenic shock (10%) respectively. RUSH had 94.2% sensitivity in the diagnosis of unstable polytrauma patients; the accuracy of RUSH in shock patients was 95.2%.;CONCLUSIONS:RUSH is accurate in the diagnosis of unstable polytrauma patients; and 4% of patients were diagnosed during follow-up after admission by RUSH and pan-CT.

Purpose:Sepsis is a common acute life-threatening condition that emergency physicians routinely *** options within the Emergency Department(ED)are limited due to lack of infrastructure,consequently limiting the use of invasive hemodynamic monitoring or imaging *** mortality rate due to sepsis can be assessed via multiple scoring systems,for example,mortality in emergency department sepsis(MEDS)score and sepsis patient evaluation in the emergency department(SPEED)score,both of which quantify the variation of mortality rates according to clinical findings,laboratory data,or therapeutic *** study aims to improve the management processes of sepsis patients by comparing SPEED score and MEDS score for predicting the 28-day mortality in cases of emergency ***:The study is a cross-sectional,prospective study including 61 sepsis patients in ED in Suez Canal University Hospital,Egypt,from August 2017 to June *** were selected by two steps:(1)suspected septic patients presenting with at least one of the following abnormal clinical findings:(a)body temperature higher than 38℃ or lower than 36℃,(b)heart rate higher than 90 beats/min,(c)hyperventilation evidenced by respiratory rate higher than 20 breaths/min or PaCO2 lower than 32 mmHg,and(d)white blood cell count higher than 12,000/μL or lower than 4000/μL;(2)confirmed septic patients with at least a 2-point increase from the baseline total sequential organ failure assessment(SOFA)score following *** inclusion criteria included adult patients with an age≥18 years regardless of gender and those who had either systemic inflammatory response syndrome or suspected/confirmed *** were shortly follow-up for the 28-day *** patient was subject to SPEED score and MEDS score and then the results were compared to detect which of them was more effective in predicting *** receiver operating characteristic curves were also done for MEDS and SPE

OBJECTIVE:Maxillofacial injuries are commonly encountered in the practice of emergency medicine. More than 50% of patients with these injuries have multisystem trauma that requires coordinated management between emergency physicians and surgical specialists in oral and maxillofacial surgery, otolaryngology, plastic surgery, ophthalmology, and trauma surgery. The aim of this study is to identify the patterns of brain injuries associated with maxillofacial trauma and its outcome.;METHODS:This descriptive study (cross-sectional) was carried out among 90 polytrauma patients with maxillofacial fractures attending the Emergency Department at Suez Canal university Hospital and fulfilling our inclusion and exclusion criteria.;RESULTS:This study demonstrates the relation between type of maxillofacial fracture and type of traumatic brain injuries in which the majority of patients with epidural hemorrhage presented with mid face fractures (60%), while the minority of them presented with upper and lower face fractures (20% for each of them). The majority of patients with subdural hemorrhage were associated with mid face fractures (75%), the majority of patients with brain contusions associated with mid face fractures (75%), and all of the patients presented by pneumocephalus were associated with mid face fractures (100%).;CONCLUSIONS:The results of this study confirm the value of quick diagnosis and early intervention, which is fundamental to prevent morbidity as well as mortality especially with regards to prevention of traumatic brain injury as even a short duration of hypoxia and edema will lead to significant permanent neurological deficits.

Background: The treatment of brain metastases with radiotherapy has shifted to the use of Stereotactic Radio-surgery (SRS). The technical issue of expanding the treatment volume around the Gross Tumor Volume (GTV) is a current debate. Radiotherapy centers use variable GTV-PTV margins, ranging from one to 2 mm. Material and Methods: We performed a dosimetric comparison in plans of twenty patients using three margins: PTV zero, PTV1, and PTV2. We also developed imaginary Peel volumes. These volumes are described as follows: Peel1 = PTV1 − GTV, Peel2 = PTV2 − GTV. Results: Our results showed that the mean PTV volume differed significantly across the different margins (p = 0.000). The V12 of the brain significantly varied as a function of PTV margin (p = 0.000). The target coverage and plan quality indices were not significantly different. The Peel volume dosimetric analysis showed that the mean dose was significantly higher in the nearby normal brain tissue: Peel1 (p = 0.022) and Peel 2 (p = 0.013). Conclusion: According to our dosimetric analysis, expanding the GTV into a PTV by 1 mm margin is more convenient than 2 mm.

The deodorizer distillate(DD)is a byproduct of vegetable oil processing industry and is rich in functional bioactive *** study aimed to employ phosphorylation modification for DD to produce a new sustainable *** bioactive ingredients in DD,namely fatty acids,sterols,and tocopherols,were determined by using HPLC and *** results revealed that the DD sample contained a high percentage of unsaturated fatty acids(72.3%)and high levels ofγandδ-tocopherols(54.8%and 31.60%,respectively).Mechanical parameters(tensile strength,elongation at break,and tear strength)of leather were improved after being treated with the prepared fatliquor ***,SEM showed that the texture of the fatliquored leather had been remarkably ***,the fatliquored leather possessed effective antibacterial effect against the specified+ve,-ve bacteria,and Candida albicans *** strength,fullness,soft handle,and elasticity of leather were all improved,and the grain of leather was protected from becoming loose after drying.

Iran is located on a silver, lead, and zinc belt and according to the latest studies holds 11 million tons of lead, zinc, and silver stones which constitute 4 percent of global resources. Considering that mineral materials are explored in an uncertain space, exploration investment risk is an inseparable part of these activities. The important fact is to minimize the effect of this undesired factor in exploration. To achieve this, it is required that exploration activities and withdrawals are performed in a certain framework in which risk minimization is considered. Using mineral potential modelling for determining promising zones which should be taken into consideration in more detailed stages could make achieving the purpose possibly. This work is aimed at applying fuzzy neural network and TOPSIS methods simultaneously in order to explore zinc and lead resources. In this article, geological, telemetry, geophysics, and geochemistry data is integrated using fuzzy-neural network (neuro fuzzy) and using TOPSIS method rating for lead and zinc ore deposit potential mapping in Isfahan-Khomein strip which has been introduced as one of zinc and leads mineral scopes in Iran. This area which is composed of several zinc and lead ore deposits has been considered as the target area. Fuzzy integration results of zinc and lead mineralization witness layers confirm the relatively high potential of lead and zinc mineralization in this region having a northwest-southeast trend and involving more than 90 percent of the known indices and ore deposits of the region. In this research, it was shown that the results of TOPSIS-Neuro-Fuzzy integrated model (a combination of neural network and fuzzy logic) have increased the resolution of talented areas from the areas with no mineralization potential in comparison with the fuzzy method individually.

Assessment of the Influence of Sulfuric Acid/Hydrogen Peroxide Mixture on Organic Sulfur Reduction of High Sulfur Coals and Their Chemical Composition

One of the contaminants in coal is sulphur. The adverse impact of sulfur on coal, such as environmental pollution, degradation of steel quality, and reduction of coal’s thermal value, has led to the attention of sulfur separation methods in recent decades. Leaching (chemical dissolution) is one of the best methods for desulfurising coal, reducing sulfur in coal. In this study, hydrogen peroxide as an oxidising agent on sulfuric acid yield in reducing sulfur types of coal and chemical structure and the organic texture of high sulfur coal was investigated. The experiments were designed using a three-level response surface methodology with four duplicate points and 27 experiments. The independent variables studied were temperature, time, stirring speed and ratio of sulfuric acid to hydrogen peroxide. Dependent variables included reduction percentage of total, pyritic and organic sulfur. This study showed that 99.99% of total sulfur, 30.11% of pyritic sulfur and 69.08% of organic sulfur were reduced. These values were obtained at a temperature of 60°C, time 120 min, stirring speed 200 rpm and 3:1 ratio of sulfuric acid to hydrogen peroxide. Significant changes were observed by infrared spectroscopy (FTIR) of the coal structure before and after desulphurisation. On the other hand, the studies showed no specific changes in the bonds related to the organic coal matrix. The results showed that this method could be used as a secure process for removing inorganic and organic sulfur without destroying the organic coal matrix.
