Percutaneous antegrade management of large proximal ureteral stones using non-papillary puncture

Objective:To evaluate the feasibility and the safety of medial non-papillary percutaneous nephrolithotomy(npPCNL)for the management of large proximal ureteral ***:We evaluated prospectively collected data of 37 patients with large proximal ureteral stones more than 1.5 cm in diameter treated by prone *** on stone size,in-toto stone removal or lithotripsy using the Lithoclast®Trilogy(EMS Medical,Nyon,Switzerland)was *** parameters including operative time(from start of puncture to the skin suturing),stone extraction time(from the first insertion of the nephroscope to the extraction of all stone fragments),and the stone-free rate were ***:Twenty-one males and 16 females underwent npPCNL for the management of large upper ureteral *** median age and stone size of treated patients were 58(interquartile range[IQR]:51-69)years and 19.3(IQR:18.0-22.0)mm,*** median operative time and stone extraction time were 25(IQR:21-29)min and 8(IQR:7-10)min,*** case(2.7%)of postoperative bleeding and two cases(5.4%)of prolonged fever were managed *** stone-free rate at a 1-month follow-up was 94.6%.Conclusion:The npPCNL provides a straight route to the ureteropelvic junction and proximal *** from a dilated portion of the ureter under low irrigation pressure with larger diameter instruments results in effective and safe stone extraction within a few minutes.

Introduction: Obstetric fistula (OF) is an abnormal communication between the genital and urinary tracts and/or associated with a recto-vaginal communication resulting from prolonged dystocic delivery. It is a frequent pathology in underdeveloped countries. In Zinder no study has been carried out on obstetric fistula. The aim of this study was to evaluate the epidemiological, social impact and therapeutic aspects of obstetric fistula at the CSME of Zinder. Patient and Method: This was a descriptive cross-sectional study of obstetric fistula care at CSME during the period of (January 2018 to June 2020). Results: A total of 196 cases were collected over 30 months. This represents an annual incidence of 78.4 fistulas. The median age of the patients was 18.63 years, and more than 65% were over 20 years old. One hundred and ninety-one patients (97.45%) did not attend school, and 56.12% (n = 110) lived in polygamous households. One hundred and fifty-one women (n = 151), 77.04% had full assistance from their husband during the first four (4) months of the pathology. Twenty-four (12.24%) had been notified of repudiation. Obstetrical risk of dystocia was found in 39.79% (n = 78) of the patients, of whom 24.49% (n = 48) had a focused antenatal consultation (CPNR). The labour lasted more than 24 hours in 100%. The majority of women are multiparous and 60% had at least 2 pregnancies. vesicovaginal fistula is the main pathological type with 86.23%. Fistulas were closed in 83.16% and 68.87% exited without urine loss. Conclusion: Obstetric fistula is a devastating disease affecting girls and young women in Niger, as in other countries in Africa and Asia. It is a major concern worldwide and is a social tragedy because it is disabling and depressing.

Objective: Improve the care of patients followed for acute leukemia in the Oncohematology department of the National Hospital of Niamey. Methods: This was a prospective study, over a period of 2 years from January 1, 2018 to December 31, 2019, in patients with acute leukemia in the Oncohematology department of the National Hospital of Niamey (HNN), whose diagnosis was made on a blood smear associated with a myelogram and immunophenotyping and who were consenting. Results: We collected 25 cases of acute leukemia confirmed by myelogram and immunophenotyping. The mean age of the patients was 31.32 years, with a predominance of women, a sex ratio of 0.92. Pupils and students were in the majority with 40% and most came from the Niamey region, i.e. 68%. Anemic syndrome was the most common clinical sign in 96%. ALL predominated in 64% of cases. On the blood count, the hyperleukocytosis was more marked in AML (mean white count: 197256.6 elts/mm3) than in ALL (137891.6 elts/mm3), it was the same for thrombocytopenia which is more marked in AML (75588.89/mm3) than in ALL (52156.25/mm3). Therapeutically, 52% of patients received chemotherapy. The mean overall survival was 16.223 ± 3.191 months, including a mean survival for AML of 6.853 ± 1200 months compared to 21.720 ± 5.920 months for ALL. Conclusion: Acute leukemia still remains a major problem in our context, due to the precariousness of limited financial, diagnostic and therapeutic resources. Thus reflecting in our results, the increasing number of cases, the diagnostic delay and the guarded prognosis. This is the reality in several other countries in the sub-region and even in certain developed countries.

Introduction: Hepatitis B virus (HBV) infection is a major public health problem in Cameroon. Garoua city is the headquarters of the North Region of Cameroon, where the HBV prevalence is among the highest of the country. The aim of this study was to determine the prevalence of HBsAg carriage and associated factors among persons incarcerated in the Garoua Central Prison. Methods: It was a cross-sectional study conducted from July 1 to July 31, 2023 at the Garoua Central prison. We included all prisoners willing to participate in the study and who gave their verbal consent. We collected data using a pre-established data entry form and we used rapid test for blood screening for HBV surface antigen (HBs Ag) with ELISA confirmation. Data were analyzed using the R® software for Windows. After the univariate analysis, we selected associated variables to HBV infection with p-value p-value was set at 5%. Results: We included 1389 prisoners out of which 97.6% were male. The median age (IQR) of the study population was 28 (23 - 35) years. The median (IQR) duration of incarceration was 12 (6 - 26) months and the mean (±sd) number of incarcerations was 1.24 (±0.6). HBV prevalence was estimated at 14.8% (95% CI: 13.0 - 16.7). Upon uni- and multivariate analysis, no risk factor was significantly associated with viral hepatitis B infection in our study population. Conclusion: The prevalence of Hepatitis B was high in the Garoua Central Prison, but there were no additional risk factors for HBV infection. There is a need to include the Garoua Central Prison and by the way other prisons in the country in the chronic viral hepatitis care program.

污水污泥是污水处理过程中无法避免的副产品。污泥中含有大量的磷,分布在污泥的不同相中。目前,已有一些方法被广泛应用于污泥磷组分的判别。随着全球磷储量的迅速枯竭,从污泥中回收磷越来越受到人们的关注。本研究旨在确定富铁污水污泥中的主要磷分型,研究超声波辅助碱性溶液提取污泥中磷的效果,并探讨超声波对Hedley分型中磷组分的影响。结果表明,在两种污泥样品中WAS和ADS中磷的分型均以Na OH-P为主,其含量占总磷的～55%。铁磷化合物(Fe Ps)是污泥中铁-磷结合态的主要形式,分别占WAS和ADS中总磷的62.2和67.5%。与Fe Ps结合的铁是污泥中铁的主要组分。XRD分析与顺序化学提取结果均表明,污泥样品中Fe Ps主要存在于羟基磷铁中。Visual Minteq模拟结果表明,p H值影响磷的形态,磷的沉淀受金属离子含量的影响。超声波对碱性溶液从污泥中提取磷具有促进作用。以0.1 M Na OH为提取剂,超声处理30 min,WAS和ADS中磷的提取率分别为75.2和61.4%。当仅用Na OH时,WAS和ADS中磷的提取率分别为～10%和～20%。动力学研究表明,污泥中磷的提取过程均受拟化学反应控制。NaS可以有效的从污泥中提取磷,WAS和ADS中提取的磷量分别为89.0和80.9 mg/L。单独超声波处理使HO-和Na HCO-P组分略有增加,Na OH-和HCl-P组分略有减少。本研究有助于加深对富铁污泥中磷的形态的理解,推进磷的化学回收技术的发展。

Cooperative file sharing mechanism with Network Coding in wireless Mesh networks

This article describes a new model of a cooperative file sharing system in a wireless Mesh network. The authors＇ approach is to develop an efficient and cooperative file sharing mechanism based on opportunistic random linear Network Coding. Within this mechanism, every node transmits random linear combination of its packets according to cooperative priority, which is computed in a distributed manner according to the node-possible contribution to its neighbor nodes. With this mechanism, the more a node contributes to others, the more the node has chances to recover the entire file first. The performance metrics of interest here are： the delay until all the packets in a file have been delivered to all nodes, and an ideal packet size, by the use of which the authors can get the minimum transmission delay. Through extensive simulation the authors compare their mechanism with the current transmission process in a wireless Mesh network without random linear Network Coding. The authors found that using their mechanism, the nodes can cooperatively share the entire file with less transmission time and delay than the current transmission process without random linear network.

The twin reversed arterial perfusion (TRAP) sequence is an anomaly unique to monochorionic multiple pregnancies. It is a rare complication. We present a case of acardius anephus, which was mistaken for a live anomalous singleton fetus. A 21-year-old unbooked Primigravida was seen in labour ward with a ten hours history of labour pains. Urgent transabdominal ultrasound revealed a singleton live fetus at 30 weeks gestation with gross cystic mass lesion at the level of the abdomen and polyhydramnious.?Caesarean section confirms a twin gestation with monoamniotic monochorionic single placenta. The umbilical cord of the acardiac twin was short—First twin was a live female baby, weighing 1000 g. TRAP is a rare event an early referral to feto maternal unit will improve perinatal outcome.
