Background: Ethmoidal polyposis coexisting with adenoid enlargement and tympanic membrane perforation is a rare condition in otolaryngological practice. Evidences have shown that antrochoanal polyp often times coexists with adenoid enlargement especially in the young which was in sharp contrast to ethmoidal polyposis occurring in the presence of adenoid enlargement. Moreover, the hidden location of nasopharynx housing the adenoids, coupled with the masking effect of bilateral sinonasal polyps, leads to inadvertent misdiagnosis of this complex pathology with subsequent difficulty in providing effective management. Case Presentation: A 24 year-old lady presented with recurrent bilateral nasal obstruction that became persistent associated with nasal discharge and anosmia. She also had recurrent right otorrhoea with associated hearing loss. Diagnostic rigid nasal endoscopy revealed mucoid discharge with bilateral polypoid masses filling both nasal cavities. Otoendoscopic finding revealed a small (about 5%) central tympanic membrane perforation. A clinical assessment of chronic rhinosinusitis with nasal polyposis complicated by chronic suppurative otitis media (CSOM) was made. A non-contrast CT scan of the paranasal sinuses showed isodense lesions in the nasal cavities, all paranasal sinuses and the entire nasopharynx. She had endoscopic sinus surgery and a nasopharyngeal clearance biopsy. The nasal, paranasal and nasopharyngeal masses had histologic confirmation of inflammatory nasal polyps and lymphoid (adenoid) hyperplasia respectively. Her condition improved remarkably with subsequent medical treatment. She was followed up for 8 months and no recurrence was observed. Conclusion: Sinonasal polyposis can coexist with adenoid hypertrophy and middle ear disease as a single pathological condition. Hence, a high index of suspicion and thorough evaluation become necessary for making timely diagnoses and instituting effective management.

This paper developed an optimization technique for groundwater vulnerability in Kano Metropolis, North-Western Nigeria. A combination of DRASTIC is taken from initial letters of seven parameters namely depth to water table (D), net recharge (R), aquifer media (A), soil media (S), topography (T), impact of vadose zone (V) and hydraulic conductivity (C), while GOD also represents groundwater confinement (G), overlaying strata (O), depth of water (D) and multi-criteria evaluation (MCE) techniques were used in the optimization method by integrating other important and sensitive parameters for groundwater pollution, principally the anthropogenic point source pollution parameters (dump site, petroleum stations, automobile shops and under storage tanks). Geographic Information System was used to perform the sensitivity analysis (SA) using the single parameter and map removal sensitivity methods. Result of sensitivity optimization revealed the depth to groundwater (D), net recharge (N), impact of vadose zone (V) from DRASTIC model, and groundwater conferment (G) from GOD model having significant impact on the groundwater vulnerability, respectively. A combination of these four parameters was used to generate DNVG groundwater vulnerability for the area. This suggests that an integration of other point source pollution parameters can enhance the influence of DRASTIC and GOD model parameters on groundwater vulnerability condition. The paper recommends for the application of the optimization method used in this study in another area with similar geological and anthropogenic point source of pollution with a view to validating or improving on it. In this study, several input data, such as anthropogenic point sources of contamination, are added to the existing DRASTIC and GOD model parameters as part of a sensitivity analysis aiming to optimise the performance of the resultant models.

This research work developed and evaluated the mechanical properties of coconut fibre reinforced low density polyethylene (LPDE) composite material. The effect of fibre loading on the mechanical properties: tensile, flexural, and impact of the developed composite material have been investigated. Also carried out was the effect of fibre loading on the water absorptivity of the developed material. Sample categories of the developed composite were prepared by varying the fibre contents by weight at 0%, 10%, 20%, and 30%. The aim is to reduce the excessive waste disposal of LDPE materials that are largely found in the form of disposed water package materials (or pure water sachets) that usually affects the environment in the form of pollution. The water retting process was applied in extracting and cleaning fibre (or coir), while the mixed coir-LDPE (or developed composite material) was prepared by Compression Moulding Technique (CMT). The tensile and flexural properties were tested using Hounsfield Monsanto Tensometer (type w) while the impact properties were tested using the Charpy Impact testing machine. The microstructure of the composite was investigated using Scanning Electron Microscopy (SEM). The fractured surface morphology of the composite samples indicated a homogeneous mixture of the coir fibre and LDPE matrix. However, weak interfacial bonding between the coir fibre and LDPE matrix was also observed. The analysis of the water absorptivity showed that the developed composite materials have low water absorptivity at low fibre loading. However, at higher fibre loading, the water absorptivity increases significantly.

With population growth around the world,municipal waste disposal and continued energy demand becomes some of the major challenges to deal *** order to address these,an approach is required for an optimal waste management system that offers the population benefit with a lower environmental *** study evaluates the technical-economic and environmental impact analysis of a grid-connected waste to energy(WtE)plant to power a Univerisiti Teknologi Malaysia(UTM)*** energy recovery potential of the waste stream was assessed using the life cycle assessment(LCA)method with GaBi^(TM) software(version 4).A technical,economic and environmental analysis was then carried out for the grid-connected WtE system using HOMERPro software with gasification conversion *** cash flow analysis was based on levelized costs of energy(LCOE)and total net present value(NPV).The results gave an NPV for the system at USD 1.11×10^(7),with most of the effects resulting from the grid operating costs and the LCOE of USD 0.43/kWh compared to the grid unit price of USD 0.7/kWh which corresponds to a saving of$0.27/kWh in energy *** an environmental point of view,the results showed a significant reduction in carbon dioxide emissions from around 2,000 tons per year to around 400 tons per *** regard to the amount of waste sent to landfills,the results show a significant improvement from 142,605.5 kg/year to 0.13 kg/year.

Nonlinear Deterministic Chaos in Benue River Flow Daily Time Sequence

The Various physical mechanisms governing river flow dynamics act on a wide range of temporal and spatial scales. This spatio-temporal variability has been believed to be influenced by a large number of variables. In the light of this, an attempt was made in this paper to examine whether the daily flow sequence of the Benue River exhibits low-dimensional chaos;that is, if or not its dynamics could be explained by a small number of effective degrees of freedom. To this end, nonlinear analysis of the flow sequence was done by evaluating the correlation dimension based on phase space reconstruction and maximal Lyapunov estimation as well as nonlinear prediction. Results obtained in all instances considered indicate that there is no discernible evidence to suggest that the daily flow sequence of the Benue River exhibit nonlinear deterministic chaotic signatures. Thus, it may be conjectured that the daily flow time series span a wide dynamical range between deterministic chaos and periodic signal contaminated with additive noise;that is, by either measurement or dynamical noise. However, contradictory results abound on the existence of low-dimensional chaos in daily streamflows. Hence, it is paramount to note that if the existence of low-dimension deterministic component is reliably verified, it is necessary to investigate its origin, dependence on the space-time behavior of precipitation and therefore on climate and role of the inflow-runoff mechanism.

A common assertion is that alkaline solution aids oil mobilization by generating in situ soap,or by lowering interfacial tension(IFT)to ultra-low values in synergy with *** study takes a different approach that involves the alkaline dissolution of detrital quartz grains of sandstone reservoirs to create pathways for oil migration and *** dissolution via alkaline injection will result in changes in permeability and *** study performed high-pH core flooding on Berea sandstones using core displacement *** molybdate spectrophotometry was applied to measure the amount of dissolved *** and confining pressure variations were also *** molar concentration of NaOH varied at 0.5 M and 1.0 *** results show higher initial silica dissolution for 0.5 M NaOH(˃200mg/L)compared to 1.0 M NaOH(20 mg/L),which can be attributed to the presence of pre-existing dissolved silica and precipitates in the system prior to the first injection ***,a steady quartz dissolution rate of 0.4 mg/L/hr for 20 h was only achieved at 1.0 ***,an abrupt drop in quartz dissolution to below 0 mg/L was recorded for 0.5 M NaOH after 3 h of *** higher molar concentration of injected alkaline solution,both confining and inlet pressures increased from 8 and 5 bars to 12 and 11 bars as a result of the increased secondary phase of(hydr)oxides or precipitates in the ***,it can be inferred that the effect of alkaline solution on quartz dissolution is strongly dependent on molar concentration.

Introduction: Viral load suppression is a key determinant of successful anti-retroviral therapy. The study aimed to determine virologic response to Antiretroviral therapy in the large cohort of children and adults living with Human Immune deficiency Virus. Materials and Methods: Viral Load results from the HIV Ribonucleic Acid Polymerase Chain Reaction register of 10,887 children and adults on cART in 4 states in Northern Nigeria between 2017 and 2019 were retrieved and analyzed in the PCR Molecular Laboratory of the Federal Teaching Hospital, Gombe. Results: 10,887 children and adults were analyzed. Males were 28.4% (3094) and 71.6% (7793) females. 2.9% (311);3.5% (386);7.3% (797);65.2% (7098);14.5% (1583);5.2% (562) and 1.3% (150) were aged 0 - 9 years, 11 - 18 years;19 - 25 years, 26 - 45 years;46 - 55 years;56 - 65 years and 10 years. The most recent CD4count before viral load request was ≥1000/μL in 7.4% (810/10887);500 -999/μL in 39.0% (4240);350 - 499 μL in 22.7% (2466) and 1000 c/mL in 26.5% (821/3094) males and 24.1% (1876/7793) females. Viral load was significantly lower among females (p-value 0.007). 50.5% (157/311);52.1% (201/386);28.5% (227/797);23.5% (1670/7098);19.9% (315/1583);17.8% (100/562) and 18.0% (27/150) aged 0 - 9 years, 11 - 18 years;19 - 25 years, 26 - 45 years;46 - 55 years;56 - 65 years and 1000 c/mL respectively. Viral load was >1000 c/mL in 28.2% (229/811) for those on HAART for 6 months - 1 year and 23.6% (1243/5275) after receiving Highly Active Antiretroviral Therapy (HAART) for 1 - 5 years. 26.3% (1072/4075) and 21.1% (153/726) had viral load > 1000 c/mL after receiving HAART for 6 - 10 and >10 years respectively (p-value 0.001). Conclusion: HIV viral suppression was below the WHO recommended threshold.

Experimental Investigation on the Thermal Properties of Gypsum Plaster-Rice Husk Ash Composite

This experimental investigation aims at evaluating the thermal properties of rice husk ash (RHA)—filled gypsum plaster composite for potential applications, as insulating materials. The thermal conductivities of composites of gypsum plaster reinforced with RHA at 0%, 10%, 20%, 30%, and 40% volume fractions were determined experimentally using Lee’s disc method. The experimental results show that integrating RHA reduces the thermal conductivity of gypsum plaster and improves its insulation capacity. The results obtained from the experiments were compared with the Rule of Mixture Model, Maxwell Model, and Russell Model. It was observed that the thermal conductivities obtained from experiments and the theoretical models decreased with an increase in the volume fraction of RHA. The errors associated with the models with respect to experimental results are on the average of 28.7% for Mixture Rule, 31.6% for Russel Model, and 18.8% for the Maxwell Model. An agricultural waste like RHA can be beneficially used for the preparation of composites and, due to improved insulation capability, these composites can be used for applications such as insulation boards and sheathing, hardboard, ceilings of roofs, decorations, etc.

Background/Aim: Blood culture is critical in the diagnosis and treatment of blood stream infections (BSIs) especially in children. BSIs are among the most common cause of morbidity/mortality and blood culture has remained the gold standard for diagnosis. We sought to compare Blood Culture Isolates (BCI) from conventional and Bactec automated blood culture system (ABCS) among paediatric patients at the Federal Teaching Hospital Gombe (FTHG) Nigeria. Methods: BCI in children (0 - 18 years) by conventional method from 2008-2012 and Bactec Automated culture system from 2015-2020 were retrieved from the clinical microbiology laboratory register. Information analyzed included, age, sex, month, and year and blood culture isolates. Results: There were 5276 (56.9% males, 43.1% females) and 1169 (54% males, 46% females) Blood Culture Isolates by CM and ABCS respectively. Overall positive culture isolates were 9.7% (515/5276) in CM and 45.9% (536/1169) in ABCS (p = 0.01). Positivity rate in newborn was 13.3% (282/2114) by CM and 40.9% (219/263) by ABCS p = 0.01;under-5 was 10.5% (448/4253) vs. 37% (359/873) (p = 0.01);Gram positive 32.6% (172) vs. 65% (759) (p = 0.01;Gram negative 55% (2910) vs. 34% (397) (p = 0.01). Staph aureus 22% (114/515) by CM vs. 61.9% (332/536)) by ABCS (p = 0.01);Klebsiella 24.9% (128/515) by CM vs. 7.5% (40/536) p = 0.01) in ABCS, E. coli 8.9% (46/515) vs. 2.1% (11/536) p = 0.01;Proteus vs. 1.1% (6/515) by ABCS, Pseudomonas 3.3% (17/515) vs. 5.6% (30/536) p = 0.05, Alkaligenes 1% (5/515) vs. 8.2% (44/536) p = 0.01 and Citrobacter 1% (5/515) vs. 8.4% (45/536) p = 0.01. Conclusion: Blood culture yield was five times higher with Bactec compared with Conventional method.
