Periodontitis is the commonest oral disease affecting population worldwide. This disease is notorious for the devastation of tooth supporting structures, ensuing in the loss of dentition. The etiology for this disease is bacterial biofilm, which accumulates on the teeth as dental plaque. In addition to the biofilm microorganisms, other factors such as environmental, systemic and genetic are also responsible in progression of periodontitis. Diabetes mellitus (DM) is metabolic disorder which has an impact on the global health. DM plays a crucial role in the pathogenesis of periodontitis. Periodontitis is declared as the “sixth” major complication of DM. Evidence based literature has depicted an enhanced incidence and severity of periodontitis in subjects with DM. A “two way” relationship has been purported between periodontitis and DM. Mutual management of both conditions is necessary. Periodontal therapy (PT) may assist to diminish the progression of DM and improve glycemic control. Various advanced technological facilities may be utilized for the purpose of patient education and disease management. The present paper clarifies the etio-pathogenesis of periodontitis, establishing it as a complication of DM and elaborating the various mechanisms involved in the pathogenesis. The role of PT in amelioration of DM and application of digital communication will be discussed. Overall, it is judicious to create an increased patient cognizance of the periodontitis-DM relationship. Conjunctive efforts must be undertaken by the medical and oral health care professionals for the management of periodontitis affected DM patients.

A unique laboratory scale auto-thermal moving bed gasifier was designed for studying the thermochemical conversion of coal-biomass blends. For this purpose, two coals (lignite and sub-bituminous), two biomass materials (corn stover and switchgrass), and their respective blends were used. Gasification characteristics of the fuels were evaluated with an emphasis on improving the producer gas composition. The efficiency and product gas compositions reveal that utilizing the inner stainless-steel tubing better promotes heat transfer upwards in the axial direction when compared to utilizing quartz insulation. The H2/CO ratio at the same operating conditions is much higher due to the increase in bed temperature and heat transfer upwards in the axial direction. This improved the overall efficiency by at least 20%. Using pure oxygen and steam, efficiency greater than 50% was obtained for blends with corn stover at steam to oxygen ratio of 2:1. Also, using air as the gasifying agent greatly improved the H2/CO ratios and overall efficiency in blends with corn stover. In contrast, blends with switchgrass were not very effective with respect to the overall gasification characteristics. Blending switchgrass with coal may not be viable option from the viewpoint of generating high quality producer gas for downstream operations.

A hybrid wavelet and time plane based method for QT interval measurement in ECG signals

Here we present a method of QT interval meas-urement for Physionet's online QT Challenge ECG database using the combination of wavelet and time plane feature extraction mechanisms. For this we mainly combined two previous works one done using the Daubechies 6 wavelet and one time plane based with modifications in their algorithms and inclusion of two more wavelets (Daubechies 8 and Symlet 6). But found that out of these three wavelets Daube-chies 6 and 8 gives the best output and when averaged with the interval of time plane feature extraction method it gives least percentage of error with respect to the median reference QT interval as specified by Physionet. Our modified time plane feature extraction scheme along with the wavelet method together produces best re-sults for automated QT wave measurement as its regular verification is important for analyzing cardiac health. For the V2 chest lead particularly whose QT wave is of tremendous significance we have tested on 530 recordings of Physionet. This is because delay in cardiac repolarization causes ventricular tachyarrhythmias as well as Torsade de pointes (TdP). A feature of TdP is pronounced prolongation of the QT interval in the supraventricular beat preceding the ar-rhythmia. TdP can degenerate into ventricular fibrillation, leading to sudden death.

This paper addresses the problem of single-channel speech enhancement in the adverse environment. The critical-band rate scale based on improved multi-band spectral subtraction is investigated in this study for enhancement of single-channel speech. In this work, the whole speech spectrum is divided into different non-uniformly spaced frequency bands in accordance with the critical-band rate scale of the psycho-acoustic model and the spectral over-subtraction is carried-out separately in each band. In addition, for the estimation of the noise from each band, the adaptive noise estimation approach is used and does not require explicit speech silence detection. The noise is estimated and updated by adaptively smoothing the noisy signal power in each band. The smoothing parameter is controlled by a-posteriori signal-to-noise ratio (SNR). For the performance analysis of the proposed algorithm, the objective measures, such as, SNR, segmental SNR, and perceptual evaluations of the speech quality are conducted for the variety of noises at different levels of SNRs. The speech spectrogram and objective evaluations of the proposed algorithm are compared with other standard speech enhancement algorithms and proved that the musical structure of the remnant noise and background noise is better suppressed by the proposed algorithm.

This paper proposes a multi-band speech enhancement algorithm exploiting iterative processing for enhancement of single channel speech. In the proposed algorithm, the output of the multi-band spectral subtraction (MBSS) algorithm is used as the input signal again for next iteration process. As after the first MBSS processing step, the additive noise transforms to the remnant noise, the remnant noise needs to be further re-estimated. The proposed algorithm reduces the remnant musical noise further by iterating the enhanced output signal to the input again and performing the operation repeatedly. The newly estimated remnant noise is further used to process the next MBSS step. This procedure is iterated a small number of times. The proposed algorithm estimates noise in each iteration and spectral over-subtraction is executed independently in each band. The experiments are conducted for various types of noises. The performance of the proposed enhancement algorithm is evaluated for various types of noises at different level of SNRs using, 1) objective quality measures: signal-to-noise ratio (SNR), segmental SNR, perceptual evaluation of speech quality (PESQ);and 2) subjective quality measure: mean opinion score (MOS). The results of proposed enhancement algorithm are compared with the popular MBSS algorithm. Experimental results as well as the objective and subjective quality measurement test results confirm that the enhanced speech obtained from the proposed algorithm is more pleasant to listeners than speech enhanced by classical MBSS algorithm.

Introduction Tobacco is one of the biggest public health problems and a major risk factor for various non-communicable diseases(NCDs).An important aspect of tobacco control strategy could include modifications in the tobacco-related built *** study investigated the association between tobacco shop density and tobacco use prevalence in the urban slums of Bhopal city,*** We conducted a cross-sectional survey to obtain the distribution of tobacco-related built environment(tobacco shops)in the neighbourhood(400-m service area)of 32 urban slum clusters of *** plotted this distribution using the’network service area analysis’in ArcMap 10.7.1 ***,we used an ecological design to determine the association between tobacco shop density and tobacco use prevalence in these 32 clusters(N=6214 adult inhabitants).We used multiple linear regression analysis to estimate the regression coefficient(adjusted for socio-demographic variables)between tobacco use and tobacco shop density at the cluster *** The prevalence of tobacco use among all 32 slum clusters ranged from 22.1 to 59.6%(median 40.9%with IQR 31.8-44.2).There were 194 tobacco shops situated in the neighbourhood of all *** median density of tobacco shops was 59.40/km2(IQR 39.9-108.1/km2)in the neighbourhoods of slum *** use prevalence was significantly associated with tobacco shop density(estimate or B=0.071,p value=0.002)after adjusting for age,literacy,wealth index,and gender *** Tobacco use prevalence is significantly associated with tobacco shop density in the slums of Bhopal city in central *** need to develop appropriate built environment interventions to control rampant tobacco use.

Colorectal cancers in ulcerative colitis from a low-prevalence area for colon cancer

AIM: To determine the incidence and risk factors for colorectal cancer(CRC) in patients with ulcerative colitis from a low prevalence region for ***: Our prospective database yielded a cohort of 430 patients [age: 44 ± 14.6 years; 248 men(57.7%)] with ulcerative colitis(median disease duration 6, range: 1-39 years) for analysis. Of these, 131(30.5%) had left-sided colitis and 159(37%) extensive colitis. Patients with histologically confirmed CRC within the segment with colitis were compared with those without CRC, to determine the risk factors for the development of ***: Twelve patients(2.8%) developed CRC. The overall incidence density was 3.56/1000 patient-years of disease- 3/1000 in the first 10 years, 3.3/1000 at 10 to 20 years, and 7/1000 at > 20 years. Three of our 12 patients developed CRC within 8 years of disease onset. On univariate analysis, extensive colitis, longer duration of disease, and poor control of disease were associated with development of CRC. On multivariate analysis, duration of disease and extent of colitis remained ***: CRC occurred in 2.8% of patients with ulcerative colitis in our population- an incidence density similar to that in Western countries in spite of a low overall prevalence of colon cancer in our *** risk increased with extent and duration of disease.
