Efficacy and safety of treatment of hepatitis C virus infection in renal transplant recipients

AIm: To assess the efficacy and safety of combinedpegylated interferon and ribavirin therapy in hepatitis C virus (HCV) infection in renal transplant recipients. mETHODS: This is a retrospective chart review of post renal transplant patients who were positive for anti-HCV and HCV-RNA, and who have received treatment with combination of pegylated interferon and ribavirin between October 2003 and December 2008. Only patients with stable graft function and absence of evidence of cirrhosis and who received the therapy for continuous 48 wk were included. Nineteen patients (13 male and 6 female) were identified and included. The patient's complete blood count, liver and kidney prof ile, and cal- culated glomerular f iltration rate (GFR) were monitored every 6-8 wk while on treatment. HCV-RNA was tested at 12 wk for early virological response, at 48 wk for end of treatment response (ETR), and then retested at 24, and 48 wk after completion of therapy for sustained virological response (SVR). Liver biopsies were obtained before treatment from all patients and graft kidney biopsies were performed as required. RESULTS: Of the entire cohort, 9 patients (47.4%) showed an ETR and 8 had SVR (42.1%). Of the 8 patients with abnormal alanine aminotransferase (ALT) levels at baseline, 78.9% had their ALT normalized (including the virological non responders). ALT was normal in all responders at the end of therapy and at 24 wk post therapy (100%). Only one patient (5.3%) developed an increase in creatinine and decline in GFR from baseline towards the end of treatment. This patient's kidney biopsy revealed borderline rejection. There was no impact on response by HCV-genotype, initial HCV RNA load, age or sex of the patient or duration post transplant before commencement of therapy. All patients tolerated treatment in the same way as non-transplant with no unusual or increased occurrence of side effects. The natural radioactivity of 226Ra, 232Th and 40K and the fallout of 137Cs in soils and granite rocks of two regions in Yemen (Shabwah and Hadramout) were measured by using gamma-ray spectrometry (HPGe) detector. The average values of the radionuclides 226Ra, 232Th and 40K in the soil samples are 14.34, 25.78 and 566.05 Bq/kg respectively. For rock samples, the average activity concentration for 226Ra, 232Th and 40K are 45, 106 and 1235 Bq/kg respectively. Low concentration values of 137Cs in soil and rock samples under investigation, are not radiologically important. The radium equivalent activity (Raeq) and external hazard index (Hex) of all samples are less than the limits of 370 Bq·kg-1 and unity, respectively. The average values of total absorbed dose rate due to three primordial radionuclides in soil and rock samples are 46.5 nGy/h and 138.36 nGy/h, respectively, where the absorbed average value of the granite is higher than the permitted limit. So, the local people must avoid using these granite samples as the interior decorative materials of dwelling without radioactivity control.

The main role of this article is to describe dental assistants and the dental department’s role in the dental delivery system for Special health care needs (SCHN) by creating a structure to educate the dental assisting profession and project factors that have a significant impact on the dental assistant. The dental assisting management of SCHN patient’s profession including dental Assistance general responsibilities, regulation credentialing, and education. The SCHN patient’s management creates a challenge in all area of the dental department. Handling patient with a SCHN is underexplored throughout the dental staff especially when there is not enough theoretical foundation, education and training to deal with this category of patients. SCHN patients are defined as individuals who have abnormal behavioral impairments, mental conditions or/and chronic disease or abnormal laboratory results. Thus, the aim of this article is to guide all dental assistance staff in the best dental management choice for SCHN patients. All health care teams are part of patient care in most medical settings. A work environment supportive ensures positive outcomes for patient care. This article was considered as those categories of patients required more care and special way to deal with, moreover language, age, gender and education level of the patient are also considered significant parries. Not many studies were found to support the care of SCHN patients in dentistry department. Taking experience from other area help to design a system to handle the SCHN even if it requires hiring a Dentist who is specialized in that filed.

Oligonucleotide-based therapy has experienced remarkable development in the past 2 decades,but its broad applications are severely hampered by delivery *** used viral vectors and lipid nanovectors are suffering from immune clearance after repeating usage or requiring refrigerated transportation and storage,*** this work,amino-modified virus-mimetic spike silica nanoparticles(NH_(2)-SSNs)were fabricated using a 1-pot surfactant-free approach with controlled spike lengths,which were demonstrated with excellent delivery performance and biosafety in nearly all cell types and *** indicated that NH2-SSNs entered cells by spike-dependent cell membrane docking and dynamin-dependent *** positively charged spikes with proper length on the surface can facilitate the efficient encapsulation of RNAs,protect the loaded RNAs from degradation,and trigger an early endosome escape during intracellular trafficking,similarly to the cellular internalization mechanism of *** the fantastic properties of NH_(2)-SSNs in nucleic acid delivery,it revealed that nanoparticles with solid spikes on the surface would be excellent vehicles for gene therapy,presenting self-evident advantages in storage,transportation,modification,and quality control in large-scale production compared to lipid nanovectors.

NOx Emission Reduction Using Hydrous Ethanol-Gasoline Blend with Syngas in SI Engine

This paper presents a comparatively experimental study of nitrogen oxide (NOx) emissions from an internal combustion engine fed by gasoline available in the Saudi Arabian market rating octane number (RON 91 and RON 95) with admixtures of syngas with 0% E0, 5% E5 and 10% E10—by volume of pure ethanol—and HE5 and HE10 with water concentrations of 5%, 10%, 30% and 40%—by volume of hydrous ethanol—at stoichio-metric mixtures. An on-board plasma system used to produce syngas through the partial oxidation of gasoline with air in a plasma-assisted fuel reformer. The syngas injected in a gasoline engine with a fuel injection system modified for operation with addition of some amount of syngas. The experimental results demonstrated a significant total reduction in NOx emissions and slightly increased in fuel consumption when mixing gasoline (RON 91 and RON 95), ethanol (E5 and E10) and hydrous ethanol (HE5 and HE10) with syngas. For the use of hydrous ethanol (HE5 and HE10) along with the addition of syngas, for both RON 91 and RON 95, the lowest NOx emissions are found 72% with a water concentration of 40%.

The Diabetic Foot Research in Arabs’ Countries

Objective: To review all the studies on diabetic foot disorders (DFDs) that were published on the Pubmed? site aiming to identify the contributions of the different Arabs’ countries in the world scientific literature on this topic. methods: The Pubmed? site was searched using different key words for searching all the abstracts on Diabetes mellitus (Dm) and DFDs published from Arabs’ League countries (n = 22). For this review, the 22 countries were classified into 3 groups: Group 1 (G1): Gulf Council Countries (GCC) countries (n = 6), Group 2 (G2): African Arabs’ countries (n = 10), Group 3 (G3): Asian and/or Eastern mediterranean Arabs’ countries (n = 6). All the abstracts on Dm coming from all of the 22 Arabs’ countries were initially reviewed to locate the ones related to DFDs’ management. All of the articles related to DFDs were reviewed by the senior author. A publication index was created to allow a comparison between the productivity of various countries and correlate that to the population number. Results: By April 2012, a total of 906 articles were published on Dm, out of them 115 (11.6%) were related to DFDs. The largest number of Dm/DFDs research came from G1 countries (n = 437/51) followed by G2 (n = 307/38) and finally G3 (n = 162/26). The percentages of the studies related to DFDs were therefore: 11.6%, 12.3% and 20.6% respectively. Saudi Arabia was the top on the list of all studied countries with 31 studies related to DFDs out of the 187 on Dm (16.5%). Conclusion: more research on DFDs is needed in most of the Arabs’ countries particularly those in the GCC region which reported very high prevalence rates and are expected to hold these rates for the coming decades. Also, special attention is needed for those low-income Arabs’ countries that had no contributions in DFDs’ research.

*** strongest locus which associated with type 2 diabetes(T2D)by the common variant rs7903146 is the transcription factor 7-like 2 gene(TCF7L2).We aimed to quantify the interaction of diet/lifestyle interventions and the genetic effect of TCF7L2 rs7903146 on glycemic traits,body weight,or waist circumference in overweight or obese adults in several randomized controlled trials(RCTs).*** October 2016 to may 2018,a large collaborative analysis was performed by pooling individualparticipant data from 7 *** RCTs reported changes in glycemic control and adiposity of the variant rs7903146 after dietary/lifestyle-related interventions in overweight or obese *** treatment interaction models which used the genetic effect encoded by the allele dose and common covariates were applicable to individual participant data in all *** the joint analysis,a total of 7 eligible RCTs were included(n=4,114).Importantly,we observed a significant effect modification of diet/lifestyle-related interventions on the TCF7L2 variant rs7903146 and changes in fasting *** with the control group,diet/lifestyle interventions were related to lower fasting glucose by-3.06(95%CI,-5.77 to-0.36)mg/dL(test for heterogeneity and overall effect:I^(2)=45:1%,pmic control and adiposity traits per one copy of TCF7L2 risk *** findings suggest that carrying the TCF7L2 T risk allele may have a modestly greater benefit for specific diet/lifestyle interventions to improve the control of fasting glucose in overweight or obese adults.

背景脑电双频指数（bispectralindex，BIS）可监测患者在全麻状态下的意识水平。在不影响麻醉深度的情况下，有些因素可影响BIS数值。本实验目的在于观察改变患者体位是否对BIS值有影响。方法选取40例在全麻下行短小手术的患者。首先将患者置于平卧位（仰卧位）15分钟，记录BIS值、平均动脉压、心率、呼气末二氧化碳分压和呼气末异氟烷浓度。然后改变患者体位，将患者置于头低仰卧位（30°）。最后将患者置于头高仰卧位（30°）。每种体位保持15分钟并记录数据。结果与平卧位相比头低位的BIS值显著增高（中位数47VS40），而头高位的BIS值显著降低（39VS41）（P〈0．05）。结论患者体位的改变明显影响了BIS值，由此可影响对麻醉深度的判断。

The continuous battle between humans and the multitude of pathogenic microorganisms in the environment has sought relief in the form of antimicrobials. But the counter attack by pathogenic organisms in the form of multidrug resistance, acquired by various mechanisms such as transformation, transposition, conjugation and transduction is a major reason for concern. Bacteriophages have contributed in a significant way to dissemination of genes encoding antimicrobial resistance, heavy metal resistance and virulence factors through the phenomenon of transduction. This review aims at compiling information about the different mechanisms by which bacteriophages aid in transferring genes involved in antimicrobial resistance to Escherischia coli in various environments.
