Drainage vs. non-drainage after cholecystectomy for acute cholecystitis:a retrospective study

many surgeons practice prophylactic drainage after cholecystectomy without reliable evidence,this study was conducted to answer the question whether to drain or not to drain after cholecystectomy for acute calculous cholecystitis.A retrospective review of all patients who had cholecystectomy for acute cholecystitis in Aseer Central Hospital,Abha,Saudi Arabia,was conducted from April 2010 to April 2012.Data were extracted from hospital case files.Preoperative data included clinical presentation,routine investigations and liver function tests.Operative data included excessive adhesions,bleeding,bile leak,and drain insertion.Complicated cases such as pericholecystic collections,mucocele and empyema were also reported.Patients who needed therapeutic drainage were excluded.Postoperative data included hospital stay,volume of drained fluid,time of drain removal,and drain site problems.The study included 103 patients allocated into two groups;group A（n = 38） for patients with operative drain insertion and group B（n = 65） for patients without drain insertion.The number of patients with preoperative diagnosis of acute non-complicated cholecystitis was significantly greater in group B（80%） than group A（36.8%）（P 〈 0.001）.Operative time was significantly longer in group A.all patients who were converted from laparoscopic to open cholecystectomy were in group A.multivariate analysis revealed that hospital stay was significantly（P 〈 0.001） longer in patients with preoperative complications.There was no added benefit for prophylactic drain insertion after cholecystectomy for acute calculous cholecystitis in non-complicated or in complicated cases.

The coronavirus first reported in China in November 2002 in the form of atypical pneumonia known as the severe acute respiratory syndrome (SARS).[1] The virus then appeared in 2012 in Saudi Arabia as the middle East respiratory syndrome (mERS).[2] The year 2020 witnessed a novel β-coronavirus related to the previous detected viruses.[3] It was the severe acute respiratory syndrome coronavirus 2 (SARS-Co V-2)-a positive single stranded RNA virus.

A study of the impact of environmental loads that penetrate a passive skylight roofing system in malaysian buildings

A passive skylight system is a significant building design element that provides an ideal condition for interior spaces. However, the use of this system is limited to specific climatic regions because of its considerable effect on the indoor environment. malaysia is a tropical country that has favorable natural benefits, such as solar geometry and natural light, which can brighten building interiors throughout the year. However, harnessing this benefit affects spaces, especially those in single-story buildings, because of excessive natural loads. This study reviews a concept to understand the passive behavior of solar radiation in the form of light and heat that falls on, interacts with, and is emitted from a skylight system in a single-story building. The study method is theoretically based on descriptive analysis to assess design requirements. The review shows that designs grounded on the physical aspects of climate （influenced variables）, materials （design variables）, and human comfort （affected variables） in one process （ESI） can develop the architectural way of thinking rather than estimate the condition based on a limited perspective. This assumption indicates that the adoption of this concept in the preliminary design stage will enable designers to balance the building environment effectively.

Vector control schemes have recently been used to drive linear induction motors(LIm)in high-performance applications.This trend promotes the development of precise and efficient control schemes for individual motors.This research aims to present a novel framework for speed and thrust force control of LIm using space vector pulse width modulation(SVPWm)inverters.The framework under consideration is developed in four stages.To begin,mATLAB Simulink was used to develop a detailed mathematical and electromechanical dynamicmodel.The research presents a modified SVPWm inverter control scheme.By tuning the proportional-integral(PI)controller with a transfer function,optimized values for the PI controller are derived.all the subsystems mentioned above are integrated to create a robust simulation of the LIm’s precise speed and thrust force control scheme.The reference speed values were chosen to evaluate the performance of the respective system,and the developed system’s response was verified using various data sets.For the low-speed range,a reference value of 10m/s is used,while a reference value of 100 m/s is used for the high-speed range.The speed output response indicates that themotor reached reference speed in amatter of seconds,as the delay time is between 8 and 10 s.The maximum amplitude of thrust achieved is less than 400N,demonstrating the controller’s capability to control a high-speed LIm with minimal thrust ripple.Due to the controlled speed range,the developed system is highly recommended for low-speed and high-speed and heavy-duty traction applications.

The aggregation behavior of the mixture of cetyltrimethylammonium chloride(CTAC), a cationic surfactant, and moxifloxacin hydrochloride(mFH), a fourth-generation fluoroquinolone antibiotic drug, has been studied using the conductivity technique in aqueous and alcoholic(EtOH, 1-PrOH, and 2-BuOH)media. The study was performed at several temperatures between 298.15 and 323.15 K at 5 K intervals.The assembly has been characterized by evaluating the micellar parameters, such as the critical micelle concentration(CmC) and the counter ion binding(β), of the CTAC + mFH mixture. The values of the CmC for the assembly of the CTAC + mFH mixture were reliant on the composition of alcohols in the mixed solvents and the temperature. The CmC values of the CTAC + mFH mixture increased with increasing temperature;that is, assembly was delayed by increased temperature. The micellization of the CTAC + mFH mixed system was delayed in alcoholic media. The observed-ΔG0mvalues for the association of the CTAC + mFH mixed system demonstrated a spontaneous aggregation process under all study conditions.Based on the-ΔH^(0)_(m) and +ΔS^(0)_(m) values, the association of the CTAC + mFH mixture is exothermic and the interaction forces acting between the CTAC and mFH species are hydrophobic, ion–dipole, and electrostatic interactions. The transfer properties and enthalpy–entropy compensation were also assessed and described comprehensively.

The effect of replacing the anion from N to Bi down the group in the periodic table is investigated on Srmg2X2（X = N,P,As,Sb,Bi）.A full potential linearized augmented plane wave plus local orbitals method is used along with different exchange–correlation potentials to obtain the lattice constants,phonons,electronic,and optical properties of the Sr mg2X2（X = N,P,As,Sb,Bi） Zintl compounds.A good agreement is achieved and our calculations are validated by previous experimental and theoretical data.all compounds have shown stable dynamical behavior with gamma centered longitudinal response having no imaginary frequencies.Electronic band structures reveal the semiconducting nature of the compounds.The Pnictogen（X）-p state contributed mainly in the valence band and the Sr-d state forms the conduction of the compounds.Relative charge transfer and low overlapping of the atomic densities indicates the preferable ionic bonding character of these materials.In the optical properties,real and imaginary parts of dielectric function,complex refractive index,birefringence,reflectivity,and optical conductivity are calculated.These compounds can be utilized in the optical and optoelectronic devices.

Power Quality Improvement Using ANN Controller For Hybrid Power Distribution Systems

In this work,an Artificial Neural Network(ANN)based technique is suggested for classifying the faults which occur in hybrid power distribution systems.Power,which is generated by the solar and wind energy-based hybrid system,is given to the grid at the Point of Common Coupling(PCC).A boost converter along with perturb and observe(P&O)algorithm is utilized in this system to obtain a constant link voltage.In contrast,the link voltage of the wind energy conversion system(WECS)is retained with the assistance of a Proportional Integral(PI)controller.The grid synchronization is tainted with the assis-tance of the d-q theory.For the analysis of faults like islanding,line-ground,and line-line fault,the ANN is utilized.The voltage signal is observed at the PCC,and the Discrete Wavelet Transform(DWT)is employed to obtain different features.Based on the collected features,the ANN classifies the faults in an effi-cient manner.The simulation is done in mATLAB and the results are also validated through the hardware implementation.Detailed fault analysis is carried out and the results are compared with the existing techniques.Finally,the Total harmonic distortion(THD)is lessened by 4.3%by using the proposed methodology.

Sugarcane is one of the major important sugar yielding crops in Bangladesh.As an exhaustive crop,sugarcane removes a huge amount of plant nutrients from the soil.However,the combined use of organic and inorganic fertilizers can be a good approach to deal with nutrient depletion and promote sustainable crop production as well as improve soil health.Therefore,an attempt was made to identify the most fruitful and profitable integrated nutrient management on the aspects of growth,yield and quality of sugarcane in two consecutive growing seasons.Seven treatments:T_(1)=Control,T_(2)=165:55:120:30:10:2.5:4 kg N:P:K:S:mg:Zn:B ha^(−1),T_(3)=Poultry Litter(PL)at 5 t ha^(−1)+95:51:87:9:10:2.5:4 kg N:P:K:S:mg:Zn:B ha^(−1),T_(4)=Cow Dung(CD)at 15 t ha^(−1)+36:52:60:17:10:2.5:4 kg N:P:K:S:mg:Zn:B ha^(−1),T_(5)=Press mud(Pm)at 15 t ha^(−1)+10:50:43:0:10:2.5:4 kg N:P:K:S:mg:Zn:B ha^(−1),T_(6)=mustard Oil Cake(mOC)at 0.5 t ha^(−1)+140:54:115:25:10:2.5:4 kg N:P:K:S:mg:Zn:B ha^(−1)and T_(7)=Gm(Green manure)at 5 t ha^(−1)+140:53:100:28:10:2.5:4 kg N:P:K:S:mg:Zn:B ha^(−1)were used in this experiment.Two years data showed that treatment T_(3)produced the maximum amount of tillers,total dry matter yield,millable sugarcane,cane yield and sugar yield,followed by the T_(4)treatment.The highest stalk heights were recorded in the T_(3)treatment,which was statistically similar to all other treatments except T_(1)and T_(2).The juice quality parameters viz.,brix and pol in cane were found significant in treatment T_(3)while the highest purity was obtained in the T_(7)treatment.all the data of Jaggery(goor)quality parameters,the highest sucrose content,color transmittance,Jaggery(goor)recovery and the lowest ash content of Jaggery(goor)were observed in the T_(3)treatment,which was statistically similar to the T_(4)treatment in both seasons.The highest cost of production was obtained from the T_(6)treatment while the highest gross return,net return and BCR were recorded in the T_(3)treatment.No significant changes were found in one cycle of suga

Enhancement of Growth and Grain Yield of Rice in Nutrient Deficient Soils by Rice Probiotic Bacteria

Plant associated bacteria are promising alternatives to chemical fertilizers for plant growth and yield improvement in an eco-friendly manner.In this study,rice associated bacteria were isolated and assessed for mineral phosphate solubilization and indole-3-acetic acid(IAA) production activity in vitro.Six promising strains,which were tentatively identified as phylotaxon Pseudochrobactrum sp.(BRRh-1),Burkholderia sp.(BRRh-2),Burkholderia sp.(BRRh-3),Burkholderia sp.(BRRh-4),Pseudomonas aeruginosa(BRRh-5 and BRRh-6) based on their 16 S rRNA gene phylogeny,exhibited significant phosphate solubilizing activity in National Botanical Research Institute phosphate growth medium,and BRRh-4 displayed the highest phosphate solubilizing activity,followed by BRRh-5.The p H of the culture broth declined,resulting in increase of growth rate of bacteria at p H 7,which might be due to organic acid secretion by the strains.In presence of L-tryptophan,five isolates synthesized IAA and the maximum IAA was produced by BRRh-2,followed by BRRh-1.Application of two most efficient phosphate solubilizing isolates BRRh-4 and BRRh-5 by root dipping(colonization) of seedling and spraying at the flowering stage significantly enhanced the growth and grain yield of rice variety BRRI dhan-29.Interestingly,application of both strains with 50% of recommended nitrogen,phosphorus and potassium fertilizers produced equivalent or higher grain yield of rice compared to the control grown with full recommended fertilizer doses,which suggests that these strains may have the potential to be used as bioinoculants for sustainable rice production.

Effect of different additives on the phase separation behavior and thermodynamics of p-tert-alkylphenoxy poly (oxyethylene) ether in absence and presence of drug

Cloud point(CP) determinations of 4-(1,1,3,3-tetramethylbutyl)phenyl-polyethylene glycol(TX-100(nonionic surfactant)) was carried out in aqueous as well as in the attendance of drug(ceftriaxone sodium trihydrate(CFT))/(CFT + different inorganic salts) and discussed thoroughly. Nonionic surfactants are employed extensively in different formulations. In aqueous solution, the values of CP of TX-100 are obtained to increase by means of enhancing of their concentration in the solution. The CP values of TX-100 solutions were found to decrease in the presence of drug and their values decrease more with rising concentrations of the drug. The values of CP of CFT and TX-100 mixtures were found to further decrease in the attendance of inorganic salts in comparison to their absence. The effect of different sodium salts in decreasing CP values of TX-100 was achieved in the following order: NaCO_3 > Na_2SO_4 > Na Cl. However, in the case of potassium and ammonium salts, the decreasing order obtained is K_2SO_4 > KCO_3 > KCl and(NH4)2 SO_4 > Na_2CO_3 > NH_4Cl respectively. Various thermodynamic parameters for example standard free energy(ΔG_c~Θ), standard enthalpy(ΔH_c~Θ) as well as standard entropy(Δ Sc?)changes of phase separation were also evaluated and discussed in detail on the basis of their behavior.
