Sinonasal Polyposis: About 60 Cases at Fann University Hospital Center, Senegal

Introduction: Sinonasal polyposis (SNP) is a chronic inflammatory disease of the mucosa of the nasal cavities and facial sinuses. It is characterized by an oedematous, multifocal and bilateral degeneration of the nasosinus mucosa, which originates in the lateral masses of the ethmoid, where it causes the formation of smooth, gelatinous, translucent and pyriform polyp lesions. The objective of this study was to review epidemiological, clinical, paraclinical data and evaluate the results of endoscopic surgical treatment. Patients and Methods: This is a retrospective study on 60 patients followed at the ENT department of the Fann National University Hospital Center, from January 2010 to December 2015. All patients with sinonasal polyposis were included in the study. Results: The average age of our patients was 38 years and the sex ratio (M/F) was 0.8. In the patients’ histories, we found 18% asthma and 10% Widal’s disease. The average consultation time was 8.5 years. All patients had consulted for nasal obstruction;rhinorrhea was bilateral and found in 67.7% of cases, with olfactory disorders accounting for 50%. The CT scan performed in 58% of cases made it possible to specify the extent of the lesions;the involvement of the ethmoidal sinus was constant and extended to the other sinuses except in 2 cases. All patients had received medical treatment with local corticosteroids. Endoscopic surgical treatment was initiated in 43% of cases after failure of corticosteroid-based medical treatment. The evolution under treatment marked by the reappearance of symptoms that increased each month. At one month postoperatively, all clinical symptomatology had improved with the exception of olfactory disorders, which persisted in 3 patients. At 12 months we noted 12 cases of reappearance of nasal obstruction. Conclusion: SNP is a disease of little known etiology. The diagnosis is almost always clinical. Gallbladder Carcinoma of the Thyroid Revealed by an Acute Festered Thyroiditis: A Literature Study about a Case

The cutaneous extension of gallbladder thyroid carcinoma is uncommon and is among the aggressive forms of the disease. We are reporting the case of a woman of 54 that shows acute festered thyroiditis worsened by a necrotic ulcer wound on the skin lasting 3 weeks amid a big neglected hetero-multinodular goitre, evolving since 20 years. The anatomopathological test showed a gallbladder thyroid carcinoma of the thyroid with severe inflammation. The treatment consisted of a complete thyroidectomy with recurrent bilateral dredging. There was a favourable evolution. Gallbladder carcinoma, in its aggressive aspect, may be linked to the occurrence of acute festered thyroiditis. Therefore, the prognosis of our patient was favourable.

Fatal Cataclysmic Otorrhagia and Epistaxis Due to a Ruptured Aneurysm of the Petrous Internal Carotid Artery: A Case Report

Background: Aneurysms of the internal carotid artery within the petrous temporal bone are extremely rare;their true incidence is unknown. The exact cause is unclear: they may be congenital or result from trauma, infection, or radiation. Aim: We report a case of massive otorrhagia and epistaxis from a ruptured aneurysm of the petrous internal carotid artery. Case Presentation: A 34-year-old man presented to our department for the first time with repeated left otorrhagia ongoing for 5 years, left sided pulsatile tinnitus and left conductive hearing loss. In his history, we noted a right hemi-corporeal deficit of sudden onset one month ago and the head-CT showed a left frontoparietal subarachnoid hemorrhage without any visualised vascular malformation. Otomicroscopy showed a pulsatile mass visible at the posterior part of the hypotympanum. There was a right-sided hemiparesis estimated at 2/5 with no disorder of the sensitivity. After hemodynamic stabilization, the patient was discharged from the hospital and treatment was scheduled in interventional radiology and neurosurgery unit. Unfortunately the patient presented at home with a cataclysmic hemorrhage by massive otorrhagia and epistaxis and arrived dead at the emergency unit. Conclusion: The treatment of a petrous carotid aneurysm must be carried out quickly considering the risk of rupture leading to a cataclysmic hemorrhage that can be rapidly life threatening.

A Chronic Dysphony Caused by a Bifocal and Pulmonary Laryngeal Tuberculosis

Laryngeal tuberculosis is a rare entity. We consider the case of a woman of 62 who presented a one-year evolving chronic dysphonia chart associated with cough, febricula and general health damage. The nasofibroscopy showed a budding ulcerous lesion of the larynx a priori evoking a granulomatous lesion or cancer. The BAAR spits were strongly positive and the radiographic pictures of the thorax, typically evoked a pulmonary tuberculosis. Seeing the excellent therapeutic response to TB treatment in 4 months period, the bifocal tuberculosis diagnostic was confirmed.

Assessment of Radiofrequency in Otorhinolaryngology at the Military Hospital of Ouakam (MHO)

Introduction: Coblation® is a low temperature-controlled tissue ablation technique widely used in human surgery. Its development in otorhinolaryngology has allowed a spectacular advance in the management of diseases of the nasal and oral cavities. The objective of our work was to study the indications for radiofrequency in ENT, to describe the technical aspects and to evaluate the results of his practice at the HMO. Patients and Methods: This is a retrospective study carried out in the ENT and Head and Neck Surgery department of the Ouakam Military Hospital (MHO) over a period of 3 years (from January 2015 to the end of December 2017), involving 37 patients who received radiofrequency treatment in the ENT sphere. Results: The study was carried out on 37 patients. The sex ratio of 0.19. The mean age of the patients was 33.3 years. The indications were dominated by chronic caseous cryptic tonsillitis with or without halitosis with 24 cases (64.9%) followed by recurrent tonsillitis in 5 cases (13.5%) then we have obstructive hypertrophy of the palatal tonsils with syndrome sleep apnea (OSAS) in 4 cases (10.8%) followed by nasal obstruction (hypertrophy of the inferior turbinates) and hypertrophy of the soft palate and uvula in 2 cases (5.4%). The complication rate was 8.1% and dominated by transient dysgeusia in 1 case or 2.7%, uvula edema in 1 case or 2.7% and bleeding in 1 case or 2.7%. The postoperative follow-up was done one week, 1 month, 3 months, 6 months and 12 months after radiofrequency. Volumetric reduction begins to set in from 1 month (62.5%) and becomes maximum in 3 months (74.3%). Conclusion: Coblation® is a controlled tissue ablation technique at low temperature, patented by ArthroCare® Corporation in 1997. Very widespread in human surgery. Surgical Treatment of Hyperparathyroidism Secondary to Chronic Renal Failure: Our Experience with the 7/8 Subtotal Parathyroidectomy Technique

Introduction: Surgical management takes place, after a certain period of evolution, on particularly severe forms of secondary hyperparathyroidism, resistant to medical treatment. Subtotal parathyroidectomy is an effective technique in the treatment of these disorders. Method: Our study is retrospective of 33 cases of hyperparathyroidism secondary to chronic renal failure in dialysis patients operated by the 7/8 technique. Identified over a period of 10 years (January 2010 to December 2019), in the ENT department of the Fann University Hospital. Results: Out of 33 cases of secondary hyperparathyroidism, the average age of our patients was 51.24 years with a sex ratio of 0.43. Causal nephropathy was dominated by nephro-angiosclerosis, which was found in 27.27% of cases. Bone pain found in 23 patients or 69.69% was the predominant clinical sign. The average calcemia was 92.7 mg/l. Parathormone was dosed in all our patients and the average was 1611.05 ng/l. The consequences were clinically marked by recurrent paresis in one patient. No case of hematoma or postoperative infection was found. On the biological level 10 patients or 30.30% had a transient hypocalcaemia. The results were marked by a drop in PTH in 23 patients or 78.78% of cases. Conclusion: Subtotal parathyroidectomy remains an effective and beneficial therapeutic method for kidney failure patients with secondary hyperparathyroidism.

Appendicitis bilharzia is a very rare condition and we report 3 cases of this pathology that sense clinical and biological similarities with bacterial appendicitis. The etiological diagnosis was exclusively histopathological, this allowed to highlight eggs of Schistosome in the appendicular wall in the three patients. The surgical treatment was supplemented by a specific medical treatment based on praziquantel. The surgical suites were simple for 2 patients, complicated by superficial parietal suppuration in a patient.

In Senegal, tomato (Solanum lycopersicum L.) cultivation is affected by salinity in many agro-ecological zones. The selection of salt tolerant varieties would be an alternative solution to enhance the production. Thus, germination and growth are studied under axenic conditions for five varieties of tomato subjected to increasing concentrations of NaCl [0, 35, 70 and 105 mM], and supplemented in an MS/2 medium for 30 days. The results reveal that salt negatively affects the evaluated parameters. The Rodeo and Lady Nema varieties have the lowest final germination rates (50%) unlike the Mongal variety (55%). These last two varieties have a decrease of 71.78% and 81.28% in the height of the stem, respectively, in the presence of NaCl at [105 mM] while that of the Rodeo variety is 70%. The Xewel variety has the greatest average number of leaves in the presence of [NaCl 35 mM] (4.95 leaves) and [NaCl 70 mM] (4.77 leaves). The Lady Nema variety records the longest taproot length (2.99 cm) unlike the Rodeo variety (2.25 cm) at [NaCl 105 mM]. Residual Effects of Sucrose and Hormonal Treatments of the Tuberization Medium on in VitroGermination of Potato (Solanum tuberosumL.) Microtubers

The residual effects of sucrose concentrations (80 or 100 g·L-1) and hormonal treatments (BAP + Kinetin or Coumarin) of tuberization medium on in vitro microtubers germination of three potato varieties (Solanum tuberosum L.) so called Aida, Atlas and Odessa, are described. After 3 weeks of incubation at 28℃ ± 1℃, 70% of Aida microtubers variety, previously formed in the MT2 medium [MS/2 + 80 g·L-1 Sucrose], germinated. The best germination rate for varieties Atlas (100%) and Odessa (66.66%) was obtained on microtubers previously formed in the medium MT2 [MS/2 + 100 g·L-1 Sucrose]. The addition of hormones in the tuberization medium allowed optimizing the microtubers germination of the Aida variety unlike the other varieties. Indeed, for the Aida variety, the combination M5 [Kin 2.5 mg·L-1 + Coum 0.025 mg·L-1 + Sucrose 80 g·L-1] increased the germination rate from 70% up to 93.33%. The best germination rate (90%), noticed with microtubers of Atlas variety, initially formed in M2 medium [Kin 1 mg·L-1 + BAP 1 mg·L-1 + Sucrose 100 g·L-1], was lower than that one (100%) obtained on medium without hormones. For Odessa variety, the maximum germination rate (53.33%) of microtubers, from the medium M4 [Kin 2.5 mg·L-1 + BAP 1 mg·L-1 + Sucrose 100 g·L-1], was also lower than that one (66.66%) observed in the medium without hormones. Aida and Atlas varieties thus offer a better germination rate than Odessa after their cold storage.

In arid and semi-arid regions, the growth and development of cultivated plants, especially tomato (Solanum lycopersicum L.), are severely limited by water deficit. Thus, to cope with this constraint, the plant establishes symbiotic relationships with arbuscular mycorrhizal fungi (AMF) in the soil whose extension of the hyphae allows a better and deeper exploration;this notably improves the hydromineral nutrition of the plant. Therefore, the choice of fungal partner becomes crucial for the establishment of a crop in water-deficient soil. In this context, the contribution of AMF to the water stress tolerance of two varieties of tomato plants was assessed under semi-controlled conditions. Parameters, such as the mycorrhizal frequency, intensity of mycorrhization, relative mycorrhizal dependency, growth, and biochemical parameters (carbon, nitrogen, phosphorus, and proline contents) of plants subjected to three levels of water stress (T100, T70, and T30), were evaluated. The highest frequencies and intensities of mycorrhization and relative mycorrhizal dependencies were obtained with plants of the Xewel variety inoculated with Rhizophagus fasciculatus (F: 95.24%, 88.35%, and 13.64%;M: 40.52%, 37.52%, and 11.22%;D: 23.7%, 54.4%, and 78.82%) and in those of the Lady Nema variety inoculated with Claroideoglomus etunicatum (F: 95.12%, 87.01%, and 15.25%;M: 40.66%, 37.99%, and 11.42%;D: 19.27%, 57.01%, and 70.98%), respectively at water regimes of T100, T70 and T30. These same symbiotic couples recorded, at T30, the best survival rates (+ 40%) and the higher aerial (77% and 74%) and root dry weights (80% and 59%). Plants of the Xewel variety inoculated with R. fasciculatus recorded the highest contents of carbon (T70: 30.59% and T30: 21.55%) and phosphorus (T70: 0.18% and T30: 0.17%).
