Synthesis of silver nanoparticles using seeds of Nigella sativa as a capping agent was evaluated in this study. Different concentrations of the aqueous extract of N. sativa with silver nitrate solution were exposed to sunlight;as a force for acceleration of the formulation. Then the silver nanoparticles were characterized by UV-Vis, scanning electron microscope (SEM) and X-ray diffraction (XRD) techniques. Antibacterial activity of the nanoparticles was investigated against Staphylococcus aureus and Escherichia coli by the disc diffusion method. The characterization of nanoparticles was detected by the change in color to yellow-brown which indicated the formulation of silver nanoparticles. Irregular shapes within range of nanoscale were detected using SEM and XRD techniques. The finding suggests that silver nanoparticles may be effectively used as antibacterial agent.

Therapeutic Mammoplasty in Management of Breast Cancer: A Prospective Clinical Study

Background: The introduction of oncoplasticbreast surgery, with a concomitant contralateral symmetrization procedure, allows the surgical oncologist the freedom to perform wider excisions, with more favorable cosmetic outcomes. Objective: To assess the feasibility of bilateral breast reduction for the management of breast cancer, and to determine the most suitable surgical technique of therapeutic mammoplasty for each patient. And to evaluate the cosmetic outcome during the postoperative period. Methods: This study was from June 2014 until June 2016, and included 86 female patients with breast cancer;divided in 3 groups;group A;had bilateral superior pedicle reduction mammoplasty, group B;had bilateral inferior pedicle reduction mammoplasty, group C;had bilateral batwing mastopexy. 1 month postoperative, patients were asked to answer a 5-scale questionnaire evaluating their own cosmetic outcome. Criteria they were asked to evaluate were: symmetry, shape and volume, projection, correction of ptosis, visibility of the scars and overall satisfaction. A similar questionnaire was answered by a panel made up of a surgeon and breast-care nurse after seeing the pre- and post-operative photos of the patients in question. Results: Group A had a mean tumor size of 2.7 ± 0.69 cm, least resected margins 1.2 - 4.3 cm, Group B had a mean tumor size of 3.2 ± 1.19 cm, least resected margins 0.7 - 3.8 cm, Group C had a mean tumor size of 3.4 ± 1.12 cm, least resected margins 1.7 - 5.2 cm. Cosmetic results fared high above average. Group C fared better in overall satisfaction but Group B fared better in ptosis correction, projection and symmetry. The panel answers showed difference regarding shape and volume;the only two criteria to be almost of the same result among the three groups. Background: Surgical resection of upper gastrointestinal malignancies occasionally carries substantial morbidity due to inaccurate pre-operative staging. The potential to prevent needless laparotomy by means of precise staging is the pouring force behind the use of diagnostic laparoscopy (DL). Objective: To assess the role of laparoscopy and laparoscopic ultrasound (LUS) in proper staging of upper gastro intestinal malignancies, and in potential palliation in advanced cases for pain (by neurolytic celiac plexus block) or gastric outlet obstruction (by laparoscopic bypass surgery). Study design: In this prospective study, 62 patients with lower esophageal, gastric and peri-pancreatic carcinomas were joined after written informed consent. All patients were examined with laparoscopy and LUS with the help of frozen section analysis to any doubtful metastatic site, peritoneal fluid and ascitic fluid analysis. Results: DL helped us to avoid needless laparotomy in 22.5% of patients, reducing its post-operative complications, hospital stay and cost. DL also helped us to do palliative management either in the form of gastric bypass or laparoscopic celiac plexus block. Conclusion: we praise the use of DL as a safe, effective and complimentary method to the other routine imaging modalities, in proper staging and palliation for upper gastrointestinal malignancies.

Objective： Over 50% of the world populations are infected with Helicobacter pylori （H. pylori）. Most subjects are asymptomatic; however, in 1994, H. pylori has been categorized as group I carcinogen. The aim of the study was to investigate the relationship between H. pylori infection and gastric cancer. Methods： Thirty gastric cancer patients （GCs） and 30 gastritis patients were enrolled in the study. H. pylori was cultured on non-selective and selective medias, infection density was assessed by quantitative culture. Antibiotic sensitivity testing was performed. PCR was done for the H. pylori 16S rRNA gene in addition to cagA, vacA and iceA genes. Results： H. pylori could be cultured from 100% of specimens obtained from all patients. The density of H. pylori was higher in cancer cases than in gastritis patients. The 16S rRNA was detected in all GC patients （100%） while it was only detected in 70% of gastritis patients. The cagA gene was found in 53.3% vs 13.3% of GC and gastritis patients, respectively. The vacA gene was present in all GC patients （by at least one of its alleles） while it was only found in 33.3% of gastritis patients. The vacA slml combination was the most predominant genotype in GC patients, while m2 was the commonest allele in gastritis patients （10%）. The iceA gene was found in 86.7% vs 40% of GC and gastritis patients, respectively. Simultaneous presence of multiple H. pyiori strains was proved, both phenotypically and genotypically. Conclusion： The development of GC is linked to infection with H. pylori harboring certain virulence genes. Higher infection density of H. pylori was found in GC patients. Co-existence of more than one strain of H. pylori in the same patient occurs in both malignant and benign lesions.

Chromatographic Analysis of Thiophenes in Calli and Suspension Cultures of Tagets spp.

The thiophene isomer fractions were investigated by TLC （thin layer chromatography） and GLC （gas liquid chromatography）. The TLC analysis revealed that, 5-（4-acetoxy-l-butynyl）-2,2-bithenyl （BBTOAc, Rf 0.78 I）; 5-（3-buten-l-ynyl）-2,2-bithineyl （BBT, Rf 0.39 II） and 2,2,5,2-oc-terthienyl （∝-T, Rf 0.32 III） were detected in extracts of leaf, stem and root calli or suspension cultures of Tagetes erecta and T. patula as well as leaf cell culture of T. patula which treated with DL-tryptophan under light or dark conditions. The GLC quantitative analysis of thiophenes in subjected revealed that the percentage distribution of the major thiophenes in all the analysed samples were determined as follows： BBT II 〉 ∝ -TllI 〉 BBTOAc I. Another thiophene was determined in relatively small amounts and found to be 5-（3-penten-l-ynyl）-2,2-bithienyl （PBT IV）. Other thiophenes were quantitated in trace amounts with unidentified or unknown chemical structure from the investigated extracts.

Condensation of β-Oxoanilide 1 with active methylene derivatives 2a,bafforded the pyridine derivative 5, and with crotononitrile afforded the pyridine 8. Compounds 9 and 11a-c were obtained by reaction of 1 with malononitrile dimer and arylidinemalononitrile 10a-10c. In contrast, when compound 1 reacted with ethoxymethylen malononitrile afforded the pyridine derivative 13. On the other hand, treatment of 1 with anthranilic acid gave the quinoline derivative 14. Also, reactions of 1 with isothiocyanate derivatives afforded compounds 16-18. The reaction of 1 with chalcone derivative afforded the pyridine derivative 22. Treatment of compound 1 with thiourea produced pyrimidine derivative 23. Furthermore, compound 1 converted into pyrimidinethione 24a and pyrimidinone 24b on treatment with a mixture of aromatic aldehydes and thiourea or urea respectively. Reaction of 24a with hydrazonyl halide, thiosemicarbazide and arylidinecyanothioacetamide afforded compounds 26, 28 and 29. Compound 29 was treated with chloroacetonitrile to afford compound 30. Six compounds from the newly synthesized were screened for antibacterial and antifungal activity against bacteria staphylococcus aureus, bacillus cereus and klebsiella pneumonia and fungi aspergillus flavus and aspergillus ochraceous, respectively. Some of the tested compounds showed significant antimicrobial activity. IR, 1H NMR, mass spectral data, and elemental analysis elucidated the structures of all the newly synthesized compounds.

The area to the southeast of the Western Desert of Egypt has been subjected to considerable development activities over the last few years. The development includes the cultivation of about 2260 km2 of the desert lands “the well-known Toshka Project”. The hydrogeological conditions of the area are subjected to detailed investigation based upon the construction of the water table maps, hydrologeologic cross-sections, pumping tests, aquifer geometry, and recharge-discharge relationship. The study revealed that the Quaternary and the Nubia sediments are the main water bearing layers in the area. The Quaternary aquifer is of limited potential and made of mixed sand with clay deposit ranges in thickness between 5 to 10 m. The Nubia aquifer is the oldest sedimentary formation and the main groundwater resources in the area. It is represented by multilayered of sand and silt exists generally under artesian conditions. It is composed of three water bearing horizons partially separated by two confining horizons and extends in thickness ranges between 70 and 230 meters. The thickness increases away from the high dam lake. The analysis of pumping tests of the aquifer indicated that its potentiality is increasing north of the High Dam Lake (HDL) whereas it decreases in the other direction. This is due to high hydraulic conductivity and aquifer thickness in the area northeast of Khor Toshka and at west of Garf hussein. The hydraulic conductivity of the aquifer ranges between 12.73 and 0.9 m/day. The review of the changes in groundwater levels in the area showed that there is a drop in ranges between 1 and 14 meters in the last few years indicating that the extraction from the groundwater is much more higher that the replacement rate. Also, the analysis of the fluctuation of water levels of the HDL and the groundwater level indicated that the influence of water on groundwater level in the area is observed only at a distance less than 10 km from the lake shore line. Today, mammography is the best method for early detection of breast cancer. Radiologists failed to detect evident cancerous signs in approximately 20% of false negative mammograms. False negatives have been identified as the inability of the radiologist to detect the abnormalities due to several reasons such as poor image quality, image noise, or eye fatigue. This paper presents a framework for a computer aided detection system that integrates Principal Component Analysis (PCA), Fisher Linear Discriminant (FLD), and Nearest Neighbor Classifier (KNN) algorithms for the detection of abnormalities in mammograms. Using normal and abnormal mammograms from the MIAS database, the integrated algorithm achieved 93.06% classification accuracy. Also in this paper, we present an analysis of the integrated algorithm’s parameters and suggest selection criteria.
