Background: Confusion often arises in caring for diabetic foot infections and ulcers, especially with antimicrobials;we aim to shed light on this entity and alert healthcare workers to its stewardship. Methods: Records were reviewed between February 2016 and September 2023. Data for patients diagnosed with diabetes and foot ulcers, infected or not, were examined following ICD 9 search terms. Records for patients were included if they were prediabetic/diabetic adults with foot ulcers, more than 18 years old, and on antidiabetic treatment. Patients were excluded if they insulin resistant, with normal HgbA1c levels, wheel-chair dependent, bed-bound, non-diabetic patients, diabetic patients who had vascular lower limb surgery earlier to ulcers, diabetic patients who had aortocoronary bypass, deep venous thrombosis within six months, malignancy, and severe clinical depression. A modified IWGDF/IDSA guidelines definitions for DFI and DFU was considered. Statistical analysis was done using R programming. Statistical methods were employed as appropriate, and a significant P-value was considered for P Results: Most characteristics were well balanced between DFI and DFU, on imaging osteomyelitis and tissue swelling were significantly more in DFI. Endovascular radiological procedures showed angiograms to be considerably more in DFI, while angioplasty was more in DFU, in addition to smoking. Bacteremia was uncommon, and swab cultures were mostly polymicrobial in both ulcers;no clear association with blood bacteria was detected with the polymicrobial growth, though few were concordant. Antimicrobials prescribed for both ulcers were not statistically different except for carbapenems, which were more in DFI (P Conclusion: Attention should be paid to best practices while caring for diabetic ulcers. These include swab culture interpretations, the use of antimicrobials, and plan management according to DFI or DFU to utilize either local care or combination with antimicrobials.

Surgical Management of Mitral Valve Infective Endocarditis with Accidental Finding of Open Pulmonary Tuberculosis

Objective: This case report aimed to highlight intersections of TB and Cardiovasular diseases which carry high morbidity and mortality rates. Methods: We are reporting the surgical management of forty seven years female who had back ground history of IDDM (Insulin dependent diabetic Mellitus), ESRD (End stage renal disease) on HD (haemodialysis) also she had left subclavian artery stenosis, and paroxysmal atrial fibrillation. She was diagnosed with mitral valve infective endocarditis and found accidently to have an open pulmonary tuberculosis (TB) on the day before surgery. Results: She was started on first line anti-TB treatment. She was isolated in her private room and airborne precautions measures applied. The patient underwent a tissue Mitral Valve replacement and tricuspid valve repair annuloplasty. Special precautions were applied in Theatre and on cardiopulmonary bypass Machine guided by KAMC-J disinfection protocol. The patient made good recovery postoperatively. She was discharged well on day 7 post operatively. Conclusion: Intersections of TB and cardiovasular diseases carry high morbidity and mortality rates. Early diagnosis and early anti tuberculosis treatment can surely improve the patient prognosis. Our decontamination and disinfective procedures are recommended. Cases like this should be monitored long term for the development of further cardiovascular complication.

Four isolates of Bacillus subtilis coded,B4,B7,B8 and B10 were examined as biocontrol agents for their abilities and antagonistic effect on the in vitro growth of certain phytopathogenic fungi of peanut,Rhizoctonia solani and Sclerotium *** subtilis isolate B4(GenBank accession ***150884)was the highly effective one for inhibiting the fungal mycelial *** fermentation of *** isolate B4 was carried out and the maximum biomass achieved was 4.53 g L-1 at 11 *** subtilis isolate B4 was formulated and evaluated as a biofungicide to reduce peanut soil-borne diseases under greenhouse and field conditions at the side of Rizolex-T(fungicide)as *** by formulated plant growth-promoting rhizobacteria(PGPR)*** B4 and Rizolex-T in a soil infested with ***,*** and mixture of them were more effective in decreasing percentage of damping-off,root and pod rot disease incidence(%)in greenhouse and open field environment during the two seasons 2015 and *** by PGPR gave highly dry weight and number of healthy pods compared to control of fungi treatment which was nearby to dry weights of healthy pods achieved by treatments by Rizolex-T in a soil infested with ***,*** and mixture of *** PGPR *** B4 gave higher increasing of yield percentage than treatment by Rizolex-T in the two evaluated seasons 2015 and *** can conclude that the produced bioforumlated agent was more efficient as fungicide when compared with the other chemical synthesized fungicides,safe for human and the environment and economy.

Agents for Smart Power Grids

The future of electricity systems will compose of small-scale generation and distribution where end-users will be active participants with localized energy management systems that are able to interact on a free energy market. Software agents will most likely control power assets and interact together to decide the best and safest configuration of the power grid system. This paper presents a design of agents that can be deployed in real-time with capabilities that include optimization of resources, intensive computation, and appropriate decision-making. Jordan 51-bus system has been used for simulation with a total generation capacity of 4050 MW of which 230 MW represents renewable energy. The economic analyses demonstrated the use of smart grid technologies with 2016 generation—load profiles for nominal liquified gas (NLG) prices and ±20% sensitivity analysis. The results have shown variations in the range of 1% in the price of MWh with smart grid technologies. These variations are mainly driven by the fact that agents shift power generation to renewable power plants to produce maximum power at peak hours. As a result, there is a positive economic impact in both NLG ± 20% sensitivity analysis, due to the fact that agents coordinate to better displace expensive thermal generation with renewable generation. It is evident that renewable resources compensate for power at peak times and provide economic benefits and savings.

Comparative Analysis of the Separation Variation Influence on the Hydrodynamic Performance of a High Speed Trimaran

This paper numerically investigates the influence of separation variation of the outriggers on the hydrodynamic performance of a high speed trimaran （HST） aiming at improving its applicability in diverse realistic disciplines. The present investigation was performed within the framework of the 2-D slender body method （SBM） by calculating the resistance of three symmetric trimaran series moving in a calm free surface of deep water. Each trimaran series comprises of 4681 configurations generated by considering 151 staggers （-50%≤a≤＋ 100%）, and 31 separations （100%≤β≤400%） for 81 Froude numbers （0.20≤Fn≤ 1.0）. In developing the three trimaran series, Wigley-st. AMECRC-09, and NPL-4a models were used separately for both the main and side hulls of each individu;d series models. A computer macro named Tri-PL was created using the Visual Basic for Applications~. Tri-PL~ sequentially interfaced Maxsurfe then Hullspeed to generate the models of the three trimaran series together with their detailed hydrostatic particulars, followed by their resistance components. The numerical results were partially validated against the available published numerical calculations and experimental results, to benchmark the Tri-PL macro and hence to rely on the analysis outcomes. A graph template was creaLed within the framework of SigmaPlot to visualize the significant results of the Tri-PL properlv.

The antioxidant status of the plasma in patients with breast cancer undergoing chemotherapy

The aim of the present study was to investigate the status of oxidative stress in the serum of patients affected with cancer breast. Changes in the levels of total antioxidant capacity (TAC), uric acid, malondialdehyde (MDA), nitric oxide (nitrite/nitrate), cupper and iron were measured in serum of patients affected by non-metastatic as well as metastatic cancer breast. Significant decrease in TAC (32.7-37.5%), uric acid (28.1%-49.2), MDA (20.7%-25.2%) and nitric oxide (50.4%-61.9) were found in both groups of cancer breast patients compared to the control group. Serum Cu++ concentrations were significantly lower in metastatic cancer patient group compared with both control and non-metastatic cancer groups. Fe++ in serum was significantly lower in patients with non-metastatic cancer compared to normal subjects and patients with metastatic cancer. Significant differences were also observed between patients with non-metastatic and metastatic cancer breast as regards serum uric acid, nitric oxide were higher were observed in non-metastatic compared with metastatic cancer breast.

Evaluation of Surgical Outcome after Resection of Pancreatic Tumors, Our Experience in Mansoura Oncology Center, a Middle-Volume Center in Egypt

Background: Pancreatic cancer is the fourth most common cause of cancer related deaths in the world. Surgical resection remains the only potentially curative treatment for adenocarcinoma of the pancreas;only 10% - 20% of patients are candidate for standard pancreatic resection. Objective: To evaluate perioperative mortality, morbidity and survival for patients underwent PD in middle-volume center. Patients and Methods: Sixty patients with pancreatic tumors were enrolled in this study for different surgical procedure according to type of tumors. Results: No early postoperative complications were observed in 60% of patients, whereas 40% of patients developed one or more complication, the median survival for this group was 12.3 months with standard deviation 3.8 months.

Synthesis of Novel Series of Benzothieno [2,3-d] Pyrimidine Derivatives, Promising Anticancer Agents

An extension of the authors＇ previous discovery of in vitro antitumor activity of substituted thino [2,3-d] prymidine derivatives is reported. The synthesis of some new spirothino [2,3-d] prymidine （4a-f）, imidazolidin, substituted prymidinyl and substituted thiazolidine thino [2,3-d] prymidine derivatives have been described. Thirteen of the obtained compounds were selected by the NCI and evaluated for their in vitro anticancer activity. Seven of the investigated compounds, 4a, 8a, 9a, （12a, b）, 14a and 15a, displayed high anticancer activity in the primary assay. These compounds have been selected for a full anticancer screening against a 60-cell panel assay where they showed non-selective broad spectrum and promising activity against all cancer cell lines. Compounds 12a and 12b proved to be the active members in this study compared to 5-fluorouracil and cyclophosphamide as reference drugs, respectively. Compounds 12a and 12b were identified as promising lead compounds, evaluated for their in-vitro antitumor activity.
