Assessment of KL-6 as a tumor marker in patients with hepatocellular carcinoma

AIM： To investigate the clinical significance of KL-6 as a tumor marker of HCC in two different ethnic groups with chronic liver disease consecutively encountered at outpatient clinics. METHODS： Serum KL-6 was measured by the sandwich enzyme immunoassay method using the KL-6 antibody （Ab） as both the capture and tracer Ab according to the manufacturer＇s instructions （Eisai, Tokyo, Japan）. Assessment of alpha fetoprotein （AFP） and protein induced vitamin K deficiency or absence （PIVKA-II） was performed in both groups using commercially available kits. RESULTS： A significantly higher mean serum KL-6 （556±467 U/L） was found in HCC in comparison with non-HCC groups either with （391±176 U/L; P〈0.001） or without （361±161 U/L; P〈0.001） liver cirrhosis （LC）. Serum KL-6 level did not correlate with either AFP or PIVKA-II serU/Levels. Using rec：eiver operating curve analysis for KL-6 as a predictor for HCC showed that the area under the curve was 0.574 （95%CI = 0.50-0.64） and the KL-6 level that gave the best sensitivity （61%） was found to be 334 U/L but according to the manufacturer＇s instructions; a cut-off point of 500 U/L was used that showed the highest specificity （80%） in comparison with AFP and PIVKA-II （78% vs 72% respectively）. Combining the values of the three markersimproved specificity of AFP for HCC diagnosis from 78% for AFP alone; 93% for AFP plus PIVKA-II to 99% for both plus KL-6 value （P〈0.001）. Mean serum alkaline phosphatase level was significantly higher in KL-6 positive （564＋475） in comparison with KL-6 negative （505＋469） HCC patients （P = 0.021）, but such a difference was not found among non-HCC corresponding groups. CONCLUSION： KL-6 is suggested as a tumor for HCC. Its positivity may reflect HCC-associated cholestasis and/ or local tumor invasion.

Eco-toxicity and health risk assessment of polycyclic aromatic hydrocarbons in surface sediments of Burullus Lake in Egypt

Polycyclic aromatic hydrocarbons(PAHs)are ubiquitous environmental contaminants of growing concern due to their potential ecological and human health *** study presents a comprehensive assessment of PAHs contamination in the surface sediments of Burullus Lake,a vital and second largest delta lake in *** aim was to evaluate the eco-toxicity and potential health risks associated with the presence of these *** seven sediment samples were collected from various drains in the southern part of Burullus *** extraction method was employed to extract PAHs(16PAHs)from the sediment ***,target compounds were located using *** results showed that samples contained PAHs levels ranging from 0.038×10^(-6)to 0.459×10^(-6),which is considered heavily polluted by the European standard for PAHs ***,there was no apparent source of PAHs in the elKhashah drain or the Brinbal Canal,as HPLC found none of the *** most prevalent compound in sediment samples along the study area was *** diagnostic indices in the present study indicated that the hydrocarbons in the region originated from pyrolytic and man-made sources along the drains of Burullus *** principal component analysis(PCA)and diagnostic ratios revealed that coal combustion and pyrolytic sources were responsible for the PAHs contamination in the surface *** non-carcinogenic risk(HI),which is the product of the HQs for the adult and child populations,respectively,was *** values under 1,therefore,demonstrated that they had no carcinogenic effects on human *** and MEQs in the sediments of Burullus Lake do not have a cancer-causing impact on *** the safety of nearby wildlife,aquatic life,and people,all activities that raise petroleum hydrocarbon levels in Burullus Lake must be adequately regulated and *** to the ecological risk assessment,there is little chance that PAHs will be found

OBJECTIVE:To characterize the chemical profile of methanolic crude extract and its fractions(Ethyl acetate,n-butanol and aqueous)using liquid chromatographymass spectrometry(LC-MS)analysis,to evaluate their biological and pharmacological properties:antioxidant(1,1-diphenyl-2-pycrylhydrazyl(DPPH),2,2′-azino-bis(3-ethylbenzothiazoline-6-sulphonic)(ABTS·+),galvinoxyle free radical scavenging,reducing power,phenanthroline andβcarotene-linoleic acid bleaching assays),enzymes inhibitory ability against several enzymes[acetylcholinesterase(AChE),buthyrylcholinesterase(BChE),urease and tyrosinase].METHODS:Secondary metabolites were extracted from Tamarix africana air-dried powdered leaves by maceration,the crude extract was fractionated using different solvents with different polarities(Ethyl acetate,n-butanol and aqueous).The amount of polyphenols,flavonoids and tannins(hydrolysable and condensed)were determined using colorimetric assays.A variety of in vitro biochemical tests were carried out to assess antioxidant and oxygen radical scavenging properties using DPPH,ABTS,galvinoxyle free radical scavenging,reducing power,phenanthroline andβcarotene-linoleic acid bleaching *** effect was examined against acetylcholinesterase and buthyrylcholinesterase *** anti-urease and antityrosinase activities were performed against urease and tyrosinase enzymes *** extract’s components were identified using LC-MS and compared to reference ***:The results indicated that Tamarix africana extracts presented a powerful antioxidant activity in all assays and exhibited a potent inhibitory effect against AChE and BChE as well as urease and tyrosinase ***-MS analysis identified amount of eight phenolic compounds were revealed in this analysis;Apigenin,Diosmin,Quercetin,Quercetine-3-glycoside,Apigenin 7-O glycoside,Rutin,Neohesperidin and Wogonin in methanolic extract and its different fractions of Tamarix africana from ***

Genotypic characterization of Helicobacter pylori isolates among Egyptian patients with upper gastrointestinal diseases

Objective： Over 50% of the world populations are infected with Helicobacter pylori （H. pylori）. Most subjects are asymptomatic; however, in 1994, H. pylori has been categorized as group I carcinogen. The aim of the study was to investigate the relationship between H. pylori infection and gastric cancer. Methods： Thirty gastric cancer patients （GCs） and 30 gastritis patients were enrolled in the study. H. pylori was cultured on non-selective and selective medias, infection density was assessed by quantitative culture. Antibiotic sensitivity testing was performed. PCR was done for the H. pylori 16S rRNA gene in addition to cagA, vacA and iceA genes. Results： H. pylori could be cultured from 100% of specimens obtained from all patients. The density of H. pylori was higher in cancer cases than in gastritis patients. The 16S rRNA was detected in all GC patients （100%） while it was only detected in 70% of gastritis patients. The cagA gene was found in 53.3% vs 13.3% of GC and gastritis patients, respectively. The vacA gene was present in all GC patients （by at least one of its alleles） while it was only found in 33.3% of gastritis patients. The vacA slml combination was the most predominant genotype in GC patients, while m2 was the commonest allele in gastritis patients （10%）. The iceA gene was found in 86.7% vs 40% of GC and gastritis patients, respectively. Simultaneous presence of multiple H. pyiori strains was proved, both phenotypically and genotypically. Conclusion： The development of GC is linked to infection with H. pylori harboring certain virulence genes. Higher infection density of H. pylori was found in GC patients. Co-existence of more than one strain of H. pylori in the same patient occurs in both malignant and benign lesions.

Landscape Evaluation Based on Gaofen Satellite in the Southern Part of the Nile Delta, Egypt

Landscape segmentation and classification is fundamental to landscape research because it provides an important frame of reference for researchers to communicate and compare their work. Anthropogenic human activities mainly lead to landscape changes. The present study aims to assess the impact of anthropogenic activities on landscape classification of the Nile Delta using remote sensing and GIS techniques. Field survey, digital databases and GIS capabilities are applied for landscapes classification. Vector data using a lot of maps and raster data using satellite image have the ability to give obvious classification about landscape. Results showed that the anthropogenic impacts affect negatively on the landscape classification. Using GF2, landscapes are classified into major eight classes: cultivated land, garden land, woodland, grassland, bare land, urban land, water bodies and mining land. It was showed that the urban occupies the highest percentage of the study area. Urban construction and development areas centered on the capital Cairo city and the city of Giza are dumbbell-shaped to the east. Bare lands occupy the second percentage of the study area, and it may be distributed on around the Nile Delta, southeast of Cairo City and southwest of Giza City. According to vegetation cover, three classes were applied as the sequence: Cultivated land > Garden land > Grass land. These classes depend mainly on the River Nile. Vegetation cover may be based mainly on the water from the Nile River. In addition, mining land occupies the least percentage of the study area. The main distribution of mines and mineral exploration is also very small, but it is distributed on the edge of the city. Landscape metric as Fractal Dimension (Frac) and the Square Pixel (SqP) was applied to validate the segmentation and classification. Design of a Microcontroller-Based Data Acquisition System for Ground Weather Observations: Evaluation of Radio Refractivity of Air

A meteorological data acquisition system is built using a PIC16F877A microcontroller and electric output-based sensors. This system is programmed in C language using the PCWH compiler. It is smart, accurate, cheap, small, light and easily movable. It is designed to automatically get instantaneous measurements of air temperature, atmospheric pressure, and relative humidity. Depending on their utilisation, these measurements are collected with repetition rate tuneable from one minute to one hour. They are displayed on LCD screen and transferred to a personal computer where they are stored and processed. As an application, such data are recorded in situ and used to estimate the radio refractivity of air near the ground in real time. This parameter is necessary to account for the propagation of radiofrequency waves through the lower atmosphere, during wireless transmissions.

The two most important challenges facing banks today are attracting new customers and retaining their existing ones. Research shows that 30 percent of banks cited customer loyalty as their biggest challenges. Thus, given that customer loyalty is completely connected to customer delight. The challenging question is: How do banks achieve customer delight by making every interaction a pleasant experience? In our viewpoint “The key is to stop treating customers as segments and personalize all customer interactions and services which can be achieved by using the latest technological advancements in Big Data Analytics, Artificial Intelligence (AI) and Machine Learning”. With the rapidly increasing usage of social media like Facebook, Twitter, LinkedIn, and Instagram, business organizations are now moving towards adapting this technology to drive business advantages. This research will explore the power of social media and how it can be used by banks to provide an edge over their competitors by providing improved products and services to their customers thereby making their experience easy and responsive. It also proposes a framework for social media analytics and its important components to address all the technical and business aspects of the retail and online banking, however, what customer expects from this medium and what banks offer to them needs to be widely studied and understood.

Timed Intercourse versus Intrauterine Insemination with Mild Ovarian Stimulation for Unexplained Infertility

Background: Infertility is unexplained in about 22% - 28% of infertile couples. Spontaneous pregnancy may occur in them. If did not happen, Expectant management will be via clomiphene citrate (CC) administration, intrauterine insemination (IUI), and IVF & ICSI. Aim: to assess the effectiveness of intrauterine insemination with mild controlled ovarian stimulation compared with expectant management in couples with unexplained infertility more than one year. Methods: 160 couples with unexplained infertility were selected, The couples were randomly divided into two groups: Group (A) “80 couples”: Intrauterine insemination (IUI) with mild controlled ovarian stimulation (combination of CC + hMG) using prepared semen and was performed 36 hours after hCG injection, Group (B) “80 couples”: Couples had no ovulation induction. They encouraged for timed intercourse in the most fertile days of female cycle guided by folliculometry for 6 months. Data were collected quantitatively, coded and analyzed using SPSS. The power of study is 80% and 95% confidence interval. Result: The pregnancy rate was calculated in both groups: In group (A) (IUI/COS): OPR (Ongoing pregnancy rates) was 27.5% and PR (pregnancy rates)/cycle was 8.6%. In group (B) (Expectant management): OPR was 25% and PR/cycle was 5.3%. Conclusion: Mild controlled ovarian stimulation CC + hMG with IUI offers no statistical significance in terms of pregnancy outcomes over expectant management in this study however significance in the response to stimulation.
