Objective: To report ocular complications of Rift Valley fever (RVF) during its first reported outbreak in southwest Saudi Arabia in autumn 2000. Design: Cross- sectional study of patients in a referral hospital. Participants: One hundred forty- three consecutive patients with confirmed RVF serologic test results and ocular lesions were enrolled in the study. Methods: Hospitalized patients (n=30) and outpatients (n=113) with clinical symptoms consistent with RVF, positive RVF serologic test results, and ocular abnormalities were studied. Ophthalmologic examinations, including fundus photography and fluorescein angiography, were performed. Patients were followed up at regular intervals to determine the prognosis and outcome of identified ocular abnormalities. Main Outcome Measures: Visual acuity at initial presentation and course of anterior and posterior segment complications. Results: Among 143 patients (78% males; mean age,53.2 years), 212 eyes were affected, comprising 47 eyes in 30 inpatients and 165 eyes in 113 outpatients. The mean interval between the onset of RVF and visual symptoms ranged from4 to 15 days (mean, 8.8 days). Macular or paramacular retinitis was identified in all the affected eyes (n=212) at the time of initial assessment. Lesions included retinal hemorrhages (40% ), vitreous reactions (26% ), optic disc edema (15% ), and retinal vasculitis (7% ). Anterior uveitis was present in 31% of outpatients. Fluorescein angiography of the retinitis showed early hypofluorescence with late staining of retinal lesions and blood vessels. Initial visual acuity was less than 20/200 in 80% of eyes in the outpatient group; their vision improved, deteriorated, or remained the same in 13% , 15% , or 72% , respectively. evaluation at the last follow- up showed macular (60% ) or paramacular (9% ) scarring, vascular occlusion (23% ), and optic atrophy (20% ) in the outpatient group. bACKGROUND Distal humerus e.b.w fractures are one of the most common traumatic fractures seen in pediatric patients and present as three main types:Supracondylar(SC),lateral condyle(LC),and medial epicondyle(Me)*** To evaluate the epidemiology of pediatric distal humerus fractures(SC,LC,and Me)from an American insurance claims *** A retrospective review was performed on patients 17 years and younger with the ICD 9 and 10 codes for SC,LC and Me fractures based on the IbM Truven MarketScan®Commercial and IbM Truven MarketScan Medicare Supplemental *** from 2015 to 2020 were queried for treatments,patient age,sex,length of hospitalization,and *** A total of 1133 SC,154 LC,and 124 Me fractures were *** fractures had the highest percentage of operation at 83%,followed by LC(78%)and Me fractures(41%).Male patients were,on average,older than female patients for both SC and Me *** In the insurance claims databases used,SC fractures were the most reported,followed by LC fractures,and finally Me *** was identified to be a factor for how a pediatric distal humerus fractures,with patients with SC and LC fractures being younger than those with Me *** peak age per injury per sex was similar to reported historic central tendencies,despite reported trends for younger physiologic development.

background:Severe trauma is associated with systemic inflammation and organ *** rodent trauma models are the mainstay of postinjury research but have.b.en criticized for not fully replicating severe human *** aim of this study was to create a rat model of multicompartmental injury which recreates profound traumatic ***:Male Sprague-Dawley rats were subjected to unilateral lung contusion and hemorrhagic shock(LCHS),multicompartmental polytrauma(PT)(unilateral lung contusion,hemorrhagic shock,cecectomy,bifemoral pseudofracture),or na?ve ***,plasma toll-l ike receptor 4(TLR4),hemoglobin,spleen to body weight ratio,bone marrow(bM)erythroid progenitor(CFU-GeMM,bFU-e,and CFU-e)growth,plasma granulocyte colony-stimulating factor(G-CSF)and right lung histologic injury were assessed on day 7,with significance defined as p valuesesults:Polytrauma resulted in markedly more profound inhibition of weight gain compared to LCHS(p=0.0002)along with elevated plasma TLR4(per hemoglobin(penlarged spleen to body weight ratios(p=0.004).both LCHS and PT demonstrated suppression of CFU-e and bFU-e growth compared to naive(pelevated in PT compared to both na?ve and LCHS(pemonstrated significant histologic right lung injury with poor alveolar wall integrity and interstitial ***:Multicompartmental injury as described here establishes a reproducible model of multicompartmental injury with worsened anemia,splenic tissue enlargement,weight loss,and increased inflammatory activity compared to a less severe *** may serve as a more effective model to recreate profound traumatic injury to replicate the human inflammatory response postinjury.
