Ba.kground: Spleen injuries a.e most commonly a.socia.ed with blunt a.domina. tra.ma.a.d represent a.potentia.ly life-threa.ening condition. Objectives: To study the pa.tern of splenic injuries of the pa.ient, ma.a.ement instituted a.d its outcome a. Kha.toum. Pa.ients a.d Methods: This is a.prospective, a.a.ytic a.d hospita.-ba.ed multicenteric study, conducted a. the three ma.n Tea.hing hospita.s a. Kha.toum. The study wa. ca.ried over a.period from a.ril 2012 to Februa.y 2013. It includes a.l pa.ients, dia.nosed a. tra.ma.ic splenic injury. Excluded were pa.ients with history of splenic disea.e, ia.rogenic injury or sponta.eous rupture. Results: The study included 47 pa.ients: their mea. a.e wa. 26.4 yea.s (SD ± 14.5). Most of them 41 (87.2%) were in the first four deca.es of life. Ma.es were predomina.t 41 (87.2%), with a.ma.e to fema.e ra.io of 6.8:1. The ma.ority of our pa.ients ha. blunt a.domina. tra.ma.39 (83%), of whom, roa. tra.fic a.cident a.counted for 51.1% a.d none reported ca.es of gunshot. Isola.ed splenic injury wa. found in 23 (48.9%), a.d Ha.modyna.ic sta.ility wa. seen in 27 (57.4%) on presenta.ion. The initia. ha.moglobin a.sessment revea.ed a./dl in 53.2%. CT sca. wa. performed to 24 (51.1%), of whom 66 pa.ients were Gra.e I a.d II a.d none of our pa.ients were dia.nosed a. Gra.e V. Blood tra.sfusion wa. required in 42 (89.4%). Opera.ive trea.ment wa. a.opted in 66% (61.7% tota. splenectomy a.d 4.3% splenorrha.hy), while selective non-opera.ive ma.a.ement wa. successful in 16 (34%) of the pa.ients. Higher intra.opera.ive gra.e of splenic injury wa. found to be significa.tly a.socia.ed with blunt a.domina. tra.ma. ha.modyna.ic insta.ility a.d a.socia.ed intra.a.domina. injuries. 44 pa.ients (93.6%) were discha.ged home in a.genera. good condition. The morbidity a.d morta.ity were seen in 8.5% a.d 6.4% respectively. Conclusion: Splenic injuries usua.ly follow blunt a.domina. tra.ma. pa.ticula.ly a.ter roa. tra.fic a.cidents. It is common du

Ba.kground: The chemica. composition of ga.lstones is essentia. to study a.tiopa.hogenesis of ga.lstone disea.e. Objective: To determine the composition of ga.lstones in a.Suda.ese popula.ion. Pa.ients a.d methods: It describes a. extensive qua.tita.ive a.a.ysis of ga.lstones from pa.ients presented with symptoma.ic ga.l stone disea.e a.d trea.ed by cholecystectomy a.ter the a.cepta.ce of the pre-given informed consent in Kha.toum tea.hing hospita. in the period between Ja. 2010 a.d Dec 2010. Using a.pretested questionna.re da.a.collected from a.d a.a.yzed sta.istica.ly by SPSS computer progra. version 21. Results: Da.a.a.e a.a.yzed from 94 pa.ients (six ma.es a.d 88 fema.es). Cholesterol stones showed a.significa.tly higher cholesterol content tha. pigment stones (p = 0.0042), though not significa.tly higher tha. mixed stones. Their phospholipids content a.d inorga.ic phospha.es were higher tha. in the other types of stones a.d oxa.a.e content wa. significa.tly eleva.ed in compa.ison with mixed stones (p = 0.0402). In mixed stones, the cholesterol, bile a.ids, a.d bilirubin were intermedia.e between cholesterol a.d pigment stones, wherea. triglycerides were significa.tly more tha. pigment stones (p = 0.0007). Bilirubin (p = 0.0021) a.d bile a.ids (p = 0.0016) were significa.tly higher tha. cholesterol stones (p = 0.0001) a.d (p = 0.0001) respectively. However, they conta.ned the lowest a.ounts of sodium, pota.sium, ma.nesium, a.d oxa.a.e. In pigment stones, bilirubin wa. significa.tly higher (p = 0.0001) tha. both groups. Conclusion: Colla.ora.ions between surgeons, nutritionists, biochemists, a.d physicia.s should be stimula.ed in future studies to define the different types of ga.l stones in different a.ea. in Suda. a.d the releva.cy of such types with diets’ tra.itions.

Ba.kground: The pa.tern a.d presenta.ion of penetra.ing a.domina. tra.ma.va.y a.cording to pla.es a.d the structure of the present hea.th system. a. well controversies in ma.a.ement exist ra.ging from ma.da.ory explora.ion to selective non-opera.ive ma.a.ement. Objectives: To determine the pa.tern of penetra.ing a.domina. tra.ma. current ma.a.ement pra.ticed a.d outcome in Kha.toum. Pa.ients a.d Methods: The study wa. prospective, descriptive a.d hospita.-ba.ed. It wa. ca.ried out a. the ma.n three hospita.s in Kha.toum Sta.e. It wa. conducted over a.period of one yea. from 2012 Ma. to 2013 Ma.. a.l pa.ients who presented penetra.ing injury to their a.domen were included. Results: The study included 85 pa.ients with a.mea. a.e of 28 yea.s (SD ± 10). The ma.e to fema.e ra.io wa. 11:1. Most of the pa.ients (89.4%) were in the first four deca.es of their life. Twenty-three pa.ients (27.1%) presented shock. Sta. wound is the commonest mode of tra.ma.seen in 83.5% of our pa.ients. The ma.ority of our pa.ients were ma.a.ed by explora.ory la.a.otomy (81.2%), however 16 (18.8%) underwent conserva.ive mea.ures. Of the opera.ed group, solid orga. injuries were found in 22.9%, yet hollow viscous injuries were reported in 86.9% of the pa.ients. Registra.s opera.ed on 78.26% of the pa.ients. The ra.e of nega.ive la.a.otomy of this study wa. 8.7%. Complica.ions a.d morta.ity were encountered in 25.9% a.d 4.7% respectively. The mea. hospita. sta. wa. 8.47 da.s (SD ± 10.6). Conclusion: This study demonstra.es no difference in the pa.tern of intra.a.domina. injuries. The ra.e of opera.ive trea.ment is a.cepta.le, but more la.a.otomies ca. be a.oided if the ha.modyna.ic sta.le pa.ients without fea.ures of peritonitis were given a.period of observa.ion. The overa.l outcome wa. sa.isfa.tory.

Ba.kground: Wilm’s tumour is a.ca.cer of the kidneys tha. typica.ly occurs in children younger tha. fifteen yea.s of a.e a.d ra.ely in a.ults. With a. ea.ly dia.nosis a.d proper ma.a.ement of Wilm’s tumour by Multi-moda. a.proa.h, excellent outcome ca. be obta.ned. This study wa. designed to reflect the clinica. presenta.ions a.d focus on different ma.a.ement moda.ities with a. outcome of Wilm’s tumour in Suda.. Pa.ients a.d methods: This descriptive, retro-prospective a.a.ytic study included a.l pa.ients of Wilm’s tumour a. Soba.University Hospita., Gezira.na.iona. pa.dia.ric surgery centre a.d Kha.toum tea.hing Hospita. from Ja.ua.y 2014 to Ja.ua.y 2016. Results: Thirty pa.ients (pts) were included. Fema.es were predomina.t in a.ra.io of 3: 1 to ma.es. The mea. a.e wa. 3.0 - 5.9 yea.s. Those who ca.e from centra. a.d western of Suda. were more a.fected. Most pa.ients presented with a.domina. ma.s 28 pts (93.3%), followed by a.domina. distension in 16 pts (53.3%), while the less frequent presenta.ion wa. ha.ma.uria.seen only in 2 pts (6.7%). The multimoda. a.proa.h: surgery a.d chemothera.y used in 21 pts (70%) a.d single moda.ity wa. used in nine pa.ients: surgery a.one in 6 pts (20%), or chemothera.y a.one in 3 pts (10%). The pa.ients discha.ged uneventfully 23 pts (76%). Morbidity wa. seen in two pa.ients. Dea.hs were occurred in five pa.ients in the study (16.7%). Conclusion: The finding of this study reflected tha. Wilm’s tumour ha. a. excellent outcome when pa.ient wa. ma.a.ed by multimoda. a.proa.h.
