Evaluating the Impact of the universal Test and Treat Strategy on the Survival of Patients in the Northwest Region of Cameroon

Introduction: The main outcome of efficiently implemented universal test and treat (UTT) program is improved survival. UTT implementation has been ongoing in Cameroon since 2016 but evaluation data are scarce. This study aims to assess the survival of antiretroviral therapy (ART) patients initiated under UTT in Northwest region of Cameroon. Methods: This retrospective cohort study included HIV-positive patients initiated in 2016 at 27 purposefully selected sites and followed until 2021. Data was anonymously abstracted from ART registers and patients’ charts. Kaplan-Meier survival estimates and Cox model were used to compare the survival of patients initiated under UTT with those initiated otherwise, using stata version 14.0. Results: In total, 2490 HIV-positive patients (median age 42.7 years, 94.7% adults, and 69.0% female) participated in the study. Of 1389 patients with viral load (VL) test results, 55% were initiated on ART late. The VL suppression rate of patients initiated late and those initiated early were similar. During follow-up, 1020 (40.9%) participants censored. The survival curves of patients initiated early on ART and those initiated late were similar during the first 2.5 years of follow-up but significantly (p Conclusions: This study confirms the expected impact of UTT. Programs only need to close existing implementation gaps along the critical pathways (diagnosis and treatment) of UTT, focusing more on males.

Cytomegalovirus enteritis is most usually associated with patients positive for human immunodeficiency virus or immunosuppressed transplant patients. The gastrointestinal tract may be affected anywhere from the esophagus to the colon, but the small bowel involvement is rare. We report a case of cytomegalovirus ileitis in an immunocompetent adult, which was confirmed by histopathologic findings through colonoscopic biopsy.

Crystallinity,polytype,and morphology of clay minerals in the Luochuan loess-palaeosols in Shaanxi Province,northwest China were studied in order to have an insight into their origin using X-ray diffraction(XRD)analysis and scanning electron microscopy(SEM)*** SEM observations showed that the morphology of some illites seems to be lath-shaped *** analysis of illite crystallinity(IC)on the Luochuan section indicated that the origin of illite was related to the rocks of an *** illite in the Luochuan loess-palaeosol section were of 2M1 polytype,but some were of 1M polytype formed by weathering of feldspar in the process of *** in the Luochuan section has undergone both physical and chemical *** results revealed that most illite were of detrital origin related to the source area of an anchizone,but parts of the illite were of an authigenic origin formed during pedogenesis after *** in the samples of the Luochuan section were mainly composed of irregular flaky grains and their crystallinity was *** showed that chlorite had the detrital origin formed by physical *** crystallinity was relatively *** value of CIA on the Luochuan section ranges from 61.9 to 69,and therefore kaolinite could not be formed during weathering and *** results indicated that the kaolinite had a detrital *** of smectite seems to be *** XRD patterns of all samples contained diffraction peaks at 1.5218Å(nontronite)and 1.5052Å(montmorillonite),thus indicating an intermediate composition between trioctahedral and dioctahedral *** smectite crystallinity was very *** results revealed that smectite in the study area was authigenic rather than detrital origin.

Phenolic resin/montmorillonite intercalation composites were prepared by using the methods of pressing intercalation and melt *** and structure of the composites were investigated by using XRD,TG and test of softening *** is indicated that both the pressing intercalation and melt intercalation can be used to prepare the phenolic resin/organo-montmorillonite intercalation *** with phenolic resin,the intercalation nanocomposites have better heat-resistance,higher decomposition temperatures and less thermal ***,these two intercalation methods have different effects on the softening point of the intercalation *** intercalation almost does not affect the softening point of the intercalation nanocomposites,while melt intercalation significantly increases the softening point of the intercalation nanocomposites, probably due to the chemical actions happening in the process of melt intercalation.

The chemical and petrological correlation of metamorphic nappes and klippes overlying the Proterozoic sedimentary units in the kumaun Himalaya is still debated. The Ramgarh and Almora gneisses, not previously distinguished in the Askot Klippe, show distinct field, petrological and chemical signatures markedly similar to the tectonostratigraphic disposition of the Almora Nappe. A negative Eu anomaly in the Ramgarh granitic gneisses indicates lesser plagioclase fractionation while the Eu anomaly in the Almora pelitic gneisses is likely to have been controlled by feldspar crystallization in restites. During the anatexis at > 776°C temperature and >6.6 kbar pressure, the melt moved slightly away to its crystallization sites. The Rb/Sr ratio ?0.54 and Nb ?10 ppm is consistent with the granodioritic composition. The negative Sr anomaly in the underlying Ramgarh granitic gneisses indicates a distinct mantle derived source/plagioclase fractionation with a notable correspondence to other late orogenic granites, particularly the basement Ulleri gneisses from the Nepal Himalaya. Ramgarh gneisses plot in the late-and post-COLG field. The Askot ensemble is likely to be the tectonometamorphically reworked basement, viz. the Ramgarh Group along with its metapelitic cover o f the Almora Group, together comprising southward thrust remnants of the leading edge of the Indian Plate that collided with Tibet during the Tertiary Himalayan orogeny.

航空发动机轴承在高速、高温、高载荷等极端工况下易发生机械故障,为了提前预警,提出了一种基于自适应粒子群优化(Adaptive Particle Swarm Optimization,APSO)算法的最小二乘支持向量机(APSO Least Squares Support Vector Machine,APSO-LSSVM)对滑油系统中轴承磨屑进行在线监测的故障诊断及寿命预测。通过主成分分析法(Principal Components Analysis,PCA)对滑油磨屑信息进行降维处理,构建特征向量,并将特征向量输入APSO-LSSVM模型,对轴承故障状态进行分类并对轴承剩余寿命进行预测。结果表明:使用PCA可以保留数据样本99.9%的信息,同时还能极大地降低数据维度;与遗传算法(Genetic Algorithm,GA)、灰狼优化(Grey Wolf Optimization,GWO)算法、粒子群优化(Particle Swarm Optimization,PSO)算法的支持向量机相比,所提算法因采用了自适应调节粒子移动步幅,在进行轴承状态分类时准确率更高,分类正确率可达95.56%,同时在进行轴承剩余寿命预测时具有较好的准确度和泛化性。
