Multivariate Statistical Analysis of Dominating Groundwater Mineralization and Hydrochemical Evolution in Gao,Northern Mali

Population growth and expanding urbanization have caused persistent shortages and contamination of groundwater resources in Mali,*** increase in groundwater salinity makes it more difficult for residents to obtain drinking water,it is necessary to clarify the causes and control factors of groundwater mineralization in Gao region,northern *** on the analysis of the hydrochemical composition of groundwater in 24 boreholes,Piper and Sch?eller diagrams,principal component analysis(PCA)and hierarchical cluster analysis(HCA)are used to carry out multivariate statistical analysis on the main *** results show that the groundwater samples are weakly alkaline,with pH values ranging from 5.83 to 8.40,and the average values of boreholes are 7.50,*** average electrical conductivity(EC)value is 354.4(μS/cm),and the extreme value is between 124.0 and 1247(μS/cm).Water is usually mineralized and presents nine types of water *** three principal components explain 84.42%of the total variance for 13 *** factor F1(58.85%),the factor F2(16.88%)and the factor F3(8.69%)present for the majority of the total data *** addition,multivariate statistical analysis confirmed the genetic relationship among aquifers and identified three main *** related to groundwater mineralization(F1),clustering related to oxide reduction and iron enrichment(F2),and clustering of groundwater pollution caused by nitrate and magnesium(F3).We found that agriculture,weathering activities and dissolution of geological materials promote the mineralization of *** quality in the Gao region is becoming less and less potable because of increasing salinity.

Clinical and Therapeutic Aspects of Inguinal Hernia in Children in the General Surgery Department of Reference Health Center in Commune I of Bamako Mali

Inguinal hernia in children is a congenital pathology in children linked to the persistence of the peritoneo-vaginal canal in children and the NüCK canal in girls;persisting into adulthood. This work aimed to study inguinal hernia in children in the general surgery department of the Reference Health Center of Commune I of the Bamako District;to determine the frequency of inguinal hernia;describe the epidemiological, clinical and therapeutic aspects of inguinal hernias and in order to evaluate the cost of treatment. This was a prospective study from April 2017 to March 2018 during which 60 children with an inguinal hernia were collected. Inguinal hernias in children represented 9.23% of patients operated on in the department. The average age was 59.23 months with extremes of one month and 180 months and a standard deviation of 49.23 years. The male sex was dominant with a sex ratio equal to 9:1. The notion of prematurity was found in 11.7% and was associated with a testicle not in place in 1.7% of cases. 41.7% of our patients were referred by a doctor and 66.7% of patients presented with painless, intermittent inguino-scrotal swelling (83.3% of cases). The hernia was discovered before one week of life in 50% of cases. Inguinal swelling was absent in 11.7% but observed intraoperatively. The inguinal hernia was unilateral in 91.7% of cases and 70% of hernias were discovered by the parents during pushing efforts. The inguinal hernia was located on the right in 61.7% of cases. The hernia swelling was soft, painless, impulsive and reducible in 78.3% of cases. The swelling was inguino-scrotal in 58.3% of cases. The hernia was simple in 95% of patients and hernial strangulation was observed in 3 cases or 5% of cases. 98.3% of patients were ASA I. The treatment was carried out openly in all our patients including closure of the vaginal peritoneal canal in 95% of cases under general anesthesia in 98.3% of cases. Morning Glory Syndrome in Two (02) Malians: Case Report from IOTA-University Hospital

Background: First described by Handmann in 1929, the “Morning Glory Syndrome” is a rare congenital anomaly of the optic papilla whose notable ophthalmoscopic sign is papillary excavation associated with other ocular and/or non-ocular anomalies. Purpose: The main purpose of this study is to report 2 cases of this rare congenital papilla anomaly in 2 young Malian adults diagnosed in the IOTA-University Hospital respectively in January and May 2020. Materials and methods: In our study, we have included two cases of male gender. Results: In the 1st case the defect concerned both papillae. It is also associated with other ocular malformations (bilateral macular reshaping and unilateral cataract), without associated cerebral defects. In the 2nd case, only one papilla was affected by the malformation and there was an absence of associated malformations of other ocular and cerebral structures. Discussion: The Morning Glory Syndrome is part of the group of congenital defects of the optic nerve. Ultrasound in Ti-Rads Classification of Thyroid Nodules at the Marie Curie Medical Clinic

Introduction: A thyroid nodule is a localized hypertrophy within the thyroid parenchyma. The aim of our study was to study the benefit of ultrasound in the Ti-rads classification of thyroid nodules. Methodology: This was a prospective study with a descriptive aim, with prospective collection, which took place over a period of 17 months at the “Marie Curie” medical clinic. The ultrasound machine used was a Voluson E8 from 2011 and the examinations were carried out by two radiologists and two experienced sonographers. The parameters studied were sociodemographic data;clinical data and ultrasound aspects of the Ti-rads classification in the management of nodules. Results: We collected 235 patients out of 738 patients referred to the service for a cervical ultrasound, i.e. a frequency of 31.84% of cases. There was a female predominance with 95.7% of cases and a sex ratio of 0.04. The average age of our patients was 50 years. We found on cervical ultrasound: Isthmo-lobar glandular hyperplasia in 99 patients, i.e. a frequency of 42.1%. The Ti-rads 3 classification was the most represented in 69.4% of cases. The benignity criterion represented 85.6% of cases in our patients and the malignancy criterion represented 14.4% of cases. Conclusion: The precise description of a thyroid nodule provided by ultrasound (Ti-rads) is essential in the management of nodules.

Introduction: Radiology and medical imaging examinations bring together the means of acquiring and restoring images of the human body from different physical phenomena such as the absorption of X-rays, nuclear magnetic resonance, reflection of ultrasound waves or radioactivity, with which we sometimes associate optical imaging techniques such as endoscopy. The objective of our study was to study the profile of radiological examinations in the radiology and medical imaging department of the “KALALE” medical clinic. Methodology: This was a descriptive, prospective study that took place over a period of six (6) months in the radiology and medical imaging department of the “KALALE” medical clinic and the centers of health of communes III and IV of Bamako in Mali. Data collection was carried out on a survey form. The parameters studied were sociodemographic data, clinical data, radiological data. Results: We recorded 6030 radiology examinations of which 19% of the examination frequency cases came from the “KALALE” medical clinic and 81% of the cases were referrals from the reference health centers of communes III and IV. There were 1500 X-rays or 24.9% of cases including 54% cases of bone X-rays;5% of cases of skull and face X-rays;5% of cases of Intravenous Urography (IVU);4% of cases of Hysterosalpingography (HSG);3% of cases of Retrograde Ureterocystography (UCR) and 1% of cases of X-ray of the Abdomen without preparation (ASP). We found 4530 ultrasounds or 75.1% of cases. Conclusion: Medical imaging has become essential in the study of the human body and now constitutes an essential tool for the detection and treatment of most pathologies, from simple fractures to the most serious tumors.

Prevalence of Refractive Errors among School Children Aged 5 to 15 Years Old at CHU-IOTA

Introduction: Undetected refractive errors constitute a health problem among school children who cannot take advantage of educational opportunities. The authors studied the prevalence of refractive errors in school children aged 5 to 15 at CHU-IOTA. Patients and Method: This is a prospective, descriptive cross-sectional study carried out in the ophthalmic-pediatrics department of CHU-IOTA, from October to November 2023. Results: We received 340 school children aged 5 to 15, among whom 111 presented ametropia, i.e. a prevalence of 32.65%. The average age was 11.42 ± 2.75 years and a sex ratio of 0.59. The average visual acuity was 4/10 (range 1/10 and 10/10). We found refractive defects: astigmatism 73.87%, hyperopia 23.87% of cases and myopia 2.25%. The decline in distance visual acuity was the most common functional sign. Ocular abnormalities associated with ametropia were dominated by allergic conjunctivitis (26.13%) and papillary excavation (6.31%) in astigmatics;allergic conjunctivitis (9.01%) and papillary excavation (7.20%) in hyperopic patients;turbid vitreous (0.90%), myopic choroidosis (0.45%) and allergic conjunctivitis (0.45%) in myopes. Conclusion: Refractive errors constitute a reality and a major public health problem among school children.

Introduction: The authors wanted to study the prevalence of ocular trauma in the ophthalmology unit of the CSREF of Kati. Patients and method: This is a retrospective study, covering the period from January to December 2015, carried out in the ophthalmology unit of the CSREF in Kati. Results: The sample consisted of 568 patients, or 6.82% of ophthalmological consultations. Children aged 0 to 14 years accounted for 154 (27.11%) of the trauma cases. Bruises constitute the main circumstance (58.1%) of eye trauma, followed by domestic accidents (15.6%). Closed globe ocular trauma was the main type of trauma (61.6%), followed by ocular adnexal trauma (30.1%) and open globe trauma (8.3%) in which the penetrating wound was the lesion, the least common (2.6%) but the most serious. Medical treatment was the most used therapeutic modality in 82.4% of cases, followed by surgery with 17.6% of cases. The evolution of the lesions after treatment was favorable in 85.6% of cases and unfavorable in 14.4% of cases. Complications were observed in 5.8% of cases and sequelae in 8.6% of cases. Conclusion: Ocular trauma constitutes an important reason for ophthalmological consultation at the CSREF of Kati. The severity of some of these lesions requires frequent recourse to surgery. The complexity of the treatment should encourage us to favor preventive measures.

Secondary Stress Induced Central Serous Chorioretinopathy in Bamako: Report of Two Cases

Introduction: Idiopathic central serous chorioretinopathy (ICSC) is characterized by the buildup of fluid leading to circumscribed elevation of the retina within the posterior pole. This condition affects young people between the ages of 20 - 50 years. The disease is seen predominantly in men as compared to women. We report the cases of two (02) patients received in consultation at IOTA-Teaching Hospital between July 2019 and February 2020. Case Presentation: We report two cases of Idiopathic stress central serous chorioretinopathy. The first case is a 37-year-old man that we received in consultation for visual acuity decrease from a sudden installation for 1 hour. The clinical and paraclinical arguments permitted us to retain the diagnosis of Idiopathic stress central serous chorioretinopathy. The patient benefited from laser treatment and the clinical evolution was good. The second case is a 42-year-old man, who consulted for a sudden reduction of visual acuity that had been evolving for 3 weeks. The clinical and paraclinical contexts were in favour of the diagnosis of Idiopathic stress central serous chorioretinopathy. The patient was treated with spironolactone. The clinical evolution was favourable. Conclusion: Our study confirms the existence of central serous chorioretinopathy (CSC) in Black African patients. Studies show the connection between CSC and stress.

Introduction: Acute coronary syndromes (ACS) are a diagnostic and therapeutic emergency. Objective: Studying the feasibility, difficulties and results of coronary angioplasty in acute coronary syndromes at the Luxembourg Mother-Child University Hospital in Bamako. Patients and Methods: Cross-sectional, descriptive study with prospective recruitment over 18 months from September 2020 to February 2022. All patients aged at least 18 years old admitted for SCA and having undergone PCI during the study period were included. Result: We collected 249 patients for SCA, of whom 160 underwent angioplasty, either an angioplasty/SCA ratio of 0.64. The average age of the patients was 59.54 ± 11.62 with extremes of 32 and 92 years. The age group of 45 to 65 years was the most representative. The predominance was male, sex ratio of 2.4. The main cardiovascular risk factors were high blood pressure (58.23%) and diabetes (45.78%). Persistent ST-segment elevation on the electrocardiogram was present in 71.48%. The treatment time was more than 12 hours after the first medical contact in 95.5%. The approach was radial in 96.5% of cases. Coronarography was pathological in 91.16% of our patients. The lesions were tri-truncular in 34.13% with the anterior inter ventricular as culprit artery in 72% of cases. The majority of patients (64%) had undergone angioplasty with implantation of an active stent. Angioplasty was performed successfully in 98% and per procedural mortality was 1.87%. Only 6.45% of ACS with ST elevation benefited from primary angioplasty. Conclusion: Percutaneous coronary intervention is performed routinely in our center with satisfactory results. Difficulties exist, related to the diagnostic delay of ACS and the high cost of angioplasty.

Despite existing policies on training health professionnels in essential newborn care (ENC), neonatal mortality still remains high in Mali. Our work aimed to assess the level of knowledge of health staff about ENC. Material and methods: From March 20th to April 20th, 2016, we interviewed newborn care providers at the six reference health centers and the Gabriel Touré University Hospital Center in Bamako. Results: In total, we interviewed 407 newborn care providers with a sex ratio of 0.52. Interviewees had over five years work experience in 62.1%. They considered a low Apgar score as an indication for neonatal resuscitation in 89%, regardless of profile (p = 0.1583). They knew the good aspiration technique in 54%, with nurses and midwives more knowledgeable (p ) of the reference health centers (p = 0.0000). The interviewees knew the indication and rate of ventilation in 30.2% and 16.0%, respectively. About one third (34%) thought oxygen administration should be systematic during ventilation. The knowledge level on ventilation was the lowest in the group of general practitioners (p = 0.0063 for oxygen indication and p for the technique). Knowledge level for other ENC components (temperature maintenance, eyes care, breastfeeding) were higher. The knowledge of the delay of the breasting did not correlated with either the profile (p = 0.0857) or the place of practice. The knowledge of the first bath was dependent on both the professional profile (p = 0.0002) and the reference level (p = 0.0238). Conclusion: The level of knowledge of health professionnels on ENC should be improved. This will involve the integration of ENC in initial training curricula along with an appropriate continuing training policy thereafter.
