Determination of riding comfort on cycleways using a smartphone application

Well maintained cycleways will encourage more people to cycle, as the condition of cycleways is important for the safety, accessibility and riding comfort of cyclists. Despite that, only a few models used to describe the quality of service for cyclists take the surface condition into account. Objective measuring methods are needed to enable reliable and effective assessment of surface conditions, and measurable performance criteria related to the needs of cyclists should be developed. The purpose of this study has been to test the reliability and validity of using accelerometers in smartphones to assess the riding comfort on cycleways. A smartphone application converting three-dimensional accelerometer measurements into a single indicator for cycleways has been used to assess road surfaces in two field studies, in Sweden and Norway, respectively. Both studies assessed test sections of varying quality. To relate the measurements to subjective riding comfort assessments by cyclists, recruited cyclists collected quantitative data using the app, whilst also rating their perceived riding comfort by completing a survey. Measurements were also related to standard road surface condition indicators, generated from a road surface tester equipped with 19 laser sensors: international roughness index(IRI), mega-and macrotexture. The results show that it is possible to describe the unevenness of a cycleway using the technology present in smartphones. A software application can be used to collect and analyse data from the acceleration sensors in the phone, which can then be used to describe the riding comfort of cyclists. It is mainly the unevenness in the 50-1000 mm sizerange that create the greatest discomfort for cyclists, and intermittent vibrations are perceived as more uncomfortable than more evenly distributed vibrations. Therefore, IRI is not a relevant measurement for describing the riding comfort of cyclists.

Professional Domiciliary Oral Care for Elderly in Nursing Homes—A Randomized Controlled Pilot Trial

Domiciliary dental care (DDC) makes regular dental visits possible for people with different functional limitations who otherwise would not be able to access a dental clinic. DDC also facilitates cooperation with nursing staff. To our knowledge, the effect of DDC on oral health among elderly people in nursing homes has not been studied. As part of a project to establish recommendations for DDC, the aim of this randomized, controlled, single-blind, pilot trial with three parallel arms was to study the effect of professionally conducted oral care intervention, in domiciliary setting, on gingival bleeding on probing, dental plaque and oral mucosa. For three months, the participants were given either A) monthly professional DDC with oral care intervention, B) monthly individual oral care instructions, or C) oral care as usual. A total of 102 (n = 107) individuals completed the study. After three months, gingival bleeding on probing was significantly reduced among more participants in groups A and B compared with group C (p nal DDC has the most favourable effect on gingival bleeding, whereas individual oral care instructions result in the highest reduction of dental plaque. Both professional oral care interventions and individual oral-hygiene instructions should be included in a domiciliary oralcare programme.

Myeloid-derived suppressor cells(MDSCs)comprise heterogeneous myeloid cell populations with immunosuppressive capacity that contribute to immune regulation and tolerance *** previously reported impaired MDSC function in patients with primary sjögren’s syndrome(pSS)and mice with experimental SS(ESS).However,the molecular mechanisms underlying MDSC dysfunction remain largely *** this study,we first found that aryl hydrocarbon receptor(AhR)was highly expressed by human and murine polymorphonuclear MDSCs(PMN-MDSCs).Indole-3-propionic acid(IPA),a natural AhR ligand produced from dietary tryptophan,significantly promoted PMN-MDSC differentiation and suppressive function on CD4^(+)T *** contrast,feeding a tryptophan-free diet resulted in a decreased PMN-MDSC response,a phenotype that could be reversed by IPA *** functional importance of PMN-MDSCs was demonstrated in ESS mice by using a cell-depletion ***,AhR expression was reduced in PMN-MDSCs during ESS development,while AhR antagonism resulted in exacerbated ESS pathology and dysregulated T effector cells,which could be phenocopied by a tryptophan-free *** regulatory factor 4(IRF4),a repressive transcription factor,was upregulated in PMN-MDSCs during ESS *** immunoprecipitation analysis revealed that IRF4 could bind to the promoter region of AhR,while IRF4 deficiency markedly enhanced AhR-mediated PMN-MDSC ***,dietary supplementation with IPA markedly ameliorated salivary glandular pathology in ESS mice with restored MDSC immunosuppressive ***,our results identify a novel function of AhR in modulating the PMN-MDSC response and demonstrate the therapeutic potential of targeting AhR for the treatment of pSS.

目的探讨原发性干燥综合征(pSS)合并动脉粥样硬化(AS)患者的临床特点。方法回顾性分析2019年1月至2022年1月上海中医药大学附属岳阳中西医结合医院风湿科214例pSS患者的临床资料,根据是否合并AS分为合并组(108例)和未合并组(106例)。比较两组一般资料、实验室指标和颈动脉内膜中层厚度(IMT)的差异,并进行危险因素分析。结果与未合并组比较,合并组年龄[66.00(61.25,69.00)岁vs.59.00(50.75,65.25)岁]更大,病程[128.00(108.25,141.75)个月vs.94.00(74.75,112.25)个月]更长,ESR[27.00(14.10,48.00)mm/h vs.22.00(12.00,36.50)mm/h]、单核细胞[0.40(0.30,0.50)×10~9/L vs.0.30(0.30,0.50)×10~9/L]、白细胞介素-6(IL-6)[3.56(2.17,5.95)pg/mL vs.2.62(1.00,5.15)pg/mL]、白细胞介素-2受体(IL-2R)[519.00(403.00,662.00)U/mL vs.441.00(348.00,592.00)U/mL]、总胆固醇[(4.86±1.17)mmol/L vs.(4.55±1.10)mmol/L]、低密度脂蛋白-胆固醇[(3.07±0.80)mmol/L vs.(2.82±0.78)mmol/L]水平更高,IMT[左侧0.80(0.80,0.90)mm vs.0.80(0.78,0.80)mm,右侧0.80(0.80,0.90)mm vs.0.80(0.80,0.80)mm]更大,差异有统计学意义(P<0.05)。多因素logistic回归分析结果显示,病程长(OR=1.080)、ESR水平升高(OR=1.021)是pSS发生AS的独立危险因素(P<0.05)。结论pSS合并AS患者与未合并AS患者的血清学指标存在差异,病程、ESR水平对其有影响。

Higher frequency of brain abnormalities in neuromyelitis optica spectrum disorder patients without primary sjogren's syndrome

Neuromyelitis optica spectrum disorder often co-exists with primary sjogreffs syndrome. We compared the clinical features of 16 neuro- myelitis optica spectrum disorder patients with （n = 6） or without primary sjogreffs syndrome （n = 10）. All patients underwent extensive clinical, laboratory, and MRI evaluations. There were no statistical differences in demographics or first neurological involvement at onset between neuromyelitis optica spectrum disorder patients with and without primary sjogren＇s syndrome. The laboratory findings of cerebrospinal fluid oligoclonal banding, serum C-reactive protein, antinudear autoantibody, anti-sjogren＇s-syndrome-related antigen A an- tibodies, anti-sjogren＇s-syndrome-related antigen B antibodies, and anti-Sm antibodies were significantly higher in patients with primary sjogren＇s syndrome than those without. Anti-aquaporin 4 antibodies were detectable in 67% （4/6） of patients with primary sjogren＇s syndrome and in 60% （6/10） of patients without primary sj6gren＇s syndrome. More brain abnormalities were observed in patients without primary sj6gren＇s syndrome than in those with primary sj6gren＇s syndrome. Segments lesions （〉 3 centrum） were noted in 50% （5/10） of patients without primary sj6gren＇s syndrome and in 67% （4/6） of patients with primary sjogren＇s syndrome. These findings indicate that the clinical characteristics of neuromyelitis optica spectrum disorder patients with and without primary sjogren＇s syndrome are similar. However, neu- romyelitis optica spectrum disorder patients without primary sjogreffs syndrome have a high frequency of brain abnormalities.

风湿免疫病的发病机制与B细胞异常活化相关。利妥昔单抗是一种特异性耗竭B细胞的单克隆抗体，近年来在风湿免疫病中得到越来越多的应用。为了规范利妥昔单抗在风湿免疫病领域的应用，中国初级卫生保健基金会风湿免疫学分会成立了专家小组，对利妥昔单抗在风湿免疫病治疗中的应用制定了共识建议。本共识共有12条，分别针对利妥昔单抗治疗类风湿关节炎、系统性红斑狼疮、抗中性粒细胞胞质抗体相关性血管炎等自身免疫病、特殊人群应用以及应用的注意事项给出了建议，为我国临床医生规范和安全用药提供参考。

目的:从分子水平探讨IgG4相关性疾病(IgG4-RD)和原发性干燥综合征(pSS)的相关细胞因子特征。方法:获得了17个小唾液腺(9个来自pSS患者,8个来自非pSS患者作为对照)、7个颌下腺(4个来自IgG4-RD患者,3个来自切除的颌下腺肿瘤周围的肉眼正常组织作为对照)的样本Rna-Seq数据,分析IgG4-RD和pSS来源样品中的差异表达基因和富集途径,以揭示这两种疾病的免疫炎症特征。结果:尽管三级淋巴结构(TLS)相关细胞因子(例如CXCL13、CCL19等)的表达显著增加,但对于受累组织的先天免疫反应特征,在IgG4-RD中观察到更显著成纤维细胞相关细胞因子及其受体的表达,巨噬细胞和中性粒细胞相关的细胞因子及其受体在pSS中有更为显著的表达。此外,IgG4-RD受累唾液腺中经典信号通路的富集模式也与pSS不同:在IgG4-RD中,JAK-STAT信号通路、NF-κB信号通路显著富集,而在pSS中,与细胞趋化性相关通路显著富集。结论:在IgG4-RD和pSS之间的转录水平上,相关细胞因子间有不同的分子和生物过程模式,这可能指导其后续的机制研究和药物靶点选择。

sjgren综合征(sjgren syndrome,SS)是一种主要累及外分泌腺、慢性炎症性的自身免疫性疾病,发病初期主要表现为泪液和唾液分泌减少所致的眼干、口干综合征,但在发病的晚期,会出现多系统损害的症状和体征。SS发病机制复杂,其确切发病机制国内外报道较少。目前,随着动物模型和实验检查技术的不断更新,关于SS发病机制的研究取得了新的进展,为早期诊断SS提供了新的理论证据。本文从遗传学、病理生理学方面就SS的发病机制做一综述,以期对SS相关性干眼的诊断和治疗有更新的认识。
