To the editor The interesting report by Redfors, et aL[1] in the June issue of the Journal of Geriatric Cardiology, about an 88-year-old woman who presented with chest pain, found on echocardiography （eCHO） to have transient left ventricular apical akinesis with a hypercontractile base, and an occluded first diagonal coronary branch with suspected acute plaque rupture, on coronary angiography, treated with a percutaneous coronary intervention, for which the authors concluded that the observed extent of akinesis was disproportionally large for the occluded coronary lesion, letting them to deduce that the acute myocardial infarction （AMI） caused the episode of Takotsubo syndrome （TTS）, reminds me of a similar case of an AMI due to stent thrombosis and a resultant associated TTS.

Watching the winds in northwest Iowa during more than 30 summers has led me to two conclusions about the local atmosphere at ground level: there is a net northward transport of heat and water taking place throughout the summer;warm humid winds from the south continually alternate with cool dry winds from the north. The proposed northward heat transfer is consistent with the constraint, placed on the motions of the oceans and the atmosphere, of the earth’s heat balance due to the increased absorption of solar radiation at low latitudes compared to that at high latitudes. At mid-latitudes in the interior of continents, like North America, it is the job of the atmosphere alone to constantly help satisfy the global heat balance. Although qualitative in nature, the predicted northward heat flux is strongly based on frequent observations over lengthy time intervals.

It was a great honour and privilege to be elected President of WASeR during the first Council meeting, held in Beijing last Saturday. I hope that I will be able to fulfil the expectations of those that elected me and I look forward to serving the Association and promoting its development. As a hydrologist/geomorphologist who started working in the field of erosion and sedimentation some 35 years ago, I have long appreciated the need for, and importance of, interaction with those with similar interests working in adjacent disciplines, for example, civil engineers, hydraulic engineers, soil scientists, geochemists, agricultural engineers, soil conservation specialists, watershed managers, estuarine scientists, and increasingly social scientists and environmental managers. I therefore strongly believe that one of WASeR's key roles, and indeed probably it's most important role, should be to facilitate and promote such interaction and interchange.

In vitro Ruminal Gas Production Kinetics of Four Fodder Trees ensiled With or Without Molasses and Urea

This study investigated if the addition of urea （U）, molasses （M） or their 1：1 （v/v） mixture during ensiling increases the nutritional value of forage from four fodder trees （Prunus persica, Leucaena esculenta, Acacia farnesiana, and Prunus domestica）. Forage samples of fodder trees were collected in triplicate （three individual samples of each species） and subjected to an in vitro gas production （GP） procedure. Fermentation at 24 h （GP 24）, short-chain volatile fatty acids （SCFA）, and microbial crude protein production （MCP）, in vitro organic matter digestibility （OMD）, metabolizable energy （me） and dry matter degradability （DMD） were estimated. Forage samples were incubated for 72 h in an incubator at 39oC and the volume of GP was recorded at 2, 4, 6, 8, 10, 12, 24, 48, and 72 h of incubation using the reading pressure technique. The rumen fermentation profiles were highest for P. persica, which showed the highest （P〈0.0001） DMD, me, OMD, SCFA, GP 24 and MCP. On the other hand L. esculenta had the lowest （P〈0.0001） DMD, SCFA, MCP; P. domestica had the lowest （P〈0.0001） OMD. The addition of M to silage increased （P〈0.0001） me and OMD, as well as GP. However, the addition of U and the mixture of U and M reduced （P〈0.0001） DMD, me, OMD, SCFA, GY 24 and MCP. These results show that P. persica has the highest nutritive value and L. esculenta the lowest for ruminants. Additionally, the addition of M to forage from fodder trees increases rumen GP and fermentation, which may improve nutrient utilization in ruminants.

目的:探讨实验性早期糖尿病鼠视网膜水通道蛋白4(AQP4)及其mRNA的表达变化及其意义。方法:用链脲菌素(streptozotocin,STZ)腹腔注射的方法建立大鼠糖尿病模型,运用墨汁灌注实验检测视网膜微血管渗漏情况;用免疫荧光组织化学检测AQP4在视网膜内的分布和表达变化;用原位杂交和RT-PCR检测AQP4 mRNA在视网膜内的分布及表达变化。结果:与同龄正常对照组大鼠相比,早期糖尿病鼠视网膜未见明显血管渗漏现象,但存在部分细胞水肿、节细胞数目减少、感光细胞外节排列紊乱等病理改变;免疫组织化学、原位杂交和RT-PCR结果显示,模型鼠视网膜内AQP4及其mR-NA的表达明显增强。结论:实验性早期糖尿病鼠视网膜,在微血管病变发生前,已存在细胞水肿等病理改变;AQP4及其mRNA的表达明显上调。AQP4表达上调可能是糖尿病早期视网膜水肿形成、视功能下降的重要原因。

目的 :评价含有 1%焦磷酸钠、7%三聚硫酸钠、0 .2 4 3 %氟化钠及 10 %高清洁二氧化硅的牙膏减少外源性牙色斑的效果。方法 :采用双盲、分层、二单位平行观察的临床研究方法。根据改良 L obene色斑指数 (L obene stain in-dex m ethod)均数、性别、吸烟习惯将 91名年龄为 18～ 5 5岁的研究对象平衡地分为两组 ,随机地分配使用试验或对照牙膏。于基线、第 4周及第 8周固定由一名检查者完成研究对象的色斑指数测定。结果 :86人完成了该研究。8周临床研究表明 ,使用美白牙膏的研究对象较对照组的牙齿色斑指数均数降低了 3 7.1% (P=0 .0 0 0 1)。结论 :与对照组牙膏相比 ,使用含有 1%焦磷酸钠、7%三聚硫酸钠、0 .2 4 3 %氟化钠及 10 %高清洁二氧化硅的牙膏可以有效地减少牙外源性色斑。

Introduction: Urinary tract infections (UTI) are frequent and of polymorphous clinical symptomatology in elderly subjects both in and out of hospital. In Mali, to our knowledge, no study concerning UTIs in the elderly has been conducted, hence the interest in this innovative work. Objectives: To determine the prevalence and clinical and paraclinical aspects of urinary tract infections in the elderly. Materials and methods: This was a prospective descriptive and cross-sectional study from September 1, 2013 to August 31, 2014, i.e. duration of 12 months. All patients aged 65 years and over, hospitalized or ambulatory in the internal medicine department with a documented urinary tract infection were included. Results: We collected and examined 194 patients. The cytobacteriological study of urine (CBSU) was positive in 28 patients, i.e. a prevalence of 14.43%. The male sex represented 59.8% of the cases, the sex ratio was equal to 1.46. The age groups between 65 - 69 and 70 - 74 years were the most affected, respectively 28.57% and 39.28%. The main clinical signs were: asthenia, anorexia, dependence, fever, urinary burning, dysuria. Hospitalization was associated with urinary tract infection with P = 0.01. The group of enterobacteria were incriminated in 75% of cases. escherichia coli, Klebsiella pneumoniae and Pseudomonas aeruginosa represented 39.28%, 14.28% and 10.71% of cases. Urinary tract infections were represented by pyelonephritis, acute prostatitis, orchi-epididymitis and simple cystitis. Conclusion: Urinary tract infection is frequent in the elderly, its clinical presentation is polymorphic and enterobacteria are the most incriminated group of bacteria.
