Background:Preoperative patient selection in Associating Liver Partition and Portal vein ligation for Staged hepatectomy(ALPPS)is not always reliable with currently available scores,particularly in patients with primary liver *** study aims to(i)to determine whether comorbidities and patients characteristics are a risk factor in ALPPS and(ii)to create a score predicting 90-day mortality ***:Thirteen high-volume centers participated in this retrospective multicentric study.A risk analysis based on patient characteristics,underlying disease and procedure type was performed to identify risk factors and model the Comprehensive ALPPS Preoperative Risk Assessment(CAPRA)score.A nonparametric receiver operating characteristic analysis was performed to estimate the predictive ability of our score against the Charlson Comorbidity index(CCi),the age-adjusted CCi(aCCi),the ALPPS risk score before Stage 1(ALPPS-RS1)and Stage 2(ALPPS-RS2).The model was internally validated applying ***:A total of 451 patients were *** was 14.4%.The CAPRA score is calculated based on the following formula:(0.1×age)−(2×BSA)+1(in the presence of primary liver tumor)+1(in the presence of severe cardiovascular disease)+2(in the presence of moderate or severe diabetes)+2(in the presence of renal disease)+2(if classic ALPPS is planned).The predictive ability was 0.837 for the CAPRA score,0.443 for CCi,0.519 for aCCi,0.693 for ALPPS-RS1 and 0.807 for *** 1,000 cycles of bootstrapping the C statistic was *** accuracy plot revealed a cut-off for optimal prediction of postoperative mortality of ***:Comorbidities play an important role in ALPPS and should be carefully considered when planning the *** assessing the patient’s preoperative condition in relation to ALPPS,the CAPRA score has a very good ability to predict postoperative mortality.

Management of missile injuries to the maxillofacial region: A case series

AiM: To assess our management of patients suffering from missile injuries to the maxillofacial ***: From December 2009 to September 2012, 40 patients with missile injuries(high velocity gunshot and bullet wounds, explosive injuries and shrapnel etc.) affecting the maxillofacial region were treated. All except for 2 patients were males. All had soft tissue injuries with or without bone injuries. These patients were referred to the plastic and maxillofacial surgery ward of our hospital. The patients were 19 to 65 years of age(mean 45 years). in 19 cases, there were missile injuries to other parts of the body, especially the lower extremities. All of the patients were managed by early soft tissue debridement, comprehensive reconstruction and antibiotics. This retrospective study was approved by the irB and ethical ***: The majority of injuries were caused by high velocity projectiles(88%) and the remaining by car explosions or dynamite blasts(12%). 40 patients were treated surgically. Thirty patients had soft tissue loss(75%) and 20 patients(50%) had bone loss; there was combined soft tissue and bone loss in 10(25%) patients. Facial fractures were in the orbital bones in 10 cases, maxillary in 7, nasal in 5 and the mandible in 3 cases. We used primary repair in the majority of soft tissue defects(25 of 40 cases). Bone repair was done primarily at the same stage using miniplates, titanium screws or wires. in some cases with a bone defect, iliac bone grafts were used simultaneously or in the later stages(mandibular defects). There was no failure of bone reconstruction in our cases. infections occurred in two cases and were treated with systemic antibiotics and dressing changes, without any long term ***: Our principles for soft tissue reconstructions were according to the reconstructive ladder and included primary repair, local flaps, skin grafts and regional flaps depending on the extent of damage. Role of cytomegalovirus on the maturation and function of monocyte derived dendritic cells of liver transplant patients

AiM: To study the impact of association between cytomegalovirus(CMV) pathogenesis with dendritic cell(DC) maturation and function was evaluated in CMV reactivated liver transplanted patients in comparing with non-reactivated ones, and healthy controls. METHODS: Monocyte derived dendritic cells(Mo DCs) was generated from collected ethylenediaminetetraacetic acid-treated blood samples from patient groups and controls. in these groups, expression rates and mean fluorescent intensity of DC markers were evaluated using flowcytometry technique. Secretion of cytokines including: interleukin(iL)-6, iL-12 and iL-23 were determined using enzyme-linked immunosorbent assay methods. The gene expression of toll-like receptor 2(TLR2), TLR4 and iL-23 were analyzed using in-house real-time polymerase chain reaction ***: Results have been shown significant decreases in: Expression rates of Mo DC markers including CD83, CD1 a and human leukocyte antigen DR(HLA-DR), the mean fluorescence intensitys for CD1 a and HLA-DR, and secretion of iL-12 in CMV reactivated comparedwith non-reactivated liver transplanted patients. On the other hand, significant increases have been shown in the secretions of iL-6 and iL-23 and gene expression levels of TLR2, TLR4 and iL-23 from Mo DCs in CMV reactivated compared with non-reactivated liver transplanted recipients. CONCLUSiON: DC functional defects in CMV reactivated recipients, such as decrease in expression of DC maturation markers, increase in secretion of proinflammatory cytokines, and TLRs can emphasize on the importance of CMV infectivity in development of liver rejection in transplanted patients.

Objective:To evaluate the bioefficacy ofα-cypermethrin impregnated into long lasting insecticide treated nets(LLiTNs- iNTERCEPTOR?) against main malaria vector,Anopheles stephensi（***）.Methods:The effectiveness of bed net impregnated withα-cypermethrin(iNTERCEPTOR?) with washing was *** washing procedure and bioassay tests were carried out according to the WHO-recommended *** vector, *** was exposed to impregnated bed net for three minutes and then mortality measured after 24 h recovery *** was also measured according to the logarithmic times. Results:Result of cone bioassay method showed that bioefficacy ofα-cypermethrin decreased from 100%in unwashed to 15%in 20 ***50 was measure as one minute in one wash and increased to 40 min in 20 ***:Findings of this study provide guideline for malaria vector control authorities and people using pyrethroid-impregnated bed nets.

目的:选择北京海奥思凯科技公司生产的电热砭石仪与传统的艾灸进行对比研究,观察两者对体表微血管血流灌注量的影响。方法:选择20名志愿者(平均30岁)为受试对象,分别在下腹部接受传统艾盒灸与电热砭灸两种不同的刺激,利用激光多普勒血流成像仪观察体表微血管灌注量的变化。结果:在相同作用时间内,传统的艾盒灸对全身微血管灌注量的即刻升高效果以及被灸局部的远期升高效果要优于电热砭灸仪,但电热砭灸对被灸局部的体表微血管灌注量升高效果与传统艾盒灸相同。结论:电热砭灸不能完全代替传统艾灸,在一些疼痛局限性的疾病治疗上,可能效果接近。

in traditional iranian medicine,accurate determination of Mizaj plays a very important role in the etiology and treatment of *** are a lot of evidences and documents in original iranian traditional medicine books and documents related to the relationship between Mizaj and the facial character and *** this study,we investigated the validity of determining Mizaj based on facial dimensions and *** this purpose,a software was developed to determine Mizaj based on an algorithm of facial dimensions and *** cross-sectional study investigated the validity of identifying Mizaj based on facial dimensions and appearance(using the facial-based Mizaj recognition software)by comparison with the Mizaj determined by a standardized questionnaire and by using the traditional method of experts’opinion on determining the *** inclusion criteria of the study were being healthy,using no medication,and consent to participate in the *** sample size of the study was 83 *** analyzing the data,SPSS software version 19 was ***,for examining the level of internal agreement of the data,Kappa statistics were *** results of the study showed that the level of agreement between determining Mizaj by facial characteristics with the experts’distinction of Mizaj is almost complete,and the level of agreement with the Mizaj questionnaire was *** results showed that the facial-based Mizaj recognition software has a good validity to detect the Mizaj of individuals in the study ***,studies with a larger sample size are needed to confirm the results of this study.
