Introduction Antibiotic use for acute otitis media(AOM)is one of the major sources of antimicrobial ***,the effective minimal antibiotic duration for AOM remains ***,guidelines often recommend broad ranges(5-10 days)of antibiotic use,yet the clinical impact of such a wide window has not been *** We systematically searched PubMed/MeDLINe,embase,Scopus,Web of Science,and Cochrane Library from database inception to 6 October *** meta-analysis was conducted on randomized controlled trials that assessed antibiotic treatment for AOM in children(PROSPeRO CRD42020196107).Results For amoxicillin and amoxicillin-clavulanate,7-day regimens were noninferior to 10-day regimens in clinical responses[amoxicillin:risk ratio(RR)0.919(95%CI 0.820-1.031),amoxicillin-clavulanate:RR 1.108(0.957-1.282)],except for≤2 *** the third-generation cephalosporins,7-day and 10-day regimens had similar clinical responses compared to placebo[7-day:RR 1.420(1.190-1.694),10-day:RR 1.238(1.125-1.362)compared to placebo].However,5-day regimens of amoxicillin-clavulanate and third-generation cephalosporins were inferior to 10-day *** to amoxicillin,a shorter treatment duration was tolerable with *** Our findings indicated that 1O days of antibiotic use may be unnecessa.ily long,while the treatment duration should be longer than 5 ***,5-day regimens would be sufficient for a modest treatment *** findings revealed that the current wide range of recommended antibiotic durations may have influenced the clinical outcome of AOM,and a narrower antibiotic duration window should be re-established.

Tremor occurs in about half of multiple sclerosis(MS)*** tremor has a broad frequency range of 2.5-7 Hz,with a higher prevalence of postural tremor(44%)compared to intentional tremor(6%)(Alusi et al.,2001).Tremor may affect the upper and lower extremities,head,and trunk,and may even affect the vocal cords in isolated cases of palatal *** tremor is classically attributed to lesions of the brain stem,cerebellum,or cerebellar peduncles,and tremor intensity has been shown to correlate with the number of lesions or their functional ***,recent work has demonstrated that inflammatory damage to the cerebello-thalamic and cortico-thalamic pathways might also play an important role in causing tremor,as it co-occurs with other signs and symptoms of MS such as dysarthria,dysmetria,dysdiadochokinesia,and dystonia(Alusi et al.,2001).

Late Pleistocene spore-pollen record and climatic change at the Chongphadae Cave Site,Democratic People's Republic of Korea

We present a detailed pollen record and interpretations of late Pleistocene climatic change at the Chongphadae Cave Site, Democratic People's Republic of Korea. The mean annual paleotemperature and mean annual paleoprecipitation of the site were calculated using the temperature index and precipitation index based on ecological features and geographical distribution of each taxon. Temperature index and precipitation index range from 8.8℃ to 10.4℃ and from 805.0 mm to 963.1 mm, respectively. Four dates(radiocarbon, uranium series, fission track, and paleomagnetic excursion dating) of the deposit profile investigated yield a range of ~21.3 ka BP to ~117 ka BP, geochronologically corresponding to the late Pleistocene, and extend from the last interglacial highstand through the Last Glacial maximum. Our results are thus consistent with the climatic shift from interglacial to glacial conditions, provide evidence that the environments of the region, which was reconstructed from the paleoclimatic index, changed from a mild and humid to a cool and dry climate during the late Pleistocene, and suggest trends similar to those of several parts of the Northern Hemisphere which lie in the same latitudinal zone as our study area.

傅里叶变换红外光谱(FTIR)是鉴别物质和分析物质结构的有效手段,可对样品进行定性和定量分析,广泛应用于医药化工等众多领域。FTIR作为一种操作简便、实用、准确率高和费用低的鉴别方法,如今已经普遍应用于各种药品检验中,而有关其应用于药理研究中还非常少,目前仅有采用红外光谱分析法对各种肿瘤组织进行研究的报道。高尿酸血症肾病是人体血尿酸升高后,过高的尿酸负荷超过肾脏的清除能力,尿酸结晶沉积于集合管、肾盂和尿道等。采用FTIR技术评价高尿酸血症肾病模型,并评价药物的药效,具有一定的可行性。该实验为探讨FTIR评价高尿酸血症肾病模型和当归痛风方疗效的科学性,将60只KM小鼠随机分为6组,正常对照组、模型组、阳性对照组和当归痛风方低、中、高组,采用氧嗪酸钾复制高尿酸血症肾病模型,灌胃当归痛风方,检测血清uA、尿液uA、血清CRe、尿液CRe、血清il-1β和肾脏il-1β、肾脏FTIR谱图等,并观察肾脏的病理变化。结果表明模型组各项指标与正常组比较有统计学意义(puA、血清CRe、血清il-1β和肾脏il-1β等与模型组比较且有统计学意义(p<0.05)。当归痛风方高剂量组肾脏的病理变化有所改善,无炎性细胞浸润,管腔内无管型,说明对肾损伤具有保护作用。各组FTIR谱有明显的差异,模型组的强度均低于正常组,阳性对照组和当归痛风方高剂量组谱接近正常组,进一步说明当归痛风方对肾损伤具有保护作用。研究表明FTIR可以作为一种简便快捷灵活经济的评价动物模型及药物疗效的方法之一,也为高尿酸血症肾病模型及有效药物的筛选提供了一个有意义的技术手段。该实验拓展了FTIR检测方法的使用范围。

effect of intramolecular energy transfer on the rate of unimolecular isomerization: HCN --HNC

This paper reports calculations of the rate of isomerization of HCN - HCN based on the theory of Gary and Rice as extended by Zhao and Rice. The major task is to determine the effect of intramolecular energy transfer on the prediction of the rate of isomerization. Both the full three-dimensional (3D) system and the reduced two-dimensional (2D) system obtained from freezing CN bond at 1.159 A are analyzed to check the validity of the freezing bond approximation. Meanwhile, RRKM rates are calculated to test RRKM choice of the transition state by comparing to Gary-Rice three-state model. The comparison shows that the rates from 2D model and 3D model are differing up to 20% with 2D rates consistently larger. The intramolecular energy transfer modifies the isomerization rate for HCN system up to 30% that is modestly small by the expectation. The isomerization rate predicted from RRKM theory is greater than those of Gary-Rice three-state model theory up to 65%, and it overstimates the rates under all considerations, including the contribution from intramolecular energy transfer, for the studied system by about a factor of 2.
