The global prevalence of obesity is increa.ing ra.idly with an exponential rise in incidence of type 2 diabetes mellitus in recent years.‘Diabesity’,the term coined to show the strong interlink between obesity and diabetes,is the direct cons-equence of the obesity pandemic,and poses significant challenges in the management of the *** addressing the clinical and mecha.istic complica.ions of obesity such as metabolic-associated fatty liver disease and obstructive sleep apnoea,a ra.ional management algorithm for diabesity cannot be *** classes of anti-diabetic medica.ions including insulins,sulphonylureas,thiazolidinediones and meglitinides are associated with the risk of weight gain and may potentially worsen ***,appropriate selection of antidiabetic drug regimen is crucial in the medical management of *** role of non-pharmacological measures such as dietary adjustments,exercise interventions and ba.iatric procedures should also be ***,the importance of appropriate and optimal management of diabesity is often overlooked by medical professionals when achieving adequate glycemic control which results in inappropriate management of the disease and its *** review provides a narra.ive clinical update on the evidence behind the management of diabesity.

Diabetes care is often difficult without a proper collabora.ion between the pa.ient and the care provider as the disease is mostly self-managed by pa.ients through adjustments in their lifestyles,and medica.ion doses to optimise glycaemic *** clinical guidelines on the management of diabetes mellitus(DM)provide only broad principles on diabetes care,and the blind follow-up of such principles without a proper review and considera.ion of pa.ient characteristics often results in inadequate glycaemic control and diabetes complica.ions ***,a proper understanding of the pathobiology,clinical situa.ion,and comorbidities of the individual case is of paramount importance to tailoring the most appropriate management strategy in real-world diabetes *** the aid of five unique cases of DM[(1)Medically managed type 2 diabetes mellitus(T2DM)with severe obesity;(2)Management of T2DM with unreliable glycated haemoglobin(HbA1c);(3)Obesity in a pa.ient with type 1 diabetes mellitus(T1DM);and(4)Late diagnosis and subsequent management of monogenic diabetes and *** worsening of well-controlled T2DM)]we elaborate on the importance of individua.ised diabetes care and the practica.ities in these *** review also provides an evidence update on the management of different forms of DM to guide physicians in optimising the care of their pa.ients in day-to-day clinical practice.

Several epidemiological studies have clearly identified diabetes mellitus(DM)as a major risk factor for cognitive dysfunction,and it is going to be a major public health issue in the coming years because of the alarming rise in diabetes prevalence across the *** and neural tissues predominantly depend on glucose as energy substrate and hence,any altera.ions in carbohydrate metabolism can directly impact on cerebral functional output including cognition,executive capa.ity,and *** affects neuronal function and mental capa.ity in several ways,some of which include hypoperfusion of the brain tissues from cerebrovascular disease,diabetes-related altera.ions of glucose transporters causing abnorma.ities in neuronal glucose uptake and metabolism,local hyper-and hypometabolism of brain areas from insulin resistance,and recurrent hypoglycemic episodes inherent to pharmacotherapy of diabetes resulting in neuronal *** decline can further worsen diabetes care as DM is a disease largely self-managed by ***,it is crucial to understand the pathobiology of cognitive dysfunction in rela.ion to DM and its management for optimal long-term care plan for pa.ients.A thorough appraisal of normal metabolic characteristics of the brain,how altera.ions in neural metabolism affects cognition,the diagnostic algorithm for pa.ients with diabetes and dementia,and the management and prognosis of pa.ients when they have this dangerous combina.ion of illnesses is impera.ive in this *** evidence-based narra.ive with the backup of latest clinical trial reviews elaborates the current understanding on diabetes and cognitive function to empower physicians to manage their pa.ients in day-to-day clinical practice.

Recent geological and paleontological explora.ion in the indus ba.in of Pa.istan allowed the discoveries of numerous remains of non-ma.ine reptiles( titanosaurian sauropod,abelisaurian and noasaurian theropod dinosaurs),and ma.ine reptiles( crocodiles),flying reptiles( pterosaurs),ma.ine and non-ma.ine mammals,fishes,invertebrates,and plants,especially Pa.istan is rela.ively rich in footprints / trackways in the *** vertebrates of indo-Pa.istan are very significant for paleobiogeographic study due to the present-day connection of this continent with a.ia.in Northern Hemisphere,whereas during past( Jurassic and pre-Jurassic) it was connected to the Gondwana. The Mesozoic vertebrates show close affinities with Gondwanan *** Cenozoic vertebrates show Eura.ian affinity and migrated from indo-Pak subcontinent to Eura.ia or vice versa via Paleo indus River systems along Western indus Suture,after long journey of about 6 000 km the first collision of indo-Pak subcontinent with a.ia occurred at terminal Cretaceous.

A dark pulse mode-locked laser is experimentally demonstrated using the indium tin oxide(iTO)coated Dshape fiber as a saturable absorber(sa).Using the polishing wheel technique,a D-shape single mode fiber was fabricated.A 60-nm-thick layer of iTO was deposited over the D-shape fiber using the electron beam deposition *** sa has a satura.ion intensity of 40.32 MW/cm^2 and a modula.ion depth of 3.5%.A stable dark pulse mode-locked laser was observed at a central wavelength of 1559.4 nm with repetition rate 0.98 MHz,pulse width 370 ns and signal-to-noise ra.io 61 dB.
