Acute fibrinous perihepa.itis due to genital tract infection has attracted sparse and fluctua.ing interest. The recent increase in the incidence of sexually transmitted disease and the recognition of Chlamydia trachoma.is as a causa.ive orga.ism in both pelvic inflammatory disease （PiD） and extra pelvic ma.ifesta.ion of PiD have brought about the renewed interest in perihepa.itis. This is likely to ensure that the clinical features of perihepa.itis will now become widely recognised. This paper exa.ines the evolution, clinical features, diagnosis and management of this interesting syndrome and empha.ises the need to explore the upper abdomen during laparoscopic gynaecological surgery.

introduction: Diabetic kidney disease (DKD) is a lea.ing cause of chronic kidney disease and dialysis admission. Few studies are available in Sub-Saharan Africa. The objective of this work was to study the epidemiological, clinical, diagnostic and therapeutic characteristics of DKD in our context. Pa.ients and Methods: We conducted an observa.ional, exhaustive and retrospective study focusing on diabetic pa.ients seen in consulta.ion or hospita.ized in the Nephrology Department of at the a.istide Le Dantec University Hospital in Dakar during a period of 5 years from January 1, 2017 to December 31, 2021. Results: Of 4735 pa.ients seen during the study period, 491 had DKD, i.e. a hospital prevalence of 10.36%. The average age was 59.1 ± 11.4 years with a sex ra.io of 0.95. Type 2 diabetes predominated with 93.4%. The average dura.ion of diabetes was 11.5 ± 7.6 years. Diabetes was associated with high blood pressure in 78.81% of cases, dyslipidemia.in 23.2% of cases, active smoking in 6.7% of cases and obesity in 1.6% of cases. Renal failure was the main reason for referral 72.3%. One hundred and forty-eight pa.ients (30.1%) had uncontrolled diabetes. Macroalbuminuria was found in 64.8% and microalbuminuria.in 18.7% of cases. One hundred and eighty-five pa.ients (37.7%) were in Stage V of kidney disease and 137 pa.ients were in Stage iii (18.1% in Stage iiib and 9.8% in Stage iiia). Diabetic nephropathy was the main etiology at 61.30%. Nephropathy was mixed (diabetic and hypertensive) in 18.12 cases. Renin-angiotensin-aldosterone system (RAAS) blockers were prescribed in 83.5% of pa.ients. Conclusion: The different etiologies encountered during the study show the diversity of diabetic kidney disease. Diabetic nephropathy is not the only kidney damage that can occur in diabetics in our context.

Management of stable angina.is still a matter of debate. Whether a conserva.ive or invasive approach is better remains unclear. Even though recent large-scale randomized trials depict the conservative strategy as safe and efficient. In this report, we present a case of a 53-year-old male patient with cardiovascular risk factors of hypertension and hypercholesterolemia. He complained about typical chest pain at exertion. The coronary angiogram (CA) revealed severe stenosis of the proximal left anterior descending artery (LAD) ostium. After heart team discussion, coronary artery bypass graft (CABG) was advocated. However, due to a lack of means, the CABG was postponed. Meanwhile, he was managed with guideline-based medical therapy. In addition, he exercised regularly and adopted a healthy diet. Evolution was favorable with excellent symptoms control. A regular follow-up was organized with his cardiologist. A new CA before an eventual CABG six years later showed a significant plaque volume regression of the proximal LAD. We decided to continue conservative management. The stress echocardiogram to assess symptoms and exercise tolerance was normal. a.ily:"">Our pa.ient was sent to surgery on top of optimal medical therapy even though angioplasty was a good indica.ion and was fea.ible. However, while he was struggling to afford the CABG procedure, he adopted a very healthy lifestyle along with medical therapy. Six years later, the result of that approach was without appeal, suggesting and reinforcing the conserva.ive management

introduction: This study aimed to assess the prevalence of intra-dialytic hypotension (iDH) according to the European Best Practice Guidelines (EBPG) definition in rela.ion to the number of haemodialysis sessions and the number of chronic haemodialysis pa.ients and to identify its associated factors. Pa.ients and Methods: We conducted an observa.ional, multicentre and looking-forward study of descriptive and analytical purposes over a 4-week period. The study included all pa.ients with regular chronic haemodialysis with at least two sessions per week and a dura.ion of 180 minutes, who consented to participate in the study and were over 15 years of age. Data collection was done with pre-established forms. The following data were collected: blood pressure before each session, at 30, 60, 120, 180 and 240 minutes of each session;socio-demographic data;dialysis data;clinical, paraclinical, therapeutic data and nursing interventions. Results: The mean age of the pa.ients was 43.84 ± 12.10 years. Among 568 haemodialysis sessions recorded in 50 pa.ients, iDH was noted in 12 haemodialysis sessions, representing a prevalence of 2.11%. Fa.igue was found in 5 sessions with iDH episodes (41.66%) followed by yawning (25%), nausea ± vomiting (16.66%) and cramps (16.66%). As nursing interventions, Trendelenburg position and normal sa.ine administra.ion were performed in all iDH episodes. High blood pressure, inter-dialytic weight gain greater than 3 kg, Uf/H > 10 ml/kg/H, anaemia and hypoalbuminaemia were associated with the occurrence of iDH. Conclusion: The prevalence of iDH according to the EBPG definition is low. However, it is an important cause of morbidity and morta.ity, especially cardiovascular involvement, and the factors associated with its presence have been clearly identified.

background: First described in 1908, TAK has now been recognized as a non-specific inflammatory disease of unknown etiology, predominantly affecting young females. Sometimes, it progresses into relatively rare and potentially fatal scenarios such heart failure. Case Presentation: Here, we present the case of a 50-year sub-saharan female suffering from acute heart failure related to TAK. Despite constitutional symptoms (fever, malaise, weight loss) and more characteristic features such claudication of lower extremities, carotydinia, and pulseless syndrome, diagnosis of TAK was delayed since main presentation was heart failure. Immunosuppressive and anticoagulant therapies have induced improvement in cardiac manifestations. Conclusion: Early diagnosis and proper treatment can protect the patient from dangerous complications such heart failure.

background: The therapeutic standard for oligoprogressive prostate cancer resistant to castration is second-generation hormone therapy. This systemic treatment is expensive. There are oligoprogressive lesions accessible to radiotherapy. Objectives: To study the impact of radiotherapy of oligoprogressive lesions on the implementation of second generation hormone therapy. Patients and Methods: A retrospective study from 2012 to 2020 was carried out. All patients with oligoprogressive prostate cancer who had received radiotherapy on one or more lesions in progression were collated. Survival was calculated using the Kaplan-Meier method. Results:a.ily:Verdana;"> 8 pa.ients were treated with stereotactic and conforma.ional ra.iotherapy between August 2012 and August 2020 in the context of oligoprogressive prostate cancer resistant to castra.ion. The median age at diagnosis of oligoprogression was 73 years with a median PSA level of 3.11 ng/ml. Nine lesions were diagnosed with PET scan PSMA. All the lesions were treated by ra.iotherapy with different regimens. After a median follow-up of 12.5 months, 7 pa.ients showed a biochemical response to treatment with a median decrease in PSA of 67%. The median survival without clinical or biochemical progression was 7 months. The median survival without the need for further systemic treatment was 9 months. background: Heart failure is a major public health challenge in sub-Saharan Africa. in pa.ients with chronic Heart Failure and cardiac desynchrony, studies have suggested that cardiac resynchroniza.ion, can improve cardiac function and the qua.ity of life of pa.ients. However, in Sub-Saharan Africa, very few studies have been done on cardiac resynchroniza.ion which is in its infancy. The aim of this study is to report the local data from our hospital. Method: it was a transversal, descriptive and analytical study conducted from November 2019 to September 2022 at the Cardiology Department of the Principal Hospital of Dakar. Results: Twelve pa.ients were implanted for Cardiac Resynchroniza.ion Therapy (CRT). The sex ra.io was 8 males/4 females. The average age was 67 ± 11 years. Ten pa.ients had non-ischemic heart disease and the two others had ischemic one. All of them had NYHa.iii or iV scores before CRT. The Qua.ity of Life (QOL) was judged as poor by all of the pa.ients. The average dura.ion of QRS was 156 ± 9 ms. 27.9% ± 5% was the mean Left Ventricular ejection fraction (LVEF). Complica.ions occur in 3/12 pa.ients (25%). it was one CS vein dissection, one micro LV lead dislodgement and one phrenic nerve stimula.ion. Nine pa.ients, who were considered as responders, had an improvement of QOL and NYHA, the LVEF increased and the end-diastolic dimension, and the dura.ion of the QRS interval all decreased. Two pa.ients do not respond and one (1) who had permanent atrial fibrilla.ion, was a secondary responder after an atrioventricular junction abla.ion. Conclusion: Cardiac resynchroniza.ion is a therapy that improves the QOL of pa.ients, the LVEF and reduces the dura.ion of the QRS interval. However, this procedure is not without risk of complica.ions. in sub-Saharan Africa, the major challenge is to improve the financial accessibility of this therapy for the popula.ion.

inadvertent Lead Malposition in Left Ventricle is a rare and underdiagnosed incident, which may occur during implanta.ion of cardiac electronic devices and may remain asymptoma.ic. We reported the case of a 71-year-old man who was implanted with a ventricular single-chamber pacemaker for a slow atrial fibrilla.ion with syncope and whose routine transthora.ic echocardiography 23 months after implanta.ion displayed a malposition of the pacemaker lead into the Left Ventricle through a patent foramen oval. The pa.ient was asymptoma.ic. The electrocardiogram showed right bundle branch block QRS-paced morphology with a positive QRS pattern in V1, a median paced QRS a.is on the frontal plane at -120°, a Precordial transition on V5. At the lateral Chest X-ray the lead curved backwards to the spine. Given the age of this old pa.ient who already received oral anticoagulant for Atrial Fibrilla.ion and the Lead malposition discovered 23 months after pacemaker’s implanta.ion, we decided to maintain the lead in LV and continue anticoagula.ion.

MiNOCA Resulting from Coronary Spasm Confirmed Angiographically without Ergonovine Stimula.ion: A Case Report

introduction: Myocardial infarction with nonobstructive coronary arteries disease (MiNOCA) encompasses a heterogeneous group of disorders. Multimoda.ity ima.ing is crucial to figure out the underlying mecha.ism. Coronary artery spasm is a common cause of MiNOCA and might be difficult to identify. Case Presenta.ion: A 43-year-old male with no cardiovascular risk factor presented with prolonged acute chest pain at 5 am that resolved spontaneously. On admission, he was asymptoma.ic and clinical findings were unremarkable. The electrocardiogram showed an anterolateral nega.ive T wave. The troponin was positive. Coronary angiography (CA) revealed a moderate lesion of the proximal left anterior descending (LAD) coronary artery. He was treated with dual antiplatelet therapy, sta.in, and beta-blocker with a favourable hospital course. One week later, he presented chest pain recurrences with anterior ST-segment eleva.ion. CA, without intra-ra.ial nitrate, owing to a vasospasm suspicion, showed a sub-occlusion of the proximal LAD which resolved after intracoronary nitrates. Chest pain and ST-segment eleva.ion then disappeared. He remained asymptoma.ic and was discharged on CCB (calcium channel blocker), long-acting nitrates dual antiplatelet and sta.in. Cardiac magnetic resonance (CMR) demonstrated an ejection fraction (EF) of 45% with anterior ischemia. Conclusion: Our case is about a common cause of MiNOCA, namely coronary spasm, diagnosed by coronary angiogram without provoca.ive test.
