Hepatoblastoma in Developing Countries;Eight Years of Single Centre Experience

Background and objectives: Although hepatoblastoma (HB) is a rare childhood tumor and constitutes only 0.9% of all pediatric cancers, there was an obvious improvement in risk stratification and prognosis over the last two decades. This study aimed to assess the outcome of HB patients treated in our center and to investigate the impact of different prognostic factors on the survival of these patients. Patients and methods: This was a retrospective study included newly diagnosed patients with HB presented to the Children Cancer Hospital Egypt (CCHE 57357), from July 2007 to June 2015. Patient’s data were analyzed for the clinical characteristics and survival outcome of the included patients. Results: One hundred twenty-four children were included during the study period with a median age of 14 months. The tumor was found occupying the entire liver in 25 patients (20%);while it was confined to one lobe in 80% of them, portal vein thrombosis was diagnosed in 10 patients, and there were 30 patients (24%) had metastatic disease at presentation. Only five patients (4%) underwent primary surgical excision, and all of them were grossly excised (stage I);77/119 (64.7%) experienced delayed surgery after two to six courses (median, four courses) of C5VD and the overall resection rate was 66%. There were 42 patients (35.3%) failed to do surgical excision either because they still had evident metastatic disease with poor chemotherapy response, or because their tumor remained unresectable after six courses of chemotherapy. The 3-year event-free and overall survivals for the whole studied patients were 45.7% (95% CI, 36.9% - 56.7%), and 66.7% (95% CI, 57.1% - 77.8%) respectively. The 3-year EFS and OS were significantly better in those who underwent surgical excision (68.63% and 80.74% respectively, P-value 0.001). Also, the survival rates were significantly affected by the presence of metastatic disease at presentation, tumor stage and initial risk grouping of the studied patients

Total Hip Prosthesis in Dakar and Forensic Implication

The total hip prosthesis is the ultimate treatment of the hip joint’s wear. It is a delicate surgery and the most common replacements. This study was performed in the Orthopaedic-Traumatology Department of Public Health Establishment of Aristide Le Dantec in Dakar. It aimed to determine the complications of our total hip and highlight their forensic implications. This is a retrospective study from patients collected from January 2000 up to December 2010. The collection of elements records was performed for each patient, based on a file including age, sex, primary-location’s etiology, and types of complications of total hip prostheses. The analysis of these complications has shown that infections, poorly lit information, poor operational planning and technical foul may be the main cause of a forensic implication. This is a retrospective study from patients collected from January 2000 up to December 2010. No lawsuit has been recorded for the complications identified in this work. This is due to the belief in fate, ignorance of the victims and solidarity of the medical profession, the high cost and slow pace of judicial proceedings. The diagnosis of total hip prosthesis complications is mainly based on imaging tests. The absence of scintigraphy in our center remains a limit to the early diagnosis of multiple complications and the proper establishment of the epidemiological profile of these lesions. Taking consciences patients of their rights encourages a legitimate requirement repair the harm inflicted.

Integrating Remote Sensing and Field Survey to Map Shallow Water Benthic Habitat for the Kingdom of Bahrain

Identification and classification, as well as mapping of marine habitats, are of primary importance to plan management activities, especially in disturbed ecosystems like the ones in the marine areas of Bahrain. Remotely sensed Landsat-8 imagery coupled with field survey was used to identify, classify and map the benthic habitats in Bahrain marine area. The used geospatial techniques include advanced image processing procedures, which comprise of radiometric and atmospheric corrections, sun glint removal, water depth correction and image classification. Extensive ground-truthing analyses through in-situ field surveys by a team of scuba divers were conducted in October 2014 and June 2015 to inform and refine the classifications. The variables collected from this survey included physical and chemical characteristics of the water, habitat type, substrata, fauna and flora. A total of 176 field points were collected and utilized to perform an accurate assessment of the image classification. Initial habitat classification resulted in 20 habitat categories. However, due to the inability of the Landsat-8 sensors to accura.ely discriminate that level of classification, categories were merged into seven classes. The derived map shows that the benthic marine habitats of Bahrain consist of deep water （2,523 km2）, rock （1,738 km2）, sand （1,191 km2）, deep water/sand （1,006 km2）, algae （922 km2）, seagrass （591 km2） and corals （275.50 km2）. Although limited by the spatial and spectral resolutions of Landsat 8, the used methods produced a suitable map of the benthic habitats within the marine area of Bahrain with an overall accuracy of 84.1%. The use of very high spatial resolution sa.ellite imagery will most likely increase such accuracy significantly.

Hartree-Fock Calculations of <sup>16</sup>O and <sup>40</sup>Ca Nuclei Using Fish-Bone Potential

Hartree-Fock calculations are carried out to describe some properties of 16O and 40Ca nuclei using the two forms of fish-bone potential (I and II). A computer simulation search program has been introduced to solve this problem. The Hilbert space was restricted to three dimensional variational space spanned by single spherical harmonic oscillator orbits. Binding energies, root mean square radii and form factors are found to have a good fit with experimental data.

Synthesis and Screening of New 5-Substituted-1,3,4-oxadiazole-2-thioglycosides as Potent Anticancer Agents

A series of newly 1,3,4-oxadiazole-2-thioglycoside derivatives were synthesized. The key step of this protocol is the coupling between 5-herteroaryl-1,3,4-oxadiazole-2-thione and activated sugars (cyclic or acyclic sugar analogues) in the presence of basic medium. Among of the synthesized compounds, compounds 7, 10, 11 and 13 were screened for them in vitro anticancer activity against four human cancer cells. MCF-7 (Breast), HEPG2 (Liver), HCT116 (Colon) and HEP2 (Larynx) carcinoma cell lines with IC50 values ranging from 2.08 - 8.72 μg/well. Compounds 11 and 13 were highly specific and potent for four cell lines (MCF-7, HCT116, HEPG2 and HEP2).

In the present study, we investigated the biochemical alterations and gene expression of carbohydrate and lipid metabolism after oral administration of vitamin A and E for 2 months in diet-induced obese Wistar rats. Vitamin A and E administration reduced significantly the increase in body weight and food intake and normalized the alterations in lipid profiles in obese rats compared to normal rats. Moreover, both vitamins decreased the fat accumulation in liver tissues of obese rats. Finally, they up-regulated mRNA expression of Pyruvate Kinase (PK) and Glucose Transporter-2 (GLUT-2), and increased lipolysis and cholesterol metabolism through up-regulation of lipoprotein lipase (LPL), Sterol Responsible element Binding Protein-1a (STREBP-1a) and STREBP-1c mRNA expression. In conclusion, vitamin A and E regulate gene expression of carbohydrate and lipid metabolism, and also a.eliorate changes associated with obesity induced by high fat diet in Wistar rats.

Production of Citric Acid from Corncobs with Its Biological Evaluation

Corncobs could serve as a substrate for citric acid production using solid state fermentation technique. The culture optimization concerning substrate concentration, culture duration, pH, temperature and substrate hydrolysis was carried out for maximum productivity of citric acid. Under the optimized conditions, 48.4 g of citric acid was produced from 1 kg dry corncobs. Biological evaluation was carried out for citric acid such as melanin synthesis inhibitory, anti-allergy, anti-bacterial, and hyaluronic acid production activities. The results showed that citric acid has potent melanin inhibitory activity, good inhibition for β-hexosaminidase release and potent stimulatory effect for the production of hyaluronic acid. These activities (melanin synthesis inhibitory, anti-allergy and hyaluronic acid productive activities) of citric acid have been reported for the first time.

The current work concerns the optimization process of phenolic compounds solid liquid extraction from grape byproducts at high temperatures and short incubation times. The effect of five experimental parameters (solidliquid ratio, particle size, time, temperature and solvent mixture) mostly believed to affect the extraction process was undertaken. A first response surface methodology experimental design was used to optimize the solid-liquid ratio and milling time parameters. A second design was used for the optimization of the quantitative and qualitative parameters. The quantitative parameters studied are: total phenolic compounds, flavonoid content, total monomeric anthocyanin composition and tannin concentration. The qualitative parameters analyzed are: antiradical activity and antioxidant capacity. The second design was based on the use of time, temperature and solvent mixture as optimization parameters. The assays were first conducted separa.ely revealing the best experimental conditions for the maximization of each response variable alone. A simultaneous response surface methodology of all the responses taken together was then conducted, showing the optimal extraction conditions to be: 93 minutes at 94?C and in 66% ethanol/water solvent. The maximal response values obtained for each parameter are: Total Phenolic Compounds yield (5.5 g GAE/100g DM), Flavonoid Content (5.4 g GAE/100g DM), Total Monomeric Anthocyanin yield (70.3 mg/100g DM), Tannin Concentration (12.3 g/L), Antiradical Activity (67.3%) and Total Antioxidant Capacity (393 mgAAE/L). All of the optimal values were acquired at 3 mL/g solid-liquid ratio and 6.8 min milling time. The obtained extracts could be used as natural bioactive compounds in several industrial applications.

Designing and Synthesis of New Fluorine Substituted Pyrimidine-Thion-5-Carbonitriles and the Related Derivatives as Photochemical Probe Agents for Inhibition of Vitiligo Disease

A new biocidal agents fluorine substituted-3-thioxopyrimidine-5-carbonitriles (2-9) and/or the related fluorine substi- tuted pyrimido (4,5-d) pyrimidines (10-14) were synthesized by the cycloaddition of fluorinated β- arylidine malo- nonitriles (1a-c) followed by a nucleophilic attack against α,β-bifunctional reagents in different conditions. Structures of the fluorine targets were characterized by their elemental analysis and spectral data (UV, IR, 1H NMR, 13C NMR and mass measurements) and further evaluated as photochemical probe for inhibition of Vitiligo, it was found that compounds 5, 9, 11 and 12 exhibited high potency over the investigated compounds.

Midterm Results of Leaflet Augmentation in Mitral Valve Repairin Rheumatic Valves Experience in One Center

Patients with severe mitral regurgitation (MR) should undergo surgery when they present symptoms or if asymptomatic when there is objective evidence of left ventricular dysfunction. In this work, we analyze the midterm results of leaflet augmentation in mitral valve repair of rheumatic valves with gluteraldehyde preserved autologous pericardium. Patients and Methods: In our department 48 patients were exposed to mitral valve repair by leaflet augmentation either anterior or posterior beside other technique and all patients supported by flexible annuloplasty ring and followed for five years clinically and by echocardiography. Results: Age of the patients ranging from 12 to 47 years, mean age 25.9 ± 8.9 and there were 12 males (25%) and 36 females (75%) with male to female ratio of 1:3. All patients presented with shortness of breath (100%);with 14 patients were in NYHA class III (29.17%) and 34 patients were in NYHA class IV (70.83%). During follow-up period 5 patients needed reoperation by valve replacement, causes of reoperation were restrictive valve motion in one patient, left atrial thrombus in 1 patient and sever mitral regurgitation in 3 patients. Freedom from reoperation was 87.5%. At 5 years, (92.9%) were in New York Heart Association functional class I, three patients (7.1%) were in class II. Echocardiography at follow-up showed satisfactory mitral valve function. Conclusion: leaflet augmentation is a simple and reproducible method of valve repair for rheumatic MR with good midterm result.
