Sodium-glucose cotransporter-2 inhibitors(SGLT2i)are novel oral hypoglycemic agents garnering much attention for their substantial *** recent data have positioned SGLT2i at the forefront of diabetic chronic kidney disease(CKD)and heart failure ***2i use post-kidney transplant is an emerging area of *** from this mini review include the following:Empagliflozin is the most prescribed SGLT2i in kidney transplant recipients(KTRs),median time from transplant to initiation was 3 years(range:0.88-9.6 years).Median baseline estimated glomerular filtration rate(eGFR)was 66.7 mL/min/1.73 m2(range:50.4-75.8).Median glycohemoglobin(HgbA1c)at initiation was 7.7%(range:6.9-9.3).SGLT2i were demonstrated to be effective short-term impacting HgbA1c,eGFR,hemoglobin/hematocrit,serum uric acid,and serum magnesium *** are shown to be safe in KTRs with low rates of infections,hypoglycemia,euglycemic diabetic ketoacidosis,and stable tacrolimus *** data is needed to demonstrate long-term ***2i appear to be safe,effective medications for select *** present literature,though limited,is founded on precedent robust research in CKD patients with *** research/utilization of SGLT2i is vital to not only identify long-term patient,graft and cardiovascular outcomes of these agents,but also to augment management in KTRs.

Obj.ctive: To report a case of naso-sinus tuberculosis and discuss its diagnosis and management in our context. Clinical Case: A 37-year-old patient, with no previous history, visited us for a swelling under left orbital including a nasal obstruction and intermittent frontal headaches, which has been developed since about one year. Clinical Examination: The anamnesis founds no notion of epistaxis, visual disability, nor notion of fever. It has been noted a swelling under left angulo orbital during physical examination, a burgeoning mass filling the left nasal including a Lymphadenopathy under homolateral mandibular. Para clinical investigations in particular CT scan of the sinuses have highlighted a tissue process under orbital with a left naso-ethmoido-maxillary extension. The anatomical pathologic examination of the biopsy carried out through endoscopy permitted to highlight epithelioid giganto-cell granulomas with caseous necrosis evoking tuberculosis. The patient has been prescribed an anti-tuberculosis treatment. The course of illness is marked by the healing of the patient.

Dobutamine Infusion and Absence of Pulmonary Hypertension Are Associated with Decreased Mortality in a Cohort of 249 Patients with Cardiogenic Shock

Obj.ctive: Prognostic analysis of 249 patients admitted for cardiogenic shock (CS) of various origins. Background: Little is known about prognosis of CS from non-ischemic cardiomyopathy. Methods: Retrospective monocentric study of patients referred to an ICU during 2 years. Results: Despite aggressive management including intra-aortic balloon pump (31%), extra-renal replacement therapy (36%), extra-corporeal life support (8%), and catecholamine infusion (97%), in-hospital mortality was 46%. Toxic CS or CS related to deficiency carried a better outcome (mortality 5%). Post-myocardial infarction or post-cardiac arrest CS was associated with higher mortality. In the multivariate analyses, only SAPS II (OR 1.037;1.013 - 1.056;p = 0.0001), pulmonary hypertension (OR 4.8;1.3 - 17;p = 0.02), extra-renal replacement therapy (OR 2.9;1.3 - 6;p = 0.006), and dobutamine infusion (OR 0.44;0.2 - 0.96;p = 0.04) were significantly associated with in-hospital mortality. Conclusion: Dobutamine infusion was associated with a better outcome. Higher SAPS II, pulmonary hypertension, and extra-renal replacement therapy were associated with increased in-hospital mortality.

Frequency and Sociodemographic Factors Associated with Congenital Malaria at the Borgou Regional University Teaching Hospital (CHUD-B) in Benin in 2015

Background: Every minute a child dies from malaria in Sub-Saharan Africa. Despite this high infant and child mortality associated with malaria, congenital malaria is still not adequately understood as regarding its frequency, severity and management. The obj.ctive of this research work was to determine the frequency and identify the socio-demographic factors associated with the onset of congenital malaria at the gynecological and pediatric units of the Borgou Regional Teaching Hospital (CHUD-B) in Benin in 2015. Methods: This study was a cross-sectional survey with analytical purpose, based on exhaustive census performed from April to j.ly 2015 on newborns delivered at CHUD-B and their mothers. The dependent variable was the presence of congenital malaria and the independent variables were socio-demographic characteristics. The data were processed and analyzed using Epi Info software version 7.0 with a significant threshold of 0.05. Findings: The frequency of congenital malaria in newborns was 19%. No sociodemographic factor was associated with congenital malaria. Conclusion: Nearly one in five newborns delivered at CHUD-B had congenital malaria. Due to malaria’s contribution to perinatal morbidity and mortality and subsequent receptivity of child victims to malaria, a longer study covering a whole year and focused on the determinants of congenital malaria should be carried out.

Protein phosphorylation plays an important role in physiological processes, such as muscle contraction. Phospho-specific antibodies have become powerful tools to study these processes. Cardiac myosin binding protein-C (cMyBP-C) is one of the proteins that make up the contractile apparatus of cardiomyocytes. Phosphorylation of cMyBP-C is essential for normal cardiac function, since dephosphorylation of this protein leads to its degradation and has been associated with cardiomyopathy. One of the upstream kinases, which phosphorylate cMyBP-C, is protein kinase D (PKD). While studying the role of PKD in cMyBP-C phosphorylation, we tried to analyze phosphorylation of PKD with a phospho-specific PKD-Ser744/748 antibody. Contrary to the expected 115 kDa, a signal was found for a 150-kDa protein. By MALDI-TOF mass spectrometry, we identified this protein to be cMyBP-C. These data were confirmed by immunostaining using the p-PKD-Ser744/748 antibody, which displayed a striated pattern similar to the one observed for a regular cMyBP-C antibody. To our knowledge there are no antibodies commercially available for phosphorylated cMyBP-C. Thus, the p-PKD-Ser744/748 antibody can accelerate research into the role of cMyBP-C phosphorylation in cardiomyocytes.

Rice genes OsDj.2 and OsERF104,encoding a chaperone protein and an APETELA2/ethylene-responsive factor,respectively,are strongly induced in a compatible interaction with blast fungus,and also have function in plant susceptibility validated through gene ***,we reported the CRISPR/Cas9 knockout of OsDj.2 and OsERF104 genes resulting in considerable improvement of blast resistance.A total of 15 OsDj.2(62.5%)and 17 OsERF104(70.8%)T_(0)transformed lines were identified from 24 regenerated plants for each target and used in downstream *** of homozygous T1 mutant lines revealed not only a significant decrease in the number of blast lesions but also a reduction in the percentage of diseased leaf area,compared with the infected control *** results supported CRISPR/Cas9-mediated target mutation in rice susceptibility genes as a potential and alternative breeding strategy for building resistance to blast disease.
