Duodenal biopsy may be avoided when high transglutaminase antibody titers are present

AIM:To evaluate the predictive value of tissue transglutaminase (tTG) antibodies for villous atrophy in adult and pediatric populations to determine if duodenal biopsy can be ***: A total of 324 patients with celiac disease (CD; 97 children and 227 adults) were recruited prospectively at two tertiary centers. Human IgA class anti-tTG antibody measurement and upper gastrointestinal endoscopy were performed at diagnosis. A second biopsy was performed in 40 asymptomatic adults on a gluten-free diet (GFD) and with normal tTG ***: Adults showed less severe histopathology (26% vs 63%, Ps than children. Levels of tTG antibody correlated with Marsh type in both populations (r=0.661, Pstic regression revealed that only tTG antibody was an independent predictor for Marsh type 3 lesions, but clinical presentation type and age were not. A cut-off point of 30 U tTG antibody yielded the highest area under the receiver operating characteristic curve (0.854). Based on the predictive value of this cut-off point, up to 95% of children and 53% of adults would be correctly diagnosed without biopsy. Despite GFDs and decreased tTG antibody levels, 25% of the adults did not recover from villous atrophy during the second year after ***: strongly positive tTG antibody titers might be sufficient for CD diagnosis in children. However, duodenal biopsy cannot be avoided in adults because disease presentation and monitoring are different.

AIM:To assess the efficacy of allopurinol to prevent hyperamylasemia and pancreatitis after endoscopic retrograde cholangiopancreatography(PEP).METHODs:One hundred and seventy patients were enrolled and randomized to two groups:a study group(n=85)who received 300 mg of oral allopurinol at 15 h and 3 h before endoscopic retrograde cholangiopancreatography(ERCP)and a control group(n=85)receiving an oral placebo at the same *** Outcome Measurements included serum amylase levels and the number severity of the episodes of *** amylase levels were classified as normal(semia(>151 IU/L).Episodes of PEP were classified following Ranson's criteria and CT severity ***:Gender distribution was similar between *** age was 53.5±18.9 years for study group and 52.8±19.8 years for ***,the distribution of benign pathology was similar between *** was more common in the control group(P=0.003).Mild PEP developed in two patients from the study group(2.3%)and eight(9.4%) from control group(P=0.04),seven episodes were observed in high-risk patients of the control group(25%) and one in the allopurinol group(3.3%,P=0.02).Risk factors for PEP were precut sphincterotomy(P=0.02),pancreatic duct manipulation(P=0.002)and multiple procedures(P=0.000).There were no deaths or side ***:Oral allopurinol before ERCP decreased the incidences of hyperamylasemia and pancreatitis in patients submitted to high-risk procedures.

黄原胶作为一种用途很广的生物增稠剂和发泡剂,具有抗剪切性和耐酸、耐碱和抗氧化等特点,这些特点恰好满足静态泡沫去污对增稠剂的要求。本文测量并分析了碱性体系中(Na2CO3体系)影响发泡液粘度的几种因素(搅拌速度、黄原胶浓度、碳酸钠浓度及温度);以及酸性体系中(HNO3+H3PO4体系)黄原胶浓度和发泡液放置时间对泡沫稳定性的影响;比较分析了铝片在酸性发泡液及其生成泡沫中的失重情况。试验结果表明黄原胶无论是在碱性溶液还是在酸性溶液中均具有一定的稳定性,能够满足泡沫去污的要求;其稳定性随时间推移而降低的特性有利于泡沫去污产生的二次废液的处理。黄原胶酸性发泡液能够产生稳定的泡沫,对铝片具有良好的腐蚀效果。在本实验中,当铝片处于距底端4cm高度位置时,腐蚀速率达到1.0 mg/(cm2.h)。因此,黄原胶有望作为放射性污染大体积空腔容器静态泡沫去污的增稠剂。

Nonlinear Coefficient Determination of Au/Pd Bimetallic Nanoparticles Using Z-scan

In this paper we present the nonlinear optical characterization of Au/Pd nanoparticles in order to obtain the nonlinear refractive indices using the Z-scan technique. The experiments were performed using a 514 nm laser beam Ar+, with 14 Hz of modulation frequency, as excitation source. By using a lens the excitation beam was focused to a small spot and the sample was moved across the focal region along the z-axis by a motorized translation stage. seven samples with different concentration ratio of Au/Pd nanoparticles were prepared by simultaneous reduction of gold and palladium ions in presence of poly (N-vinyl-2-pirrolidone) (PVP) using ethanol as a reducing agent. In this work, we report the application of the Z-scan technique, to generate optical transmission of laser light as a function of the z position for solutions containing bimetallic nanoparticles of Au (core)/Pd (shell) with average sizes ranging from 3 to 5 nm. The magnitude of the obtained nonlinear refractive index was in the order of 10?8 cm2/W. Our results show that the nonlinear refractive index has a nonlinear behavior when the (Au/Pd) ratio was increased.

sharka, it is considered one of the most serious diseases of stone fruits worldwide. It is transmitted long distances by propagating material and by aphids close to an infection focus. In san Rafael, Mendoza, Argentina, it made a study of dispersion＇s degree of the disease in a plum plantation CV D＇Agen. samples were taken in the spring of the basal part of open leaves. A DAs-ELIsA test was made using a protocol in agreement with sENAsA and INAsE （2007） on 750 plants. Infection percentages found were 4.4%; 6.4%; 8.53% and 10.1%; between the first until last year of study. These numbers indicate a degree of dissemination of the disease similar than described by the Chileans and slower than described by the spanish and French. several factors can be responsible for the speed of dispersion of diseases like sharka： vector quantities, presence of parasites and predators, closeness to inoculants sources, different degree of susceptibility of the species or cultivar to the disease or insect or others. This work can serve as a base for further studies on the incidence of diverse factors in the process of disease dispersion.

Amphibians are thought to be highly susceptible to perturbed environments. However, recent studies show that many of them are successful inhabitants of disturbed, fragmented habitats. The source of this resilience is yet unclear, but it may be the byproduct of having a robust phenotype and/or the result of phenotypic plasticity. We then assessed the contribution of each by evaluating cytological features of two brain nuclei that modulate reproductive behavior and of the ovary, using female specimens of the frog Diaglena spatulata prospering in conserved and disturbed areas of a tropical dry forest. Our results in the brain show that the medial amygdala, but not the preoptic area, had a reduced size in frogs collected in disturbed forests compared to specimens collected in conserved forests. Both brain nuclei displayed, however, neurons with a reduced size in frogs captured in disturbed forest patches. In contrast, ovarian cytological features were similar between groups. Our preliminary results lead us to propose that Diaglena spatulata female specimens might combine robust ovary and plastic brain’s phenotypic traits to confront disturbed environments. This, however, is still a working hypothetical framework that needs to be experimentally confirmed.

Reduction of the closure time of postoperative enterocutaneous fistulas with fibrin sealant

AIM: To assess whether the use of fibrin sealantshortens the closure time of postoperative enterocutaneous fistulas (ECFs). METHODs: The prospective case-control study included 70 patients with postoperative ECFs with an output of stulous tract of > 2 cm and without any local complication. They were divided into study (n = 23) and control groups (n = 47). Esophageal, gastric and colocutaneous fistulas were monitored under endoscopic visualization, which also allowed fibrin glue application directly through the external hole. Outcome variables included closure time, time to resume oral feeding and morbidity related to nutritional support. REsULTs: There were no differences in mean age, fistula output, and follow-up. Closure-time for all patients of the study group was 12.5 ± 14.2 d and 32.5 ± 17.9 d for the control group (P support was 8.6% and 42.5%, respectively (P s with colonic fistulas, complete closure occurred 23.5 ± 19.5 d after the first application of fibrin glue, and spontaneous closure was observed after 36.2 ± 22.8 d in the control group (P = 0.36). Recurrences were observed in 2 patients because of residual disease. One patient of each group died during follow-up as a consequence of septic complications related to parenteral nutrition. CONCLUsION: Closure time was significantly reduced with the use of fibrin sealant, and oral feeding was resumed faster. We suggest the use of fibrin sealant for the management of stable enterocutaneous fistulas.

The Phenomenological Approach as Epistemologically support to Legitimize the Position of Oneself to the Other of Psychoanalysis

This work aims to show, in the phenomenology of Husserl, the relevance to seek the elements that give epistemological legitimacy and validity to the conceptualization that brings psychoanalytic theory to account for processes whose explanation necessarily imply to clarify that psychoanalysis, in contrast to psychological treatments, is not based on the effects of a relationship between the participants but in the positioning that each of them occupies relative to the other, particularly when having in mind the concept of transference and recognizing in it the position from which the analyst can give conditions of possibility--through his interventions--for which the patient may realize, analyze, and rethink his own position on what constitutes his discomfort. Husserl recaptures the Cartesian base from which opens philosophical reflection to a new area of research ＂in＂ consciousness where it is allowed to understand the relationship between subject and object from a new perspective that enables establishing an epistemology that substantiates the rigorous analysis of human subjectivity. Husserl＇s proposal introduces us to an epistemic field in which it can be shown and, that the psychoanalytic method which enables its effects is precisely the position of the analyst, while by his presence and intervention leads to the other in question, without having to abide by an alien desire, be able to position himself before his own desire. This analysis regarding the desire of oneself is indispensable to understand the elements at play in contemporary psychopathology, as in the present context the intensity of the demands generated in the economic apparatus, are experienced by the subject as arising from himself, erasing traces of his desire that is superseded by social imperatives that crushed and fragmented him.
