Objectives: The main objective was to evaluate antibiotic prescribing in the Department of Pediatrics at gabriel Touré teaching hospital. Methods: This was a prospective descriptive study conducted from September 1 to November 31, 2018, in the Department of Pediatrics at gabriel Touré teaching hospital. All children aged 0 to 15 years hospitalized for any pathology during the study period and having received at least one antibiotic had been included. Results: We collected 445 children’s records out of 1032 admissions during the study period, representing a hospital frequency of 43.1%. The sex ratio was 1.3%. The 2 - 5 age group accounted for 48.1%. Fever was the reason for consultation in 45.6% of cases. Patients’ general condition was altered in 60% of cases. The fathers were blue-collar workers in 65.4% and the mothers housewives in 85%, and had no education in 42.9% and 64.5% respectively. Hepatomegaly was present in 18.2%, splenomegaly in 9.6% and peripheral adenopathy in 3.1%. The site of infection was pulmonary in 37.6% and ENT in 9.2%. bacterial infection was assumed on admission in 54% of cases, and meningitis in 57.7%. The discharge diagnosis was ma.a.ia in 54.6%, severe acute malnutrition in 18.6% and meningitis in 6.7%. The death rate was 3.8%. Neutrophilic leukocytosis was present in 47.3% of patients. CRP was positive in 85% of patients. Blood cultures taken in 27.6% of patients were positive in 5. CSF analysis in 30% of patients showed elevated leukocytes in 6.5%. No cultures were positive. Antibiotic prescription was justified by infectious hypotheses in 43.1% of patients. β-lactam antibiotics were prescribed in 98.6%. Antibiotic therapy was not adapted to national/international recommendations in 68.3% of cases, and was not justified in 16.3% of cases on D5 of hospitalization. 目的:分析不同脂肪组织来源干细胞源性外泌体(ADSC-Exos)蛋白质组学差异,为深度创面修复提供理论依据。方法:取烧伤痂组织与正常皮下脂肪组织,分离脂肪干细胞(ADSC),通过光镜观察、Western blot(WB)、成骨分化、成脂分化鉴定ADSC。差速离心方法收集脂肪干细胞外泌体(ADSC-Exos),通过电镜观察、WB法、Nanosight分析仪鉴定ADSC-Exos;通过非标记定量蛋白质谱(LFQ)技术分析两组患者ADSC-Exos蛋白质表达水平差异。结果:成功分离ADSC,光镜下呈纤维状,WB显示细胞稳定表达CD29及CD105,成骨诱导分化细胞内见红色密集钙结节、成脂诱导分化细胞内存在折光性好的透亮脂滴。成功分离ADSC-Exos,电镜下呈双膜性结构,WB显示外泌体可稳定表达表面标志物CD63及CD81,Nanosight显示外泌体均匀对称分布,直径30~150 nm。两组表达水平差异蛋白质184种,差异倍数显著上调前10种蛋白:丝裂原活化蛋白激酶3、丝裂原活化蛋白激酶1、转化生长因子β-1前蛋白、细胞凋亡调节因子baX、热休克蛋白HSP 90α、微管蛋白α-1B链、腺病毒E1B19kDa相互作用蛋白2、钙调素-3、热休克蛋白HSP 90β、蛋白激酶Cβ型;差异倍数显著下调前10种蛋白:激肽源B1、激肽源1、补体家族(C3、C1q C链、C1q B链、C5、C1q A、C4B、C4A、C9)。结论:烧伤痂下组织来源ADSC-Exos蛋白与正常组织的表达存在明显差异,可能与烧伤创面修复、抑制瘢痕过度增生及抗炎与机体免疫等密切相关。

Study on daily surface evapotranspiration with SEBS in Tibet Autonomous Region

The estimation of surface evapotranspiration （ET） with satellite dataset is one of the main subjects in the understanding of climate change, disaster monitoring and the circulation of water vapor and energy in Tibet Autonomous Region （TAR）. This research selects satellite images on January 11, April 6, July 31 and October 19 in 2010 as the representative of winter, spring, summer and autumn respectively, estimates the distribution of daily surface ET based on the surface energy balance system （SEBS） along with potential evapotranspiration （PET） and ET derived from Penman-Monteith （P-M） method. The results are obtained as follows. （1） The seasonal distribution of ET and PET basically decreases from the southeast part to the northwest part of TAR. Although ET and PET have similar spatial distributions, there are still some differences to estimate the extreme values especially the maximum value in the middle and southeastern parts of TAR. No matter what kind of methods we adopted, the maximum value of ET and PET always appears in summer, followed by autumn or spring while that in winter is the smallest. （2） In order to better understand the accuracy of SEBS model in the estimation of ET, we compared the ET from SEBS and the ET obtained from P-M method. Results show that the ET from SEBS could estimates the variation trend of actual ET, but it slightly underestimates or overestimates the value of ET as a whole, especially for those areas with thick forest. （3） The spatial distribution of Normalized Difference Vegetation Index （NDVI） exhibits a decreasing trend from the southeast part to the northwest part of TAR which displays remarkable consistency of distributions between ET and vegetation index. ET is well positively related to NDVI, minimum, mean, maximum air temperature and sunshine duration in different seasons while negatively related to precipitation, relative humidity and wind speed in summer.

Appendicitis bilharzia is a very rare condition and we report 3 cases of this pathology that sense clinical and biological similarities with bacterial appendicitis. The etiological diagnosis was exclusively histopathological, this allowed to highlight eggs of Schistosome in the appendicular wall in the three patients. The surgical treatment was supplemented by a specific medical treatment based on praziquantel. The surgical suites were simple for 2 patients, complicated by superficial parietal suppuration in a patient.

Head and Neck Cellulitis of Dental Origin and HIV Infection: 24 Cases

Cervico-facial cellulitis is a serious poly-microbial infection of the deep fascial spaces of the face and neck which may be life-threatening. The existence of an underlying ground that causes immune failure may promote or complicate this infection. The aim of this work was to describe the socio-demographic and clinical aspects of dental cervicofacial cellulitis associated with HIV in our context. It was a descriptive cross-sectional study that concerned all cases of cervico-facial cellulitis with positive HIV serology encountered at the CHU-CNOS of bamako from may 2017 to January 2018. Twenty-four cases were collected. Cellulite with positive HIV accounted for 1.14% of consultations in the service. Females accounted for 67.66% of cases with a sex ratio of 0.5. The average age was 33.58 years old. Housewives accounted for 33.33% of cases. The bride and groom represented 83% of the cases. The triad (pain, swelling, and trismus) was the most common reason for consultation with 41.7% of cases. The tooth 37 was involved in 25% of the cases. In 87% of cases, patients were screened in the service. HIV-1 was found in 75.0% of cases. Complications such as peripheral facial palsy were found in three cases. The medico-surgical treatment was performed in 23 cases with a favorable evolution. The search for an underlying ground, in particular HIV infection, must be systematic in the face of diffuse cervicofacial cellulitis.

Introduction: Chronic renal failure is a real public health problem because of its prevalence, the cost of its ma.a.ement and the high morbidity and mortality rate associated with it. The objective of this study was to determine the frequency, causes and main aggravating factors of chronic renal failure in the medical department of the hospital of Sikasso. materials and Methods: This was a cross-sectional, retrospective, descriptive study conducted in the medical department of the Sikasso hospital from January 1, 2021 to November 30, 2022. It covered all the records of patients with chronic renal failure hospitalized in the department during this period. The MDRD formula was used to estimate the glomerular filtration rate. The KDIGO 2012 classification was used to stage CKD. Results: Of 820 patient records reviewed, we retained 197 records. The mean age of our patients was 42.25 ± 10 years. The male sex represented 114 (57.9%). The socio-economic level was considered low and precarious in 79.8% of cases. The frequency of chronic renal failure was 24%. The reason for hospitalization was mostly hyper creatinine > 185 μmol/l (92.4%). Herbal medicine was reported in 103 cases (52.3%). Medical history was hypertension 101 cases (51.3%), undocumented gastro duodenal ulcer (UGD) 14 cases (7.6%), hypertension and diabetes 11 cases (5.1%), diabetes 8 cases (4%), lower limb edema 9 cases (4.6%). Hypocalcemia was 147 cases (75.6%) with hyper phosphoremia was 153 cases (77.7%). Hemoglobin level was: <6 g/dl, 44 cases (22.3%);between 6 - 8 g/dl, 77 cases (39.1%), from 8 - 10 g/dl, 54 cases (27.4%). The etiologies of CKD were vascular nephropathy 106 cases (53.8%), interstitial nephropathy, 44 cases (22.3%), glomerular nephropathy, 33 cases (16.8%), diabetic nephropathy, 12 cases (6.1%) and polycystic kidney disease 2 (1%). CKD was classified as stage 5, 171 cases (86.8%), stage 4, 11 cases (5.6%), stage 3, 13 cases (6.6%) and stage 2, 2 cases (1%.) Dialysis was performed in 1095

Objective: The main objective was to study the disturbances of the Blood Count of children hospitalized in the general pediatric of the gabriel Touré teaching hospital. Methods: This was a prospective and descriptive study conducted from September 1 to November 30, 2018 in the general pediatrics department of the gabriel Touré teaching hospital in bamako. Data were collected on patient records using a survey sheet. Results: We collected 512 files of children out of 1030 admissions during the study period;the rate of completion of the blood count is 50%. The male sex was predominant with a sex ratio of 1.3. The majority of patients were under 5 years old (58%). The majority of fathers (56%) and mothers (64%) of children had no education;they are mainly farmers (61%) and housewives (88%). Pallor was the reason for consultation in 29% of patients and present in 60%. On blood count, anemia was present in 92% of patients, half of whom (50%) had severe anemia with a hemoglobin level below 7 g/dL. The anemia was mainly microcytic (72%) and hypochromic (66%). Hyperleukocytosis (62%), eosinophilia (68%) and basophilia (58%) were the abnormalities observed in the white line. Thrombocytopenia accounted for 40%. Severe ma.a.ia (53%) was the main discharge diagnosis and almost all patients (99%) were alive at discharge. Conclusion: The characteristics of anemia require a study of the complete blood count in healthy children with dosage of serum iron and ferritin for a better understanding of the phenomenon.
