Temporal and spectral contributions to musical instrument identification and discrimination among cochlear implant users

UNASSIGNED:To investigate the contributions of envelope and fine-structure to the perception of timbre by cochlear implant (CI) users as compared to normal hearing (NH) listeners.;UNASSIGNED:This was a prospective cohort comparison study. Normal hearing and cochlear implant patients were tested. Three experiments were performed in sound field using musical notes altered to affect the characteristic pitch of an instrument and the acoustic envelope. Experiment 1 assessed the ability to identify the instrument playing each note, while experiments 2 and 3 assessed the ability to discriminate the different stimuli.;UNASSIGNED:Normal hearing subjects performed better than CI subjects in all instrument identification tasks, reaching statistical significance for 4 of 5 stimulus conditions. Within the CI population, acoustic envelope modifications did not significantly affect instrument identification or discrimination. With envelope and pitch cues removed, fine structure discrimination performance was similar between normal hearing and CI users for the majority of conditions, but some specific instrument comparisons were significantly more challenging for CI users.;UNASSIGNED:Cochlear implant users perform significantly worse than normal hearing listeners on tasks of instrument identification. However, cochlear implant listeners can discriminate differences in envelope and some fine structure components of musical instrument sounds as well as normal hearing listeners. The results indicated that certain fine structure cues are important for cochlear implant users to make discrimination judgments, and therefore may affect interpretation toward associating with a specific instrument for identification.

对准各向同性纤维复合材料的动态抗压强度进行试验和数字模拟。采用分离式霍普金森压杆,在应变率400/s左右进行准各向同性层压板的平面压缩试验。与静态值相比,准各向同性层压板的动力强度大大增强。采用ABAQUS软件中实体8结点单元C3D8I和作者自定义的8结点界面单元建立有限元模型。界面单元连接三维实体单元来模拟复合材料层,其包括允许初始剥离和扩展的粘结破坏模式。假定模型在这些条件下的破坏现象主要取决于界面的剥离,这一结论通过试验得到验证。试验显示所有的准各向同性层压板几乎劈裂成完整的几块。模型结果与试验结果十分吻合,证实了假设:层内破坏不是准各向同性层压板破坏的主要原因。

Discriminating sterile inflammation from infection, especially in cases of aseptic loosening versus an actual prosthetic joint infection, is challenging and has significant treatment implications. Our goal was to evaluate a novel human monoclonal antibody（mAb） probe directed against the Gram-positive bacterial surface molecule lipoteichoic acid（LTA）. Specificity and affinity were assessed in vitro. We then radiolabeled the anti-LTA mAb and evaluated its effectiveness as a diagnostic imaging tool for detecting infection via immuno PET imaging in an in vivo mouse model of prosthetic joint infection（PJI）. In vitro and ex vivo binding of the anti-LTA mAb to pathogenic bacteria was measured with Octet, ELISA, and flow cytometry. The in vivo PJI mouse model was assessed using traditional imaging modalities, including positron emission tomography（PET） with [^（18）F]FDG and [^（18）F]Na F as well as X-ray computed tomography（CT）, before being evaluated with the zirconium-89-labeled antibody specific for LTA（[^（89）Zr]SAC55）.The anti-LTA mAb exhibited specific binding in vitro to LTA-expressing bacteria. Results from imaging showed that our model could reliably simulate infection at the surgical site by bioluminescent imaging, conventional PET tracer imaging, and bone morphological changes by CT. One day following injection of both the radiolabeled anti-LTA and isotype control antibodies, the anti-LTA antibody demonstrated significantly greater（P 〈 0.05） uptake at S. aureus-infected prosthesis sites over either the same antibody at sterile prosthesis sites or of control non-specific antibody at infected prosthesis sites. Taken together, the radiolabeled anti-LTA mAb, [^（89）Zr]SAC55, may serve as a valuable diagnostic molecular imaging probe to help distinguish between sterile inflammation and infection in the setting of PJI. Future studies are needed to determine whether these findings will translate to human PJI.

近年来,临床上已广泛应用造血细胞移植治疗急性白血病、急性再生障碍性贫血等疾病,其治疗急性再生障碍性贫血的成功率约有50％。对急性白血病治疗,也有缓解病例的报告。可是,在造血细胞移植治疗之前必须采取适当措施,使造血细胞能够植入而不致被排斥,在一定程度上也能减轻移植治疗后发生急性免疫反应,这就使我们想到性激素类药物,它是有效的抗辐射药物,对造血可能有一定的影响,在造血细胞移植中是否能产生良好的作用,是值得我们探索的。本文报导了3,17-β棕榈酸雌二醇(简称“棕二”)、丙酸睾丸酮(简称“丙睾”)两类性激素对致死剂量照射Laca小鼠骨髓移植疗效的影响。

Purpose:We offer our injecting experience on a new technique with hyaluronic acid cross linked to autologous fat graft with a specific periorbital cannula. method:Retrospective study on reconstructive and aesthetic injections with hyaluronic acid or autologous fat graft in the periorbital region to treat hollow eye rings,hollow cavities and eyebrow zone. Treatments were realised regarding Coleman lipostructure same protocol using autologous fat graft or hyaluronic acid. The hyaluronic acid used in each case was the same(Restylane *by Q-med).Indications were:hollow eye rings and hollow cavities,the valley of tears,the orbital part,the temporal area and the region of the eyebrow. Results:The advantage of microplasty is due to its execution in simple office as a classic injection of hyaluronic acid for wrinkle treatment with same aseptic rules as microlipostructure. We noticed less bruise and fewer surface irregularities,a better distribution of the product,retro orbicular pre periosty and less lymphatic stasis. Conclusion:We propose hyalurostructure or microlipostructure treatment for dark circles and oculofacial rejuvenation.

Running head:Tear trough management with hyalurostructure Precis:This is a comparative study of the hyalurostructure technique and its modification leading to less complication between 2009 to 2013 for tear trough treatment. Purpose:Tear troughs represent a main aesthetic characteristic within the aging process,which leads many patients to the wish for a safe and efficient rejuvenation method that will fill their eye *** this study we analyzed results and complication rates of two different methods for hollow eye ring treatment. methods & Results:In this study we used two different methods on two patient *** group A was treated according to the hyalurostructure protocol,but using the hyaluronic acid gel Restylane?(by Q-med-Sweden) and a reinforced 25g Pix’l+ micro cannula,group B received a novel method that included cooling of the periorbital area,no local anesthesia and post-interventional corticoid therapy for 48h. Our results clearly indicate that protocol B is associated with significantly less complication *** also analyzed in how far smoking,the age or skin properties may affect the clinical ***,we found out that the choice of treatment and a medical record of blepharoplasty are significant predictive factors that correlate to complications. Conclusion:Tear trough treatment by hyalurostructure shows satisfying and efficient results with few complications. Our novel technique used in group B,that includes pre-injection cooling of the periorbital area,as well as incision with displacement of the malar fat 10 mm below the orbital border,applying gentle back and forth movements while injecting and oral corticosteroid therapy 48h after intervention,proves to be more effective and safe with significantly low complication *** technique makes hyalurostructure a competent and promising treatment for tear trough management.

Immunosenescence is marked by accelerated degradation of host immune responses leading to the onset of opportunistic infections, where senescent T cells show remarkably higher ontogenic defects as compared to healthy T cells. The mechanistic association between T-cell immunosenescence and human immunodeficiency virus(HIV) disease progression, and functional T-cell responses in HIV-tuberculosis(HIV-TB) co-infection remains to be elaborately discussed. Here, we discussed the association of immunosenescence and chronic immune activation in HIV-TB co-infection and reviewed the role played by mediators of immune deterioration in HIV-TB coinfection necessitating the importance of designing therapeutic strategies against HIV disease progression and pathogenesis.

Interventions to improve treatment adherence among adolescents: A meta-analysis

AIm: To examine the overall effectiveness of interventions designed to improve medical treatment adherence among adolescent ***: Pub med and Psyc INFO databases were searched to retrieve and analyze empirical journal articles(from 1948-2013). Only peer-reviewed, English language journals that defined a measure of adherence(or compliance), assessed an intervention aimed at improving adherence among adolescents, and provided information to calculate an r effect size were included. Studies were excluded if they lacked assessment of the effectiveness of interventions on improving adherence in adolescents as compared to no interventions or standard care. Case studies or journal articles that examined substance abuse or psychological disorders were also excluded. Analyses were conducted with fixed and random-effects methods, and moderators of intervention efficacy were also examined. RESULTS: For each study that met the inclusion crite-ria(n = 45), an effect size r, reflecting the strength and direction of the interventions' relationship to adherence was recorded; a positive r indicated that the intervention increased adolescent adherence, whereas a negative r indicated that the intervention decreased adolescent adherence. The overall effectiveness of adolescent adherence interventions was positive and significant(unweighted mean r = 0.27, 95%CI: 0.21-0.33, P = 0.001). moderator analyses at the fixed effects level revealed that interventions were less effective when adolescents reported their adherence behaviors, when the type of adherence regimen was a medication regimen, and when the type of intervention was cognitivemodification based. CONCLUSION: These findings contribute to understanding interventions for enhancing adolescent adherence. Future research should continue to examine the specific challenges faced by adolescents and create targeted interventions.

Diagnosis and management challenges of in-stent restenosis in coronary arteries

Over the course of the 3 decades, percutaneous coronary intervention(PCI) with stent implantation transformed the practice of cardiology. PCI with stenting is currently the most widely performed procedure for the treatment of symptomatic coronary disease. In large trials, drugeluting stents(DES) have led to a significant reduction in in-stent restenosis(ISR) rates, one of the major limitations of bare-metal stents. Due to these favorable findings, DES was rapidly and widely adopted enabling more complex coronary interventions. Nevertheless, ISR remains a serious concern as late stent complications. ISR mainly results from aggressive neointimal proliferation and neoatherosclerosis. DES-ISR treatment continues to be challenging complications for interventional cardiologists.
