Hepatitis B virus (HBV) chronic infection represents a significant cause of morbidity and mortality worldwide. While traditional intramuscular (IM) HBV vaccination is an excellent method for robust and sustained seroconversion in healthy individuals, its efficacy in chronic liver disease is sub-optimal and scant data exists in the post-liver transplant state. Importantly, HBV complications are even more severe in these same immunocompromised populations. Intra-dermal (ID) vaccination has shown initial promise as a successful alternative to achieving HBV seroconversion in patients refractory to standard vaccination protocols. Herein is a case report of a 61 year-old female who underwent liver transplantation for chronic HBV infection and achieved HBsAg seroconversion with a robust HSsAb titer with ID vaccination after having failed both standard and double dose IM vaccination.

Community Hospital Experience of Surgical Times and Outcomes in Patients Undergoing Cesarean Deliveries for Non-Reassuring Fetal Tracing: A Retrospective Cohort

Background: Current guidelines recommend regional anesthesia versus general as a method of choice for women undergoing cesarean deliveries (CS). However, little is known about the surgical times in the operating room and a choice of anesthesia for cesarean deliveries. Objective: This study was designed to compare times from the arrival to the OR to the delivery of the fetus between regional and general anesthesia along with maternal and fetal outcomes, for patients undergoing cesarean sections for non-reassuring fetal tracing. Study Design: Records were reviewed for patients who underwent cesarean delivery for non-reassuring fetal heart rate tracing from February 2012 to May 2018. A total of 190 charts were selected. Seven patients who received epidural or spinal anesthesia and then converted to general anesthesia (GA) were excluded. The primary outcomes were: 1) entering the operating room to skin incision (min);2) the time from entering the operating room to delivery of the fetus (min). These times were compared among the patients who underwent epidural, spinal and general anesthesia. The secondary criteria included time from skin incision to delivery of the fetus (min), estimated blood loss (ml), Apgars scores, Arterial/venous cord pH, NICU admissions and fetal complications. ANOVA or kruskal-Wallis Test was used for the continuous variable and Fisher’s exact test was used for the categorical variable to test the differences between groups. Logistic regression model was used for the binary outcomes after adjusting for age, BMI and number of prior laparotomies. Results: Infants in the GA group were delivered significantly faster when compared to epidural and spinal group separately with a P-value of 0.001. The mean time from arrival to OR to delivery of the newborn in GA group was 12.7 minutes, compared to 27 minutes in epidural group and 32.7 minutes in the spinal group. 电力系统运行规划主要使用以小时为单位运行的机组组合。目前,可再生能源在用户侧的渗透率越来越高,导致电力系统负荷产生巨大波动。因此,为了维持电力系统运行的经济性和安全需求、确保电力供应的可靠性,我们有必要进行以小时为间隔的最优潮流计算,并且考虑各种可能情况。传统的元启发法比较适合解决最优潮流问题,但是由于耗时太长,实际上并不实用。为此,本文提出了一种进化型多任务处理架构,根据不同负荷需求并行处理多个最优潮流问题。仿真结果表明该架构极大地提升了进化算法在最优潮流问题的利用率,与正统进化算法相比具有更快的计算速度。

Biliary stenting is clinically effective in relieving both malignant and non-malignant obstructions. However, there are high failure rates associated with tumor ingrowth and epithelial overgrowth as well as internally from biofilm development and subsequent clogging. Within the last decade, the use of prophylactic drug eluting stents as a means to reduce stent failure has been investigated. In this review we provide an overview of the current research on drug eluting biliary stents. While there is limited human trial data regarding the clinical benefit of drug eluting biliary stents in preventing stent obstruction, recent research suggests promise regarding their safety and potential efficacy.

Objective: To investigate the pattern and distribution of stroke in Trinidad and Tobago from 2000-2009. To identify the prevalence of co-morbid conditions among new stroke patients during the period under surveillance. Methods: Data were collected from May 2010 to July 2010 from the clinic of 728 new persons treated stroke at one of the main treatment centers. Variables measured included age, gender ethnicity, smoking status and co-morbid conditions. SPSS (Version 17) for Windows and ARC GIS version 9.3 were used to facilitate both descriptive and inferential data analysis. Results: Of the 728 new hospital admissions for the period January 2000-December 2009 for stroke, 369 (50.7%) were males and 359 (49.3%) were females. 59.8% were South-East Asian;30.5% were African and 9.7% were mixed ethnicity. The predominant age group was 60 - 69 years (n = 215, 29.5%) while less than 1% were under 30. Ischemic stroke accounted for 352 (48.4%) of all new cases for the period;Hemorrhagic stroke accounted for 14.6% (n = 107), with 37% (n = 269) classified as other unspecified condition (including unknown). Of the 728 cases examined, 171 patients died before being discharged and 552 were treated and discharged. Information of 5 cases was not available. Using this data, the overall case fatality ratio was calculated as 23.5%, with the case fatality ratio for males being 23.2% and the equivalent ratio for females being 22.9%. Using a standard classification, the majority, (n = 389, 53.4%), of cases were classified as mild;246 (33.8%) were deemed moderate, and 93 (12.8%) were severe cases. Hypertension was clinically diagnosed in 80.9% of the cases;56.3% were diabetic, and 21.7% were classified as smokers having been either past or current smokers. Other lifestyle risk factors such as obesity and exercise were not examined due to the lack of the relevant data. 额外牙是较为常见的牙齿形成障碍．其发病形式多种多样：可单发或多发．可位于单侧或双侧．可能出现在双颌或仅限于单颌。上颌正中额外牙是指发生在上颌前部两中切牙之间的额外牙。本文介绍了一例12岁男孩因4颗上颌正中额外牙导致上颌中切牙异位阻生的多学科联合治疗。经临床及X线检查后．外科拔除4颗上颌正中额外牙同时暴露阻生的右侧上中切牙，术后两中切牙间间隙达14mm。随后．根据头影测量分析行正畸固定矫治：将中切牙牵引至耠平面高度后继续排齐．根据牙量调整切牙间间隙，通过复合树脂修复切牙间少量残余间隙。
