BACkGROUNG For a long time,laryngopharyngeal reflux disease(LPRD)has been treated by proton pump inhibitors(PPIs)with an uncertain success *** To shed light the current therapeutic strategies used for LPRD in order to analysis the rationale in the LPRD *** Three authors conducted a PubMed search to identify papers published between January 1990 and February 2019 about the treatment of *** prospective or retrospective studies had to explore the impact of medical treatment(s)on the clinical presentation of suspected or confirmed *** criteria for considering studies for the review were based on the population,intervention,comparison,and outcome *** The search identified 1355 relevant papers,of which 76 studies met the inclusion criteria,accounting for 6457 patients.A total of 64 studies consisted of empirical therapeutic trials and 12 were studies where authors formally identified LPRD with pH-monitoring or multichannel intraluminal impedance-pH monitoring(MII-pH).The main therapeutic scheme consisted of once or twice daily PPIs for a duration ranged from 4 to 24 *** most used PPIs were omeprazole,esomeprazole,rabeprazole,lansoprazole and pantoprazole with a success rate ranging from 18%to 87%.Other composite treatments have been prescribed including PPIs,alginate,prokinetics,and H2 Receptor *** Regarding the development of MII-pH and the identification of LPRD subtypes(acid,nonacid,mixed),future studies are needed to improve the LPRD treatment considering all subtypes of reflux.

The current pandemic due to the severe acute respiratory syndrome coronavirus 2 has caused an extreme burden for health care systems globally,and the number of cases is expected to continue to increase,at least in the immediate *** virus is estimated to have infected more than 1.5 million *** available reports suggest that gastrointestinal(GI)involvement in coronavirus disease 2019(COVID-19)is common and in some cases the GI symptoms may precede the respiratory *** addition to direct effects of severe acute respiratory syndrome coronavirus 2,the infected patients remain at risk for the complications commonly managed by gastroenterology and hepatology *** most commonly reported GI manifestation of COVID-19 is diarrhea,which is reported in a third to up to more than half of the *** to moderate elevation of the liver enzymes are also common,although no case of acute liver failure has been reported so *** of the medications used for treatment of COVID-19 can also be associated with GI symptoms or liver injury and can be included in the differential diagnosis in these *** the diagnosis of the infection is currently based on RNA analysis in respiratory samples,the available literature on fecal shedding of this virus suggests that fecal RNA testing might prove to be a useful diagnostic *** is reasonable to delay all non-urgent endoscopic procedures during the peak of the pandemic and use additional protective equipment such as N95 respirators during endoscopy while most patients can be considered high risk for having been exposed to the virus.

GIS-Based Situational Analysis of Cutaneous Leishmaniasis Disease (CLD) in Sri Lanka

Cutaneous Leishmaniasis (CL) is a severe health problem and a parasitic disease on human dermal and widely pervades tropical and subtropical developing counties. The study is mainly focused on Geographic Information System (GIS) based Situational Analysis (SA). The clinically recorded 394 CL patients’ information was obtained from the District General Hospital of Polonnaruwa (DGHP) for 2017 and 2018. The spatial distribution of these patients was collected using Global Positing System (GPS). Moran’s I Index spatial autocorrelation technique and Getis-Ord Gi were used to identify the study site’s hot spot and cold spot areas. More than 75% of the CL patients’ population were highly involved with agricultural activities, and they are the highly exposed group of the CL in the study area. Also, 75% of the CL population were men, and the highly vulnerable age group was 35 - 39 men and 40 - 44 women. The generated Moran’s I Index indicates 0.0321, representing a randomly distributed pattern of CL patients over the District, and the Getis-Ord Gi Z Score value was >1.96 (p ks earlier than the reported month. Hence to prevent the disease spread, it is essential to implement an awareness program regarding sandflies’ behaviour and CLD.

Efficient Power Conditioner for a Fuel Cell Stack-Ripple Current Reduction Using Multiphase Converter

Integration of fuel cell stack with an inverter is complex in nature. A number of factors have to be taken into account in designing the inverter as well as during the integration. One of these factors is ripple current which could affect the life of the fuel cell stack if there is fuel and/or oxidant starvation. In this paper an inverter topology is investigated which significantly reduces or even nullifies the ripple content in the fuel cell system. The investigations have been carried out using indigenously developed 1 kW PEMFC stack and a 4 kW PEMFC stack with single and multi phase inverter. The results are presented here.

目的探讨腹腔巨噬细胞功能平衡对小鼠腹腔微环境和腹腔异位子宫内膜细胞清除的影响。方法给小鼠、裸鼠腹腔注射子宫内膜上皮和间质细胞,不同时间观察腹腔募集巨噬细胞数,D iI-Ac-LDL鉴定分化巨噬细胞吞噬功能的清道夫受体(SR-A1)。同步反转录-聚合酶链技术(Real-Tim e RT-PCR)检测巨噬细胞的MCP-1/JE和IL-1αmRNA表达。结果腹腔注射子宫内膜细胞后的小鼠、裸鼠募集巨噬细胞增多,上皮细胞的刺激作用强于间质细胞。24 h点为腹腔巨噬细胞募集高峰、MCP-1/JE和IL-1α的基因表达高峰,免疫正常小鼠表达反应吞噬功能的巨噬细胞清道夫受体(SR-A1)强于免疫缺陷裸鼠。结论子宫内膜细胞募集腹腔巨噬细胞为一独立免疫防御反应,募集巨噬细胞的清道夫功能和细胞因子分泌功能的平衡可能对维持正常腹腔微环境和有效清除异位子宫内膜细胞起重要作用,这在内异症的发病机制中具有重要意义。

Acaricidal activity of synthesized titanium dioxide nanoparticles using Calotropis gigantea against Rhipicephalus microplus and Haemaphysalis bispinosa

Objective:To assess the acaricidal activity of titanium dioxide nanoparticles(TiO_2 NPs)synthesized from flower aqueous extract of Calotropis gigantea(***)against the larvae of Rhipicephalus(Boophilus)microplus[R.(B.)microplus]and the adult of Haemaphrysalis bispinosa(***).Methods:The lyophilized *** flower aqueous extract of 50 mg was added with 100 mL of TiO(OH_2)(10 mM)and magnetically stirred for 6 *** TiO_2 NPs were characterized by X-ray diffraction(XRD).Fourier transform infrared spectroscopy(FTIR),Scanning electron microscopy(SEM),and Energy dispersive X-ray spectroscopy(EDX).The synthesised TiO_2 NPs were tested against the larvae of R(B.)microplus and adult of *** were exposed to filter paper impregnated ***:XRD confirmed the crystalline nature of the nanoparticles with the mean size of 10.52 *** functional groups for synthesized TiO_2NPs were 1405.19,and 1053.45 cm^(-1)for-NH_2 bending,primary amines and amides and 1053.84and 1078.45 cm^(-1)for *** micrographs of the synthesized TiO_2 NPs showed the aggregated and spherical in *** maximum efficacy was observed in the aqueous flower extract of *** and synthesized TiO_2 NPs against R.(B.)microplus(LC_(50)=24.63 and 5.43 mg/L and r^2=0.960 and 0.988)and against ***(LC_(50)=35.22 and 9.15 mg/L and r^2=0.969 and 0.969).***:The synthesized TiO_2 NPs were highly stable and had significant acaricidal activity against the larvae of R.(B.)microplus and adult of *** study provides the first report of synthesized TiO_2 NPs and possessed excellent anti-parasitic activity.

目的:探讨腹腔微环境受子宫内膜细胞刺激改变中腹膜间皮细胞的作用及其对子宫内膜异位症发病机制的意义。方法:注射小鼠子宫内膜上皮和间质细胞入小鼠腹腔,酶联免疫吸附法(ELISA)检测注射后4、24和72h时点腹腔冲洗液细胞因子MCP-1/JE、IL-1α和IL-6蛋白表达,同步逆转录-聚合酶链式反应技术(Real-Time RT-PCR)检测腹膜组织(主要含腹膜间皮细胞)和腹腔巨噬细胞的细胞因子MCP-1/JE、IL-1α和IL-6mRNA表达。结果:子宫内膜细胞刺激腹腔细胞因子蛋白含量迅速一过性升高,4h时点为表达高峰,腹膜细胞因子基因表达与腹腔液蛋白表达同步,腹腔巨噬细胞基因表达高峰滞后于腹腔液蛋白表达。子宫内膜上皮细胞刺激腹腔炎症反应作用强于间质细胞。结论:子宫内膜细胞刺激腹腔发生非特异性炎症反应,腹膜间皮细胞可能是其细胞因子效应的主要来源,提示腹膜在子宫内膜异位症中除作为病灶依附体外,还可能在腹腔微环境无菌性炎症反应中起重要作用。

Objective: To describe the proportion of women presenting with undiagnosed anaemia at confinement and to explore associations between anaemia and socio economic factors, dietary pattern and compare maternal and neonatal outcome among term mothers with anaemia compared to non-anaemic pregnant women at a tertiary care centre in Sri Lanka. Method: A comparative cross-sectional study was performed by recruiting anaemic and non-anaemic term mothers who delivered at the Teaching Hospital Peradeniya during the period March 2018-March 2019. Pregnant mothers whose haemoglobin level was less than 10.5 g/dl were considered as anaemic and compared with the mothers whose haemoglobin level was above this level. With written consent, demographic data, etiological factors, maternal and neonatal outcomes of term mothers were evaluated by an interview and blood samples were withdrawn to carry out anaemia related investigations. Results: Among 2854 pregnancies, a total of 234 (8.19%) term pregnant mothers were anaemic and they were and compared with 199 non-anaemic mothers. High-Dose Intra-Dermal Hepatitis B Vaccine in a Liver Transplant Patient Who Failed Prior Intramuscular Vaccination: A Brief Case Report

Hepatitis B virus (HBV) chronic infection represents a significant cause of morbidity and mortality worldwide. While traditional intramuscular (IM) HBV vaccination is an excellent method for robust and sustained seroconversion in healthy individuals, its efficacy in chronic liver disease is sub-optimal and scant data exists in the post-liver transplant state. Importantly, HBV complications are even more severe in these same immunocompromised populations. Intra-dermal (ID) vaccination has shown initial promise as a successful alternative to achieving HBV seroconversion in patients refractory to standard vaccination protocols. Herein is a case report of a 61 year-old female who underwent liver transplantation for chronic HBV infection and achieved HBsAg seroconversion with a robust HSsAb titer with ID vaccination after having failed both standard and double dose IM vaccination.
