The Internet of Things(IoT)is a recent technology,which implies the union of objects,“things”,into a single worldwide *** promising paradigm faces many design challenges associated with the dramatic increase in the number of *** identification is one of these challenges that becomes complicated with the increase of network *** this,there is still no universally accepted method of identifying things that would satisfy all requirements of the existing IoT devices and *** this regard,one of the most important problems is choosing an identification system for all IoT devices connected to the public communication *** unique soft-ware and hardware solutions are used as a unique global identifier;however,such solutions have many *** article proposes a novel solution,based on the Digital Object Architecture(DOA),that meets the requirements of identifying devices and applications of the *** work analyzes the benefits of using the DOA as an identification platform in modern telecommunication *** propose a model of an identification system based on the architecture of digital objects,which differs from the well-known *** proposed model ensures an acceptable quality of service(QoS)in the common architecture of the existing public communication networks.A novel interaction architecture is developed by introducing a Middle Handle Register(MHR)between the global register,i.e.,Global Handle Register(GHR),and local register,i.e.,Local Handle Register(LHR).The aspects of the network interaction and the compatibility of IoT end-devices with the integrated DOA identifiers in heterogeneous communication networks are *** developed model is simulated for a wide-area network with allocated registers,and the results are introduced and discussed.

Aim:Studies of clinical outcomes in chronic hepatitis C(CHC)patients with pretreatment advanced liver fibrosis(F3)after sustained virologic response(SVR)are scarce,and most studies are of small cohorts and retrospective in *** aim was to assess the clinical outcomes following direct acting antiviral(DAA)treatment among hepatitis C F3 patients after ***:This study included 1517 chronic hepatitis C patients with F3 fibrosis receiving DAAs in the out-patient clinics at the Egyptian Liver Research Institute and Hospital(ELRIAH).We included patients 18 years or older with HCV who received DAAs,have F3 by transient elastography,and have no history of hepatocellular carcinoma(HCC).Patients were followed up every six months after end of treatment using ultrasonography and ***:Significant improvement of fibrosis occurred with decreases in LSM,FIB-4,APRI,and *** changes in LSM were categorized depending on delta LSM,873 patients(57.5%)showed regression,454(29.9%)were stable,and 190 patients(12.5%)showed progression of ***,33 cases developed HCC during follow-up with incidence rate of 0.915/100 py(95%CI:0.64-1.27).Incidence was high with progression of liver fibrosis(6.17/100 py)compared to patients with stable fibrosis(1.09/100 py)and regression of liver fibrosis(0.75/100 py).There were no significant differences as regards fibrosis indicators at baseline(LSM,FIB-4,APRI,and FIB-6)between those who developed HCC and those who did ***:CHC Patients with F3 fibrosis showed a high rate of regression of fibrosis and decreased HCC incidence after achieving SVR following DAAs.

The present study was designed to investigate the potential protective effect of melatonin as an antioxidant separately or in combination with antigens （cerearial; CAP or soluble worm; SWAP） against Schistosoma mansoni infection in hamsters. Each hamster was sensitized with an initial immunization of 0.6 ml of the extracted antigen （30μg protein/mL）. After four days, a second injection of 0.4 mL was given （20μg protein/mL）. Then, each hamster was exposed to 260 ± 20 S. mansoni cercariae followed with melatonin treatment （3.5 mg/kg） for thirty days from the 1st day of post infection. Levels of lipid peroxidation （LPO） products, eatalase （CAT） activity, hepatic glutathione （GSH） and biochemical changes in the liver and kidneys functions were investigated. The results revealed a high significant increasing of LPO and decreasing of CAT and GSH in liver of infected hamsters. Biochemical observations showed severe damage in the liver enzyme activities and increasing cholesterol level in infected animals. Melatonin co-treatment with antigen to the infeeted-hamster attenuated the increase of LPO and restored the activity of CAT and levels of hepatic GSH. Also, the biochemical damages in the liver and kidneys funetions were reduced. The present study suggests that melatonin may be useful in combating free radical-induced damage due to infection toxicity. The immunization with previous antigens resulted in a remarkable improvement on the liver enzyme activities, which were increased after infection. Thus, vaccination of hamsters with antigens （both CAP and SWAP） and melatonin treatment has more potent effect on the enhancement of antioxidant and biochemical of S. mansoni infected-hamster than each treatment separately. Immunization of the hamster with SWAP followed by melatonin was the best way among the other regime treatments to improve the biochemical and antioxidant parameters of the infected-hamsters.

Herein, the fabrication of cellulose acetate (CA) silica-based nanocomposite membranes via the dry-wetphase inversion procedure for water desalination was investigated. The modified and unmodified silicananoparticles (MSNPs and SNPs) were prepared by the sol-gel technique. The effect of the SNPs andMSNPs was investigated on the CA membrane's properties and their performance for water *** CA nanocomposite membranes were characterized to study their structure, hydrophilicity, andmorphology. The fabricated nanocomposite membranes showed hydrophilic surface properties. Theperformance of reverse osmosis (RO) membranes was measured using a crossflow RO unit at 10 bar(1 bar = 0.1 MPa). The membrane with 10 mg of SNPs enhanced permeate water flux compared to thepristine CA membrane by 1.6 L/(m2·h). The effect of MSNPs on the nanocomposites' performance waslower than their counterpart in the case of adding SNPs. The membrane with 30 mg of MSNPs showedthe highest permeate water flux among other nanocomposite membranes with a value oAQSf 35.7 L/(m2·h)at 24 bar.

Two low alloy steels 0.5Cr-0.5Mo-0.25V and H85 were pack-aluminized at 900°for 4 h by using Fe-Al powder mixture containing 48% Fe, 20.6% Al- 29.4% Al2O3 and 2% NH4Cl by weight. The microhardness and oxidation resistance at 900℃ of the aluminide coatings were studied. It was found that pack-aluminizing improves the microhardness of the 0.5Cro.5Mo-0.25V steel while it reduces the microhardness of the H85 steel. Pack aluminizing highly improves the oxidation resistance after 20h exposure at 900℃ in air for the investigated steels.

One of the leading causes of mortality worldwide is liver *** earlier the detection of hepatic tumors,the lower the mortality *** paper introduces a computer-aided diagnosis system to extract hepatic tumors from computed tomography scans and classify them into malignant or benign *** hepatic tumors from computed tomography scans is considered a challenging task due to the fuzziness in the liver pixel range,intensity values overlap between the liver and neighboring organs,high noise from computed tomography scanner,and large variance in tumors *** proposed method consists of three main stages;liver segmentation using Fast Generalized Fuzzy C-Means,tumor segmentation using dynamic thresholding,and the tumor’s classification into malignant/benign using support vector machines *** performance of the proposed system was evaluated using three liver benchmark datasets,which are MICCAI-Sliver07,LiTS17,and *** proposed computer adided diagnosis system achieved an average accuracy of 96.75%,sensetivity of 96.38%,specificity of 95.20%and Dice similarity coefficient of 95.13%.

Evaluation of GALAD Score in Diagnosis and Follow-up of Hepatocellular Carcinoma after Local Ablative Therapy

Background and Aims:Strategies for detection of early hepatocellular carcinoma(HCC)are still *** GALAD score is a serum biomarker-based model designed to predict the probability of having *** aimed to assess the ability of GALAD score to diagnose early HCC and its validity to follow patients after local ablation ***:This multicenter prospective study included 108 patients in two groups,58 HCC patients(67 focal lesions)with local ablative therapy(study group),and a control group of 50 patients with liver *** GALAD scores of the study and control groups,and of the HCC patients before and after ablative therapy were ***:Most patients were men(74.1%in study group and 76%in controls)with hepatitis C virus infection(98.30%in the study group,and 94%in controls).GALAD scores were significantly higher in HCC patients than in those with benign cirrhosis(2.65 vs.−0.37,p=0.001).Ablative therapy was successful in 94.4%of focal lesions<2 cm,and in 86.10%of 2–5 cm *** GALAD score was also significantly lower at 1 month after ablation in patients with well-ablated tumors(2.19 vs.0.98,p=0.001).The best cutoff values of GALAD score for diagnosis of early HCC,and for prediction of well ablation of HCC were 0.74 and≤3.31(areas under the curve of 0.92 and 0.75,sensitivities of 84.48%and 76.19%,specificities of 89.13%and 83.33%,positive predictive values of 90.74%and 94.1%,and negative predictive values of 82%and 35.7%respectively).Conclusion:The GALAD score was effective for the diagnosis of early HCC and for followup after ablative therapy.

The active researchers, till now, did not find a curative treatment for many diseases threatening lives of the patients. Proper management and treatment for those patients can give them longer and healthier lives. Over the years, auto-haemotherapy has been used to treat illnesses and diseases such as： arterial circulatory disorders, autoimmune diseases as rheumatoid arthritis, gout, bronchitis, allergies, ache vulgaris, skin eczema, leg ulcers, chronic sinusitis, skin lesions, HIV infection, HCV, Lyme disease, connective tissue diseases, fibromyalgia, asthma, history of heart attack and fungal infections. It helps in cessation of smoking, treatment of drug and alcohol addiction, decreasing the appetite, losing weight in obese patients and speeding up the recovery after a long period of illness. It has also been used in combination with conventional cancer treatment. Auto-haemotherapy is a simple technique where the blood is drawn from a vein and re-injected into a muscle. The blood in the muscle works as a foreign body that will be rejected by the reticulo-endothelial system. It stimulates the immune system and the bone marrow to increase the production of macrophages which are the body＇s cleaners. The macrophages cleanse everything, they eliminate bacteria, viruses, cancerous cells called neoplastic cells, this is a spring cleaning, and they even eliminate fibrin which is a clotted blood. Auto-haemotherapy is a very powerful technique with a low cost. It requires a syringe and a physician who knows how to withdraw blood from a vein and straight away does an intra-muscular injection with this blood under aseptic condition which increases and stimulates the immune system. Musculoskeletal pain is a distressing, unpleasant feeling and an emotional experience associated with actual or potential tissue damage. Prolotherapy is an injection therapy designed to stimulate tissue healing. Lyftogt PIT （perineural injection therapy） treats the neurogenic inflammation. Therefore it stops the pain and promotes the release of NGFs （nerve growth factors） which are important for tissue regeneration. Vitamin D3 receptors are ubiquitous in tissue distribution, which opens the possibility for unforeseen biological functions of vitamin D endocrine system. Dextrose is the most commonly used tissue proliferant in prolotherapy. It has different ways of action aiming to stimulate the tissue regeneration. Platelet Rich Plasma is also used in the IROM （interventional regenerative orthopedic medicine）, and it is very powerful because it delivers ＂growth-factors＂ which stimulate the local tissue progenitor cells （adult mesenchymal stem cells） enhancing the regeneration of the tissues. Adult Mesenchymal stem cell is the hot topic nowadays in the non-surgical field of pain management. In this paper, 250 patients suffering from chronic pain and dysfunction of different body joints （knee, hip and elbow）, were treated with one or two injection therapy techniques, to evaluate the patient improvement regarding the pain and function. The effect of adding an intra-articular injection of vitamin D to the above techniques was also evaluated regarding the joint pain and function improvement. Some of the patients were treated with Platelet Rich Plasma and others were treated with Bone Marrow Stem cells. The results of the 250 patients were compared with those obtained from 30 matching control patients, having same conditions and were treated with physiotherapy, Conclusions show that the therapeutic benefit from combining the two techniques, Lyflogt Perineural Injection Therapy and Prolotherapy, is better than using only one of them regarding the pain and function for the di
