Intestinal alkaline phosphatase (IAP) plays an essential role in intestinal homeostasis and health through interactions with the resident microbiota, diet and the gut. IAP’s role in the intestine is to dephosphorylate toxic microbial ligands such as lipopolysaccharides, unmethylated cytosine-guanosine dinucleotides and flagellin as well as extracellular nucleotides such as uridine diphosphate. IAP’s ability to detoxify these ligands is essential in protecting the host from sepsis during acute inflammation and chronic inflammatory conditions such as inflammatory bowel disease. Also important in these complications is IAP’s ability to regulate the microbial ecosystem by forming a complex relationship between microbiota, diet and the intestinal mucosal surface. Evidence reveals that diet alters IAP expression and activity and this in turn can influence the gut microbiota and homeostasis. IAP’s ability to maintain a healthy gastrointestinal tract has accelerated research on its potential use as a therapeutic agent against a multitude of diseases. Exogenous IAP has been shown to have beneficial effects when administered during ulcerative colitis, coronary bypass surgery and sepsis. There are currently a handful of human clinical trials underway investigating the effects of exogenous IAP during sepsis, rheumatoid arthritis and heart surgery. In light of these findings IAP has been marked as a novel agent to help treat a variety of other inflammatory and infectious diseases. The purpose of this review is to highlight the essential characteristics of IAP in protection and maintenance of intestinal homeostasis while addressing the intricate interplay between IAP, diet, microbiota and the intestinal epithelium.

Time to clinical response and remission for therapeutics in inflammatory bowel diseases: what should the clinician expect, what should patients be told?

An awareness of the expected time for therapies to induce symptomatic improvement and remission is necessary for determining the timing of follow-up, disease(re)assessment, and the duration to persist with therapies, yet this is seldom reported as an outcome in clinical trials. In this review, we explore the time to clinical response and remission of current therapies for inflammatory bowel disease(IBD) as well as medication, patient and disease related factors that may influence the time to clinical response. It appears that the time to therapeutic response varies depending on the indication for therapy(Crohn's disease or ulcerative colitis). Agents with the most rapid time to clinical response included corticosteroids, calcineurin inhibitors, exclusive enteral nutrition, aminosalicylates and anti-tumor necrosis factor therapy which will work in most patients within the first 2 mo. Vedolizumab,methotrexate and thiopurines had a longer time to clinical response and can take several months to achieve maximal efficacy. Factors affecting the time to clinical response of therapies included use of concomitant therapy, disease duration, smoking status, disease phenotype and advanced age. There appears to be marked variation in time to clinical response for therapies used in IBD which is further influenced by disease and patient related factors. Understanding the expected time to therapeutic response is integral to inform further decision making, maintain a patientcentered approach and ensure treatment is given an appropriate timeframe to achieve maximal benefit prior to cessation.

Aims We used a 10-year field experiment that consisted of mowing and fertilizer treatments to evaluate the role of niche limitation in seedling establishment of species from different functional groups and of varying local abundance in an old field undergoing *** Seedlings of nine different species were planted into a successional field subjected to mowing and fertilizer treatments for 10 years that resulted in different plant communities and resource *** representative of the factorial combination of three functional groups(C4 grasses,C3 grasses and legumes)and three abundance categories(abundant,present,or absent in the old field)were planted in four treatments resulting from the factorial combination of annual spring mowing(mowed and unmowed)and fertilizer application(annually fertilized and unfertilized).Survivorship,relative growth rate(RGR)and biomass were measured to determine the role of niche limitation on recruitment and *** Findings Mowing increased the establishment success of *** had little influence on seedling performance and survivorship.C3 grasses had the highest survivorship,while C4 grasses and legumes had equivalent RGRs,but higher than C3 *** contrast,survivorship of legumes was unrelated to mowing or fertilizer,suggesting that establishment of this functional group was dependent on other,unmeasured conditions or *** already present,but at low abundance,performed better than locally abundant or absent *** limitation may restrict the arrival of a ***,recruitment and establishment was subject to niche limitation,which varied among species,functional groups and whether a species is already resident at the site and its ***,species interactions restrict establishment during old-field succession,supporting the niche limitation hypothesis.
