背景因生活方式和手术干预而推荐患者进行转诊是全科医学临床肥胖管理的一部分。然而,目前的相关实践效果不佳。这一定性研究旨在描述影响全科医生为肥胖患者转诊的因素。方法对澳大利亚新南威尔士州4个地区的24名全科医生进行半结构定性访谈,访谈内容为针对肥胖患者的管理。应用归纳主题分析法进行定性分析。结果影响全科医生转诊决策的首要因素是态度、精力以及患者的积极性。生活方式干预转诊通常是经全科医生建议,受到患者和当地卫生系统的影响。转诊进行肥胖外科手术的要求通常由患者提出,受到医生以往有限经验、患者期待、支付能力和专业法律问题的影响。转诊与地区偏远程度或者手术转诊服务的可及性并无紧密联系。结论全科医生报告的因生活习惯和手术干预而进行转诊的行为存在不同。全科医生的转诊态度通常由以往有限的病例经验而形成,而不是通过更多的系统证据回顾,尤其是手术干预。可针对患者转诊结局,对全科医生进行培养和交流,从而改善这一状况。

The result shows tha. the region experienced cooling/exhuma.ion during the La.e Creta.eous a.d Ea.ly Tertia.y period. a.e-eleva.ion rela.ionships for different da.ing systems a.d different minera.s suggest a.pulse of ra.id exhuma.ion a. ～110 Ma.before present, preserved in the structura.ly highest sa.ples. a. lower eleva.ions, a.es begin to decrea.e with decrea.ing eleva.ion, suggesting lower exhuma.ion ra.es since 90 Ma. Two periods of different exhuma.ion ra.es a.e identified since 90 Ma. The a.era.e a.pa.ent exhuma.ion ra.e for the period of 43.4-22.5 is 0.062 km/Ma. wherea. tha. for the period of 76.4-47.4 Ma.is 0.039 km/Ma.

受新型冠状病毒肺炎疫情影响,许多有组织性筛查的国家暂停了结直肠癌筛查服务,这可能会导致癌前病变和结直肠癌的诊断延迟,进展期腺瘤和结直肠癌发病率升高。然而随着疫情的控制,筛查项目的重启,其对结直肠癌发病率及其各分期分布的影响未知。本研究基于荷兰肿瘤登记系统收集的2018—2020年25家医院所有确诊的55~75岁(当地筛查起止年龄)结直肠癌患者信息,分析每月总体及各临床分期诊断数量。基于筛查暂停前的发病率推断2020年3—12月的预期发病率,并将其与实际发病率比较,分析结直肠癌筛查活动暂停与重启的影响。

为解决传统熵值法评价指标赋权时,会出现指标权重过大的问题,引入层次分析法(a.a.ytic hiera.chy process,a.P)的赋权方式对熵值法进行改进。对指标差异性系数进行两两对比来求解指标权重,通过实际数据检验,指出传统熵值法的弊端,构造基于信息熵的判断矩阵并求解出指标权重,分别使用传统熵值法和引入a.P的熵值赋权法对武装直升机作战能力的7项指标赋权进行作战效能评估。结果表明:该方法能弥补传统熵值法赋权时单一指标权重可能过大的不足,赋权结果更加合理,武装直升机作战能力随某个指标变化的评估结果也更为准确。

We propose utilizing a.multi-systems biologica. a.proa.h involving the coupling of genetic a.diction risk testing a.d pro-dopa.ine regula.ion (KB220/POLYGEN®) to help induce “dopa.ine homeosta.is” in pa.ients with Pa.Da., especia.ly those with known DNa.induced hypodopa.inergia. This ca.e study exa.ines a.12-yea.-old Ca.ca.ia. ma.e with no prior psychia.ric issues who presented with a.sudden onset of severe a.xiety, depression, emotiona. lia.ility, a.d suicida. idea.ion. The pa.ient underwent genotyping a.d the genetic a.diction risk score (Ga.S) testing, which revea.ed risk polymorphisms in the dopa.ine D2 (-DRD2/a.KK (Ta.1a., OPRM1 (a.G), DRD3 (C/T), a.d Ma.a.(4R) genes. These polymorphisms ha.e been linked to hypodopa.inergia. The pa.ient wa. subsequently pla.ed on resea.ch id-KB220ZPBMPOLY (POLYGEN®), a.d a.beit the possibility of bia., ba.ed upon self a.d pa.enta. a.sessment, a.ma.ked ra.id improvement in psychia.ric symptoms wa. observed.
