Background: Esophageal strictures are considered to be one of the most challenging matters for gastroenterologist, general and thoracic surgeons in diagnosis and management. They can be grouped into three general categories: intrinsic diseases, extrinsic diseases, and diseases that disrupt esophageal peristalsis and/or lower esophageal sphincter (LES) function. Crohn’s disease (CD) is a very rare cause of esophageal stricture. The prevalence of esophageal CD ranges from 1% to 2% in adults with CD. It is almost diagnosed lately when complications have occurred as Strictures, fissures, esophagobronchial fistulas, and mediastinal abscesses. Case Report: Thirty-nine years old Kurdish patient, referred to our department for evaluation. Although many consultations during the last two years, the Pt was still undiagnosed. She had progressive dysphagia, and weight loss of about 25 kg. She had no other digestive or extra digestive complaint, nor caustic ingestion history and nor drug history. Retained placenta is a known cause of post-partum haemorrhage and maternal mortality. A recent systemic review has confirmed that the incidence of retained placenta had increased all over the world, which is more common in developed countries. Failure of retroplacental myometrium contraction is the main cause of retained placenta. Maternal age greater than 35 years, grandmultipara, preterm labor, history of previous retained placenta, and caesarean section were the risk factors for retained placenta. Manual removal of the placenta has been the treatment of choice. Attempts had been made by clinician and researchers to find a safe, effective and reliable method to avoid the need for surgical intervention. The efficacy and safety of prostaglandin, nitroglycerin or acupuncture in the management of retained placenta are yet to be further evaluated. Nonetheless, till date only intraumbilical vein oxytocin has been studied extensively but with varied success. More randomized clinical trials are needed to address this issue. However, if immediate manual placenta removal service is unavailable, a trial of intra-umbilical vein oxytocin 100 IU at a totalvolume of at least 40 m L while preparing for transfer to a tertiary center or theatre may result in spontaneous expulsion of the placenta.

A new type of calcium-based regenerable carbon dioxide （CO_2） sorbent, CaO-NiO, was synthesized via the sol-gel method. The as synthesized CO_2 sorbent was in the form of nanoparticles. The CO_2 sorption temperature and capacity of the sorbent were examined using thermogravi- metric analysis （TGA）. The CaO-NiO sorbent is able to capture CO_2 at a lower sorption temperature （465 ℃） than pure calcium oxide （CaO）（600 ℃）. The role of NiO in the CaO-NiO sorbent in lowering the CO_2 sorption temperature was also investigated. The sorbent was char- acterized by X-ray diffractometer （XRD）, N_2 adsorption-desorption analysis, high resolution transmission electron microscopy （HRTEM） and scanning electron microscopy （SEM）. CaO and NiO were found to coexist in the sorbent. Neither solid solution nor mixed metal oxide was formed. NiO did not react with CO_2 in the sorption process; but it worked like a catalyst to promote the CaO carbonation reaction. It is suggested that this new CaO-NiO sorbent may have a promising application as an effective CO_2 sorbent with lower energy consumption.

Several markers have been studied to diagnose PROM but there is still limitation with a gold standard test for the diagnosis [1]. Objective: To evaluate and compare the accuracy of urea, creatinine and hCG in vaginal fluid washing for diagnosis of PROM and to determine cut off levels. Type of the study: Prospective study performed on pregnant women in their third trimester who are coming to Kasr El Ainy maternity outpatient clinic with history of fluid leakage. Patients and methods: 150 patients between 26 - 40 weeks were included in the study. Group I included 50 patients with clinically confirmed ROM, group II included 50 with unconfirmed ROM and 50 controls with no complaints (group III). All patients were subjected to speculum examination for amniotic fluid pooling, nitrazine paper test, vaginal fluid washing for measurement of creatinine, urea and β-hCG, and ultrasound examination. The results were compared as regards the 3 study groups. Results: There was no statistically significant difference among the 3 study groups as regards age, parity BMI, nor gestational age. There was a statistically significant difference in creatinine level among confirmed, suspected and control groups. Urea levels showed also a statistically significant difference among the three groups. There was a positive correlation between vaginal creatinine and urea and gestational age in the 3 groups (r = 0.43, and 0.51 respectively). There was a statistically significant difference between vaginal fluid wash urea and creatinine levels and β-hCG as sensitivity, specificity, positive and negative predictive values and accuracy for urea and creatinine were all 100% and for qualitative β-hCG 83%, 100%, 100%, 85.6%, and 91% respectively. Conclusion: Urea and creatinine in vaginal washings can be accurately used in diagnosing suspected PROM and are more accurate than β-HCG.

The χ^2 family of signal fluctuation distributions represents the main fluctuation models which most radar targets follow it in their reflections. This family can be categorized as fluctuation distribution with two degrees of freedom and those with four degrees of freedom. The first category represents all important class of fluctuation models which when illuminated by a coherent pulse train, return a train of fully correlated pulses （Swerling Ⅰ model） or fully decorrelated pulses （Swerling Ⅱ model）. The detection of this type of fluctuating targets is therefore of great importance. This paper is devoted to the analysis of Cell-Averaging （CA） based detectors for the case where the radar receiver noncoherently integrates M square-law detected pulses and the signal fluctuation obeys 2 statistics with two degrees of freedom. These detectors include the Mean-Of （MO）, the Greatest-Of （GO） and the Smallest-Of（SO） schemes. In these processors, the estimation of the noise power levels from the leading and the trailing reference windows is based on the CA technique. Exact formulas for the detection probabilities are derived, in the absence as well as in the presence of spurious targets. The primary and the secondary interfering targets are assumed to be fluctuating in accordance with the χ^2 fluctuation model with two degrees of freedom （SWI ＆ SWII）. The numerical results show that the MO version has the best homogeneous performance, the SO scheme has the best multiple-target performance, while the GO procedure does not offer any merits, neither in the absence nor in the presence of outlying targets.

We report a case of calculous eosinophilic cholecystitis (EC) without symptoms suggesting the presence of Eosinophilic Granulomatosis with Polyangiitis (EGPA). A 68 years old male immunocompetent, presented to the Emergency Department (ED), with a picture of acute calculous cholecystitis. Upon his presentation, laboratory investigations showed no leukocytosis nor elevation of eosinophils count in blood serum, however, neutrophils count in serum was found to be high (76%, normal range up to 70%), then two days later patient underwent laparoscopic cholecystectomy. His final histopathology showed a picture of Eosinophilic Cholecystitis, post-operative detailed history, laboratory test including Antinuclear Antibody (ANA), Antineutrophil cytoplasmic Antibody (ANCA), immunoglobulin, and chest X-ray ruled out the possibility of the systemic disease (EGPA, Churg Strauss syndrome), and confirmed that the eosinophilia was localized to the gallbladder, which caused the picture of acute cholecystitis, and was treated sufficiently with the laparoscopic cholecystectomy.

During May 2022,a total of 237 cases,131 confirmed cases and 106 suspected cases of monkeypox virus have been reported to WHO from many countries that are not endemic for monkeypox virus(MPXV).Epidemiological investigations demonstrated new cases neither had travel history to endemic areas nor direct contact with infected animals,mostly but not totally these new cases within groups of men who have sex with men(MSM)seeking care in primary care and sexual health clinics[1,2].

Objectives: Study the effect of adding a delayed blastocyst to a transferred good quality one on ICSI cycle outcomes. Study design: Prospective cohort study. Participants/materials, setting, methods: 90 infertile patients aged from 20 - 35 years due to mild male factor, unexplained infertility or tubal factor. Patients with PCOS, endometriosis, RIF, poor responder and azoospermia were excluded. Setting: Duration 6 months from October 2019 to April 2020 in a private IVF center in Egypt. 30 cases were subjected to elective single embryo transfer and the other 60 with two embryo transfers, one good quality and another poor quality blastocyst. Results: (clinical pregnancy rate) was comparable between the two groups, being 47.7% and 36.7% in SET and DET groups respectively (p = 0.361). Miscarriage and ectopic pregnancy rate were observed in 10% of the cases pregnant after SET (group I) and in only 6.7% of the cases pregnant after DET (group II) denoting no significant statistical difference between the two groups. The highest incidence was missed miscarriages in the two groups and ectopic pregnancy was observed only in one case in group II (DET). Twin pregnancy occurred in 22% of pregnant females in the second group. Late pregnancy complications in the form of preterm labour, premature rupture of membrane and preeclampsia occurred mainly in pregnant females in the second group 18%. Water has a pH of around (7.0), making it neither alkaline nor acidic. Alkaline is defined as a pH value greater than (7.0). The power of water to neutralize acids is measured by its alkalinity. The pH scale ranges from (pH = 0 to 14), with anything above (pH = 7.0) being alkaline, and anything below (7.0) being acidic. Much of the research up to now has been investigating the effect of adding alkaline substances to the water to increase the pH levels. Recent evidence suggests that using baking soda (sodium bicarbonate), which is an alkaline substance with a pH of about (9). While studies have shown that adding soda to water increases its alkalinity, it has a number of negative health consequences. For instance, Hypokalemia, Hypochloremia, High levels of sodium in the blood, Worsening kidney disease, Worsening heart failure, Muscle weakness and spasm, and Increased stomach acid production. The main aim of this study is to investigate a natural substance consisting of environmental elements i.e., it is derived from silica by the thermal fusion method (prepared by the authors) that raises the pH of water with a physical field of 80 cm without adding any components to the water. The results showed the effectiveness of this natural substance in its solid and liquid forms in raising the pH of water to reach 8.5 from a distance range = 80 cm, without any side effects on human health.
