Organ-specific metastasis is the primary cause of cancer patient death.The distant metastasis of tumor cells to specific organs depends on both the intrinsic characteristics of the tumor cells and extrinsic factors in their microenvironment.During an intermediate stage of metastasis,circulating tumor cells(CTCs)are released into the bloodstream from primary and metastatic tumors.CTCs harboring aggressive or metastatic features can extravasate to remote sites for continuous colonizing growth,leading to further lesions.In the past decade,numerous studies demonstrated that CTCs exhibited huge clinical value including predicting distant metastasis,assessing prognosis and monitoring treatment response et al.Furthermore,increasingly numerous experiments are dedicated to identifying the key molecules on or inside CTCs and exploring how they mediate CTC-related organ-specific metastasis.Based on the above molecules,more and more inhibitors are being developed to target CTCs and being utilized to completely clean CTCs,which should provide promising prospects to administer advanced tumor.Recently,the application of various nanomaterials and microfluidic technologies in CTCs enrichment technology has assisted to improve our deep insights into the phenotypic characteristics and biological functions of CTCs as a potential therapy target,which may pave the way for us to make practical clinical strategies.In the present review,we mainly focus on the role of CTCs being involved in targeted organ metastasis,especially the latest molecular mechanism research and clinical intervention strategies related to CTCs.

Lenvatinib,a second-generation multi-receptor tyrosine kinase inhibitor approved by the FDA for first-line treatment of advanced liver cancer,facing limitations due to drug resistance.Here,we applied a multidimensional,high-throughput screening platform comprising patient-derived resistant liver tumor cells(PDCs),organoids(PDOs),and xenografts(PDXs)to identify drug susceptibilities for conquering lenvatinib resistance in clinically relevant settings.Expansion and passaging of PDCs and PDOs from resistant patient liver tumors retained functional fidelity to lenvatinib treatment,expediting drug repurposing screens.Pharmacological screening identified romidepsin,YM155,apitolisib,NVP-TAE684 and dasatinib as potential antitumor agents in lenvatinib-resistant PDC and PDO models.Notably,romidepsin treatment enhanced antitumor response in syngeneic mouse models by triggering immunogenic tumor cell death and blocking the EGFR signaling pathway.A combination of romidepsin and immunotherapy achieved robust and synergistic antitumor effects against lenvatinib resistance in humanized immunocompetent PDX models.Collectively,our findings suggest that patient-derived liver cancer models effectively recapitulate lenvatinib resistance observed in clinical settings and expedite drug discovery for advanced liver cancer,providing a feasible multidimensional platform for personalized medicine.

Unlike healthy, non-transformed cells, the proteostasis network of cancer cells is taxed to produce proteins involved in tumor development. Cancer cells have a higher dependency on molecular chaperones to maintain proteostasis. The chaperonin T-complex protein ring complex(TRiC) contains eight paralogous subunits(CCT1-8), and assists the folding of as many as 10% of cytosolic proteome.TRiC is essential for the progression of some cancers, but the roles of TRiC subunits in osteosarcoma remain to be explored. Here, we show that CCT4/TRiC is significantly correlated in human osteosarcoma,and plays a critical role in osteosarcoma cell survival. We identify a compound anticarin-β that can specifically bind to and inhibit CCT4. Anticarin-β shows higher selectivity in cancer cells than in normal cells. Mechanistically, anticarin-β potently impedes CCT4-mediated STAT3 maturation. Anticarin-β displays remarkable antitumor efficacy in orthotopic and patient-derived xenograft models of osteosarcoma.Collectively, our data uncover a key role of CCT4 in osteosarcoma, and propose a promising treatment strategy for osteosarcoma by disrupting CCT4 and proteostasis.

Despite the advance in targeted therapy,lung adenocarcinoma(LUAD)remains one of the leading causes of death in patients with cancer.CD155,a member of poliovirus receptor–related(PRR)family,has been identified as the ligand of the Ig-like receptors tactile(CD96)and DNAM-1(CD226)on T cells.The interaction of CD155 with CD96 transmits an inhibitory signal and suppresses immune response.The interaction of CD155 with CD226 enhances the immune response.1,2,3 CD155 expression has been shown to be elevated in many types of cancers4,5 and is involved with immune suppression in melanoma.6 However,how CD155/CD96 is involved in immune response in the tumor microenvironment of LUAD remains unknown.

Purpose: The aim of this study was to evaluate the prognostic value of the preoperative platelet-lymphocyte ratio and the neutrophil-lymphocyte ratio in patients with oral squamous cell carcinoma undergoing surgery. Methods: In this study, 306 patients with oral squamous cell carcinoma who had surgery were enrolled. The optimal cutoff value of platelet-lymphocyte ratio and neutrophil-lymphocyte ratio was determined by receiver operating(ROC) curve analysis. The prognostic significance of both markers was determined by uni-and multivariate analysis. Results: The results showed that high neutrophil-lymphocyte ratio and platelet-lymphocyte ratio were classified using a cut-off value of 2.7 and 135, respectively, based on ROC curve analysis. Only platelet-lymphocyte ratio was associated with decreased disease-free survival [hazard ratio(HR) = 2.237;95 % confidence interval(CI): 1.401–3.571;P =0.001] and overall survival [HR = 2.022;95 % CI: 1.266–3.228;P = 0.003] by both uni-and multivariate analysis. Conclusion: In conclusions, preoperative platelet-lymphocyte ratio is superior to neutrophil-lymphocyte ratio as an independent indicator in predicting disease-free survival and overall survival in patients who undergo oral cancer resection for oral squamous cell carcinoma.

AEG-1在恶性肿瘤中的研究进展

星形胶质细胞升高基因(Astrocyte elevated gene-1,AEG-1)最初通过快速消减杂交技术(rapid subtraction hybridization,RaSH)鉴定发现,在HIV-1或肿瘤坏死因子-α诱导的原代人胎脑星形胶质细胞(primary human fetal astrocvtes,PHFA)中其表达升高。AEG-1的cDNA由3 611个碱基组成,定位于8q22,分子量64kDa,等电点是9.33。研究发现,AEG-1定位于核周和类内质网结构区。AEG-1是Ha-ras和c-myc的下游靶分子。此外,近几年研究证实,AEG-1的致癌作用与激活PI3K-Akt和NF-κB信号通路有关。AEG-1最初在2002年被成功克隆,目前已被证实在多种器官的致癌作用过程中发挥关键作用。AEG-1在神经母细胞瘤、乳腺癌、肝癌和前列腺癌等多种恶性肿瘤中高表达,且与患者的生存率负相关。在体内、外通过提高或抑制AEG-1表达的研究进一步探明AEG-1调节细胞的多种生理、病理过程,包括增殖、侵袭、转移、血管新生和基因表达等。虽然AEG-1的致癌作用已经得到证实,但它作为一个癌基因的潜在功能和作用机制需要进一步探讨。本文就AEG-1与恶性肿瘤的关系做一综述,旨在以AEG-1为合理的靶点介入肿瘤治疗提供新的视野。

甲状腺未分化癌中PD-1和PD-L1的表达及临床病理意义

目的:探究程序性死亡受体-1(programmed death-1,PD-1)和程序性死亡受体配体-1(programmed deathligand-1,PD-L1)在甲状腺未分化癌(undifferentiated thyroid carcinoma,UTC)中的表达及其临床病理意义。方法:回顾性选取2014年1月-2021年12月佛山市三水区人民医院及中山大学精准医学科学中心收治的48例甲状腺癌患者作为研究对象。按照病理类型将其分为观察组(28例UTC)及对照组[20例甲状腺乳头状癌(papillary thyroid carcinoma,PTC)]。收集两组临床特征,检查两组PD-L1、PD-1表达情况。比较两组临床特征。比较两组及观察组不同临床特征患者PD-L1、PD-1表达情况。分析观察组中肿瘤细胞、间质免疫细胞PD-L1表达与间质免疫细胞PD-1表达的相关性。结果:观察组年龄41~85岁,平均年龄为69岁;肿物最大径为2.1~7.0 cm,平均5.0 cm。对照组年龄23~59岁,平均年龄为43岁;肿物最大径为0.7~4.0 cm,平均1.3 cm。观察组年龄≥60岁、肿物最大径≥4 cm和血管侵犯占比均显著高于对照组(P0.05)。观察组不同临床特征患者肿瘤细胞、间质免疫细胞的PD-L1阳性率及间质免疫细胞中PD-1阳性率比较差异均无统计学意义(P>0.05)。肿瘤细胞中PD-L1的表达与间质免疫细胞PD-1表达无相关性(P>0.05);间质免疫细胞中PD-L1的表达与间质免疫细胞PD-1的表达无相关性(P>0.05)。结论:UTC肿瘤细胞及间质免疫细胞中有较高的PD-L1阳性表达率。

治疗前NLR和PLR对宫颈鳞癌患者预后的影响

目的探讨经手术治疗的宫颈鳞癌患者治疗前中性粒细胞与淋巴细胞比(neutrophil-tolymphocyte ratio,NLR)及血小板与淋巴细胞比(platelet-to-lymphocyteratio,PLR)与总生存期(overall survival,OS)、无病生存期(disease-free survival,DFS)和各临床病理因素的相关性。方法回顾性分析中山大学附属第一医院于2005年1月1日至2009年12月31日期间初治的并且经过手术治疗的143例宫颈鳞癌患者的临床病理资料,根据统计学方法选取NLR和PLR截断值,将患者根据截断值分组,分析NLR和PLR与患者生存、复发情况的相关性以及与临床病理因素的相关性。结果选取NLR=2.8和PLR=125分别作为截断值,高NLR组5年生存率为30.3%,低NLR组5年生存率为80%,差异有统计学意义(P=0.000)。高NLR组复发率为69.7%,低NLR组复发率为24.5%,差异有统计学意义(P=0.000)。高PLR组5年生存率为53.7%,低PLR组5年生存率为81.6%,差异有统计学意义(P=0.000)。高PLR组复发率为50%,低PLR组复发率为22.4%,差异有统计学意义(P=0.000)。经过单因素和多因素分析,国际妇产科联盟(International Federation of Gynecology and Obstetrics,FIGO)分期、术前化疗、病灶大小、外周血血小板计数(platelet,PLT),NLR值和PLR值是影响宫颈鳞癌患者总生存率和无病生存率的独立危险因素。结论治疗前NLR及PLR均是影响宫颈鳞癌患者总生存时间和无病生存时间的独立危险因素。

BALB/c与SCID裸小鼠高侵袭性人结肠癌肝转移模型的比较研究

目的建立裸小鼠人结肠癌肝转移的高侵袭性模型,比较BALB/c鼠与SCID鼠的人结肠癌肝转移模型,为侵袭性人结肠癌的研究寻找可靠的动物模型。方法首先将人结肠癌细胞株HT-29细胞悬液接种于免疫功能正常BALB/c鼠的脾包膜下,然后将其肝转移肿瘤组织通过原位移植方式分别种植于免疫缺陷BALB/c鼠和SCID鼠的盲肠浆膜下,而第三组直接将HT-29种植于结肠,最后观察三组的成瘤及死亡情况,以及淋巴转移率、肝转移率、肺转移率及腹腔种植率等。结果术后21 d观察,40只BALB/c鼠有38只成瘤,其余2只死亡;40只SCID鼠有33只成瘤,其余7只死亡;第三组40只(20只BALB/c鼠/20只SCID鼠)裸小鼠虽有29只成瘤,且仅1只死亡,但肝转移率(24.1%)较低,不适于建立高侵袭模型。前两组在原位移植瘤大小、转移和种植率上差异无统计学意义(P>0.05),而两组成瘤率和死亡率的差异有统计学意义(P<0.05)。结论将筛选出的人结肠癌肝转移组织用于建立模型,能有效模拟高侵袭性等生物学特性,再结合BALB/c鼠价格较低的优势,所以更适宜作为人结肠癌肝转移的可靠动物模型。
