Benefits of implementing a rapid access clinic in a high-volume inflammatory bowel disease center:Access, resource utilization and outcomes

BACKGROUND Emergency situations in inflammatory bowel diseases(IBD)put significant burden on both the patient and the healthcare *** To prospectively measure Quality-of-Care indicators and resource utilization after the implementation of the new rapid access clinic service(RAC)at a tertiary IBD *** Patient access,resource utilization and outcome parameters were collected from consecutive patients contacting the RAC between July 2017 and March 2019 in this observational *** comparing resource utilization and healthcare costs,emergency department(ED)visits of IBD patients with no access to RAC services were evaluated between January 2018 and January *** to appointment,diagnostic methods,change in medical therapy,unplanned ED visits,hospitalizations and surgical admissions were calculated and *** 488 patients(Crohn’s disease:68.4%/ulcerative colitis:31.6%)contacted the RAC with a valid medical *** time to visit with an IBD specialist following the index contact was 2 *** had objective clinical and laboratory assessment(C-reactive protein and fecal calprotectin in 91%and 73%).Fast-track colonoscopy/sigmoidoscopy was performed in 24.6%of the patients,while computed tomography/magnetic resonance imaging in only 8.1%.Medical therapy was changed in 54.4%.ED visits within 30 d following the RAC visit occurred in 8.8%(unplanned ED visit rate:5.9%).Diagnostic procedures and resource utilization at the ED(n=135 patients)were substantially different compared to RAC users:Abdominal computed tomography was more frequent(65.7%,P<0.001),coupled with multiple specialist consults,more frequent hospital admission(P<0.001),higher steroid initiation(P<0.001).Average medical cost estimates of diagnostic procedures and services per patient was$403 CAD vs$1885 CAD comparing all RAC and ED *** Implementation of a RAC improved patient care by facilitating easier access to IBD specific medical care,optimized resource uti

Prevalence and predictors of hospitalization in Crohn's disease in a prospective population-based inception cohort from 2000-2012

AIM: To analyze the prevalence, length and predictors of hospitalization in the biological era in the populationbased inception cohort from Veszprem ***: Data of 331 incident Crohn's disease(CD) patients diagnosed between January 1, 2000 and December 31, 2010 were analyzed(median age at diagnosis: 28; IQR: 21-40 years). Both in- and outpatient records were collected and comprehensively ***: Probabilities of first CD-related hospitalization and re-hospitalization were 32.3%, 45.5%,53.7% and 13.6%, 23.9%, 29.8%, respectively after one, three and five years of follow-up in Kaplan-Meier analysis. First-year hospitalizations were related to diagnostic procedures(37%), surgery or disease activity(27% and 21%). Non-inflammatory disease behavior at diagnosis(HR = 1.32, P = 0.001) and perianal disease(HR = 1.47, P = 0.04) were associated with time to first CD-related hospitalization, while disease behavior change(HR = 2.38, P = 0.002) and need for steroids(HR = 3.14, P = 0.003) were associated with time to first re-hospitalization in multivariate *** CD-related hospitalization(within the year of diagnosis) was independently associated with need for immunosuppressives(OR = 2.08, P = 0.001) and need for surgeries(OR = 7.25, P < 0.001) during the disease ***: Hospitalization and re-hospitalization rates are still high in this cohort, especially during the first-year after the diagnosis. Non-inflammatory disease behavior at diagnosis was identified as the pivotal predictive factor of both hospitalization and rehospitalization.

Burden of Clostridium difficile infection between 2010 and 2013:Trends and outcomes from an academic center in Eastern Europe

AIM:To analyze the incidence and possible risk factors in hospitalized patients treated with Clostridium difficile infection(CDI).METHODS:A total of 11751 patients were admitted to our clinic between 1 January 2010 and 1 *** hundred and forty-seven inpatients were prospectively diagnosed with *** the risk analysis a 1:3 matching was *** of 732 patients matched for age,sex,and inpatient care period and unit were compared to those of the CDI *** records were collected from an electronic hospital database and comprehensively ***:Incidence of CDI was 21.0/1000 admissions(2.1%of all-cause hospitalizations and 4.45%of total inpatient days).The incidence of severe CDI was 12.6%(2.63/1000 of all-cause hospitalizations).Distribution of CDI cases was different according to the unit type,with highest incidence rates in hematology,gastroenterology and nephrology units(32.9,25 and24.6/1000 admissions,respectively) and lowest rates in 1.4%(33/2312) in endocrinology and general internal medicine(14.2 and 16.9/1000 admissions)*** of CDI was 11.3%within 12 wk after *** of hospital stay was longer in patients with CDI compared to controls(17.6 ± 10.8d vs 12.4 ± 7.71 d).CDI accounted for 6.3%of allinpatient deaths,and 30-d mortality rate was 21.9%(54/247 cases).Risk factors for CDI were antibiotic therapy[including third-generation cephalosporins or fluoroquinolones,odds ratio(OR) = 4.559;P < 0.001],use of proton pump inhibitors(OR = 2.082,P< 0.001),previous hospitaiization within 12 mo(OR = 3.167,P < 0.001),previous CDI(OR = 15.32;P < 0.001),while presence of diabetes mellitus was associated with a decreased risk for CDI(OR = 0.484;P< 0.001).Treatment of recurrent cases was significantly different from primary infections with more frequent use of vancomycin alone or in combination(P < 0.001),and antibiotic therapy duration was longer(P < 0.02).Severity,mortality and outcome of primary infections and relapsing

In contrast to large-effect qualitative disease resistance,quantitative disease resistance(QDR)exhibits partial and generally durable resistance and has been extensively utilized in crop *** molecular mechanisms underlying QDR remain largely unknown but considerable progress has been made in this area in recent *** this review,we summarize the genes that have been associated with plant QDR and their biological *** QDR genes belong to the canonical resistance gene categories with predicted functions in pathogen perception,signal transduction,phytohormone homeostasis,metabolite transport and biosynthesis,and epigenetic ***,other"atypical"QDR genes are predicted to be involved in processes that are not commonly associated with disease resistance,such as vesicle trafficking,molecular chaperones,and *** diversity of function for QDR genes contrasts with qualitative resistance,which is often based on the actions of nucleotidebinding leucine-rich repeat(NLR)resistance *** understanding of the diversity of QDR mechanisms and of which mechanisms are effective against which classes of pathogens will enable the more effective deployment of QDR to produce more durably resistant,resilient crops.

A Time-Dependent Wavepacket Method for Photodissociation Dynamics of Triatomic Molecule

We report a time-dependent quantum wavepacket theory employed to interpret the photoabsorption spectrum of the N20 molecule in terms of the nuclear motion on the upper 21A＇ and 11A＂ potential energy surfaces. The N2-O bond breaks upon excitation leading to dissociation. The total angular momentum is treated correctly taking into account the vector property of the electric field of the exciting radiation.

Maize is the world＇s most produced crop, providing food, feed, and biofuel. Maize production is constantly threatened by the presence of devastating pathogens worldwide. Characterization of the genetic compo- nents underlying disease resistance is a major research area in maize which is highly relevant for resistance breeding programs. Quantitative disease resistance （QDR） is the type of resistance most widely used by maize breeders. The past decade has witnessed significant progress in fine-mapping and cloning of genes controlling QDR. The molecular mechanisms underlying QDR remain poorly understood and exploited. In this review we discuss recent advances in maize QDR research and strategy for resistance breeding.

The efficacy of a verification stage for determining VO_(2max)and the impact of sampling intervals

It is unknown whether oxygen uptake(VO_(2))sampling intervals influence the efficacy of a verification stage following a graded exercise test(GXT).Fifteen females and 14 males(18-25 years)completed a maximal treadmill *** a 5min recovery,the verification stage began at the speed and grade corresponding with the penultimate stage from the *** oxygen consumption(VO_(2max))from the incremental GXT(iVO_(2max))and VO_(2max)from the verification stage(verVO_(2max))were determined using 10 seconds(s),30s,and 60s from breath×breath *** was no main effect for VO_(2max)measure(iVO_(2max)***_(2max))10s([47.9±8.31]ml∙kg^(−1)∙min^(−1)vs[48.85±7.97]ml∙kg^(−1)∙min^(−1)),30s([46.94±8.62]ml∙kg^(−1)∙min^(−1)vs[47.28±7.97]ml∙kg^(−1)∙min^(−1)),and 60s([46.17±8.62]ml∙kg^(−1)∙min^(−1)vs[46.00±8.00]ml∙kg^(−1)∙min^(−1)].There was a stage×sampling interval interaction as the difference between(verVO_(2max)−iVO_(2max))was greater for 10-s than 60-s sampling *** verVO_(2max)was>4%higher than iVO_(2max)in 31%,31%,and 17%of the tests for the 10-s,30-s,and 60-s sampling intervals *** for the plateau was90%for all sampling intervals;while specificity was<25%.Findings from the present study suggest that the efficacy of verification stages for eliciting a higher VO_(2max)may be influenced by the sampling interval utilized.
