The extent to which specific climatic factors influence evapotranspiration under subhumid conditions in Hungary was investigated. The reference evapotranspiration, calculated with the internationally accepted Penman-Monteith equation proposed by FAO, was considered. The results show that the influence of radiation, which provides energy for evaporation, is the strongest factor and that the influence of global radiation alone is very strong. Taking into account that radiation was measured under rather limited conditions in space and time, global radiation was calculated using the Hargreaves method based on temperature. Accordingly, we have defined a formula based on temperature-based global radiation and verified the data obtained with the Penman-Monteith formula calculated for 14 meteorological stations. The verification gave good results, therefore the method can be used for practical purposes in the subhumid conditions of Hungary based on the data of the nearest meteorological station.

Objective: Early recognition is a key step in decreasing morbidity and mortality in sepsis. Like in other European countries an ever growing number of septic patients enter hospitals via emergency departments. Our goal was to assess the number and severity of patients with sepsis, severe sepsis and septic shock attending the Hungarian emergency departments. Methods: Two-hundred and ninety-nine patients were entered into the Hungarian Emergency Sepsis Register, focusing on age, disease severity, monitoring and oxygenation. Application and time of administration of antibiotics, use of biomarkers, i.e. lactate and procalcitonin, disposition of patients were assessed using a web-based electronic questionnaire. Results: Data arriving from twenty emergency departments revealed that with severity of the disease oxygen administration and level of monitoring increased, along with antibiotic administration and use of biomarkers. Time spent in the ED did not differ between septic, severely septic and septic shock patients;however higher level of care was provided for the sickest patients. Conclusion: Our register data indicate that sepsis presents a substantial challenge to Hungarian emergency departments. The set standards for timely, adequate treatment are not yet met. The results are however also influenced by characteristics of the Hungarian health care system, most notably the lack of hospice systems. Further studies are needed to separate the subset of patients who would require and of life care.

Effect of herpesvirus infection on pancreatic duct cell secretion

AIM： To examine the effect of acute infection caused by herpesvirus （pseudorabies virus, PRV） on pancreatic ductal secretion. METHODS： The virulent Ba-DupGreen （BDG） and non- virulent Ka-RREp01acgfp （KEG） genetically modified strains of PRV were used in this study and both of them contain the gene for green fluorescent protein （GFP）. Small intra/ interlobular ducts were infected with BDG virus （10^7 PFU/mL for 6 h） or with KEG virus （10^10 PFU/mL for 6 h）, while non-infected ducts were incubated only with the culture media. The ducts were then cultured for a further 18 h. The rate of HCO3 secretion [base efflux -λ（B）] was determined from the buffering capacity of the cells and the initial rate of intracellular acidification （1） after sudden blockage of basolateral base loaders with dihydro-4,4,- diisothiocyanatostilbene-2,2,-disulfonic acid （500 μmol/L） and amiloride （200 μmol/L）, and （2） after alkali loading the ducts by exposure to nH4Cl. All the experiments were performed in HCO3-buffered Ringer solution at 37 ℃ （n = 5 ducts for each experimental condition）. Viral structural proteins were visualized by immunohistochemistry. Virallyencoded GFP and immunofluorescence signals were recorded by a confocal laser scanning microscope.RESULTS： The BDG virus infected the majority of accessible cells of the duct as judged by the appearance of GFP and viral anUgens in the ductal cells. KEG virus caused a similarly high efficiency of infection. After blockage of basolateral base loaders, BDG infection significantly elevated -λB-） 24 h after the infection, compared to the non-infected group. However, KEG infection did not modify -λB-）. After alkali loading the ducts, -λB-） was significantly elevated in the BDG group compared to the control group 24 h after the infection. As we found with the inhibitor stop method, no change was observed in the group KEG compared to the non-infected group. ＜正＞ 1 InTRODUCTIOn The owner’s intention of WWER-440/213 units in Central Europe is to keep the generating capacity in operation as long as technically feasible and reasonable from business point of view. The basis of Long Term Operation （LTO） is the safety and good plant condition. The last depends on the past practice of in-service inspection, maintenance, testing and recording the lifetime relevant data as well as on the knowledge of the design basis of the lifetime limiting structures, systems and components （SSC）. In the paper an overall picture of LTO of WWER-440/213 units is given on the examples of Paks nPP.

Visceral leishmaniasis in an immunocompetent Hungarian adult patient

Visceral leishmaniasis caused by L. infantum nicolle 1908 (Kinetoplastida, Trypanosomatidae) is very rare in immunocompetent adults. Clinical manifestation of the infection occurs in children and immune-compromised patients associated with AIDS, haemopoietic malignancies or after renal, liver, and heart transplantations in Europe. Hungary is regarded free of leishmaniasis. Except for a single infection in a small girl without a travel history, only a few imported cases have been recorded among Hungarians returning from endemic areas. Case presentation: Visceral leishmaniasis was diagnosed in a 32 years old immumocompetent Hungarian man who had spent his holidays during the previous 3 years in Dalmatian territories (Dubrovnik, Makarska and Trogir) of Croatia. He had two months history of fever, chills, and night sweating associated with weight loss. Physical examination showed mild hepatomegaly and extreme splenomegaly. The protozoon infection was confirmed by parasitological, serological and molecular biological methods. Clinical recovery of the patient was observed after treatment with amphotericin B. Conclusion: We want to highlight the effects of climate changes and to call attention of health care professionals in Central and Eastern European countries to consider the possibility of leishmaniasis as an emerging disease of tourists returned from endemic re-gions.

Comparison of Economically Favourable and Further Development Friendly DnA Isolation Methods from Microbial Cultures

Microorganisms, regardless of whether pathogenic or not, may cause enormous economic losses due to adverse effects on human and animal health, or by damaging the quality of the agricultural and food products. Based on these effects, the development of prompt molecular methods and their involvement in the practical pathogen diagnostic diagnostics is more than actual. This paper is focused on the evaluation of easy-to-perform and highly budget-friendly, PCR-related DnA purification protocols for diagnostic purposes especially in water or similar simple matrices. The slight modifications of earlier described DnA isolation methods, which rely on chelate exchange resin and/or ethanol-sodium-based heat lysis, we reevaluated in comparison with a widely used commercial kit. The efficiency of DnA purification techniques was assessed from Gramnegative as well as Gram-positive bacteria and yeast using quantitative PCR. The effectivity of different methods tested may vary depending on the bacterial or yeast species in question. nevertheless, in our hands, the chelate exchange resin-based methods were found to be the most robust and/or satisfying at least by an acceptable reproducibility rate. Our presented results support the potential of low-cost but still sensitive molecular microbe detection procedures consisting of only a few pipetting steps resulting in good reproducibility and the least possible environmental burden, serving as a good starting point for developments of matrix-specific processes and methods.
