Background Despite the increasing focus on strengthening One Health capacity building on global level, challenges remain in devising and implementing real-world interventions particularly in the Asia-Pacific region. Recognizing these gaps, the One Health Action Commission (OHAC) was established as an academic community for One Health action with an emphasis on research agenda setting to identify actions for highest impact.Main text This viewpoint describes the agenda of, and motivation for, the recently formed OHAC. Recognizing the urgent need for evidence to support the formulation of necessary action plans, OHAC advocates the adoption of both bottom-up and top-down approaches to identify the current gaps in combating zoonoses, antimicrobial resistance, addressing food safety, and to enhance capacity building for context-sensitive One Health implementation.Conclusions By promoting broader engagement and connection of multidisciplinary stakeholders, OHAC envisions a collaborative global platform for the generation of innovative One Health knowledge, distilled practical experience and actionable policy advice, guided by strong ethical principles of One Health.

Background One Health approach is crucial to tackling complex global public health threats at the interface of humans, animals, and the environment. As outlined in the One Health Joint Plan of Action, the international One Health community includes stakeholders from different sectors. Supported by the Bill & Melinda Gates Foundation, an academic community for One Health action has been proposed with the aim of promoting the understanding and real-world implementation of One Health approach and contribution towards the Sustainable Development Goals for a healthy planet.Main text The proposed academic community would contribute to generating high-quality scientific evidence, distilling local experiences as well as fostering an interconnected One Health culture and mindset, among various stakeholders on different levels and in all sectors. The major scope of the community covers One Health governance, zoonotic diseases, food security, antimicrobial resistance, and climate change along with the research agenda to be developed. The academic community will be supported by two committees, including a strategic consultancy committee and a scientific steering committee, composed of influential scientists selected from the One Health information database. A workplan containing activities under six objectives is proposed to provide research support, strengthen local capacity, and enhance global participation.Conclusions The proposed academic community for One Health action is a crucial step towards enhancing communication, coordination, collaboration, and capacity building for the implementation of One Health. By bringing eminent global experts together, the academic community possesses the potential to generate scientific evidence and provide advice to local governments and international organizations, enabling the pursuit of common goals, collaborative policies, and solutions to misaligned interests.

Identifying depression and its determinants upon initiating treatment: ChatGPT versus primary care physicians

Objective To compare evaluations of depressive episodes and suggested treatment protocols generated by Chat Generative Pretrained Transformer(ChatGPT)-3 and ChatGPT-4 with the recommendations of primary care physicians.Methods Vignettes were input to the ChatGPT interface.These vignettes focused primarily on hypothetical patients with symptoms of depression during initial consultations.The creators of these vignettes meticulously designed eight distinct versions in which they systematically varied patient attributes(sex,socioeconomic status(blue collar worker or white collar worker)and depression severity(mild or severe)).Each variant was subsequently introduced into ChatGPT-3.5 and ChatGPT-4.Each vignette was repeated 10 times to ensure consistency and reliability of the ChatGPT responses.Results For mild depression,ChatGPT-3.5 and ChatGPT-4 recommended psychotherapy in 95.0%and 97.5%of cases,respectively.Primary care physicians,however,recommended psychotherapy in only 4.3%of cases.For severe cases,ChatGPT favoured an approach that combined psychotherapy,while primary care physicians recommended a combined approach.The pharmacological recommendations of ChatGPT-3.5 and ChatGPT-4 showed a preference for exclusive use of antidepressants(74%and 68%,respectively),in contrast with primary care physicians,who typically recommended a mix of antidepressants and anxiolytics/hypnotics(67.4%).Unlike primary care physicians,ChatGPT showed no gender or socioeconomic biases in its recommendations.Conclusion ChatGPT-3.5 and ChatGPT-4 aligned well with accepted guidelines for managing mild and severe depression,without showing the gender or socioeconomic biases observed among primary care physicians.Despite the suggested potential benefit of using atificial intelligence(AI)chatbots like ChatGPT to enhance clinical decision making,further research is needed to refine AI recommendations for severe cases and to consider potential risks and ethical issues.

Background: Glycogenic hepatopathy(GH) is a disorder associated with uncontrolled diabetes mellitus,most commonly type 1, expressed as right upper quadrant abdominal pain, hepatomegaly and increased liver enzymes. The diagnosis may be difficult, because laboratory and imaging tests are not pathognomonic. Although GH may be suggested based on clinical presentation and imaging studies, the gold standard for diagnosis is a liver biopsy, showing a significant accumulation of glycogen within the hepatocytes. GH may be diagnosed also after elevated liver enzymes in routine blood tests. GH usually regresses after tight glycemic control. Progression to end-stage liver disease has never been reported. This review aims to increase the awareness to this disease, to suggest a pathway for investigation that may reduce the use of unnecessary tests, especially invasive ones.Data sources: A Pub Med database search(up to July 1, 2017) was done with the words "glycogenic hepatopathy", "hepatic glycogenosis", "liver glycogenosis" and "diabetes mellitus-associated glycogen storage hepatopathy". Articles in which diabetes mellitus-associated liver glycogen accumulation was described were included in this review.Results: A total of 47 articles were found, describing 126 patients with GH. Hepatocellular disturbance was more profound than cholestatic disturbance. No synthetic failure was reported.Conclusions: GH may be diagnosed conservatively, based on corroborating medical history, physical examination, laboratory tests, imaging studies and response to treatment, even without liver biopsy. In case of doubt about the diagnosis or lack of clinical response to treatment, a liver biopsy may be considered.There is no role for noninvasive tests like fibroscan or fibrotest for the diagnosis of GH or for differentiation of this situation from nonalcoholic fatty liver disease.

Mucin gene expression in bile of patients with and without gallstone disease,collected by endoscopic retrograde cholangiography

AIM:To investigate the pattern of mucin expression and concentration in bile obtained during endoscopic retrograde cholangiography(ERC) in relation to gallstone disease.METHODS:Bile samples obtained at ERC from 29 consecutive patients,17 with and 12 without gallstone disease were evaluated for mucin content by gel filtration on a Sepharose CL-4B column.Dot blot analysis for bile mucin apoproteins was performed with antibodies to Mucin 1(MUC1) ,MUC2,MUC3,MUC5AC,MUC5B and MUC6.Staining intensity score(0-3) was used as a measure of antigen expression.RESULTS:MUC1,MUC2,MUC3,MUC5AC,MUC5B and MUC6 were demonstrated in 34.4%,34.4%,51.7%,51.7%,55.1% and 27.5% of bile samples,respectively.The staining intensity scores were 0.62 ± 0.94,0.58 ± 0.90,0.79 ± 0.97,1.06 ± 1.22,1.20 ± 1.26 and 0.41 ± 0.73,respectively.Mean mucin concentration measured in bile by the Sepharose CL-4B method was 22.8 ± 24.0 mg/mL(range 3.4-89.0 mg/mL) .Mean protein concentration was 8.1 ± 4.8 mg/mL(range 1.7-23.2 mg/mL) .CONCLUSION:High levels of MUC3,MUC5AC and MUC5B are expressed in bile aspirated during ERC examination.A specific pattern of mucin gene expression or change in mucin concentration was not found in gallstone disease.

Gastric mucin expression in Helicobacter pylori-related, nonsteroidal anti-inflammatory drug-related and idiopathic ulcers

AIM: To determine the pattern of secreted mucin expression in Helicobacter pylori (H. pylori)-related, nonsteroidal anti-inflammatory drug (NSAID)-related and idiopathic gastric ulcers. METHODS: We randomly selected 92 patients with H. pylori-associated (n = 30), NSAID-associated (n = 18), combined H. pylori and NSAID-associated gastric ulcers (n = 24), and patients with idiopathic gastric ulcers (n = 20). Immunohistochemistry for T-cell CD4/CD8, andfor mucin 5AC (MUC5AC) and mucin 6 (MUC6), was performed on sections of the mucosa from the ulcer margin. Inflammation score was assessed according to the Sydney system. RESULTS: MUC5AC was expressed on the surface epithelium (98.9%) and neck glands (98.9%) with minimal expression in the deep glands (6.5%). MUC6 was strongly expressed in the deep glands (97.8%), variable in the neck glands (19.6%) and absent in the surface epithelium (0%). The pattern of mucin expression in idiopathic ulcer margins was not different from the expression in ulcers associated with H. pylori, NSAIDs, or combined H. pylori and NSAIDs. CD4/CD8 ratio was higher in H. pylori-positive patients (P = 0.009). Idiopathic ulcers are associated with hospitalized patients and have higher bleeding and mortality rates. CONCLUSION: Idiopathic ulcers have a unique clinical profile. Gastric mucin expression in idiopathic gastric ulcers is unchanged compared with H. pylori and/or NSAID-associated ulcers.

BACKGROUND Although the association of attention deficit hyperactivity disorder(ADHD)with psychiatric disorders is well known,its association with somatic diseases is unclear.Only few studies have investigated the gastrointestinal(GI)morbidity in adult patients with ADHD.AIM To measure gastrointestinal comorbidity and its burden on healthcare in young adults with ADHD.METHODS The cohort included subjects aged 17-35 years recruited to the Israel Defense Forces in 2007-2013,33380 with ADHD and 355652 without(controls).The groups were compared for functional and inflammatory conditions of the gastrointestinal tract and clinic and specialist visits for gastrointestinal symptoms/disease during service(to 2016).Findings were analyzed by generalized linear models adjusted for background variables.RESULTS Compared to controls,the ADHD group had more diagnoses of functional gastrointestinal disorders(referred to as FGID),namely,dyspepsia[odds ratio(OR):1.48,95%confidence interval(CI):1.40-1.57,P<0.001],chronic constipation(OR:1.64,95%CI:1.48-1.81,P<0.001),and irritable bowel syndrome(OR:1.67,95%CI:1.56-1.80,P<0.001)but not of organic disorders(inflammatory bowel disease,celiac disease).They had more frequent primary care visits for gastrointestinal symptoms[rate ratio(RR):1.25,95%CI:1.24-1.26,P<0.001]and referrals to gastrointestinal specialists(RR:1.96,95%CI:1.88-2.03,P<0.001)and more episodes of recurrent gastrointestinal symptoms(RR:1.29,95%CI:1.21-1.38,P<0.001).Methylphenidate use increased the risk of dyspepsia(OR:1.49,95%CI:1.28-1.73,P<0.001)and constipation(OR:1.42,95%CI:1.09-1.84,P=0.009).CONCLUSION ADHD in young adults is associated with an excess of FGID and increased use of related health services.Research is needed to determine if an integrative approach treating both conditions will benefit these patients and cut costs.

运用光刻胶为注模的多次互不干扰金属电镀技术实现了惯性微型电学开关的低温制造与封装.电镀技术的低温过程可使微型开关直接成形于预先制作好的含有电子信号处理电路的基底上,加上同样借助于低温金属电镀技术的基于整个硅晶片的倒装封装,直接形成环绕各个器件的密封腔体.这一技术最终将使得模块化生产成为现实.微型开关的高度和它的密封腔的高度可以分别控制.电子信号可以通过金属互连线进入密封腔体.为了便于设计,建立了一个既简单又相对准确的“弹簧质量块”模型.以此设计的惯性开关,即使在未封装的常温、常压条件下,均可工作109次以上.本文对密封腔体的强度和密封性,以及金属互连线的可靠性,都作了详细的检测,各项指标均达到其各自的标准.

Inflammation and coagulation are tightly interconnected in the pathophysiology of neuronal diseases.Thrombin,a pro-coagulant serine protease is associated with neurodegeneration and its indirect inhibitor,activated protein C(aPC),is considered neuroprotective.While levels of thrombin and aPC activity are readily measured in the blood,similar assays in the cerebrospinal fluid(CSF)have not been described.The aim of this study was to establish a specific and sensitive enzymatic assay to measure both thrombin and aPC activity in the CSF.CSF was collected from 14 patients with suspected normal pressure hydrocephalus served as a control group,while seven patients with central nervous system infections served as an acute neuro-inflammatory study group and one sample of CSF following traumatic lumbar puncture served as a positive control.Thrombin and aPC activities were measured by fluorescence released by specific proteolytic cleavage in the presence of endopeptidase and amino-peptidase inhibitors to ensure specificity.Specificity of the method was verified by thrombin and serine-protease inhibitors N-alpha-((2-naphthylsulfinyl)glycyl)-DL-p-amidinophenylalanylpiperidine and phenylmethanesulfonyl fluoride.Inhibition of thrombin activity by CSF samples and levels of specific thrombin inhibitors were also assessed.Thrombin and aPC activities were reliably measured and were significantly higher in the CSF of patients with central nervous system infections compared to normal pressure hydrocephalus controls,suggesting the involvement of these factors in neuro-inflammation.CSF thrombin activity levels in the presence of known thrombin concentration were high in patients with central nervous system infections,and low in normal pressure hydrocephalus patients.Quantification of endogenous thrombin inhibitors protease nexin 1,amyloid precursor protein and anti-thrombin III in CSF by western blot indicated a significant elevation of amyloid precursor protein in infectious CSF.In conclusion,this study describes a novel and sensitive

在世界范围内,癌症的死亡率仍在逐年上升.循环肿瘤细胞(circulating tumor cells,CTCs)是指从原发肿瘤脱落并进入血液循环系统的细胞,可能引发肿瘤转移并入侵其他正常组织和器官.因此,CTCs的检测结果可以作为癌症病人疗效和预后的评价指标.但是CTCs的数量及其稀少,使得CTCs的检测尤为困难.在癌症转移的病人中,每毫升血液约含有10～100个CTCs.利用经生物活性材料表面修饰的微流控器件,可以从血液中分离出CTCs.这是一项跨学科的挑战,需要来自不同学科背景的专家们共同参与,如细胞生物学、表面化学、流体力学及微纳加工技术等.该文首先介绍了CTCs的细胞生物学基础,然后总结了当前分离CTCs的主要微流控技术,包括基于细胞-配体作用、磁力作用和过滤等,最后综述了基于微流控技术的CTC检测和计数、在体CTC成像等最新研究进展.
